    Impact on global health status, quality-sexual life and chronic fatigue state of risk-reducing salpingo-oophorectomy in women who are BRCA1/2 mutation carriers: experience from a third level italian center

    Objective: BRCA1 and2mutation carriers are invitedto follow intensive clinical and instrumental surveillance programs or are offered prophylactic ovarian surgery. These recommendations impact many aspects of their lives. The primary objective of this study was to assess the overall quality of life (HRQoL) before and after prophylactic bilateral salpingo-oophorectomy (BSO). Secondary objectiveswere investigating sexual health (SH) and fatigue severity state. Design: This was a single-center retrospective observational study. Setting: Women who underwent surgical treatment of bilateral salpingo-oophorectomy between 2018 and 2024 at "DAI Materno Infantile"of Azienda Ospedaliera Universitaria "Federico II"of Naples were included. Methods: These patients were tracked down to undergo specific questionnaires, such as "Global Health Status (GHS) and Quality of Life Scale (QOL) (EORTC QLQ-C30),""EORTC Sexual Health Questionnaire (QLQ-SH22),"and "Fatigue Severity Scale (FSS),"to evaluate their psychological, sexual, and general physical condition impact before the surgery, 3 and 6months later. Results: The overall mean QoL score was 88.3 ± 29.8 (mean ± standard deviation), and this score worsened when the surgery was performed at 3months (p < 0.0001) with a score of 51.7 ± 30.7 and a mean difference (MD) of 36.6 points; instead, at 6 months, the overall mean QoL score was 73.1 ± 24.3 with an MD of 21.4 points. FSS reported a score of 2.7 ± 1.15 vs 4.2 ± 1.59 (p < 0.0001) vs 3.5 ± 1.43 (p < 0.0001), respectively, before and 3-6months after surgery. EORTC QLQSH22 before and after treatment showed statistically significant changes in sexual satisfaction (p < 0.0001). Conclusion: BSO may impact the quality of life regardless of the hormonal status of patients related to age or menopause, about both the functional evaluation and the psychological and emotional assessment report. The physical change related to the surgical procedure is associatedwith a mental shift that affects both the physical and sexual energy of our patients in the first 3 months postoperatively, with a slight improvement of these data at 6 months

    Eosinophilic Annular Erythema: Clinicopathologic Analysis and Therapeutic Outcomes from a Multicenter Cohort

    Introduction: Eosinophilic annular erythema (EAE) is a rare dermatosis characterized by persistent pruritic erythematous annular plaques with dermal eosinophilic infiltrates. It typically has a chronic, relapsing course with variable treatment responses and frequent refractory cases. Objectives: This retrospective multicenter study reports the clinical and histopathological features of EAE and the treatment outcomes in a case series of 10 patients. Methods: Ten patients with a confirmed clinical and histopathological diagnosis of EAE were referred to the Dermatology Departments of the University of Brescia and the University San Raffaele of Milan for evaluation and treatment. Results: The cohort included six females and four males, all Caucasian, with a median age of 56 years. Time from lesion onset to diagnosis ranged from two days to seven years. Patients exhibited annular, figurate, or polycyclic plaques with erythematous borders—dashed in 80% and centrally pigmented in 70% of cases. Intense itching was reported by 90%. Histopathology displayed dermal infiltrate primarily composed of lymphocytes with various numbers of eosinophils, ranging from scattered (10%) to abundant (40%) and numerous (50%). Treatment responses were variable, oral corticosteroids, either alone or in combination with hydroxychloroquine, being the most used therapies. However, flares frequently occurred following discontinuation of treatment. Dupilumab has shown promise in achieving long-term remission. Conclusion: Most patients exhibited pruritic lesions with dashed borders and central pigmentation, strongly suggesting a diagnosis of EAE. The positive response to dupilumab in refractory cases, along with long-term follow-up, reinforces the growing body of scientific evidence from case reports documented in the literature

