Of ambivalent identities: A reflexive exploration of identity construction of adolescent girls as Muslim women inside Bangladeshi madrasahs
The following paper explores how adolescent girls in Bangladeshi madrasahs conceptualize their identities as Muslim women in lieu of the author's own experiences. The conceptual framework has been constructed using three broad themes beginning with the social constructionist perspective followed by exploring the Multiple Dimensions of Identity Theory to explore the link between multiple dimensions of identity and meaning making i.e. the ability to perceive relationships between and among their multiple identities and noticing the tensions and conflicts, awareness of their inability to reconcile multiple identities. The third theme will draw upon Judith Butler's interpretation of subjectification i.e. the processes through which we are subjected, and actively take up as our own the terms of our subjection. In addition to the three themes, the research will heavily rely on Himani Bannerji (1995) for her thoughts of production of the Other in her essay "But who speaks for us- Experience and Agency in Conventional Feminist Paradigms." The concept of the social production of the other through employing two research methods i.e. writing storied poems about the author's own experiences of working closely with adolescents in a Bangladeshi madrasah boarding school and analyzing the artwork produced by two female students in response to the question, "How do you see yourself as a woman?" The research indicates that there is no one conceptualization of identities. Identities in Bangladesh are ambivalent and there is a necessity to depend our understanding of this ambivalence as academics and researchers. This ambivalence, though uncomfortable, must be engaged with in order to understand how individuals become and take up their own terms of subjection
Computational evaluation of binding property of riboswitches and optimization of self-docking of RNA-ligand complexes for drug discovery
Nucleic acids play an important role in a variety of cellular processes. Recent advances in the determination of DNA and RNA structures and functions highlighted the potential of targeting nucleic acids with small molecules for the treatment of diseases including cancer and bacterial/viral infections. Over the past two decades, nucleic acids have become popular targets for medicinal chemistry and docking. A review of the state-of-the-art methods for docking small molecules to nucleic acids is first presented in this thesis. Docking methods have been applied to discover new ligands for nucleic acids, for example riboswitches. Riboswitches are regulatory segments of mRNA molecules that bind specific metabolites and regulate the biosynthesis of the proteins encoded by the mRNA. We have built an RNA binding motif database that is fully automated and includes a detailed description of interactions with small ligands. Optimized structures of those RNA molecules for docking using FITTED, our own docking program, are available in the database, itself available on our group website. As the FITTED suite was initially designed for docking small molecules to proteins, we fully investigated the structural differences between proteins and nucleic acids and made several modifications to the ProCESS module in order to improve the accuracy of the prediction of potential interaction sites for ligands. 229 structures were tested with both previous and modified versions of FITTED. This study reveals that the modified FITTED suite is able to produce more reliable self-docking results.Les acides nucléiques jouent un rôle important dans de nombreux et variés processus cellulaires. Les avancées récentes dans la détermination des structures et fonctions d'ADN/ARN soulignent le potentiel de les cibler avec des petites molécules thérapeutiques afin de soigner certaines maladies comme le cancer ou des infections virales/bactériennes. Lors des deux dernières décennies, l'utilisation d'acides nucléiques en tant que cibles en chimie médicinale et en docking a reçu un intérêt croissant. Dans cette thèse, nous commencerons par examiner l'état de l'art des technologies employées pour le docking de petites molécules aux acides nucléiques. Les techniques de docking ont été utilisés pour découvrir de nouveaux ligands pour les acides nucléiques, par exemple: les riboswitches. Les riboswitches sont des segments d'ARNm qui régulent la biosynthèse des protéines encodées par l'ARNm lors d'interactions avec certains métabolites spécifiques. Nous avons établi une base de données de motifs de liaisons d'ARN; entièrement automatisée et incluant une description détaillée des interactions avec des petits ligands. Les structures optimisées de ces motifs d'ARN présentes dans la base de données peuvent ensuite être utilisées dans FITTED, le programme de docking de notre groupe, disponible sur notre site internet. Puisque la série de programme FITTED a initialement été développée pour le docking à des protéines, nous avons exploré les différences structurelles entre acides nucléiques et protéines et ajouté de nombreuses modifications au module ProCESS afin d'améliorer la prédiction de sites d'interactions potentiels. 