看護学生が臨地実習で遭遇した身体抑制への問いの展開に関するナラティヴ分析

研究目的：看護学生が臨地実習で遭遇する身体抑制への問いがどのように展開してゆくの
かを、看護学士課程を通した看護学生の経験から明らかにする。 
研究方法：ナラティヴ分析による質的記述的研究を行った。機縁法を用い、研究対象者は
2021年 3月に看護学士課程を卒業する卒業予定者 16名とし、インタビューガイドを使用
したオンラインによる半構成的面接法を行った。分析手法は、ナラティヴ分析におけるテ
ーマ分析と構造分析とした（倫理審査承認番号：20-A079）。 
研究結果：問いのテーマ分析では、看護学生が実習で身体抑制に遭遇した際の問いの結果が
示された。全 16 事例中、62 の問いがあり、その対象は患者、抑制、自分、看護師、家族、
他学生に分けられ、問いは【患者はなぜ抑制されているのか】、【患者の様子はどうか】、【患
者は不快ではないだろうか】、【患者は抑制をどこまで理解しているのか】、【このような抑制
法があるのか】、【抑制をする判断基準とは何か】、【抑制を外す判断基準とは何か】、【患者の
抑制を外すにはどうすればよいか】、【抑制を考える上での安全、自由とは何か】、【自分は何
をすべきだろうか】、【嫌だな、怖いな、かわいそうだな等】、【自分は今後どう思考や行動す
べきだろうか】、【看護師はなぜこのような行動をとるのだろうか】、【家族は抑制についてど
う思っているのか】、【他学生は抑制についてどう思っているのか】の 15のテーマがあった。 
問いの編集のテーマ分析の結果では、全 16 事例中、251 の問いの編集の語りがあった。
12の問いの編集のテーマ、32のサブテーマに類型化された。問いの編集のテーマは、【この
患者を知ろうとする】、【この患者にとっての抑制の意味を考える】、【抑制について知識を得
る】、【授業での学びを通し抑制について考える】、【その病棟における抑制について観察す
る】、【看護師の行動に意見を持つ】、【自分の考えを周囲に相談する】、【家族の抑制への思い
について知ろうとする】、【抑制しながらも快適に過ごせるよう工夫する】、【抑制は外せるの
ではないかと模索する】、【自分の行動を振り返り内省する】、【学びを今後に活かそうとする】
であった。 
構造分析では、問いの展開の 4要素（問い、問いの編集、問いの編集後の捉え、帰着）を
用い、問いのテーマ分析の結果に沿って全 16事例の問いの展開の構造を明らかにした。結
果、問いの展開の全体の構造図が示された。 
また、実習前の授業から問いの素地はつくられ、問いの編集は主に実習中に行われ、実習
後も問いが展開した。看護学生の問いの編集は、様々な 2極の視点で編集されていた。 
結論：看護学生は、患者、抑制、自分、看護師、家族、他学生を対象とした問いを発し、
4年間を通して問いが展開されていた。展開として、問いの編集から編集後の捉えを経て
帰着、問いの編集を経て帰着、問いが生まれない場合、展開なしという構造がみられた。
また、問いの展開の各要素で、看護学生の二極化する思考が明らかとなった。看護基礎教
育において倫理的課題への問いと展開に注目することが期待される。 
 
 
A Narrative Analysis of the Development of Inquiry about Physical Restraints Encountered by 
Nursing Students in Clinical Settings

Purpose: To understand how inquiries raised by physical restraints encountered by nursing students 
in clinical settings develop from student experiences during the bachelor degree program in nursing. 
Method: A qualitative descriptive study using narrative analysis was conducted, using an interview 
guide online in semi-structured interviews with 16 students scheduled to graduate from the bachelor 
degree nursing program in March 2021. I performed thematic and structural analyses in the narrative 
analysis (Ethics Review Approval Number: 20-A079). 
Results: The thematic analysis of the inquiries showed the results of the inquiries of the nursing 
students who encountered physical restraints in their clinical study. In total 16 cases yielded 62 
inquiries, which were organized into 15 themes.  
The thematic analysis of the inquiry editing yielded 251 items in the 16 cases, categorized into 12 
inquiry editing themes and 32 subthemes.  
The structural analysis of four elements of the inquiries (inquiry, inquiry editing, inquiry post-editing 
understanding, and resolution) identified the structure of inquiry development in the thematic analysis, 
and obtained an overall structural diagram of the development of the inquiries. 
The basis of the inquiries was in a pre-training lecture, and the editing of the student inquiry 
proceeded to a generally bi-polar understanding. The students developed the impressions mainly 
during the training, and it proceeded through strong emotional fluctuations, family suppression 
experiences, and post-training lectures. 
In the inquiry development, the students showed non-cognitive abilities in persistently thinking about 
a single line of inquiry, confronting unanswered inquiries by repeatedly thinking and acting, and 
reflecting on thoughts that they had taken for granted. 
Conclusion: Nursing students asked the inquiry about a variety of topics, and these inquiries were 
developed throughout the four years. It is hoped that more attention will be paid to inquiring and 
developing ethical issues in nursing education.

