    Síndrome metabólica e disfunção eréctil - avaliação de parâmetros clínicos e hemodinâmicos

    Objetivos Calcular a prevalência de fatores de risco cardiovascular, incluindo a síndrome metabólica (SM), numa série de doentes portugueses com disfunção erétil (DE) e quantificar o impacto individual e agregado dos mesmos, nos parâmetros hemodinâmicos e no grau de severidade reportada. Material e métodos Estudo de uma série de 408 doentes com DE seguidos em consulta de Urologia, no período 2008-2010. A SM foi definida pelos critérios propostos pela National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATP III), tendo sido quantificadas as variáveis hipertensão arterial (HTA), intolerância a glicose (IG), hipertrigliceridemia (HTG), diminuição de HDL e obesidade central, sob a forma dicotómica. O estudo hemodinâmico foi efetuado por eco-doppler peniano dinâmico (D-PCDU) e a avaliação da severidade da DE recorrendo ao questionário International Index of Erectile Function 5-questions (IIEF-5). Resultados Verificou-se uma prevalência elevada de HTA (46,3%), IG (36,0%), HTG (24,8%), diminuição de HDL (22,3%) e obesidade central (41,2%). A prevalência de SM foi de 26,5%. O IIEF-5 e o pico de velocidade sistólica (PSV) apresentaram medianas de 12,0 e 34,0 cm/s, respetivamente. As análises multivariadas revelaram a HTA e a IG como fatores independentes influenciando negativamente o valor do PSV (p = 0,002) e o score do IEEF-5 (p = 0,010), respetivamente. Conclusão Enfatiza-se a elevada prevalência de fatores de risco cardiovascular numa população de doentes com DE, assim como a forte associação independente da HTA ao agravamento dos parâmetros hemodinâmicos da função erétil

    The impact of GGH -401C>T polymorphism on cisplatin-based chemoradiotherapy response and survival in cervical cancer

    AIMS: Cervical cancer is the third most frequent cancer in women worldwide, mostly treated with cisplatin-based chemoradiotherapy. Since it is known that folate metabolism might interfere with cisplatin effectiveness, we intended to study the influence of the Gamma Glutamyl Hydrolase -401C>T polymorphism in treatment response in cervical cancer. METHODS: We retrospectively reviewed the clinical data of 167 patients with bulky cervical cancer submitted to cisplatin-based chemoradiotherapy. The genotypes of GGH -401C>T SNP were determined by real-time PCR and statistical analysis was performed by χ(2) test and survival analysis. RESULTS: The genotypes of GGH-401C>T were significantly associated with the response to platinum-based chemoradiotherapy. Treatmentresponse was higher in patients carrying the CC genotype, who presented a significant increased chance of treatment response (survival time in months/genotype: 91 for CC Vs 72 for CT/TT; p=0.035, log rank test). A Cox regression analysis accordingly showed that the presence of the T allele was significantly linked to a worse treatment response (HR=3.036; CI 95% 1.032-8.934, p=0.044). CONCLUSIONS: The results of our study suggested the potential interest of GGH -401C>T as a predictive factor of the outcome of cervical carcinoma treated with cisplatin-based chemoradiotherapy

    Among cardiovascular diseases, pericardial disease has specific characteristics. Its etiology, diagnosis and medical management are not as well understood as in coronary and valvular heart disease. In most cases, its cause is benign, although the proportion decreases with more severe clinical presentation. The authors present the case of a 35-year-old man with no relevant past medical history, who went to the emergency department with what appeared to be an idiopathic case of acute pericarditis. However, over the following five months, there was an unfavorable evolution to constrictive pericarditis, requiring pericardiectomy. The final diagnosis was only made following surgery - a rare case of a primary pericardial tumor, a mesothelioma
