Abstract 
Background: This study examines the correlation between diagnostic delay (DD) and several 
variables related to endometriosis in Japanese women, while simultaneously exploring the 
cut-off periods for short or long diagnostic delays. Factors influencing this delay, including 
healthcare behavior, are mainly examined. 
Methods: This cross-sectional study was conducted using an online survey among 220 
Japanese women aged 18–49 years who were diagnosed with endometriosis. Data on 
healthcare behaviors, economic expenditures, and endometriosis specific outcomes divided 
by the period of diagnostic delay were collected and analyzed using logistic regression 
models. 
Results: All 220 endometriosis patients reported a mean age of initial symptoms onset at 24.3 
years and a mean diagnosed age at 27.7 years. The median time difference between first 
symptom onset and diagnosed age was 1.5 years, and the difference further increased when 
divided into Short DD and Long DD (P < 0.001). Among these endometriosis patients, a 
longer diagnostic delay was significantly associated with higher OTC drug use (P = 0.008) 
and higher proportion of patients with Stage 4 endometriosis (P = 0.022).  
Conclusion: These findings indicate that diagnostic delays exceeding 1.5 years are associated 
with significant disease progression, and the use of over-the-counter pain medication whose 
healthcare behavior may contribute to the postponement of professional medical consultation. 
Early intervention strategies, such as routine screenings and awareness programs, could help 
patients seek medical attention earlier and potentially reduce the burden of advanced disease. 
Future research should prioritize not only pain management but also clinical assessments in 
order to reduce burden of silent progression and improve long-term patient outcomes.

Abstract 
Background: Glucocorticoids are widely used for treating rheumatism and other 
diseases. Among their complications, glucocorticoid-induced osteoporosis (GIOP) is a 
preventable and treatable condition. At St. Luke's International Hospital, the prescription rate 
for GIOP management was established as a quality indicator (QI). To improve this QI, a 
Clinical Decision Support System (CDSS) was introduced. This study aimed to evaluate the 
impact of CDSS on QI through interrupted time series (ITS) analysis using a control group 
for comparison. 
Methods: This retrospective, single-center study evaluated the impact of a CDSS on QI 
for GIOP management. ITS analysis was conducted to assess QI trends across four defined 
periods: baseline, post-implementation, suspension, and resumption of CDSS. Difference-in-
Differences (DiD) analysis was specifically applied to compare QI levels before and after the 
resumption of CDSS, focusing solely on the resumption period. The intervention group 
consisted of patients managed with CDSS, while the control group included patients with 
rheumatoid arthritis and minimal glucocorticoid exposure. Statistical analyses evaluated 
changes in QI levels and trends, with significance set at a two-tailed alpha of less than 0.05. 
Results: The intervention group, consisting of CDSS notification recipients, included 
3,698 individuals, while the control group included 148 individuals. Observation period was 
from January 2011 to March 2024. CDSS implementation resulted in an immediate QI level 
increase of +0.47 (p < 0.05); however, the improvement slope subsequently declined (-0.053, 
p < 0.05) in the intervention group. During the suspension period, QI levels in the 
intervention group decreased significantly (-1.08, p < 0.05), while the resumption phase

showed gradual recovery without statistically significant immediate effects (-1.04, p = 0.123). 
The DiD analysis comparing the resumption and suspension phases revealed no significant 
differences in QI trends between the intervention and control groups (interaction term: p > 
0.05). Despite these findings, both groups exhibited overall upward trends in QI, with 
intervention group levels consistently outperforming the control group. 
Conclusion: CDSS demonstrated its potential to enhance QI in GIOP management 
through immediate improvements; however, its limitations in sustaining long-term effects 
highlight the need for complementary strategies. Minimizing system disruptions, addressing 
contextual factors, and implementing multifaceted approaches—including clinician education 
and personalized feedback—are critical to maximizing CDSS effectiveness and ensuring 
sustained quality improvement in clinical practice.

