Role of liposomal amphotericin B in intensive care unit: an expert opinion paper
Introduction: Invasive fungal infections (IFI) are frequent in patients admitted to the intensive care unit (ICU). The use of first-line antifungals like triazoles or echinocandins may be limited by the global spread of multi-drug resistance species, drug–drug interactions, low organ penetration, and some safety concerns in case of multi-organ failure. Liposomal amphotericin B (L-AmB) is a polyene drug with a broad activity against mold and yeast and an acceptable safety profile. To outline the role of L-AmB in the treatment of IFI in critically ill patients, a panel of experts was invited to draw up an expert opinion paper on the appropriate place in therapy of L-AmB in different clinical scenarios of patients admitted to ICU. Methods: A multidisciplinary group of 16 specialists in infectious disease, microbiology, pharmacology, and intensive care elaborated an expert opinion document through a multi-step approach: (1) the scientific panel defined the items and wrote the statements on the management of IFI in ICU, (2) a survey was submitted to an external panel to express agreement or disagreement on the statements, and (3) the panel reviewed the survey and implemented the final document. Results: The final document included 35 statements that focused on epidemiology and microbiological rationale of the use of systemic L-AmB in critically ill patients and its potential role in specific clinical scenarios in the ICU. Conclusion: Systemic L-AmB may represent an appropriate therapeutic choice for IFI in ICU patients with different underlying conditions, especially when the use of first-line agents is undermined. This expert opinion paper may provide a useful guide for clinicians

nell’officina poetica cattafiana.This essay proposes an examination of a key theme in Bartolo Cattafi’ poetry. Walls and barriers define, in fact, a recurring literary topos in Cattafi's poetry, variously modulated throughout his entire production. The poet, drawing on the lessons of other 20th-century authors and beyond, elaborates original and highly personal declinations of these theme and exhibits a restless and irrepressible tension towards their overcoming and destruction. Starting from the analysis of specific case studies, the investigation aims to track some of the most significant occurrences of this image through the different poetic seasons. The examination, while privileging published verses, does not refrain from making some incursions into Cattafi's poetic workshop

Upcycling Ante Litteram and Diffuse Design: The Textile Production of the Pezzara in Sicilian Communities
The article explores the practice of upcycling in Sicilian textile traditions, with a focus on the production of pezzara carpets. Pezzare are presented as an early example of circular economy, whereby waste materials are reused to create valuable items. This practice, rooted in rural Sicilian communities, is described as a sustainable design method that predates modern upcycling and resource regeneration concepts. Using ethnographic and visual analysis, the study explores how these crafts are passed down through generations, strengthening local identities and supporting economic stability. The production of pezzare is also linked to the concept of diffuse design, whereby innovation arises from collective, everyday practices that combine local knowledge with sustainable methods. The analysis highlights how pezzara weaving embodies a blend of tradition and innovation, shaping both the cultural heritage and contemporary economic structures of Sicily. Ultimately, the study positions the pezzara as a paradigmatic example of sustainable design, demonstrating how historical craft practices can offer effective solutions to contemporary challenges in sustainability and resource management
On the locative meaning of the verb ‘to be’ in early Greek philosophy. A typological perspective
This paper investigates on the syntactic and semantic continuity between the lexical and the copulative uses of the verb εἰμί in ancient Greek. Kahn (2003[1973]) challenged the traditional view according to which the lexical meaning of the verb is more ancient than its copulative use (Meillet 1908). In particular, he was against the hypothesis of a primitive locative meaning of the original Indo-European root *h1es-, since there would be no diachronic development that explains the diversity of usage from an original word with a spatial meaning. The main function of εἰμί would be primarily syntactic, as a mark of ‘sentencehood’ used to distinguish between a simple NP and a sentence. However, if the problem is tackled from a typological perspective, it turns out that many languages of the world typically exhibit the ‘demonstrative-to-copula’ path (Diessel 1999), a grammaticalization process that originates from the ‘topic mechanism’ (Li & Thompson 1977). After considering a similar diachronic path for *h1es-, I will hypothesize a common deictic predicative source for both its verbal and nominal uses (cf. Stassen 1997; Dixon 2003; Killian 2022). I will then analyse the distribution of εἰμί in a sample of early Greek philosophical texts to determine the role of the locative meaning in the foundation of being
HPCNet: Hybrid Pixel and Contour Network for Audio-Visual Speech Enhancement with Low-Quality Video
To advance audio-visual speech enhancement (AVSE) research in low-quality video settings, we introduce the multimodal information-based speech processing-low quality video (MISP-LQV) benchmark, which includes a 120-hour real-world Mandarin audio-visual dataset, two video degradation simulation methods, and benchmark results from several well-known AVSE models. We also propose a novel hybrid pixel and contour network (HPCNet), incorporating a lip reconstruction and distillation (LRD) module and a contour graph convolution (CGConv) layer. Specifically, the LRD module reconstructs high-quality lip frames from low-quality audio-visual data, utilizing knowledge distillation from a teacher model trained on high-quality data. The CGConv layer employs spatio-temporal and semantic-contextual graphs to capture complex relationships among lip landmark points. Extensive experiments on the MISP-LQV benchmark reveal the performance degradation caused by low-quality video across various AVSE models. Notably, including real/simulated low-quality videos in AVSE training enhances its robustness to low-quality videos but degrades the performance of high-quality videos.The proposed HPCNet demonstrates strong robustness against video quality degradation, which can be attributed to (1) the reconstructed lip frames closely aligning with high-quality frames and (2) the contour features exhibiting consistency across different video quality levels. The generalizability of HPCNet also has been validated through experiments on the 2nd COG-MHEAR AVSE Challenge dataset

