Nouveaux traitements médicamenteux pour le surpoids et l’obésité. Prescription et suivi
La prise en charge de l’obésité est complexe et repose sur un suivi interdisciplinaire. Les nouveaux traitements médicamenteux favorisent la perte de poids sans traiter les causes du surpoids ou de l’obésité. Le traitement médicamenteux seul, sans approche interdisciplinaire, peut desservir le patient en raison d’un risque important d’échec thérapeutique. L’indication de ces médicaments ne correspond pas aux critères de remboursement et le médecin prescripteur doit faire une demande d’autorisation de remboursement puis faire régulièrement des rapports à l’assurance maladie, selon l’évolution du poids. L’initiation, le suivi et l’arrêt des traitements médicamenteux de l’obésité doivent se faire en partenariat entre la personne en situation d’obésité et le spécialiste de l’obésité.The treatment of obesity is complex and relies on interdisciplinary follow-up. The new medications promote weight loss without addressing the underlying causes of overweight or obesity. Medication alone, without an interdisciplinary approach, can be detrimental to the patient due to the significant risk of therapeutic failure. These medications do not meet the reimbursement criteria, and the prescribing physician must submit a request for reimbursement authorization and periodically report to the health insurance. The initiation, follow-up, and discontinuation of pharmacological treatment of obesity must be undertaken in partnership between the person living with obesity and the obesity specialist
STM-GNN : Space-Time-and-Memory Graph Neural Networks for Predicting Multi-Drug Resistance Risks in Dynamic Patient Networks
Hospital-acquired infections (HAIs), particularly those caused by multidrug-resistant (MDR) bacteria, pose significant risks to vulnerable patients. Accurate predictive models are important for assessing infection dynamics and informing infection prediction and control (IPC) programmes. Graph-based methods, including graph neural networks (GNNs), offer a powerful approach to model complex relationships between patients and environments but often struggle with data sparsity, irregularity, and heterogeneity. We propose the space-time-and-memory (STM)-GNN, a temporal GNN enhanced with recurrent connectivity designed to capture spatiotemporal infection dynamics. STM-GNN effectively integrates sparse, heterogeneous data combining network information from patient-environment interactions and internal memory from historical colonization and contact patterns. Using a unique IPC dataset containing clinical and environmental colonization information collected from a long-term healthcare unit, we show that STM-GNN effectively addresses the challenges of limited and irregular data in an MDR prediction task. Our model reaches 0.84 AUROC, and achieves the most balanced performance overall compared to classic machine learning algorithms, as well as temporal GNN approaches.</p

General practitioners play a central role in reviewing vaccination status, informing patients, and addressing vaccine hesitancy through empathetic, motivational communication. Improving digital tools and ensuring data privacy are strategic priorities to improve vaccine uptake and meet today's public health challenges.Les recommandations vaccinales suisses sont régulièrement actualisées, les principaux changements récents concernent les vaccins contre le Covid-19, les pneumocoques, le virus respiratoire syncytial, la grippe, le zona, la mpox et la méningo- encéphalite à tiques. Dans un contexte de fragilisation des politiques de santé publique, ces mises à jour soulignent l’importance de maintenir une couverture vaccinale élevée, notamment dans les populations à risque. Les médecins généralistes ont un rôle clé dans la vérification du statut vaccinal, l’information et la réponse à l’hésitation vaccinale via une communication adaptée. Enfin, l’amélioration des outils numériques et la gestion sécurisée des données personnelles constituent des stratégies majeures pour renforcer l’adhésion vaccinale et faire face aux défis actuels de santé publique
Prise en charge de la dyslipidémie diabétique