本研究の目的は、タンザニアのプワニ地域にある 2つの地区の病院において、思春期の妊
婦が長期間作用型で可逆性をもつ避妊法（LARC）を選択する際の産後の家族計画決定支援ツ
ール「Green Star」の効果を検証するため、Green Star（女性が納得して意思決定できる支
援教材の意思決定エイド）を用いる群と通常の家族計画の支援を用いる群を施設割付によ
り比較した準実験研究であった。介入群に用いる意思決定エイドの Green Starは、出産直
後から使用可能な選択肢の子宮内避妊具（IUCD）と避妊用インプラントを紹介した。各群の
介入の回数は 4回で、1回目は妊娠 28週頃、2回目は妊娠 32-34週頃、3回目は妊娠 36-38
週頃、4 回目は分娩後 2 日以内であった。研究参加者は、15 歳から 19 歳で、妊娠 28 週以
降、妊婦健診で通院している病院で出産の意思がある人であり、介入群、対照群ともに 32
名、合計 64 名であった。プライマリーアウトカムである 1 回目と 4 回目に測定する
Decisional conflict scale (DCS；意思決定の葛藤得点)の平均点は、4 回目において介入
群が対照群に比べて有意に低く、4回目と 1回目の差においても有意に低かった。また、DCS
の 4つのサブスケール（情報提供、明確性、支援、不確実性）のスコアにおいても、介入群
が対照群に比べて有意に低かった。セカンダリーアウトカムは、選択肢についての知識、選
択に至る過程の満足度、出産直後の家族計画の利用であったが、1回目と 3回目に測定する
知識テストにおいて、3 回目の平均点は、介入群が対照群に比べて高く、介入群の 3回目と
1 回目の平均点の差は、対照群よりも大きかった。4 回目に測定する満足度評価において、
平均点は介入群が対照群に比べて有意に高かった。出産直後の家族計画を選択した者の割
合は、介入群のほうが 29人であり、対照群 13人に比べて有意に高かった。対照群において
は、年齢要因が DCS得点、家族計画の選択の予測因子（若いほど葛藤が高く、選択できない）
であった。 
審査においては、主に以下の点について修正が必要とされた。 
1．方法論について 
1）プログラムを提供するリサーチアシスタントのトレーニング方法についての加筆が必 
  要 
2）どのようにプログラムが提供されたのか、また DAがどのように説明されたのか加筆 
が必要 
2．結果について 
1）介入群と対照群において、対象者の年齢・結婚歴・職業に差があったため、DCS、選択
肢についての知識、選択に至る過程の満足度、出産直後の家族計画の利用について重回
帰分析あるいはロジスティック回帰分析を行ってその結果の加筆が必要。 
2）介入群の全員が満足度に対し「満足」と回答していたため、質問紙への回答方法の 
加筆が必要 
3.考察について 
1）家族計画の選択では、両群ともに、IUCDを選んだ者が皆無であった理由と、今後の意
思決定エイドの改善について加筆が必要 
2）4回目の測定で、DCSのスコアが高い対象がいたことについての考察の加筆が必要 
 