    On Using Lazy Greedy Best-First Search with Subgoaling Relaxation in Numeric Planning Problems

    This paper studies the use of lazy greedy best-first search for numeric planning problems in combination with relaxation-based heuristics, helpful actions, and up-to-jumping actions. In particular, the new search schema that we study, whilst postponing evaluation of the heuristic at expansion time, focuses the search over those states that are reached by helpful and up-to-jumping actions. In addition, we revisit linear abstractions by improving the balance between computation time and information, providing guidance in non-simple numeric planning problems, too. The new search schema compares favorably over the IPC-23 benchmarks with alternative complete heuristic search planners from the literature

    Developing environmentally relevant test materials for microplastic research through UV-induced photoaging

    Microplastics (MPs), defined as plastic fragments smaller than 1 mm, are pervasive pollutants posing considerable ecological and health hazards owing to their durability and potential to cause adverse environmental effects. These particles originate mainly from the breakdown of bigger plastic debris by mechanisms such as UV-induced photodegradation, resulting in fragmentation into micro- and nanoplastics. Appropriate laboratory test materials that simulate naturally degraded plastics are essential for evaluating the environmental impact of MPs, enhancing analytical methods, and assessing remediation pathways. In this study we generated "true-to-life" MPs from commonly utilized plastic products through controlled photodegradation processes designed to accelerate polymer aging. Two aging protocols were developed: (i) UV irradiation of macroplastic fragments for up to eight weeks followed by mechanical milling, and (ii) UV exposure of pre-fragmented MPs over the same period. Five polymers, namely polystyrene (PS), polypropylene (PP), high-density polyethylene (HDPE), polyvinyl chloride (PVC), and polyethylene terephthalate (PET), were chosen for analysis, with PET investigated separately due to the presence of the carbonyl group, which complicates carbonyl index (CI) calculations used as a quantitative index to monitor the photo-oxidation. The surface morphology of aged MPs was examined using Scanning Electron Microscopy (SEM), their chemical composition was investigated by Near-Infrared (NIR) and Fourier-transform Infrared (FTIR) spectroscopy, thermal properties were also evaluated by Thermogravimetric Analysis (TGA). PET degradation was further analyzed using supplementary techniques such as X-Ray Diffraction (XRD) and Differential Scanning Calorimetry (DSC) to assess structural and thermal alterations. These findings demonstrate that the proposed protocols generate MPs with consistent physicochemical properties, providing a model system suitable for studying MP degradation and behavior in laboratory studies, ultimately supporting environmental risk assessment and mitigation strategies

    Variable-size Symmetry-based Graph Fourier Transforms for Image Compression

    Modern compression systems use linear transformations in their encoding and decoding processes, with transforms providing compact signal representations. While multiple data-dependent transforms for image/video coding can adapt to diverse statistical characteristics, assembling large datasets to learn each transform is challenging. Also, the resulting transforms typically lack fast implementation, leading to significant computational costs. Thus, despite many papers proposing new transform families, the most recent compression standards predominantly use traditional separable sinusoidal transforms. This paper proposes integrating a new family of Symmetry-based Graph Fourier Transforms (SBGFTs) of variable sizes into a coding framework, focusing on the extension from our previously introduced 8x8 SBGFTs to the general case of NxN grids. SBGFTs are non-separable transforms that achieve sparse signal representation while maintaining low computational complexity thanks to their symmetry properties. Their design is based on our proposed algorithm, which generates symmetric graphs on the grid by adding specific symmetrical connections between nodes and does not require any data-dependent adaptation. Furthermore, for video intra-frame coding, we exploit the correlations between optimal graphs and prediction modes to reduce the cardinality of the transform sets, thus proposing a low-complexity framework. Experiments show that SBGFTs outperform the primary transforms integrated in the explicit Multiple Transform Selection (MTS) used in the latest VVC intra-coding, providing a bit rate saving percentage of 6.23%, with only a marginal increase in average complexity.