229 structures ont ensuite été testés avec la version originale de et la version modifiée de FITTED. Virus-host interactions: Investigating the interferon response on Zika virus replication and Zika virus host dependencies on metabolism
The wide-spread outbreak of Zika virus (ZIKV) infection poses a significant threat to global health. A strong link between Zika virus and clinical birth defects, particularly microcephaly, warrants urgent study of the molecular mechanisms underlying ZIKV pathogenesis. Virus-host interactions determine the outcome of infection, thus we delved into the innate immune response against ZIKV infection and evaluated ZIKV dependency on host metabolism. Type 1 interferons (IFNs) are critical for host resistance against ZIKV. The first part of this study evaluates the effect of interferon and interferon-stimulated genes (ISGs) against ZIKV infection of Asian lineages (PLCal_ZV and PRVABC59). We have shown that type-1 IFN strongly inhibits ZIKV replication in human hepatoma Huh-7.5 cells and the ISG, IFITM3 contributes significantly to this inhibition, agreeing with published reports. We have demonstrated that cholesterol accumulation in endosomes does not restrict ZIKV replication, suggesting an alternative mechanism behind IFITM3 mediated ZIKV inhibition. To assess whether other ISGs contribute to restricting ZIKV infection, we tested a panel of ISGs. MxB, an interferon inducible myxovirus resistance protein, has been reported to restrict HIV-1 and herpesvirus infection. Interestingly, while both ZIKV Asian isolates are resistant to MxB, the African isolate MR766 is susceptible to MxB inhibition. We further identify IFI6 as a potent inhibitor of ZIKV replication for which the mechanism behind its antiviral activity remains to be elucidated. The second part of this study addresses the dependency of ZIKV on host cellular metabolism. We investigated the effect of two ZIKV proteins, NS4A and NS4B on mitochondrial respiration in U87-MG cells, a human primary glioblastoma cell line. Our results show that NS4B significantly decreases mitochondrial respiration, while basal respiration for NS4A expressing cells are comparable to the control. These findings expand on the role of specific ISGs in restricting ZIKV replication and highlight the need for investigating the underlying mechanisms behind viral restriction. Our results also support a model in which ZIKV infection causes an imbalance in mitochondrial homeostasis by decreasing mitochondrial respiration.L'éclosion répandue de l'infection au virus Zika (ZIKV) menace de façon significative la santé mondiale. L'établissement d'un lien important entre le virus Zika et certaines anomalies congénitales cliniques, et plus particulièrement la microcéphalie, presse l'étude des mécanismes moléculaires responsables de la pathogénicité du ZIKV. Les interactions entre l'hôte et le virus déterminent le cours de l'infection. Ainsi, nous avons étudié la réponse immunitaire innée au ZIKV, et évalué la dépendance du virus au métabolisme de l'hôte. Les interférons (IFNs) de type I sont cruciaux à la résistance de l'hôte contre le virus. La première partie de cette étude détermine l'effet des IFNs et des gènes stimulés par les IFNs (gènes appelés ISG pour Interferon-Stimulated Genes) contre l'infection au ZIKV issu de lignées asiatiques (PLCal_ZV et PRVABC59). Nous avons montré que les IFNs de type I empêchent la réplication du virus dans les cellules humaines hépatiques Huh-7.5, et que le gène IFITM3 (gène ISG) contribue significativement à inhiber le virus, ce qui concorde avec d'autres rapports déjà publiés. Nous avons également établi que l'accumulation de cholestérol dans les endosomes ne restreint pas la réplication du ZIKV, ce qui suggère un mécanisme alternatif à l'inhibition du virus induite par le gène IFITM3. Nous avons identifié la protéine IFI6 en tant qu'inhibiteur potentiel de la réplication du ZIKV; cependant le mécanisme de son activité antivirale demeure inconnu. La seconde partie de cette étudie aborde la dépendance du ZIKV au métabolisme de la cellule hôte. Nous avons examiné l'effet de NS4A et NS4B, deux protéines du virus, sur la respiration mitochondriale dans les cellules U87-MG, cellules humaines issues de glioblastome. Nos résultats montrent qu'alors que la respiration mitochondriale est significativement diminuée par NS4B, elle demeure inchangée dans les cellules exprimant NS4A comparativement à celle des cellules contrôle. Ces observations élargissent le rôle antiviral de certains ISGs dans la restriction de la réplication du virus Zika, et mettent en exergue le besoin d'en étudier les mécanismes. Nos données renforcent également un model selon lequel l'infection au ZIKV entraîne un déséquilibre dans l'homéostasie mitochondriale en diminuant la respiration mitochondriale
