    La formazione psicoanalitica in medicina

    No full text
The authors address the issue of approaching psychoanalysis at university in medical faculties in Italy today. More than a century after Freud’s “extraordinary” academic lectures, the resources available appear particularly limited within degree courses. These elements of reality often make the task of presenting the topicality of psychoanalysis difficult, even when one limits oneself to the psychodynamic “fundamentals”: the relevance of the asymmetric doctor-patient relationship, for example, that of unconscious dynamics, that of the setting, that of the problematic mind-body relationship. More space is, on the other hand, available in the specialisation schools in Child Neuropsychiatry and Psychiatry when teachers with the necessary sensitivity and qualifications are available. In these areas, alongside theoretical lectures and seminars, where SPI members could play an important role, the work of sensitising and transmitting the fundamental concepts of psychoanalysis takes place mainly through reflection on the daily clinical work of the learners. The role of personal analysis, supervision, self-analysis and other experiences of self-reflection and self-orientation in academic psychotherapy training is emphasised, as well as the importance of acquiring psychotherapeutic know-how. The presence of committed psychoanalysts in healthcare institutions belonging to university training networks thus appears crucial for the transmission of psychoanalytic sensitivity in our country

Automated vision-based concrete crack measurement system

Maintaining and ensuring the safety of ageing infrastructures is becoming of uttermost importance. This is particularly critical in Europe, where several infrastructures like tunnels and bridges are approaching their end-of-life. Such a condition justifies the need for a prompt recognition of the deterioration status of these structures as well as the identification of strategies for effective inspection and monitoring operations. This paper focuses on cracks, and proposes a vision-based measurement system aimed at precisely and accurately detecting cracks on concrete surfaces and measuring their geometric features. The system mainly leverages RGB-D cameras to make the pixel to real world measurement units conversion possible (by assessing the camera to target relative distance and pose), and exploits an image processing pipeline consisting of the following steps: (a) an artificial intelligence-based semantic segmentation model for identifying cracks on the target structure; (b) a ridge detection algorithm for identifying the centre line of the cracks; (c) a geometric feature extraction algorithm to extract key metrics of the crack, like its width and length. Variance analysis of the crack mean width measurement shows a repeatability standard deviation of 0.01 mm and a reproducibility standard deviation of 0.02 mm, with a Type A expanded uncertainty of 0.004 mm. The system performance is discussed in the paper referring both to lab testing activities, targeted to the assessment of its metrological characteristics, and to a real field application, i.e. concrete inspection in a tunnel. This latter application is of particular interests as it is targeted to demonstrate the robustness of the approach in harsh environments (i.e., strong presence of dust, total absence of natural light) and on surfaces whose roughness generates important light gradients and shadows (e.g., sprayed concrete)

Repurposing of FDA-Approved Drugs to Disrupt Iron Uptake in Mycobacterium abscessus: Targeting Salicylate Synthase as a Novel Approach

Non-tuberculous mycobacteria (NTM) are opportunistic pathogens that lead to severe, persistent infections, particularly in immunocompromised or vulnerable individuals. Infection rates are rising worldwide, highlighting NTM as an increasing threat to public health. There are currently no specific drugs, and the recommended regimens are usually ineffective. This scenario underlines the urgent need to develop new strategies to effectively combat these infections in a more innovative way. However, the development of new drugs can be a lengthy process, often taking more than a decade to identify even a single active compound. Among the new strategies that can expedite this process is the repurposing of approved drugs. In this work, we applied this approach to identify compounds inhibiting iron uptake in Mycobacterium abscessus (Mab). Specifically, we studied the targeting of salicylate synthase, an enzyme that plays a crucial role in the biosynthesis of mycobacterial siderophores necessary for iron acquisition. Performing an in silico virtual screening of three databases against the crystal structure of salicylate synthase, we identified 11 potential ligands. Then, in vitro assays on the recombinant enzyme highlighted three competitive inhibitors, namely fostamatinib, esomeprazole, and hydroxystilbamidine. These results confirm the potential of the repurposing approach and pave the way for further experimental validation and optimization of these inhibitors as promising compounds against NTM infections
