Objectives: To compare the predictive performance of the World Health Organization (WHO) 1973, WHO 2004/2022, the three-tier (low grade [LG]/Grade 1 [G1]-G2, high grade [HG]/G2, and HG/G3), and four-tier (LG/G1, LG/G2, HG/G2, and HG/G3) hybrid grading systems in Ta non-muscle-invasive bladder cancer (NMIBC), by evaluating recurrence-free survival (RFS) and progression-free survival (PFS). Patients and methods: This retrospective multicentre study included 1233 patients with pTa NMIBC treated with transurethral resection of bladder tumour, eventually followed by intravesical instillations as determined by their physicians, between 2010 and 2023, across 18 Italian hospitals. Pathologists graded resected tissues using the WHO 1973, WHO 2004/2022 classifications, and hybrid three-tier (LG, HG/G2, HG/G3) and four-tier (LG/G1, LG/G2, HG/G2, HG/G3) systems. Kaplan-Meier curves estimated RFS and PFS. Discriminative performance was assessed using Harrell's concordance index (C-index). Results: Among 1233 patients with pTa NMIBC, 890 were classified as LG and 343 as HG according to the WHO 2004/2022 grading system, while 586, 405, and 242 were categorised as G1, G2, and G3, respectively, under the WHO 1973 system. With a median (interquartile range) follow-up of 26 (14-48) months, 418 patients experienced recurrence, including 184 with HG recurrence and 42 who progressed to MIBC. The C-index values for RFS were 0.60, 0.56, 0.57, and 0.61 for the WHO 1973, WHO 2004/2022, hybrid three-tier, and four-tier grading systems, respectively. For progression, the C-index values were 0.80, 0.74, 0.75, and 0.81 across the same systems, underscoring the superior predictive capacity of the four-tier classification. Nonetheless, the low number of MIBC progression events limits the robustness of these analyses. Conclusion: Our findings highlight the superior prognostic accuracy of the four-tier hybrid classification in predicting recurrence and progression in patients with stage Ta NMIBC. By combining strengths from the WHO 1973 and 2004/2022, this hybrid model shows promise as tool for enhancing NMIBC patient management in clinical practice

Performance Evaluation of an Edge-Blockchain Architecture for Smart City

This paper presents a simulation-based methodology to evaluate the performance of a privacy-compliant edge-blockchain architecture for smart city environments. The proposed model combines edge computing with a private, permissioned blockchain to ensure low-latency processing, secure data management, and verifiable transactions. Using a discrete-event simulation framework, we analyze the behavior of the system under realistic workloads and time-varying traffic conditions. The model captures edge operations, including preprocessing and cryptographic tasks, as well as blockchain validation using Proof of Stake consensus. Several experiments explore saturation thresholds, resource utilization, and latency dynamics, under both synthetic and realistic traffic profiles. Results reveal how architectural bottlenecks shift depending on resource allocation and input rate, and demonstrate the importance of balanced dimensioning between edge and blockchain layers

Fe y humanismo en los Dialogi de Olimpia Morata

    No full text

Background and objective: The prognostic impact of lung metastases (LuMs) in metastatic castration-resistant prostate cancer (mCRPC) remains poorly defined. Our aim was to evaluate the clinical and molecular characteristics of patients with mCRPC with LuMs and their outcomes. Methods: This retrospective multicenter study included 930 patients with mCRPC across 13 centers in Italy. The primary endpoint was the impact of LuMs on overall survival (OS), progression-free survival (PFS), and prostate-specific antigen (PSA) response. As a secondary endpoint, next-generation sequencing for a subgroup with LuMs was used to identify molecular characteristics that might be useful in guiding personalized therapy. Key findings and limitations: Among mCRPC patients treated with an androgen receptor signaling inhibitor, we observed no significant differences in median OS or PFS and PSA response between the LuMs group and the group with bone ± lymph node metastases. Multivariable analyses revealed that only Eastern Cooperative Oncology Group performance status, PSA level, prior docetaxel treatment, and number of metastatic lesions were significant independent factors for both OS and PFS. Comparison of the groups with LuMs only versus liver metastases revealed a significant association between LuMs and a longer OS (15 vs 10 mo; p = 0.002) and PFS (9 vs 5 mo; p = 0.002). The proportion of patients with a ≥50% PSA decline was higher in the LuMs group (odds ratio 3.57, 95% confidence interval 1.37-9.45; p = 0.004). Molecular profile results showed that TP53 mutations accounted for a lower proportion of the pathogenic variants in LuMs than in liver metastases (15% vs 89%). Limitations include the retrospective design and clinical heterogeneity of the population, in addition to unavailability of metastatic biopsies for more in-depth analyses. Conclusions and clinical implications: Our findings suggest that patients with LuMs in mCRPC exhibit clinical and molecular features more similar to those with bone ± lymph nodal metastases than to patients with liver metastases. Further prospective studies are warranted