以上の内容に対し、審査員によって論文が適切に修正されたことを確認された。 
思春期における妊娠は健康課題であることから初回妊娠への介入研究が多い中、本研究
は次子妊娠率の高さに注目し、確実な避妊方法の選択を支援することに注目した点で非常
に先駆的である。LARCの知識や誤解等の家族計画への障壁を調査し、自立した意思決定が
困難な若い女性たちの避妊方法の選択を支援するため、意思決定支援ツールを開発したこ
と、そして、それによって適切な避妊方法を選択できる可能性があることを示したこと
は、タンザニアの女性のリプロダクティブヘルスの向上に寄与できる結果であると高く評
価できる。予備研究 2編を国際誌に公表している点、意思決定支援ツールを使用したタン
ザニアにおける最初の研究であるという点においても新規性と独自性があり、今後の発展
に期待できる研究である。 
以上により、本論文は、本学学位規程第 5条に定める博士（看護学）の学位を授与するこ
とに値するものであり、申請者は看護学における研究活動を自立して行うことに必要な高
度な研究能力と豊かな学識を有すると認め、論文審査ならびに最終試験に合格と判定する。

Effect of the Postpartum “Green Star” Family Planning Decision Aid on the Uptake of Long-Acting Reversible Contraception Among Pregnant Adolescents, Tanzania: Facility-Based Quasi-Experimental Study Design

論文要旨 
【目的】本研究の目的は、漢方医学を学んだ看護師の看護実践の経験について記述すること
である。西洋医学とは考え方も対象を見る物差しも、アプローチの仕方も異なる医学体系か
ら成るこの漢方医学を、看護師はどのように捉え、どのように自分の中に取り込み、そして
ケアに生かすことができるのだろうか。そもそも漢方医学を看護師が学ぶということはど
ういうことなのかを明らかにする。 
【方法】1.研究デザイン：現象学を手がかりとした質的記述的研究。2.研究対象者：漢方医
学の系統的な学習経験があり、それを活用した看護を行っていることを本人及び周囲が認
識している看護職 3 名。3.データ取集方法：約 90 分の非構造的インタビューを 1～2 回行
った。4.分析方法：西村（2014）「現象学的看護研究 理論と分析の実際」，村上 (2013)「摘
便とお花見 看護の語りの現象学」の分析方法を参考にした。5.データ収集期間：2018 年 3
月 1 日～2022 年 3 月末。6.倫理的配慮：聖路加国際大学研究倫理審査委員会の承認を得て
実施した（承認番号：17-A098） 
【結果】3名の研究対象者の語りから、漢方医学を学んだ看護師の看護実践の共通の経験が
捉えられた。漢方の学びは、漢方医学が人々の日常の生活に近いところにあるため、看護師
が、学んだ知識を頭で理解するだけでなく、自らの日常の経験の中でそれを深く理解するこ
とを可能にすると共に、患者と自分は互いに養生して生きる人間同士だと気づく。そして看
護師は、漢方の知識を自分のものとして、それぞれの生活や看護に活かしていく。日常の些
細な生活行動に根拠と意味を与える漢方は、看護師の視点を生活に向けさせた。そして生活
に視点を置き、トータルにその人を捉え、日常の生活行動に働きかける看護の大切さを改め
て意識した時看護師は、今度は自ら意図して一つ一つのケアを意味づけて行っていくので
ある。そしてそれは「漢方っていう看護」であり、漢方医学を根拠にもつ、生活に視点を置
いた患者の自然治癒力を引き出すケアであり、「看護の引き出し」を増やすものであった。 
【結論】漢方医学を学んだ看護師の看護実践は、「生活に視点を置き患者をトータルにみて、
漢方医学を根拠として些細な生活の援助にも意味を見出し、そして、互いに養生して生きる
人間同士として、患者が自らを気にかけ、主体的に自分をケアして生きていくことを支える」
ものだった。漢方を学ぶことは、患者の日常の暮らしやその支援に目を向けさせ、その意味
と価値を再認識させ、本来の看護に立ち戻らせる意味を持つ。漢方医学を看護師が学ぶこと
は意義のあることであり、今後さらに看護師が漢方医学を学ぶ場が広がることが求められ
る。 
Abstract 
Purpose： To describe the experiences of nurses who learned Kampo medicine. 
Method： This qualitative descriptive study used a phenomenological approach to 
describe the experiential phenomenon. The research subjects were three nurses who 
underwent a systematic learning experience in Kampo medicine. They were aware of 
their nursing practice and surroundings and provided Kampo medicine care. Data were 
collected using a 90-min nonstructural interview conducted once or twice. The nurses 
were asked to freely talk about unique practical situations. The analytical methods used 
were Nishimura’s Phenomenological Nursing Research Theory (2014) and Murakami’s 
Phenomenology of Stool Picking and Cherry Blossom Nursing Narrative (2013). Data 
collection was performed from March 1, 2018, to March 31, 2022. The study was 
conducted with the approval of St. Luke’s International University Research Ethics 
Review Board (approval number: 17-A098). 
Results： The combined nursing practice experiences of the nurses who learned Kampo 
medicine were obtained from the stories of three research subjects. Kampo medicine is 
close to people’s daily lives; it relieves them of their sufferings. Nurses should 
understand the knowledge they have learned and deeply understand it in their daily 
experiences. They must realize that the patient and nurse are human beings who cure 
each other. Nurses with knowledge of Kampo medicine use it in their daily lives and 
nursing practice. Kampo medicine provides a basis and meaning to trivial daily 
activities, turning the nurse’s perspective regarding life. Then, when the nurse adopts 
an alternative viewpoint of life, they reaffirm the importance of nursing on daily life 
behavior. The nurse will intentionally give meaning to each episode of care. This 
phenomenon in nursing is called Kampo, which means care that extracts the natural 
healing power of patients from the viewpoint of life based on Kampo medicine. 
Conclusion：The nursing practice experience of nurses who learned Kampo medicine 
became clear from three nurses’ stories. The nurses observed the patient, focused on 
their lives, and found meaning in the trivial life support based on Kampo medicine. The 
nurse recognized that they and the patients are human beings who live to cure. Kampo 
medicine motivates patients to be aware of themselves and take the initiative in caring 
and living for themselves.

