    What can real-world data teach us about treating patients with unresectable hepatocellular carcinoma?

    Introduction: Hepatocellular carcinoma (HCC) remains a major global health concern, as it is the most common primary liver cancer and the fourth leading cause of cancer-related mortality. Areas covered: Immune checkpoint inhibitors (ICIs) have significantly shifted the treatment paradigm, offering promising survival outcomes. However, the controlled conditions of randomized clinical trials (RCTs) often fail to reflect real-world complexities, emphasizing the necessity for strong real-world evidence (RWE). RWE, in most cases derived from observational studies, provides critical insights into the effectiveness, safety, and tolerability of systemic therapies across diverse populations and settings. The authors searched MEDLINE, Ovid Embase, and Scopus for full-text published articles in any language from the inception to 30 June 2024. This review evaluates RWE on systemic therapies for advanced HCC, including tyrosine kinase inhibitors (TKIs) like sorafenib and lenvatinib, ICIs such as nivolumab and pembrolizumab, and combination therapies like atezolizumab/bevacizumab and durvalumab/tremelimumab. Expert opinion: Studies reveal discrepancies in treatment efficacy and adverse event profiles between RCTs and routine clinical practice, underscoring the need for individualized treatment strategies. RWE highlights the influence of liver disease etiology, liver function, and tumor burden on treatment outcomes, guiding therapy selection

    Escape room in 3D virtual worlds: Reflections on new digital skills for innovative teaching by special educational teachers in training.

    The introduction of virtual worlds and digital escape rooms in education marks a significantmilestone for inclusive education. This study explored the impact of 3D Learning Virtual Worlds and escape roomson the motivation and digital skills of 840 special education teachers in training at the University of Foggia. Usingthe DigiComp 2.2 framework and the FrameVR.io platform, participants created immersive virtual worlds anddesigned educational escape rooms during their internships. Results showed significant improvements in digitalskills, particularly in problem-solving and digital content creation. Moreover, the System Usability Scale (SUS)analysis highlighted a positive perception of the usability of virtual worlds, with room for improvement. The studyconfirms the potential of virtual worlds in promoting active and inclusive learning, offering new opportunities forthe digital transformation of education

    PLACE: Multicenter Study for Right Ventricular Failure on Mechanical Cardiocirculatory Supports

    : Isolated acute right ventricular failure (aRVF) is associated with poor prognosis in different scenarios. In severe conditions, temporary mechanical cardiocirculatory support (tMCS) is required. PLACE is an international, retrospective, multicenter registry including 17 centers that investigated patients affected by isolated aRVF and treated with various types of tMCS from January 2000 to December 2020. The registry included 644 (69.6% males, mean age: 55 years) patients. The most frequent etiologies were post-left ventricular assist device implantation (LVAD) and postcardiotomy shock. These patients received mostly mechanical circulatory support (MCS) and veno-arterial extracorporeal membrane oxygenation. Mean tMCS duration was 9 days, weaning was achieved in 70.5% of the patients, and the major cause of death on support was multiorgan failure (50.5%). The mortality rate was 45 and 48.4% in-hospital and at 3 month follow-up, respectively. Multivariable logistic regression analysis identified age, aRVF due to acute pulmonary hypertension, bilirubin level, and oliguria or anuria at tMCS implantation as risk factors for in-hospital mortality. Conversely, aRVF after LVAD was found to be associated with a lower risk of early mortality. In-hospital and 3 months mortality occurred in less than half of the aRVF-supported subjects. Furthermore, several preimplant aspects such as age, organ function, and type of tMCS are independently associated with in-hospital and 3 month mortality