Ozonation for the improvement of wastewater quality in lagoons
Concerns over environmental sustainability in Canada have increased in recent years leading to changes to the discharge limits of certain contaminants like biological oxygen demand (25 mg/L), ammonia (1.25 mg NH3-N/L), and total suspended solids (25 mg/L) in effluent from wastewater treatment facilities collecting an average daily influent volume of 100 m3 or more. Contaminants of emerging concern (CECs) like pharmaceuticals, hormones, pesticides, herbicides, and other natural and synthetic compounds found in effluent from wastewater treatment facilities remain unregulated in Canada but may persist in the environment and lead to negative environmental outcomes. Given the number of Canadians who rely upon lagoons for their wastewater treatment in small, rural, and remote communities, there is a need to investigate and develop new, cost-effective strategies to improve the quality of wastewater treated in lagoons. Ozonation has been shown to improve the removal of a variety of wastewater contaminants (including CECs) from real wastewater through direct means (oxidation of compounds) and indirect means (increased dissolved oxygen, enhanced biodegradability) and was investigated in the present thesis as a potential strategy to improve wastewater treatment in lagoons. Two pilot tests were conducted to investigate the effects of ozonation on contaminant removal in lagoons. Samples of wastewater were collected from several locations in each lagoon prior to ozonation, during ozonation, and after ozonation and analyzed to determine the effect of ozonation on the removal of conventional contaminants and CECs. The samples were analyzed for biological oxygen demand (BOD), chemical oxygen demand (COD), total ammonia, unionized ammonia, total dissolved solids (TDS), total suspended solids (TSS), nitrite, nitrate, toxicity to Vibrio fischeri, and for the presence of fifteen CECs. For the first pilot test at New Credit First Nation, there were reductions in the BOD and total ammonia during the ozonation period. There was a decrease in toxicity to Vibrio fischeri and a further decrease in total ammonia during the post-ozonation period. The concentrations of three CECs (carbamazepine, gemfibrozil, and ibuprofen) decreased during the ozonation period, and the gemfibrozil concentration continued to decrease during the post-ozonation period. For the second pilot test at Rainy River First Nation, there were reductions in BOD, total ammonia, and unionized ammonia in the post-ozonation period. There was also a reduction in time from ice-off at the lagoon until compliance with the Wastewater System Effluent Regulations (WSERs) in 2018 compared to 2017 and 2016, but this time was still longer than in 2015 or 2014. Laboratory experiments with synthetic wastewater showed no improvements in removal of contaminants due to ozonation in a subsequent biodegradation process by Bacillus, Pseudomonas, and Rhodococcus species, and an increase in the formation nitrate at one of the dose tested. Direct removal of two indicator compounds (caffeine and sulfamethoxazole) by ozone was observed. The optimal dosing strategy for the removal of caffeine was different than for the removal of sulfamethoxazole. Overall, it is unclear if the pilot tests led to improvements in each lagoon and laboratory experiments with synthetic wastewater did provide evidence of an optimal ozone dosing strategy to improve the removal of contaminants. Monitoring at New Credit First Nation and Rainy River First Nation may provide further insight on the impact of ozonation on contaminant removal processes in lagoons. Further laboratory experiments using a more complex synthetic wastewater or real lagoon wastewater may also help determine the impact of ozonation on contaminant removal and the optimal dosing strategy to minimize costs associated with ozone production.Les préoccupations environnementales au Canada ont augmenté au cours des dernières années, ce qui a entraîné des changements aux limites de rejet concernant la demande biochimique en oxygène (25 mg/L), de l'ammoniac (1,25 mg NH3-N), et des matières en suspension (25 mg/L) pour les effluent des stations de