Social Determinants of Health: A Health Care System's Strategy to Improve the Culture of Care
No full textSocial Determinants of Health (SDoH), including housing, transportation, food, socioeconomic status, education, employment, and other factors, influence an individual's health status. Health equity occurs when “every person has a fair and just opportunity to attain their optimal level of health (Centers for Disease Control and Prevention, n.d.). Research indicates SDoH must be addressed to achieve health equity. Recognizing the unique role hospitals play in identifying and addressing health and social needs, regulatory and accrediting bodies now require hospitals to screen for SDoH; however, these entities have provided little to no guidance on how hospitals should implement and integrate SDoH screening and referral programs into their workflows. Hospitals that fail to comply with new regulations risk jeopardizing their accreditation by The Joint Commission and potentially losing reimbursement from Medicare and Medicaid. This dissertation thoroughly explores the challenge of standardizing the SDoH screening and referral system in a hospital setting. The research evaluates a pilot program that implements an SDoH screening and referral process in the ED of a medium-sized nonprofit hospital in a rural Mid-Atlantic region