The dissertation presentation by Miho Kato was conducted on Dec 16, 2024 over the 
zoom platform with the presence of three committee members, Sachiko Ohde (Chair), 
Zoie Wong, Nobuo Nishi and attendees from several GSPH faculty students as well as 
students from Nursing college.  The topic of the dissertation is titled as “Modeling to 
Improve Support System for Learning Difficulties among Childhood Cancer 
Survivors in Japan using System Dynamics Simulation”. The venue for the dissertation 
presentation included the main presentation of approximately 40 minutes followed by 
an open Q&A session which lasted for another 20 minutes. After the public presentation, 
closed Q&A session between Miho Kato and three committee members was followed. 
Prior to the presentation day, all three committee members reviewed his submitted 
manuscript and provided the comments as necessary.  
 
In the main public presentation, Miho Kato presented her dissertation contents clearly 
in an organized way. The presentation started with specific aims which were examined 
using appropriate flow of System Dynamics Simulation. The topics along with the 
intellectual contents are suitable for doctoral level research activities.  
 
Miho Kato logically presented her work with appropriate wording using simple and 
easy to understandable language to the audience. According to her study findings, 
Support System for Learning Difficulties among Childhood Cancer Survivors was 
successfully visualized that demonstrate the urgent need for early detection of 
neurocognitive impairments in CCS, as well as targeted interventions to support 
academic achievement. 
 
The committee has reached a consensus and decided that the dissertation of  Miho 
Kato is worthy of the award of the degree of Doctor of Public Health as stipulated in 
Article 6 of the Academic Degree Regulations of the University after going through the 
submitted dissertation along with the presentation. Also, the applicant has been 
considered as possessing the advanced research abilities equipped with knowledge 
necessary to independently conduct research activities in public health as well as in 
the field of global health. Finally, this applicant is judged to have passed the 
dissertation examination and the final examination by the committee members.

短時間のマインドフルネスに関する研究の背景と展望
―現代社会における実践可能性の検討―

1．はじめに
マインドフルネスとは、「現在の瞬間の体験に意図的に注意を向け
ることで生じる、判断にとらわれない気づき」と定義される
（Kabat-Zinn 1982 : 33-47）。日常語としての「マインドフルネス」
は、仏教の瞑想法に起源を持つ心の状態や実践方法を指すが、現代
では科学的な観点から、ストレス軽減や心身の健康増進のための手
法として研究・実践されている。従来のマインドフルネスプログラ
ムは長時間かつ長期間の介入が一般的であった。例えば、Jon
Kabat-Zinnによって開発されたマインドフルネスに基づくストレ
ス軽減プログラムには、週 1回の 2時間の講義と実践セッション、
そして毎日 45分の自主的な瞑想実践を 10週間続けることが推奨さ
れている（Kabat-Zinn 1982 : 33-47）。しかし近年では 5-30分程度
の短時間での実践効果を示す研究報告が増加している。
現代社会においては、時間的制約や多様なストレス要因により、従
来の長時間プログラムの実施が困難な場合が多い。そのような状況
において、短時間のマインドフルネスは、実施可能性のある代替手
法として注目されている。
2. 短時間マインドフルネスの実践方法と効果に関する先行研究
2.1 主な実践方法
先行研究では、複数の短時間のマインドフルネスの実践方法が報告
されている。基本的な方法として、マインドフルネス呼吸法が挙げ
られる。これは呼吸に注意を向け、持続的に集中することを目的と