traitement dont le volume moyen quotidien est de 100 m3 ou plus. Les contaminants d'intérêt émergent (CIE) présents dans les eaux usées ne sont pas réglementés au Canada, peuvent persister dans l'environnement et entraîner des effets sur l'environnement. Étant donné le nombre de Canadiens qui dépendent d'étangs pour le traitement de leurs eaux usées, surtout dans les petites communautés rurales et éloignées, il est nécessaire d'étudier et de développer de nouvelles stratégies peu couteuses pour améliorer la qualité des eaux usées traitées dans ces étangs. Il a été démontré que l'ozonation améliore l'élimination d'une variété de contaminants (y compris les CIEs) des eaux usées réelles par des moyens directs (oxydation des composés) et indirects (augmentation de l'oxygène dissous, biodégradabilité accrue) et a été étudiée dans la présente thèse comme une stratégie possible pour améliorer le traitement d'eau usée dans les étangs. Deux essais pilotes ont été réalisés pour étudier les effets de l'ozonation dans les étangs. Des échantillons d'eaux usées ont été prélevés à plusieurs endroits dans chaque étang avant, pendant et après l'ozonation et analysés pour déterminer l'effet de l'ozonation sur l'élimination des contaminants classiques et des CIE. Les échantillons ont été analysés pour la demande biochimique en oxygène (DBO), la demande chimique en oxygène (DCO), l'ammoniac total, l'ammoniac non ionisé, les matières dissous totales (MDT), les matières en suspension totales (MST), le nitrite, le nitrate, la toxicité envers Vibrio fischeri et la présence de quinze CIEs. Pour l'essai pilote à la Première nation de New Credit, des réductions de la DBO et de l'ammoniac total ont été observées pendant la période d'ozonation. La toxicité envers Vibrio fischeri et la concentration en ammoniac total ont diminué pendant la période suivant l'ozonation. Les concentrations de trois CIEs (carbamazépine, gemfibrozil et ibuprofène) ont diminué pendant la période d'ozonation, et la concentration de gemfibrozil a continué de diminuer pendant la période suivant l'ozonation. Pour l'essai pilote à la Première nation de Rainy River, des réductions de la DBO, de l'ammoniac total et de l'ammoniac non ionisé ont été observées au cours de la période suivant l'ozonation. Il y a eu une réduction du temps entre la fonte du couvert de glace de l'étang et la conformité aux limites imposées par le Règlement sur les effluents des systèmes d'assainissement des eaux usées en 2018 par rapport à 2017 et 2016, mais ce temps était plus long qu'en 2015 ou 2014. Des expériences de laboratoire avec des eaux usées synthétiques n'ont montré aucune amélioration de l'élimination des contaminants par l'ozonation dans un subséquent processus de biodégradation par trois espèces de bactérie Bacillus, Pseudomonas et Rhodococcus, mais une augmentation de formation du nitrate a été observée à une des doses étudiées. L'élimination directe de deux CIEs (caféine et sulfaméthoxazole) par l'ozone a été observée. La stratégie optimale pour l'élimination de la caféine était différente de celle du sulfaméthoxazole. L'absence d'essais de contrôle ne permet pas de conclure si les essais pilotes ont mené à des améliorations dans chaque étang et les expériences en laboratoire avec l'eau usée synthétique n'ont pas permis d'identifier une stratégie optimale de dosage de l'ozone pour améliorer l'élimination des contaminants. La surveillance aux Premières nations de New Credit et de Rainy River pourrait permettre de mieux comprendre l'effet de l'ozonation sur les processus d'élimination des contaminants dans les étangs des eaux usées en fournissant un point de comparaison des résultats
Defensive strike: International law and the use of force in self-defense against co-orbital antisatellite weapons
From the beginning of humankind's quest into outer space, military application and utilization has been at the forefront. Today, assets in space continue to provide militaries of spacefaring States with numerous advantages in areas such as communication, navigation and reconnaissance. While satellites that deliver these services provide benefits for terrestrial battles, they have remained relatively outside the crosshairs of a direct attack. Despite remaining free from an actual attack, the development and testing of antisatellite weapons (ASATs) has existed for decades. Primarily, the focus has been on direct-ascent ASATs, or ground to space missiles, capable of destroying an orbital satellite as it passes overhead. However, in recent years the development of a new type of ASAT has emerged. Co-orbital ASATs provide similar lethal results as direct-ascent ASATs, but with the ability to camouflage amongst other satellites until attacking quickly and simultaneously. This new