    Mapping Review of the Correlations Between Periodontitis, Dental Caries, and Endocarditis

    Background/Objectives: The correlation between cardiovascular diseases, particularly infective endocarditis, and oral disorders such as periodontitis and dental caries has been widely discussed in the scientific literature. In this mapping review, we aim to examine the available evidence on the link between these conditions, focusing on the pathogenetic mechanisms that underlie the development of endocarditis in patients with oral diseases. Methods: A systematic search was conducted across three major databases—PubMed, Scopus, and ScienceDirect—as well as grey literature in Google Scholar. Relevant articles were selected based on inclusion and exclusion criteria, focusing primarily on systematic reviews. The data extracted included study characteristics, main outcomes, and risk-of-bias evaluations. Results: A total of 13 systematic reviews were included in this mapping review. The findings suggest there is a significant connection between periodontal disease, dental caries, and the incidence of infective endocarditis. The evidence highlights that oral bacteria, particularly Streptococcus species, can enter the bloodstream during daily activities and invasive dental procedures, contributing to the development of endocarditis in susceptible individuals. However, the role of antibiotic prophylaxis in preventing endocarditis following dental procedures remains controversial. Conclusions: This review reinforces the importance of oral health in preventing cardiovascular complications, especially infective endocarditis. Although antibiotic prophylaxis may reduce the risk in high-risk individuals, further research is needed to clarify its effectiveness. Enhanced awareness of and education on the shared risks between oral and cardiovascular health could improve prevention strategies and patient outcomes

    The fast pace of globalization alongside digitalization created essential new operational and conceptual methods which affect all people and businesses. This paper examines entrepreneurial thinking as an essential multidimensional competence which enables people to tackle the intricate global economic conditions. This chapter uses modern research together with international policy frameworks along with educational best practices to understand entrepreneurial thinking development while analyzing essential skills and optimal teaching methods for youth development. The chapter examines international virtual exchanges that drive entrepreneurial mindsets specifically in Italy and within the VEHUB4YOU project. The paper presents recommendations alongside policy implications to integrate entrepreneurial thinking into educational systems which will enable youth success in the digital globalized economy

    Factors associated with reaching maintenance therapy in patients with advanced biliary tract cancer treated with durvalumab: Real-world results from a multicenter and multinational study

    Standard of care first-line systemic treatment for advanced biliary tract cancer includes chemo-immunotherapy with gemcitabine, cisplatin, and durvalumab, followed by maintenance durvalumab monotherapy. The present work aims to investigate the differences in baseline clinical and molecular characteristics between patients with early progression during chemo-immunotherapy and those who reach durvalumab maintenance therapy. The study population included patients with unresectable, locally advanced, or metastatic BTC who received treatment at 38 clinical Institutions in 12 countries from July 2021 to December 2023. The primary objective of the study was to investigate whether baseline clinical and molecular characteristics differed between patients with early progression during chemo-immunotherapy versus those reaching durvalumab maintenance therapy. Four hundred forty-eight patients were included in this study. Two hundred twenty-seven patients (50.7%) received maintenance with durvalumab monotherapy, whereas 221 (49.3%) did not receive maintenance therapy due to PD during first-line chemo-immunotherapy before completing 8 cycles. Results show that patients who received maintenance were more likely to be older (>= 70 years), have an ECOG = 0, locally advanced disease, and a neutrophil-to-lymphocyte ratio (NLR) <3. A higher proportion of patients with BAP1 mutations received maintenance, while TP53 mutations were more common in those who progressed early. According to the present analysis, a substantial proportion of patients (50.7%) with advanced BTC who were treated with chemotherapy plus durvalumab proceeded to receive maintenance therapy with durvalumab monotherapy, with a median treatment duration of 4.4 cycles. Patients >= 70 years, with ECOG PS 0, with locally advanced disease, and with NLR <3 had a higher likelihood of receiving maintenance therapy

    Gene expression kinetics in Sepsis After Cardiac Surgery (SACS): a multicentric prospective observational study

    Objectives: Surviving Sepsis Campaign (SSC) defined Sepsis as “life-threatening organ dysfunction caused by a dysregulated host response to infection” (De Backer D et al, Crit Care Med, n.d.). Sepsis remains one of the leading causes of morbidity and mortality (17–65% (De Oliveira DC, Arq Bras Cardiol Sociedade Brasileira de Cardiologia - SBC 94:352–6, 2010)) worldwide and it still remains a challenge to be defined and for which an appropriate treatment is desired (Chiu and Legrand, Curr Opin Anaesthesiol 34:71–6, 2021). Different studies have been conducted on genes coding for inflammatory cytokines whose could predispose to the development of sepsis [e.g., IL-10 PD1 and WT1] (Gupta DL et al, Infectious Process Sepsis, 202). Design: This multicentric observational prospective study aims to evaluate blinding the genetic expression kinetics of different molecules involved in the inflammatory process, IL10, PD1 and WT1, to search for a possible molecular predictive marker of sepsis. Setting: Nine University teaching Hospitals in Italy take part in this study in collaboration with the Department of Applied Science (DISBA) of the University of Basilicata. Participants: One hundred sixty-two patients, under elective cardiac and on pump surgery were enrolled in the study. Interventions: From each patient 4 blood samples were collected during and at the end of the surgery, following the study design. Measurements and main results: We observed, 30 min after the start of the surgery, lower gene expression levels of IL10 and PD1 in septic patients compared to non-septic (p < 0.05), but considering all the timepoint there are differences in gene expression modulation between the groups. Conclusion: These results confirmed the dysregulated immune response in septic patients compared to non-septic, highlight how a measurement of the gene expression could help to optimize procedures and pay attention to more susceptible patients