Diabetic dyslipidemia is a very common metabolic disorder, particularly in patients with type 2 diabetes. Characterized by hypertriglyceridemia and a decrease in HDL cholesterol, it is the cause of many cardiovascular comorbidities. It is therefore essential to detect this pathology as early as possible to provide rapid and appropriate management. However, given the increasing number of therapeutic possibilities for treating this disorder, it can be difficult to clinically guide one's choice. In this article, we will discuss the pathophysiology of diabetic dyslipidemia and the resulting management are presented.La dyslipidémie diabétique est une atteinte métabolique très fréquente, notamment chez les patients diabétiques de type 2. Caractérisée par une hypertriglycéridémie et une baisse du cholestérol HDL, elle est notamment à l’origine de nombreuses comorbidités cardiovasculaires. Il est donc essentiel de déceler le plus précocement possible cette pathologie afin d’offrir une prise en charge rapide et adaptée. Cependant, au vu des possibilités thérapeutiques de plus en plus nombreuses pour traiter ce trouble, il peut être difficile d’orienter cliniquement son choix. Dans cet article, la physiopathologie de la dyslipidémie diabétique et la prise en charge qui en découle sont décrites.</p

Plus particulièrement, les relations entre l'UE et l'Union économique et monétaire sont analysées par ce prisme afin d'en dégager les convergences et les conflits dans des situations d'urgence prolongée.</p
The REGROLF project : regeneration of olfactory neurons in post-traumatic anosmia
Post-traumatic anosmia is a common clinical condition referring to the complete loss of the sense of smell secondary to traumatic brain injury. During brain trauma, the relative movement of the brain to the skull shears the axons of the olfactory sensory neurons at the level of the cribriform plate in the anterior skull base. Post-traumatic inflammation at this level may induce scar tissue formation creating a barrier for new olfactory axons to grow from the olfactory mucosa in the nose towards the centrally located olfactory bulb. Only about 15% of affected patients experience some degree of recovery, whereas the majority suffer from several negative and permanent consequences ranging from diminished food enjoyment to the inability to detect potential hazards such as gas leaks or spoiled food. For some professionals, such as cooks or firefighters, anosmia may lead to an inability to work. Today, there is no cure, making post-traumatic anosmia a true unmet clinical need. In the normally functioning olfactory system, airborne odour molecules are carried into the nasal cavity where they activate specific neuronal olfactory receptors located in the olfactory epithelium. These olfactory neurons undergo continuous and life-long renewal from a pool of tissue-resident neuroprogenitors. During development, they project axons through the cribriform plate bone towards the olfactory bulb. The hereby presented project aims at recapitulating this concept in post-traumatic anosmia and to re-establish axonal connection between progenitor-derived olfactory neurons in the olfactory mucosa and the centrally located bulb. To overcome the scar tissue, a functionalized biomaterial is implanted at the level of the cribriform plate to open a new passage and to induce guided growth along a scaffold towards the olfactory bulb. On the way to achieve this ambitious goal, intermediate goals had to be met first within this project. First, in vitro culture conditions for olfactory neurons were successfully optimized and the results published. Second, guided growth of murine and human olfactory neurons was induced on custom-made biomaterials in vitro in 3D. Third, a mouse model for post-traumatic anosmia was created in the laboratory including a surgical approach to safely implant the biomaterial and a new behavioral test for the murine olfactory function. Finally, the overall concept of olfactory regeneration in mice with post-traumatic anosmia was assessed in a sham-controlled trial in vivo. Following post-traumatic anosmia, animals implanted with the biomaterial to induce regrowth of olfactory neurons recovered the smell function, whereas non-implanted animals did not. These encouraging functional results still lack the histological proof of axonal regeneration as the cause for recovery and must be considered still preliminary. In conclusion the concept of guided regrowth of olfactory sensory neurons through implanted biomaterials is worth to be pursued beyond the scope of the current thesis with the goal to develop a cure for