threat poses questions for spacefaring States, among which are whether and to what extent a target State whose satellites are being followed by such ASATs, could legally strike first to save its space assets. This thesis explores such a scenario and analyzes the target State's ability to use force in self-defense under the current legal structure in outer space. The introduction to this thesis provides information on the issue itself, the objectives of the thesis, terminology to be used, as well as the confines and methodology in which the thesis will operate. Part I addresses the threat from co-orbital ASATs against a historical backdrop to contextualize the uniqueness of this emerging danger. Part II discusses the law that applies to the issue, first to the use of force in outer space and then to the specific space systems of co-orbital ASATs. Finally, Part III sets forth a legal justification under current international law that it is argued would permit a State to use force against co-orbital ASATs before its own space assets suffered harm.Depuis le commencement de l'exploration spatiale, l'usage et les applications militaires ont toujours occupés une place importante dans l'espace. Aujourd'hui, les technologies spatiales continuent d'avantager grandement les forces armées des puissances spatiales, tant au niveau des communications et de la navigation, qu'à celui de la reconnaissance. Malgré le fait que ces satellites fournissent des services essentiels lors de combats terrestres, ils ont rarement été la cible d'attaque directe. Néanmoins, même en l'absence d'attaque, le développement et les tests d'armes antisatellites (ASATs) existe depuis de nombreuses années. La technologie principale, les systèmes à ascension directe, généralement constitués de missiles balistiques, ont la capacité de détruire un satellite en orbite lorsque celui-ci passe dans un cadran sus-jacent au lanceur. Cependant, ces dernières années ont vues le développement d'un nouveau type d'ASAT : les armes coorbitales. Cette nouvelle technologie, aussi destructrice qu'un missile balistique, a le net avantage de pouvoir se camoufler parmi les autres satellites jusqu'au moment de l'attaque. Cette nouvelle menace soulève maintes questions pour les puissances spatiales, entre autre sur la possible légalité et l'étendue des obligations encourues par l'État ciblé lors d'une frappe préventive contre une telle arme. La présente recherche étudiera un tel scénario et analysera l'habileté de l'État ciblé à utiliser la force dans un contexte de légitime défense, sous le cadre légal actuellement applicable dans l'espace. L'introduction de cette thèse présentera les informations relatives à la problématique, l'objectif visé ainsi que la terminologie et la méthodologie formant la base de cette recherche. La première partie couvrira la menace des armes ASAT coorbitales, en contextualisant les caractéristiques de celles-ci face aux menaces historiques. Ensuite, la seconde partie analysera les lois et traités applicables, d'une part quant à l'utilisation de la force dans l'espace et d'autre part sur ceux spécifiques aux ASATs coorbitaux. Finalement, la troisième partie présentera une prémisse qui, selon le droit international actuel, permettraient à un État ciblé d'utiliser la force, de manière préventive, contre un ASAT coorbital
The Extent, Nature and Distribution of Child Poverty in India
Despite a long history, research on poverty has only relatively recently examined the issue of childpoverty as a distinct topic of concern. This article examines how child poverty and well-being arenow conceptualized, defined and measured, and presents a portrait of child poverty in India by socialand cultural groups, and by geographic area. In December 2006, the UN General Assembly adopted adefinition of child poverty which noted that children living in poverty were deprived of (among otherthings) nutrition, water and sanitation facilities, access to basic health care services, shelter and education. The definition noted that while poverty hurts every human being ‘it is most threatening and harmful to children, leaving them unable to enjoy their rights, to reach their full potential and to participateas full members of the society’.Researchers have developed age-specific and gender-sensitive indicators of deprivation whichconform to the UN definition of child poverty and which can be used to examine the extent and natureof child poverty in low and middle-income countries. These new methods have ‘transformed the wayUNICEF and many of its partners both understood and measured the poverty suffered by children’(UNICEF, 2009). Dolomite dissolution in aqueous solutions in the presence of nucleotides and their structural components at 25°C and pCO2 ~ 1 atm.