した練習（Kabat-Zinn 2013）である。次に、ボディスキャンと呼
ばれる手法がある。この方法では、体の異なる部位の感覚に注意を
向けながら、注意が逸れた場合は対象に戻ることを繰り返し、マイ
ンドフルな気付きを育むことを目指している（McClintock 2019 :
265-278）。また、思考や感覚のマインドフルな受容という方法も実
践されている。これは思考や感情を判断せず、そのまま受け入れる
姿勢を養うものである（McClintock 2019 : 265-278）。さらに、オ
ープンモニタリングでは、より広い気づきの領域を促進し、生じて
くる様々な経験を非判断的に観察すること（McClintock 2019 :
265-278）を目指している。これらの技法は単独、あるいは組み合
わせて実践されており、その効果が様々な研究で確認されている。
以下ではこれらの実践方法を用いた研究において、どのような効果
が確認されているか概観する。
2.2 効果に関する実証研究
マインドフルネスの効果については、身体面と心理面の両側面から
実証研究が進められている。身体的効果として、炎症反応の抑制
（Rosenkranz 2013 : 174-184）や免疫機能の向上（Infante 2014 :
147-151）が報告されている。また、血圧の安定化（Steinhubl 2015 :
145）や疼痛の緩和（McClintock 2019 : 265-278）といった症状の
改善も確認されている。心理的効果については、従来の長時間プロ
グラムと同様に、短時間の実践でもストレスと不安の軽減
（Gilmartin 2017）、うつ症状の改善（Schumer 2018 : 569-583）
といったメンタルヘルスへの効果が確認されている。また、認知機
能面では、注意制御機能の向上やマインドワンダリング（意識が現
在から離れて、過去や未来の出来事に思いを巡らせてしまうこと）
の減少といった効果（Infante 2014 : 147-151）が報告されている。
これらの研究結果は、短時間のマインドフルネス実践でも効果が得
られることを示唆している。

3. 現代社会における意義
短時間のマインドフルネスの実践の特徴として、まず時間的効率性
が挙げられる。従来のマインドフルネスプログラムと比較して、よ
り短時間で効果が得られることが確認されており、多忙な現代人の
日常生活に無理なく組み込むことが可能である。この時間的効率性
は、継続的な実践を促す一因となっている。次に実践場所の柔軟性
が挙げられる。オフィスや学校といった日常的な環境で実施可能で
あり、特別な空間を必要としない。さらに、近年のデジタル化に伴
い、オンライン環境での実施も可能となっている。この柔軟性は、
実践機会の拡大につながり、より多くの人々がマインドフルネスへ
アクセスできる可能性を高めている。加えて、実践における経済的
な側面も特徴の一つである。特別な設備や道具を必要とせず、基本
的な指導があれば実施可能であり、集団での実施も容易であること
から、教育機関や企業などの組織への導入における障壁が低い。こ
れらの特徴は、マインドフルネスの普及を促進する要因となってい
る。
4. 短時間マインドフルネスの研究課題
短時間のマインドフルネスの研究において、いくつかの検討すべき
課題が残されている。まず、効果がどの程度持続するかという点で
ある。単回実施と継続実施における効果の違いや、効果を維持する
ための最適な実施頻度について、さらなる研究が求められる。特に、
日常生活における実践可能性を考慮した上で、効果的な実施頻度を
明らかにすることが重要である。また、年齢や性別による効果の違
い、個人の心理特性による適性の違いといった個人差要因を検討す
ることで、効果的な実践方法の選択や個別化が可能となると考えら
れる。実施方法の最適化も課題の一つである。実施時間の最適化は、
効率的な実践のための要素であり、異なる技法の比較検討を通じて、
各技法の特性や効果の違いを明らかにすることも必要である。これ

