    [[abstract]]回顧過去經驗,以往出門乘車時,常會目睹東南亞籍的人在車站大 廳群聚席地而坐的景象。令我所困惑的問題是,為什麼會選擇在人潮密集來往 的地點來當作交流場所,而不是在我認為合適的咖啡廳或是餐廳等地點?從小 我就對不同國情的文化差異有很大的興趣,再加上接觸了近兩年的護理學系課 程,令我對於跨文化照護有很大的好奇心。尤其在人類發展學課程中,我與同 組同學專研的主題是產後婦女的身心發展,至此對產後照護有高度的興趣。因 此,提出一個有關產後照護與跨文化護理的研究計畫,於課外時間累積與訓練 自己的研究能力。本計畫的研究題目將以文化衝擊現象為啟發點,了解來自不 同國家的產後婦女,碰到當地特有的文化時,內心的想法與態度

    [[abstract]]閱讀障礙 (developmental dyslexia) 有聲韻覺識 (phonological awareness) 及聲調覺識 (tone awareness) 的困難,進而影響言語可理解度 (speech intelligibility) 及閱讀能力的發 展,當前研究為了提升閱讀障礙者的言語理解困難,運用了降噪或強化聲學特徵的方式 調整接收到的語音,確實有些成效,但並非提升所有聲學特徵皆達顯著效果,也無研究 同時改變兩變項並探討其對於言語理解的影響。且以往研究多以單字作為了解言語理解 是否提升的語料,並無使用日常溝通是用的句子,無法反映在實際生活中的情形。所以 希望探討透過母音拉長的強化聲學特徵,並加入降噪處理,是否對閱讀障礙者在句子階 層的言語理解有所影

    [[abstract]]命名困難(anomia)是失語症個案中最常見的症狀之一,命名困難言語產出時 無法順利提取目標詞彙,直接影響溝通的流暢度。命名困難治療的核心是重新 建立物品或動作及其名稱之間的配對關係,並經由反覆練習強化語意及語音表 徵的活化程度以提升詞彙提取的成功機率。也就是說,關聯配對學習能力與命 名治療直接相關,而語意及語音觸接能力可能影響關聯配對學習的表現。然而, 目前討論失語症個案關聯配對學習能力的文獻有限。且文獻中對於關聯配對學 習表現與語言及認知功能間的關係的量測方式皆採用標準化評估量表。單純的 行為量測較難反應出執行任務時的認知歷程,對於語言及認知處理歷程變化的 敏感度較低。因此,本研究將探討關聯配對學習能力與命名能力間的關聯。並 進一步探索詞彙處理過程中的語意及語音觸接能力與關聯配對學習的潛在相 關性。[[note]]陳鈺

    [[abstract]]紫外線和 H2O2是導致皮膚老化的兩個主要因素。它們最終都會引發 DNA 損傷,氧化 應激和線粒體功能障礙。線粒體穩態的受損導致 ROS 形成,從而加劇線粒體功能障礙,導致 皮膚老化。另外,線粒體的分裂,融合在維持線粒體體內穩態中起重要作用。在已有報導中, 長壽基因 Sirt1 可以增加 Nrf2 表達,以保護細胞免受脫乙酰基酶 PPARγ 共激活因子 1α (PGC-1α)的氧化,這是線粒體生物發生的主要調節劑,可促進裂變和融合的互補性以避免 衰老。此外,線粒體的生成可以減少線粒體 ROS 產生。高良薑素(3,5,7-三羥基黃酮)是一種 類黃酮,具有抗氧化作用。我們先前的研究表明,高良薑精可以保護細胞免受氧化應激誘導 的衰老。然而,尚不清楚高良薑精是否通過激活 Sirt1 來增強線粒體生物合成以抵抗 ROS 引 起的皮膚衰老,從而發揮其皮膚保護的有利作用。[[note]]洪心

    Early Comprehensive Kidney Care in Dialysis-Requiring Acute Kidney Injury Survivors: A Populational Study