patients suffering from post-traumatic smell loss in the future.L'anosmie post-traumatique désigne la perte totale de l'odorat à la suite d’un trauma crânien. Lors de l’impact, le mouvement relatif du cerveau par rapport à la boîte crânienne cisaille les axones des neurones sensoriels olfactifs qui traversent la lame criblée se trouvant dans la partie antérieure de la base du crâne. L'inflammation post-traumatique à ce niveau peut induire la formation d'un tissu cicatriciel créant une barrière pour la croissance de nouveaux axones olfactifs. Seuls 15 % des patients atteints connaissent un certain degré de récupération, tandis que la majorité d'entre eux ne récupère pas et souffrent de conséquences négatives allant de la diminution du plaisir de manger à l'incapacité de détecter des dangers tels que des fuites de gaz ou des aliments avariés. Pour certains professionnels, comme les cuisiniers ou les pompiers, l'anosmie peut conduire à l'incapacité de travailler. Aujourd'hui, il n'existe pas de traitement et ceci représente un véritable besoin clinique non satisfait. Le but de ce projet est de rétablir les connexions axonales perdues lors du trauma pour redonner le sens de l’odorat au patient. Heureusement, les neurones olfactifs se renouvellent continuellement à partir d'un pool de neuroprogeniteurs résidant dans l’épithélium olfactif situé au toit de la cavité nasale. De plus, au cours du développement, ces neuroprogéniteurs ont la capacité de projeter des axones à travers la lame criblée vers le système nerveux central. Le projet présenté ici vise à tirer profit de ces caractéristiques physiologiques, en contournant le tissu cicatriciel post-traumatique par l’implantation d’un biomatériel fonctionnalisé au niveau de la lame criblée afin d'ouvrir un nouveau passage et d'induire une neurorégénération guidée. Pour atteindre ce projet ambitieux, des objectifs intermédiaires ont dû être atteints. Tout d'abord, les conditions de culture in vitro des neurones olfactifs ont été optimisées avec succès et les résultats ont été publiés. Deuxièmement, la croissance guidée des neurones olfactifs murins et humains a été induite sur des biomatériaux sur mesure in vitro en 3D. Troisièmement, nous avons créé un modèle de souris pour l'anosmie post-traumatique tout en développant une approche chirurgicale pour implanter le biomatériel. Afin de mesurer l’effet de cette intervention, nous avons développé un nouveau test comportemental pour mesurer la fonction olfactive chez la souris. Enfin, le concept global d’implant olfactif pour induire la neurorégénération olfactive chez les souris souffrant d'anosmie post-traumatique a été testé in vivo dans le cadre d'un essai contrôlé par sham. Les résultats montrent que les animaux implantés récupèrent la fonction olfactive, alors que les animaux non implantés ne l'ont pas récupérée. Ces résultats fonctionnels encourageants sont toutefois limités par l’absence de preuve histologique que la régénération axonale est à l'origine de la récupération et doivent être considérés comme préliminaires. En conclusion, le concept de régénération guidée des neurones sensoriels olfactifs par le biais de biomatériaux implantés mérite d'être poursuivi au-delà de la portée de la thèse actuelle dans le but de mettre au point un traitement pour les patients souffrant de perte d'odorat post-traumatique
Nanofitin as a novel shuttle for oral delivery of biologics
While it is the most convenient and less invasive route of delivery, oral administration is difficult and rarely used for biologics. Indeed, these macromolecules are sensitive to the harsh environment of the gastrointestinal tract, and their large size considerably limits their passive permeability across the intestinal epithelium (>500 Da). To address these challenges, the research conducted within my thesis aimed at using the advantageous features of 7 kDa affinity proteins, the Nanofitins, to cross actively the intestinal mucosa by targeting the leptin receptor. During this PhD project, Nanofitins with a specific affinity to the target were thus generated and their ability to cross the intestinal barrier has been investigated using an ex vivo porcine intestinal model. We obtained promising results demonstrating a specific receptor-mediated transport of Nanofitins. These data support their use for the oral delivery of targeted therapies and their further evaluation in an in vivo model. </p