The kinetics and stoichiometry of dolomite dissolution were investigated in deionized water at 25 °C and pCO2 ~ 1 atm in the absence (control) and presence of various nucleotides and their structural components at low concentrations (1 mM). Dissolutions were conducted in “free-drift” mode and rates estimated based on the accumulation rate of calcium and magnesium ions in solution. The additives considered in this study are likely present at low concentrations (< mM) in deep, carbonate aquifers targeted for CO2 geological sequestration. Results of our control experiment are consistent with rates and stoichiometries reported in earlier studies. In the presence of nucleotides, dolomite dissolution was inhibited and proceeded incongruently, yielding a Mg:Ca solution ratio ≥ 2. An investigation of the nucleotide structural components (nucleosides, nitrogenous bases and phosphates) revealed that, whereas they nearly all inhibited dolomite dissolution, only phosphate salts (mono-, di-, tri-, and hexametaphosphate) lead to incongruent dissolution. Examination of the surface morphology and phosphate analysis of the solids reacted in the presence of nucleotides and phosphate salts revealed the formation of a solid phosphate phase on the dolomite surface, likely a Ca rich-phosphate phase that accounts for the observed dissolution incongruency
Perceptions of academic librarians toward LGBTQ information needs: An exploratory study
While previous studies have examined lesbian, gay, bisexual, transgender, and queer (LGBTQ) information needs, none have addressed librarian confidence in addressing LGBTQ-themed information needs, or the factors affecting this confidence. The authors used a mixed-methods survey to assess the knowledge and perspectives of academic librarians in responding to information inquiries related to sexual orientation and gender identity. Based on an exploratory factor analysis, three variables were identified: duty of care/vulnerability of inquirer, public visibility of work conducted, and personal biases and prejudices. These factors can reduce or otherwise influence the ability to meet LGBTQ information needs
The representation of archival descriptions: An ontological approach
Archival descriptions play multiple roles in understanding and managing records in archives. However, archival descriptions are not efficiently organized and presented on the Web. In particular, archival context is often lacking in current descriptions, which makes it difficult for users to search and access records. To address the challenges of current archival descriptions in web environments, the overarching question is asked: How can archival context be improved for searching, using and understanding in web environments? In addition, it is necessary to build a conceptual/operational model for representing and restructuring the archival context and provenance information of archival descriptions. Two research questions are formulated to address the two objectives of the study, which are 1) to identify and examine the essential components for archival context and their relationships in archival descriptions, and 2) to explore whether archival context can be explicitly represented and modeled using ontology technologies. To answer the research questions, a multi-method is taken in three studies: 1) Study 1 is a content analysis of archival descriptions; 2) Study 2 is a methontology to develop an event-based ontology; and 3) Study 3 is a focus group study to evaluate the usefulness of the built ontology. In Study 1, sample archival descriptions are drawn from 40 archival institution sites. By conducting the content analysis, the ten essential entities from archival descriptions have been identified as Agent, Artifact, Event, Feature, Function, Place, Relation, Role, Situation, and Time. In Study 2, the results of Study 1 were used to build the ontology model. By taking a methontology approach, five stages are performed: to identify ontology requirements at the specification stage, to identify relationships and concepts at the conceptualization stage, to develop the ontology at the implementation stage, to align with other ontologies based on the Basic Formal Ontology at the formalization stage, and to validate the ontology at the evaluation stage. As a result, an Event Ontology for Archival Context (EOAC) is developed with 233 classes and 87 properties, which were explicitly formalized in OWL (Web Ontology Language) for implementation. In Study 3, two focus group sessions with users and professional archivists were conducted to elicit both users’ and professional opinions on the current status of archival descriptions and the ontology model. Participants explained the advantages and disadvantages of the current descriptions and the ontology model and ten out of eleven participants were willing to adopt the event ontology model if supporting tools were available.The findings of the three studies led to the development of a comprehensive conceptual model for archival context. The model consists of three levels: (1) the micro-level with static and fluid views; (2) the meso-level with three separate contexts (i.e., creation, description, and usage contexts); and (3) the macro-level with a series of activity systems. By integrating all three levels, the EOAC provides an explicit and comprehensive way to transform archival context into more structured, connected and interactive forms of entities. This research adopts an innovative event-centred approach and emphasizes that events are as important as objects. The three-level conceptual model for archival context provides a framework to deepen the understanding of archival context. The EOAC ontology provides a new way to