The issue of environmental sustainability is at the centre of a multi-level international debate in which dialogue between the scientific community and institutions must provide effective responses to the current climate crisis. In this context, this paper sets out to analyse progress in the achievement of environmental sustainability objectives and duties in the field of business activity - considering that large corporations are responsible for a large part of greenhouse gas emissions - in the light of the new Corporate Sustainability Due Diligence Directive, also addressing, in the second part, the role of new technologies, and in particular of artificial intelligence, in the achievement of objectives and in the fulfilment of sustainability obligations by companies
Winning strategies for value creation and value capture in the almond industry: a case study approach
Purpose: This study explores the strategies of value creation and value capture adopted by two companies producing and processing almonds to measure their effectiveness. Design/methodology/approach: The study takes a quantitative approach based on the analysis of two cases study. The value created and captured is esteemed by using the “transformation value” of agricultural products, developed by the Italian economic-estimative school. Findings: The research shows how vertical integration, product differentiation and direct sales can increase the value created and captured by companies. The results demonstrate that effective supply chain management allows firms to retain a greater share of the value created, thereby improving their competitiveness. Originality/value: This quantitative approach, scarcely present in the literature, makes a significant contribution to the literature on value creation and capture and proposes a replicable methodology for analyzing the impact of business strategies in the agribusiness sector
Golden hour management in the patient with intraparenchymal cerebral hemorrhage: an Italian intersociety document
Background: Spontaneous intracerebral hemorrhage (ICH) accounts for 9–27% of all strokes worldwide and is associated with high mortality and disability. The main causes include vascular malformations, small- and large-vessel angiopathies, and coagulation disorders. Mortality rates reach approximately 40% at 1 month and 54% at 1 year, largely influenced by early management decisions. Rapid intervention, particularly within the first hour, is crucial, especially for patients initially treated in peripheral hospitals. This consensus document, developed by SIAARTI with the endorsement of multiple medical societies, aims to standardize ICH management based on hospital capabilities, aligning with the “time is brain” principle and the 2022 AHA guidelines. Methods: A multidisciplinary panel of experts—including neurointensivists, neuroanesthesiologists, neurologists, neuroradiologists, emergency physicians, and neuroscience nurses—developed this consensus document. The process combined a systematic literature review with a modified Delphi method, prioritizing clinical questions using the UCLA-RAND appropriateness methodology. Literature searches were conducted on PubMed following PRISMA 2020 guidelines. Statements were formulated based on both evidence and expert consensus, and the final document underwent external peer review. Results: Computer tomography (CT) angiography, with over 90% sensitivity and specificity, is a key tool for identifying macrovascular abnormalities and detecting active bleeding, a critical factor in poor outcomes. Prognostic models, such as the ICH score, assist in clinical decision-making. Strict blood pressure control (target 130–140 mmHg) and early intubation in appropriate cases help mitigate hematoma expansion. Anticonvulsants are recommended only for patients with documented seizures. In cases of anticoagulant-related hemorrhage, prothrombin complex concentrates are effective for rapid reversal, though their long-term impact remains uncertain. Intensive care unit (ICU) admission is determined by ICH severity, with severe cases benefiting from specialized neurocritical care. Conclusion: A multidisciplinary and inter-societal discussion provided key recommendations for the immediate management of ICH, based on the available literature. While only a few topics are supported by robust evidence, experts strongly recommend early brain angio CT, risk stratification using scoring systems, clear communication of patient data, and intubation for impaired consciousness. Blood pressure should be controlled with alpha- and beta-blockers, avoiding hypotension. Anticoagulant reversal should be appropriately managed, and eligible patients should be centralized in ICU and neurosurgical centers using dedicated scoring systems