ついて考察することである． 
【方法】1. 研究参加者：循環器専門医による治療を実施している施設に通院中の療養者に
協力を依頼した．米国心臓協会(AHA)/米国心臓病学会(ACC)の心不全ステージ分類が
Stage C または D の療養者で，70 代～80 代の男性 4 名女性 1 名の計 5 名，2. 研究デザイ
ン：質的記述的研究，3. 研究期間：2020 年 5 月～2022 年 3 月，4. データ収集方法：非
構造化面接法を用いて１～2 回の対面でのインタビューを行った，5. 分析方法：現象学的
アプローチを手がかりに分析し，現象学研究者のスーパーバイズを受け真実性を高めた，
6. 倫理的配慮：研究者が研究参加者に文書と口頭にて研究内容，参加の自律性，匿名性，
個人情報の保護，公表について説明し同意を得た．本研究は，聖路加国際大学研究倫理審
査委員会の承認を得て実施した（承認番号：20－A015）． 
【結果】参加者の語りから考えられた慢性心不全療養者の病いの経験の本質は以下の通り
である．1 点目は，起こっている出来事の〈意味〉が捉えにくいこと，症状の現れが自覚
しにくく，かつ，いつ起こるかわからないという過去と未来の〈時間〉の曖昧さが，自覚
し難い身体で生きる経験を創り出していた．2 点目は，曖昧な病いの経験により，そうな
ってしまっている自分を「しょうがない」という能動/受動という態では現れてこない，つ
まり自分とはそういうものだというところに位置づけられていた．3 点目の医療との関わ
り方は，「しょうがない」「わからない」から至った「お任せする」であり，また，医師や
これまでの治療のおかげでまあまあと言える今を肯定的に意味づけたうえで選択した「お
任せ」でもあった．それだけではなく，彼らが具えている他者から自己を了解しようとす
る志向性により，医療との関わりを創り出していた．4 点目の死への関わり方は，輪郭が
はっきり見えていない死にどのように関わるのか，それは目前の日常に焦点を合わせ，過
去の経験の意味を捉え直しながら今を生きることに専念するという在り方であった． 
【結論】彼らの病いの経験は，慢性心不全のもたらす症状の捉えにくさと発生が予測でき
ないという曖昧さが大きな特徴であった．自分のなかで抗いようがないものが進んでいる
ことを実感し，それを「しょうがない」というところに位置づけて自分の存在や今生きる
意味を他者との関係によって見出していく．そして，自分にとって大切なものやその価値
を改めて見出し，大切な〈いつも〉の日常が保てるように医療とほどほどの距離を保ちな
がら付き合っていくという経験であった． 
 
 
 