transform archival contexts into Linked Open Data, which can be utilized to improve searches and retrievals of archival descriptions and meet the expectations of users.Les objectifs de cette étude sont donc les suivants : 1) identifier et examiner les composantes essentielles du contexte archivistique ainsi que leur relation aux descriptions archivistiques et 2) définir si les technologies de l’ontologie peuvent être utilisées pour représenter le contexte archivistique de manière plus explicite. Deux questions de recherche sont alors explorées : 1) quelles sont les composantes essentielles du contexte archivistique et 2) dans quelle mesure les technologies de l’ontologie peuvent-elles être utilisées afin de représenter le contexte archivistique?Afin de répondre aux questions de recherche, une approche méthodologique mixte servira lors de trois études : 1) la première étude est une analyse de contenu des descriptions archivistiques, 2) la deuxième étude utilise la méthontologie comme méthodologie pour développer une ontologie basée sur les événements et 3) la troisième étude évalue l’utilité de l’ontologie développée par le moyen de groupes de discussion. Dans la première étude, des échantillons de descriptions archivistiques sont recueillis sur les sites de quarante institutions. En effectuant l’analyse de contenu, dix composantes ont pu être identifiées comme étant essentielles aux descriptions archivistiques : exécutant, artefact, événement, caractéristique, fonction, lieu, relation, rôle, situation et temps. Dans la deuxième étude, les résultats obtenus lors de la première étude sont utilisés en suivant la méthontologie pour construire le modèle ontologique. Le modèle se développe en cinq étapes : identification des besoins de l’ontologie à l’état de spécifications, identification des relations et des concepts à l’état de conceptualisation, développement de l’ontologie à l’étape d’implémentation, alignement avec d’autres ontologies basées sur le Basic Formal Ontology à l’état d’élaboration et, finalement, validation de l’ontologie à l’étape d’évaluation. Le résultat est le développement d’une ontologie basée sur les événements, nommée Event Ontology for Archival Context (EOAC), et contenant 233 classes et 87 propriétés, formalisées explicitement dans le language OWL (Web Ontology Language) pour l’implémentation. Dans la troisième étude, deux groupes de discussion ont été dirigés auprès d’utilisateurs et d’archivistes professionnels afin de connaître les opinions des utilisateurs et des professionnels sur les descriptions archivistiques actuelles ainsi que sur le nouveau modèle ontologique. Les participants ont pu identifier les avantages et les désavantages des deux méthodes et dix participants sur onze étaient prêts à utiliser la nouvelle ontologie développée si les outils nécessaires étaient mis à leur disposition.Les conclusions de ces trois études permettent donc le développement d’un modèle conceptuel complet du contexte archivistique à trois niveaux : (1) le micro-niveau, comportant des vues statiques et fluides, (2) le méso-niveau, comportant trois contextes distincts (création, description et contexte d’usage), et (3) le macro-niveau, comportant plusieurs systèmes d’activités. En intégrant les trois niveaux, le modèle EOAC permet de transformer de manière explicite et compréhensive le contexte archivistique en une forme plus structurée, connectée et interactive.L’approche centrée sur les événements utilisée dans cette recherche est innovatrice et met l’accent sur le fait que les événements sont tout aussi importants que les objets. The Hepatitis C treatment revolution: Are key HIV-Hepatitis C Co-infected populations being left behind?
The emergence of direct-acting antivirals (DAAs), for the treatment of hepatitis C virus (HCV), marked one of the most significant advances in modern therapeutics. Unlike previous generations of therapies, DAAs are well-tolerated and cure >90% of chronically infected individuals. As such, in 2016, the World Health Organization defined targets to scale up screening, access to treatment and harm reduction to eliminate HCV as a public health threat by 2030. Nonetheless increasing HCV treatment initiation rates, particularly among marginalized populations, remains a significant public health challenge. This thesis addressed fundamental issues regarding the identification and quantification of barriers to DAA treatment initiation, in addition to assessing the real-world impact of treatment on individuals in Canada who are co-infected with HIV-HCV. To this end, I used data from the Canadian HIV-HCV Co-Infection Cohort (CCC), one of the largest prospective cohorts in the world. Manuscript #1 examined the generalizability of the clinical trials used to license DAAs. Here I found only a minority of CCC participants (6-43%) would have been eligible for enrolment into these trials. The majority of the exclusions appeared to be related to improving treatment outcomes by not including those at higher risk of poor adherence. This highlighted the need to evaluate the real-world impact of DAAs on access to treatment and health outcomes. Manuscript #2 evaluated DAA treatment uptake by key populations and their subsequent treatment response in a real-world setting. HCV treatment rates increased by more than three times after the introduction of DAAs (8 initiations to 28 per 100-person-years). But, using a multivariate Cox proportional hazards model, I found people who inject drugs (PWID) and more generally, people with lower income were less likely to initiate treatment. Reflective of reimbursement restrictions, people with significant liver fibrosis were more likely to initiate treatment. As the price of DAAs were reduced and