Préservation utérine pour le placenta accreta : nouveau standard ?
Placenta accreta spectrum (PAS) is among the most dangerous obstetric conditions due to elevated risk of massive hemorrhage. Ultrasound is PAS's preferred screening method. This article -introduces the new topographic classification of PAS, allowing for treatment selection, whether radical or conservative. This classification enables considering the One-Step Conservative Surgery (OSCS), a technique that could potentially avoid up to 80% of cesarean -hysterectomies in cases of PAS. This article details the procedure and -eligibility criteria for the OSCS. This promising technique could significantly reduce maternal morbidity and mortality. Finally, the possibility of training in these techniques through telemedicine opens new avenues for conservative uterine treatment in the -context of PAS.Le trouble du spectre du placenta accreta (PAS, placenta accreta spectrum) est l’une des pathologies obstétricales les plus dangereuses en raison du risque d’hémorragie massive. L’échographie est l’examen de dépistage privilégié. Cet article présente la nouvelle classification topographique du PAS, qui permet de planifier la prise en charge, radicale ou conservatrice. Cette classification permet d’envisager la chirurgie conservatrice en une étape (OSCS, One-Step Conservative Surgery), une technique qui pourrait éviter jusqu’à 80 % des césariennes-hystérectomies en cas de PAS. L’article détaille la procédure et les critères d’éligibilité de la OSCS qui pourrait significativement réduire la morbimortalité maternelle. Enfin, la possibilité de se former à ces techniques par télémédecine ouvre de nouvelles perspectives pour un traitement conservateur de l’utérus dans le cadre du PAS
VIH : ce qui a changé en 2024
The year 2024 saw many advances in HIV care, both in terms of treatment and prevention. The cure of a patient from Geneva made headlines and helped in understanding the complex immunology of the HIV virus. Long-acting injectable treatments improve the quality of life of peoplewith HIV (PWHIV) and area promising option for pre-exposure prophylaxis (PreP). Primary cardiovascular prevention in PLWHIV is improving, and therapeutic options have increased for pregnant and breastfeeding women living with HIV. Those advances must encourage us to insist on open discussions with our patients regarding their risk of acquiring HIV and their eligibility for PreP, and on the importance of early testing and treatment in case of infection.L’année 2024 a vu plusieurs avancées dans le domaine du VIH, sur le plan des traitements et de la prévention. La guérison d’un patient genevois a fait les gros titres et permis d’avancer dans la compréhension de l’immunologie du virus VIH. Les traitements injectables à longue durée d’action permettent d’alléger le suivi des personnes vivant avec le VIH (PVVIH), et sont une option prometteuse en prophylaxie préexpositionnelle (PreP). La prévention des événements cardiovasculaires des PVVIH est améliorée, et de plus vastes options thérapeutiques pour les femmes enceintes et allaitantes vivant avec le VIH sont ouvertes. Ces avancées doivent nous encourager à insister sur une discussion ouverte concernant le risque d’acquérir le VIH, la possibilité d’une PreP, et l’importance d’un dépistage et d’un traitement précoce en cas d’infection
Combining advanced formulation strategies and fractional laser ablation to improve ungual drug delivery
The nail plate constitutes a formidable barrier to topical therapy, often leading to suboptimal treatment outcomes in onychomycosis. This thesis explores the combined use of fractional CO₂ laser ablation and extended-release formulations to enhance ungual drug delivery. Terbinafine and amphotericin B were selected as model antifungals to address dermatophyte and non-dermatophyte infections, respectively. Nanocrystal-based and liposomal formulations embedded in thermosensitive gels were developed, characterised, and evaluated ex vivo using porcine hooves as nail models. Fractional laser ablation generated reproducible micropores that improved drug deposition and enabled sustained release from the formulations. Terbinafine nanocrystals achieved a 22-fold increase in deposition compared to commercial cream, while amphotericin B liposomes demonstrated enhanced nail penetration despite limited antifungal efficacy. The findings support the potential of integrating controlled-release strategies with laser-assisted drug delivery to overcome nail permeability barriers, improve patient compliance, and provide more effective topical treatments for onychomycosis
Claiming urban citizenship through insurgent choreographies: the queering of body protest and politics in a Slut Walk
Contemporary street protests reveal a transformation in the repertoires of activist action and the role of body politics in challenging gender and sexual inequalities and exclusions. Drawing on a case study of a recent edition of a Slut Walk, a transnational movement calling for an end to rape culture, I use the concept of insurgent choreography to explore the processes of place-making offered by the gathering of gender-nonconforming bodies in public space. This leads me to argue for greater consideration of the bodily dimension of gender in political participation and the role of the body in the creation of interstitial queer times and spaces in the city. This argument contributes to discussions that connect feminist and queer struggles and expand reflections on coalitional politics. It offers new perspectives on gender and political mobilisation, calling to explore further the role of the body in building a more inclusive city.</p