らの知見は、個人や状況に応じた適切な実施方法の選択に役立つと
考えられる。さらに、作用メカニズムの解明という課題も残されて
いる。生理学的指標との関連を明らかにすることで、短時間のマイ
ンドフルネスが身体に及ぼす影響をより詳細に理解することが可能
となる。加えて、心理的変化のプロセスを解明することで、より効
果的な実践方法の開発につながることが期待される。これらの基礎
的な研究の進展により、短時間のマインドフルネスの科学的根拠の
蓄積が進み、より効果的な実践方法の確立が見込まれる。とくに、
実践現場での知見を研究に反映させることで、実用的な展開の可能
性が広がると考えられる。
5. 実践事例：伝統と現代的解釈の融合
現代のマインドフルネスは仏教瞑想に起源を持つが、その実践方法
は前述したように大きく変容している。特に短時間のマインドフル
ネスは、効果や目的に着目した現代的なアプローチを取る一方で、
その本質的な価値である「気づき」や「現在の瞬間への注意」につ
いては、伝統的な仏教瞑想の知恵を継承している。この点において、
日本の禅宗、特に坐禅から示唆を得ることができる。以下では、ま
ず坐禅の本質的な理解を深めた上で、それを現代的に解釈し実践し
ている事例として、聖路加国際病院の取り組みを検討する。
5.1坐禅の本質的理解
坐禅は道元禅師が「最も優れた修行方法で、しかも、大安楽の法門
である」と説いているように、その本質は単純な構造を持ちながら、
実践的意義を有している。道元禅師は、坐禅によって「肉体も精神
も共に爽快になり、邪念やとらわれを離れ、心が動揺することなく、
真実のすがたをありのままに見究めることができる」と述べ、また
「賢人も愚者もえらぶことなく、すべての人が坐禅にはげむべき」
（今枝 2023 : 64-69）として、その普遍的な価値を強調している。

坐禅の特徴的な点は、特定の目的や達成目標を持たない点にある。
道元禅師は「さとりを求めない、ただ坐禅するだけで何も求めない」
（松原 2000）という姿勢を重視した。これは現代のマインドフル
ネスが特定の効果や目的を持って行われるのとは異なる点である。
また、坐禅における心身の在り方も特徴であり、「心を動かしたり、
いろいろ想像するようなことをしない。ただひたすらに坐る」（松原
2000）という基本姿勢のもと、六根（眼・耳・鼻・舌・身・意）か
らの感覚をありのままに受け入れる（藤田 2016 : 165-202）として
いる。これは特定の対象（例えば呼吸）に焦点を当てる瞑想とは異
なり、全ての感覚をありのままに受け入れる姿勢が重視される。
5.2 聖路加国際病院における現代的解釈と取り組み
このような伝統的な坐禅の本質である、「ただ坐る」という単純さ
と、「全ての感覚をありのままに受け入れる」姿勢を基盤とした上で、
聖路加国際病院のチャペルでは、命に携わり、常に多忙である医療
従事者が自らをかえりみて静かなひと時を過ごすことを目的として、
キリスト教の瞑想伝統との融合を図りながら、現代的な実践方法を
確立している。「聖路加メディテーション 〜キリスト教的座禅のス
スメ〜」と名付けられたこの取り組みでは、キリスト教における二
つの瞑想形態である、特定の主題について意識的に考察を深める「黙
想（メディテーション）」と、分析的思考を超えて、静かに心を開き、
深い精神的な気づきを得る「観想（コンテンプレーション）」を基盤
としている。本取り組みの特徴は、伝統的な坐禅の本質を保持しな
がら黙想を重点とし実施されている点にある。
本プログラムは毎週木曜日の朝 7:00から 8:30の間、旧館 2階の礼
拝堂前ロビーの一角で実施されている。参加者は時間内であれば自
由に入退場可能であり、多忙な医療従事者の勤務時間に配慮した柔
軟な参加形態となっている。実践の開始時には、時として五体投地
などの身体的なワークも取り入れられ、心身の準備を整える機会が

提供されている。参加者は表面に実施方法、裏面に黙想のヒントが
書かれたカードを 1枚受け取り、これらを参考に黙想を行う。注意
が逸れた際には、カードの単語に意識を戻すことで、集中を取り戻
すための手掛かりとして活用する。
本プログラムの特徴は、参加者の状況に応じた柔軟な実施が可能な
点にある。実施環境には椅子やクッションが設置され、参加者は座
禅や椅子座りなど、各自の快適さに応じた姿勢を選択できる。また、
実施時間は参加者のコンディションに応じて自由に調整可能であり、
黙想の方法についても、提示された単語への集中や、より自由な意
識の観察など、参加者が自身に適した方法を選択できる。
このような実践は、伝統的な坐禅の本質を保持しながら、短時間の
マインドフルネスの特徴である、時間的効率性、場所の柔軟性、低
コスト性の統合を試みた一つの例といえる。現代社会において、人々
が直面する「常に刺激や情報にさらされ、多忙な日常を送る中で、