    [[abstract]]Background: For patients with Acute Kidney Injury (AKI), a strong and graded relationship exists between AKI severity and mortality. One of the most severe entities of AKI is Dialysis-Requiring Acute Kidney Injury (D-AKI), which is associated with high rates of mortality and end-stage renal disease (ESRD). For this high-risk population group, there is a lack of evidence regarding optimal post-AKI care. We propose that post-AKI care through the combined efforts of the nephrologist and the multidisciplinary care team may improve outcomes. Our aim here is to study for survivors of dialysis-requiring acute kidney injury, the effects of implementing early comprehensive kidney care. Methods: This is a retrospective longitudinal cohort study of Taiwanese through analyzing the National Health Insurance Research Database (NHIRD). We included patients with acute dialysis during hospitalization from January 1, 2015 to December 31, 2018. Propensity match was done at 1:1, including estimated glomerular filtration rate (eGFR) based on CKD-EPI which was performed due to large initial disparities between these two cohorts. Results: After the propensity match, each cohort had 4,988 patients. The mean eGFR based on CKD-EPI was 27.5 ml/min/1.73 m2, and the mean follow-up period was 1.4 years.The hazard ratio for chronic dialysis or ESRD was 0.55 (95% CI, 0.49-0.62; p < 0.001). The hazard ratio for all-cause mortality was 0.79 (95% CI, 0.57-0.88; p < 0.001). Both outcomes favored early comprehensive kidney care. Conclusions: For survivors of dialysis-requiring acute kidney injury, early comprehensive kidney care significantly lowered risks of chronic dialysis and all-cause mortality

    Exploring Perceived Stress in Mothers with Singleton and Multiple Preterm Infants: A Cross-Sectional Study in Taiwan

    [[abstract]]Objective: The aim of this study was to explore mothers' perceived level of stress one month after hospital discharge following the birth of singleton and multiple preterm infants. Design: A cross-sectional design was used to compare mother's perceived stress in two groups of postpartum mothers and the relationship of the theoretical antecedents and these variables. Setting: A neonatal intensive care unit in a medical center in Taiwan. Participants: Mothers of 52 singletons and 38 multiple premature infants were recruited. One month after the infant was discharged, the participants completed a self-reported questionnaire that included demographic data about the mother and infant, the 21-item Social Support Scale, and the 15-item Perceived Stress Scale. This was returned by email or completed at the outpatient unit. Analysis: Descriptive and inferential analysis. Results: The mean social support scores were 76.6 and 76.5 (out of 105) for mothers with singleton and multiple birth infants, respectively. The most important supporter was the husband. The mean perceived stress scores of 25.8 and 31.0 for mothers with singleton and multiple birth infants, respectively, were significantly different (p = 0.02). Sleep deprivation and social support were predictive indicators of perceived stress in mothers with preterm infants. Conclusions: We suggest that the differences in stress and needs of mothers with singleton and multiple births should be recognized and addressed in clinics. The findings of this study serve as a reference for promoting better preterm infant care

    What motivates tourists to get involved in cyberspace? A comparative look at two countries

    [[abstract]]This paper aimed to test the influences of perceived usefulness on travelers’ willingness to create user-generated content (UGC) through attitudes toward social media, travel planning attitude, and electronic word of mouth, as mediating variables, and compare the proposed model results between two culturally and geographically distinct nations. The research model and the interactions among the constructs were tested and validated through structural equation modeling. Randomly selected qualified responses from Mexican (N=192) and Taiwanese (N=200) participants were analyzed. The comparative analysis of statistical results showed that in Mexico, travel planning attitude does not directly influence travelers’ involvement in UGC. Instead, choosing a travel destination is the reason why Mexicans would generate content. However, in Taiwan the non-significant direct relationship is between electronic word of mouth and user-generated content. Despite the massive social media affect in Taiwan, reading and collecting information online is not the reason for Taiwanese to get involved in providing online content. Research findings assist the destination marketers and tourism stakeholders to identify essential elements that positively motivate and inspire travelers’ intention to share opinions through social media in order to promote or adjust plans and adopt compatible strategies to successfully form and influence travelers’ decision-making behavior

    Effects of Poor Sleep Quality on Attention, Anxiety and Foreign Language Performance in EFL College Students

    [[abstract]]This study proposed and tested a conceptual model of academic expectation stress, sleep quality, and attention in EFL class. These variables did not receive much attention in previous studies but are considered important to medical students’ attention in EFL class. Data were collected from 496 medical students from a medical university in Taiwan. Structural Equation Modeling (SEM) technique was used to examine the path effect in the research model. The results found that (1) higher academic expectation stress leads to higher attention in EFL class; (2) higher academic stress causes poorer sleep quality; (3) poorer sleep quality leads to lower attention in EFL class. A mediator was also identified in this model: sleep quality was found to partially mediate the relationship between academic expectation stress and attention in EFL class. The results may advance the current literature in medical education and applied linguistics by moving a step closer to understand these three variables