Development of an Integrative Strategy to Leverage the Results of the Metabolomics Profiling and Anti-Trypanosomatid Screening of a Large Plant Extracts Collection
Nature is an inexhaustible source of new therapeutics, and humans have been using it to heal themselves for ages. Shaped by evolution, plants, bacteria, or fungi, among other living organisms, contain hundreds of potential bioactive metabolites, among which many are still to be discovered. Today, Ultra-High Performance Liquid Chromatography (UHPLC) coupled with High-Resolution tandem Mass Spectrometry (HRMS/MS) is the standard method to chemically profile these organisms and get an insight into their composition. Thanks to modern data processing workflows and annotation strategies, large amounts of spectral and structural data can be obtained and linked to biological screening results for each natural extract. In this thesis project, we aimed to develop innovative metabolomics workflows using UHPLC-HRMS/MS and biological screening results of large natural extracts collections to identify bioactive compounds in mixtures. The developed approach should help rationalize efforts to isolate valuable natural products (NP) only. As an application case, we used metabolomics and anti-trypanosomatid activity data - against Trypanosoma cruzi, T. brucei, and Leishmania donovani - obtained on a dataset of 1,600 plant extracts.
We developed a novel MS/MS-based sample vectorization method, called MEMO, to highlight chemical similarities among extracts in large chemodiverse libraries without needing any retention time (RT) based feature-alignment step. Because such an alignment makes the iterative addition of new samples difficult, we developed a pythonbased framework to organize automatically, through molecular networking, and annotate, through state-of-the-art annotation tools, the LC-MS features’ fragmentation spectra of each sample independently. While such a sample-centric workflow allows for better iterative addition of samples by avoiding the necessity to recompute post-featurealignment annotations steps, it hinders the direct comparison of the considered samples using LC-MS features’ relative intensity among samples. To maintain a way to compare unaligned samples, the generated and standardized data produced at the sample scale were integrated into a single Experimental Natural Product Knowledge Graph (ENPKG). Such a format allows the integration of both chemical and bioactivity data and the connection of experimental data to other graph databases such as Wikidata and ChEMBL. Using the SPARQL language to query the data, we could demonstrate how such an organization represents an efficient strategy to link and interpret heterogeneous chemical and biological data to annotate bioactive compounds in extracts before their isolation.To confirm the generated bioactivity annotations, we isolated and characterized some metabolites of interest from different extracts using semi-preparative HPLC, NMR, and chiroptical methods.
Because they do not require any RT-based feature alignment step and allow the easy addition of new samples over time, the MEMO method and the ENPKG workflow represent sustainable data exploitation and data management strategies in NP research. Integrating crosslinks to other DBs helps the ENPKG workflow avoid data loss into hermetic silos of information. Because of their versatility and integrative character, we believe that the developed tools should contribute to advancing the knowledge of Nature's fascinating chemistry.</p
Sortir de l’incertitude de la vie sans papiers et construire son avenir : entre aspirations et contraintes
Cette thèse vise à mieux comprendre les changements survenant après la régularisation du permis de séjour dans la vie des personnes ayant auparavant vécu et travaillé longtemps en situation de séjour irrégulier. Elle se concentre sur le cas de l’Opération Papyrus, un programme de régularisation mis en place à Genève, en Suisse, entre 2017 et 2018. Dans ce contexte, les personnes concernées étaient principalement des femmes originaires de pays non-membres de l’UE-AELE, vivant à Genève en situation de séjour irrégulier depuis plus de 10 ans en moyenne, travaillant comme femmes de ménage, aides aux personnes âgées ou baby-sitters ; mais aussi des hommes employés dans la restauration, le nettoyage ou la construction. Située à l’intersection de trois domaines — la sociologie des migrations (particulièrement celle axée sur les trajectoires d’intégration post-migration), la recherche sur les parcours de vie, et la recherche sur la stratification et la mobilité sociales — cette thèse vise à documenter et analyser l’expérience subjective de cette transition et l’évolution des situations des personnes régularisées, en adoptant une perspective processuelle.