静かに自身の心に意識を向ける時間を確保することの困難さ」とい
う課題に対して、実践可能な一つの解決策を提示している。
6. おわりに
本研究では、短時間のマインドフルネスの実践可能性と効果につい
て、現代社会における意義を中心に検討を行った。従来の長時間プ
ログラムと比較して、時間的効率性、場所の柔軟性、低コスト性と
いう特徴を持つ短時間のマインドフルネスは、現代人の生活様式に
適合した実践方法として注目される。特に、聖路加国際病院の実践
例は、伝統的な瞑想の本質を保持しつつ、現代のニーズに応えた実
践の可能性を示している。この知見は、教育現場をはじめとする様々
な領域への応用可能性を示唆している。特に学校教育においては、
授業の開始時や休憩時間を活用した短時間のプログラムとして、学
習者のストレス管理やメンタルヘルスケアに貢献することが期待さ
れる。教育実践における時間的制約や施設面での制限を考慮すると、
短時間のマインドフルネスの実施の容易さは導入における有利な特
徴となる可能性がある。今後は、効果の持続性や個人差要因、実施
方法の最適化などの研究課題に取り組むとともに、オンライン環境
での活用や発達段階に応じた実践方法の開発が求められる。さらに、
情報過多やデジタル機器への依存など、現代社会におけるストレス
要因への予防的介入としての可能性についても、今後の検討課題と
して挙げられる。基礎研究の深化と実践研究の蓄積を通じて、現代
社会に適合した心身の健康増進手法としての短時間マインドフルネ
スの確立が望まれる。

損益分岐点分析を用いた助産所での産後ケア事業に
関する経営の実態分析 
Management Status of A Postpartum Care Business at A Midwifery Clinic  
Using Break-even Point Analysis. 
要旨 
 
【目的】助産所における産後ケア事業に関して、損益分岐点分析を行うことによって、助
産所での産後ケア事業の経営方法について示唆を得ることを目的とした。 
【方法】産後ケア事業を実施している助産所を便宜的に抽出し、施設長を対象として質問
紙調査を行った。調査票の内容は主に、助産所の基本情報、産後ケア事業の実施状況、助
産所の収益、助産所の費用で構成した。分析は、損益分岐点売上高と損益分岐点売上高と
なる年間利用件数、1 件あたりの売上高を算出した。本研究は、聖路加国際大学研究倫理
審査委員会の承認を受けて行った（承認番号：24A-052）。 
【結果】研究対象施設は 5 施設であり、分娩を取り扱っている施設が 4 施設、取り扱って
いない施設が 1 施設であった。また、5 施設が委託契約を行っている自治体の合計数は宿
泊型 17 自治体、デイサービス型 15 自治体、アウトリーチ型 7 自治体であった。産後ケア
事業の利用者負担料金の平均は、宿泊型は 4,481（SD 3,135）円、デイサービス型は
3,219（SD 2,001）円、アウトリーチ型は 1,100（SD 273）円であった。産後ケア事業の 1
日利用あたりの売上高の平均は、宿泊型は 31,481（SD 1,855）円、デイサービス型は
25,807（SD 2,089）円、アウトリーチ型は 13,364（SD 2,965）円であった。各助産所の変
動費率と損益分岐点売上高は、B 助産所は変動費率が 25.9%、損益分岐点売上高が
19,227,177 円、C 助産所は変動費率が 15.0%、損益分岐点売上高が 10,485,335 円、D 助産
所は変動費率が 11.8%、損益分岐点売上高が 30,789,790 円、E 助産所は変動費率が
22.8%、損益分岐点売上高が 11,086,225 円であった。損益分岐点売上高を満たすために必
要な 1 日利用あたりの売上は、宿泊型 29,303 円〜61,875 円、デイサービス型 25,045 円〜
48,756 円、アウトリーチ型 14,606 円〜19,581 円となった。 
【結論】産後ケアの売上は、1 日利用あたりの売上が低く、出産の収益が産後ケアの費用
を補填している現状がある。産後ケアの種類の比較では、宿泊型はデイサービス型と比較
して、利用時間の長さに対する売上が低いことが示された。また、産後ケア事業の利用者
負担額は、助産所や自治体によってばらつきがあることが明らかとなった。したがって、
産後ケア事業の利用者負担額及び自治体助成額は産後ケア事業を黒字化し、助産所が産後
ケアの運営を持続させることのできる適正価格を検討し、自治体ごとの金額を標準化する
必要がある。また、宿泊型産後ケアに関しては、利用料金及び自治体助成額は利用時間の
長さを考慮した上で価格設定を行うべきである。