reimbursement restrictions were broadened. Manuscript #3 evaluated the impact of removing fibrosis stage restrictions on HCV treatment initiation. I applied a difference-in-differences approach using a negative binomial regression with generalized estimating equations to assess the impact of the policy change. Removing fibrosis stage restrictions, increased treatment uptake by 1.8 times (95% CI, 1.4, 2.5) accounting for temporal trends and the time-invariant difference between provinces. Among PWID, the impact appeared even stronger; adjusted incidence rate ratio (aIRR), 3.6 (95% CI 1.8, 7.4). Four years after the advent of DAAs, marginalized participants (PWID and those of Indigenous ethnicity) and those disengaged from care, remained more likely to require treatment. Manuscript #4 investigated the real-world impact of successful DAA treatment on health-related quality of life (HR-QoL) using a segmented multivariate linear mixed model. In contrast to clinical trial results, we observed only modest improvements in HR-QoL following a sustained virologic response with DAA therapy. In addition to these substantive objectives, this dissertation also contributes to the advancement of epidemiological methods by including two published tutorials detailing the methods used to answer the research questions for the third and fourth manuscripts, the difference-in-differences approach and segmented mixed effect models, respectively. This is an unprecedented time in clinical medicine. DAAs have transformed clinical practise by curing a chronic infection in the vast majority of patients in less than 12 weeks, but challenges remain. This work describes and quantifies barriers to HCV treatment uptake that can inform HCV elimination efforts currently underway worldwide.Le traitement de l'hépatite C a été révolutionné par le développement des antiviraux à action directe (AAD). Contrairement aux traitements antérieurs, les AAD sont bien tolérés et permettent de guérir plus de 90% des personnes infectées. À la lumière des AAD et du coût clinique et économique de l'inaction, l'Organisation mondiale de la santé a défini des objectifs pour renforcer le dépistage, l'accès au traitement et la réduction des risques de transmission afin d'éliminer l'hépatite C comme menace pour la santé publique d’ici 2030. Cependant, à ce jour, l'augmentation des taux d'initiation du traitement contre l'hépatite C, en particulier chez les populations marginalisées, reste un défi majeur pour la santé publique. Les objectifs principaux de cette thèse étaient d'évaluer l'accès aux traitements par AAD et aux résultats de ces traitements chez les personnes co-infectées par le VIH et l'hépatite C au Canada. À cette fin, les données de la cohorte canadienne de co-infection VIH- hépatite C (CCC) ont été utilisées. Le premier manuscrit a examiné la possibilité de généraliser les résultats des essais cliniques utilisés pour obtenir l’approbation des AAD à la population Canadienne. Nous avons constaté que seulement une minorité de participants CCC (6-43% selon les essais) aurait été éligible pour participer à ces essais. Les résultats de cette étude ont mis en évidence la nécessité d'évaluer l'impact réel de l’introduction des AAD sur l’initiation d’un traitement de l’hépatite C et sur le système de la santé. Le deuxième manuscrit a évalué le taux de traitement par les AAD et la réponse au traitement dans un contexte réel dans les populations clés. Les modèles multivariés de Cox ont été utilisés et après ajustement, les personnes qui s’injectent des drogues et plus généralement les personnes à faible revenu étaient moins susceptibles de commencer un traitement. Reflétant les restrictions de remboursement, les personnes atteintes d'une fibrose hépatique significative étaient plus susceptibles de commencer un traitement. Le troisième manuscrit a évalué de l’élimination des restrictions de remboursement en fonction du stade de la fibrose l'impact sur l’initiation du traitement de l'hépatite C. Une analyse de « Différence dans les Différences » a été utilisée avec une régression binomiale négative et des équations d'estimation généralisées pour déterminer l'impact du changement de politique. L'élimination des restrictions d’indication thérapeutique et l'augmentation de l’initiation du traitement de 1,8 fois (IC à 95%, 1,4, 2,5), expliquent les tendances temporelles et la différence invariante dans le temps entre les provinces. Parmi les utilisateurs de drogues injectables, l’impact était encore plus fort de 3,6 fois (IC à 95%, 1,8, 7,4). Le manuscrit final de cette thèse a étudié l'impact réel d'un traitement par AAD sur la qualité de vie liée à la santé (HR-QoL) en utilisant un modèle mixte multivarié linéaire segmenté pour évaluer son évolution dans le temps. Contrairement aux résultats des essais cliniques, nous avons observé que de modestes améliorations de la HR-QoL après la réponse virologique soutenue avec le traitement aux AAD. En plus de ces objectifs de fond, cette thèse contribue également à l'avancement des méthodes épidémiologiques en incluant deux tutoriels publiés détaillants les méthodes utilisées pour répondre aux questions de recherche du troisième et quatrième manuscrit. L'analyse de « Différence dans les Différences » et les modèles à effets mixtes segmentés respectivement. Nous sommes témoins d’une période sans précédent en médecine clinique. Les AAD ont transformé la pratique clinique en guérissant une infection chronique chez la grande majorité des patients en moins de 12 semaines. Toutefois, des problèmes subsistent. Ce travail décrit et quantifie les obstacles à l’utilisation du traitement du VHC qui peuvent informer et aider les efforts d’élimination du VHC dans le monde entier