Abstract   
 
[Purpose] The purpose of this study is to describe the illness experiences of older adults living 
with chronic heart failure. 
[Method] 1. Research participants: Patients with Heart Failure Stage C or D from AHA/ACC, 
four males and one female in their 70s and 80s, for a total of 5 people, 2. Study design: 
Qualitative descriptive study, 3. Research period: May 2020-March 2022, 4. Data collection 
method: Interviews were conducted using the unstructured interview method, 5. Analytical 
method: The analysis was performed based on a phenomenological theory, 6. Ethical 
considerations: This study was carried out with the approval of the Research Ethics Review 
Board of St. Luke's International University (Acceptance Number: 20-A015). 
[Results] The illness experiences of patients with chronic heart failure, which was considered 
from the participants' narratives, are as follows. First, it is difficult to grasp the "meaning" of 
what is happening and the ambiguity of past and future "time" is that the appearance of 
symptoms is hard to notice and they do not know when it will occur. Second, because of the 
vague experience of illness, they positioned it as "it can't be helped”. Third, that the way of 
getting involved with medical care was to "leave it to the doctor", which came from "it can't be 
helped", or it was chosen after giving positive meaning to the present. Furthermore, it was a 
way of being involved in medical care by understanding oneself from through others. Fourth, 
how it relates to death, it was to update the meaning of past experiences and to live in the present 
with a focus on everyday life.  
[Conclusion] Their experience of illness was largely characterized by the elusiveness and 
unpredictability of the symptoms of chronic heart failure. They positioned themselves as "it 
can't be helped" and found out their existence and the meaning of life through relationships with 
others. They found something important and valuable to them, addressed it at a reasonable 
distance from medical care, and maintained their daily lives.

論文要旨 
Ⅰ．研究目的 
本研究の目的は、帝王切開術を経験した母親が持つ母乳授乳への予想（妊娠中のイメー
ジ）と現実との違いを明らかにし、今後の母乳授乳の支援を検討することである。 
Ⅱ．方法 
研究デザインは、Web 調査にて探索する前向き観察研究である。研究対象は 6か月以内に
帝王切開術を受けた、日本語の読み書きが可能な産婦とした。内容は、参加者の基礎情報、
妊娠中の母乳授乳のイメージ、産後の母乳授乳の現状、産後の体調、母乳授乳の思いについ
て情報収集を実施した。そして項目ごとの基本統計を算出し、自由記載で得られた質的デー
タに関しては、意味内容をまとめて分析を行った。 
Ⅲ．結果 
帝王切開の母親の、母乳授乳に対する妊娠中のイメージと実際の経験とのギャップを把
握するために、55 名の母親から Web アンケートに回答を得た。その結果、ギャップが少な
かったものは、「母乳を昼夜あげることは大変である」「誰もがすぐにできるものではない」
のイメージであり、逆にギャップが大きかったものは、「乳頭が痛いという経験をイメージ
していた」「赤ちゃんは簡単に吸い付くと思っていた」「おっぱいが張って痛いという経験を
イメージしていた」であった。母乳授乳をしていて「嬉しかったこと」は、【幸福感】【十分
な栄養を与えられた安心感】【母としての特権】【生命力】であり、「悲しかったこと」は、
【身体的な疲労】【疼痛】【児に拒否されて戸惑う】【母親失格の気分】【制約がある】【周囲
から傷つけられた経験】との表現が認められた。母親がケアとして受け取っていたことは、
【授乳ケア】【術創のケア】【意思決定支援】【メンタルヘルスへの気遣い】であり、【ケアを
受けられなかった】という記述も少し認められた。帝王切開術を経験した母親が次の人へ伝
えたいこととしては、【帝王切開術は勇敢で幸せな体験】【無理をせず自分をいたわる】【身
体も心も想像以上に疲弊している】という内容であった。 
Ⅳ．結論 
帝王切開術を行った母親は、母乳授乳に対して「大変なことであり容易くない」と予測
しており、それは現実と一致していた。しかし経験をして初めて知った「乳頭や乳房の変
化や痛み」は事前にイメージすることはできずギャップを感じていた。よって、助産師
は、妊娠中の情報提供とあわせて、帝王切開後の母親が産後に初めて体験することは事前
にイメージできずギャップがあることを理解してケアする必要がある。