要旨 
 
目的 未就学児に対しセクシュアリティ教育を実施している助産師にインタビューを行い、助
産師による未就学児へのセクシュアリティ教育の実際と課題を明らかにすること。 
方法 半構造化質問紙を用いた聞き取り調査で、未就学児（3歳から5歳）に対しセクシュア
リティ教育を1回以上実施している助産師免許保持者とした。研究の参加について本人から同
意を得られた者に対しインタビューを実施し、逐語録作成、コード、カテゴリー化を行った。 
結果 5か所の施設、6名の助産師を対象にインタビューを実施した。《身体と心の権利》と
《生命の誕生》に関する内容が中心として伝えられ、【子ども達が自ら考え参加できる授業】
とし、事前打ち合わせで子ども達の情報を収集した上で【様々な背景を持つ子ども達に対する
配慮】を行い、【教育者の価値観をのせない中立な立場で科学的な話】を心がけていた。授業
に対し子ども達は【授業への積極的な姿勢】で【授業後の言動の変化】も認められた。課題と
しては【セクシュアリティに対して消極的な風土がある】や【子どもの環境とセクシュアリテ
ィ教育との乖離がある】ことから、【大人（保護者や先生）に対しセクシュアリティ教育の研
修を実施する】必要性があげられた。そして、助産師は【妊娠・出産・子育ての経過、命の尊
さや重さを知っている】、【科学的、心理的側面からエビデンスをもとに中立的立場で伝える
ことができる】、【SRHR（性と生殖に関する健康と権利）ベースで伝えることができる】とい
う利点があげられる一方で、【助産師の個々の価値観が反映されることによって教育内容が統
一されていない】現状がある。このことから、助産師自身も【色々なジャンルの人の話や実際
にセクシュアリティ教育を行っている人の話を聞く】や【包括的性教育を学ぶ】、また、これ
まで助産師として経験してきた【事例の振り返りを行う】ことの必要性をあげていた。 
結論 《身体と心の権利》と《生命の誕生》に焦点を当てて伝えており、幼児期からジェンダ
ー・アイデンティティは形成されることからジェンダーに関する内容を取り入れる必要性が示
唆された。工夫としては、子ども達が積極的に授業に参加できる方法を用い、一人ひとりが大
切な存在であると実感できる内容を取り入れることや、ポジティブな体験を提供する必要があ
る。課題として子ども達は取り巻く環境による影響が大きいことがあげられ、大人への教育の
必要性があげられた。そして、セクシュアリティ教育は幼少期から継続的に、段階的に積み上
げていく必要性が示唆された。助産師がセクシュアリティ教育を行う意義として、命の尊さ、
重さを知り、科学的、心理的側面から中立的立場で伝えることができること、SRHRの専門家で
あることがあげられるが、中には個人の価値観をのせたり、断片的な指導を行っている者もい
ることから、助産師自身、包括的セクシュアリティ教育を学ぶことや、これまで助産師として
関わってきた事例の振り返りを行う必要性が示唆された。