Fondée sur une démarche empirique qualitative longitudinale, elle est basée sur 88 entretiens qualitatifs menés sur quatre ans avec 39 personnes faisant l’expérience nouvelle de la vie en situation de séjour régulier. L'analyse approfondit, d’une part, si et comment, après des années de contraintes et d’incertitudes liées à la vie sans papiers, la régularisation permet aux individus d’aspirer à un avenir plus favorable et leur donne les moyens de le concrétiser. D’autre part, elle s’intéresse à la dynamique de la vulnérabilité dans le temps, en examinant comment certains mécanismes de vulnérabilisation spécifiques à la vie sans papiers disparaissent, se transforment, se combinent avec d’autres ou persistent même après la régularisation, ainsi que l’émergence de nouveaux mécanismes de vulnérabilisation. This thesis aims to better understand the changes occurring after the regularization of the residence permit in the lives of people who have lived and worked undocumented for years. It focuses on the case of Operation Papyrus, a regularization program implemented in Geneva, Switzerland, between 2017 and 2018. In this context, the people concerned were mainly women from non EU/EFTA countries in Geneva for an average of more than 10 years working as housekeepers, caregivers for older people, or babysitters and men employed in the restaurant, cleaning, or construction sectors. Situated at the intersection of three fields — the sociology of migration (particularly the one focusing on postmigration integration trajectories), life course research, and research on social stratification and mobility — this thesis aims to document and analyze the subjective experience of this transition and the evolution of the situations of regularized individuals by adopting a processual perspective.
Based on a longitudinal qualitative empirical approach, this thesis draws on 88 qualitative interviews conducted over four years with 39 individuals experiencing the new reality of living in a regular situation. The analysis, on the one hand, delves into whether and how, after years of constraints and uncertainties associated with undocumented life, regularization enables individuals to aspire to a more favorable future and provides them with the means to achieve it. On the other hand, it investigates the dynamics of vulnerability over time, examining how certain mechanisms of vulnerabilization specific to undocumented life disappear, transform, combine with others, or persist even after regularization, as well as the emergence of new mechanisms of vulnerabilization.</p

Il nous a semblé utile, dans cet ouvrage en hommage à l’œuvre de Nicolas Jeandin, de revenir sur la position procédurale de ces créanciers cessionnaires d’une faillite, qui suscite beaucoup de commentaires et une abondante jurisprudence, mais qui garde sa part d’ambiguïté. C’est une institution qui amène autant de questions de procédure civile que de questions de droit des poursuites, et qui fait donc écho au vaste champ des intérêts et des sujets de recherches du prof. La présente contribution se livre dans un premier temps à un rappel de la jurisprudence relative à la maxime inquisitoire asymétrique, avant d’exposer les petites variations apportées par le Tribunal fédéral dans les conditions initiales de celle-ci (infra I). Elle en examine ensuite les conséquences sur la sécurité du droit, plus exactement s’agissant du régime relatif à l’allégation des faits générateurs de la recevabilité et sur les différentes catégories de conditions de recevabilité que notre Haute Cour semble avoir créées (infra II)
General Revision of Tort Law in Switzerland. Reasons for Its Failure
What can we learn from the Swiss government’s rejection in 2009 of the general revision of civil liability? The major lessons are (1) that tort law as a relatively unattractive topic can be revised only with strong political support. (2) According to the ‘laziness-principle’ dominant in the history of law, a revision is unlikely if the existing norms do not create fundamental obstacles, if the difficulties of everyday practice are not dissuasive; doctrinal unsoundness is generally considered as irrelevant. In other words, no revision if a certain ‘pain threshold’ in legal practice is not reached. (3) Legal aesthetics and old habits can play an important role. The more the form of the new law changes and the more it overturns existing practice with its whole bunch of professional reflexes, the less successful a revision of tort law will be.Que pouvons-nous apprendre du rejet par le gouvernement suisse en 2009, de la révision générale de la responsabilité civile ? Les principales leçons sont les suivantes: (1) le droit de la responsabilité civile en tant que sujet relativement peu attractif ne peut être révisé qu’avec une forte volonté politique. (2) Selon le ‘principe de paresse’ dominant dans l’histoire du droit, il est peu probable qu’une révision se matérialise si les normes existantes ne créent pas d’obstacles insurmontables, si les difficultés de la pratique quotidienne ne sont pas dissuasives. Des divergences dogmatiques sont généralement considérées comme des éléments négligeables. En d’autres termes, pas de révision si un certain seuil de douleur dans l’application pratique du droit n’est atteint.
