Short-Term Administration of Parathyroid Hormone Improves Wound Healing Around Implants in an Osteoporotic Rat Model.
Background/Objectives: Intermittent parathyroid hormone (PTH) administration increases bone quantity. Existing animal studies have revealed improvements in tissue healing around implants after PTH administration. It is still unclear whether PTH has a beneficial short-term effect on the early healing of bone and soft tissue around implants in individuals with osteoporosis. The current study aims to examine whether short-term intermittent PTH administration accelerates and improves early tissue healing around implants in ovariectomized rats.
Methods: Ovariectomized rats received implants at the healed sites of extracted maxillary first molar sockets 12 weeks after the ovariectomy surgery. A daily dose of PTH was subcutaneously administered in the test group, whereas saline was administered for the control group. Long bones and maxillae were harvested 1 week after PTH administration. The following criteria were assessed: quantity and quality of long bones and peri-implant bone, bone healing around the implants, and soft tissue healing.
Results: PTH significantly increased the bone parameters of long bones. Moreover, the bone volume around the implant increased significantly compared to controls. Improved bone quality was indicated through PTH administration by increased numbers of osteoblasts and osteoclasts, as well as decreased quantities of sclerostin + osteocytes. Furthermore, PTH administration significantly improved soft tissue healing, promoted collagen production and angiogenesis, and increased the numbers of macrophages in the connective tissue around the implants.
Conclusions: Short-term intermittent PTH administration significantly accelerates soft tissue healing, which could lead to enhanced early osseous healing and bone formation around implants. Thus, Intermittent PTH administration might be considered as an available treatment modality for dental implants in osteoporosis patients
Narrative of a mother whose child suffered severe disabilities as a result of a school lunch aspiration choking accident at a special needs school. : Focusing on Changes in Trust in Schools and Educational Administration
本稿は、特別支援学校において、給食介助中の誤嚥窒息事故によりわが子が重度の障害を負った母親の経験の語りに着目し、事故の経験の背景にある母親の思いのありようとその変化を考察することを目的とした。インタビューから、それまで信頼を寄せていた学校や行政に対し、ある時点から訴訟を起こすほどの関係になってしまいながらも、再度学校や行政への信頼を希求する思いの変化が明らかになった。The purpose of this paper is to examine the background of the mother’s feelings and their changes in the experience of the accident, focusing on the narrative of a mother whose child suffered severe disabilities due to an accident involving aspiration and choking while being assisted during school lunch at a special needs school. The interviews revealed a change in the mother’s desire to regain trust in the school and educational administration, even though at one point the relationship had become so strained that a lawsuit was filed against the school and educational administration, in which she had previously placed her trust
Examining Support for Enrollment and Schooling of Children and Youth Experiencing Alternative Care in Hokkaido : Insights from the Fostering Higher Education Initiative in the U.S.
本稿は北海道内で暮らす社会的養護経験者の高等教育への進学や就学維持を支援について検討する研究会の活動報告である。北海道内で社会的養護経験者のリービングケア,アフターケアに関わる支援者などが参加するステーク・ホルダー・ミーティング(SHM)に加えて,支援を社会実装することを意識し,中心的な関係機関の支援者や社会的養護経験者が参加するコア・チーム・ミーティング(CTM)が開催された。2 つの研修会では北海道内の社会的養護経験者の進学や就学維持に関する現状と課題が報告された他,米国で実践が重ねられてきたFostering Higher Education の取り組みについて開発者であるSalazar 氏から報告が行われた。本稿ではSHM で報告された内容とCTM での議論を紹介する。This paper presents an activity report of the study group examining support for enrollment and schooling of children and youth experiencing alternative care in Hokkaido. In addition to the Stakeholder Meeting (SHM) attended by supporters involved in leaving-care and after-care, the Core Team Meeting (CTM) was held with key institutional supporters and children and youth experiencing alternative care, focusing on the social implementation of support measures. At these two sessions, reports were presented on the current status and challenges related to support for enrollment and schooling for children and youth experiencing alternative care in Hokkaido. Furthermore, Dr. Salazar, the developer of the Fostering Higher Education initiative, provided insights into its ongoing implementation in the United States. This paper presents the findings from the SHM and the discussions held at the CTM
Rational design of dual Chemo- and Thermo-responsive phase separation of Poly(4-hydroxystyrene) in non-aqueous media
Dual stimuli-responsiveness in non-aqueous media could allow various molecular designs using a system that is not compatible with aqueous media. However, the difficulty in controlling the desolvation of the polymer chains in non-aqueous media for lower critical solution temperature (LCST)-type phase separation at ambient temperature has hindered the attainment of dual stimuli-responsiveness based on chemical stimuli. In this study, we rationally designed dual chemo- and thermo-responsive phase separations of poly(4-hydroxystyrene) in non-aqueous media. The LCST-type thermo-responsiveness of poly(4-hydroxystyrene) in 1,4-dioxane/toluene was considerably altered by the addition of small amounts of hydrogen-bonding molecules as chemical stimuli. Secondary amines or molecules containing two hydrogen-bonding functional groups formed stronger hydrogen bonds with the polymer chains than 1,4-dioxane, which altered the solvation state and induced the UCST-type thermo-responsiveness or insolubility of PHS. This dual stimuli-responsive system could serve as a chemical sensor to detect the presence of acids. Dual chemo- and thermo-responsive phase separations of poly(4-hydroxystyrene) in non-aqueous media were demonstrated. The addition of small amounts of hydrogen-bonding molecules as chemical stimuli caused a considerable change of LCST-type thermo-responsiveness of poly(4-hydroxystyrene) in 1,4-dioxane/toluene. Secondary amines or molecules containing two hydrogen-bonding functional groups could cross-link the polymer chains through the hydrogen bonds, which altered the solvation state and induced the UCST-type thermo-responsiveness or insolubility of the polymer. The impact of preoperative psychiatric intervention for postoperative delirium after major oral and maxillofacial surgery with free flap reconstruction
Background: Postoperative delirium (POD) is a severe complication associated with various adverse outcomes, especially in older patients. Although the incidence and risk factors for POD have been explored in general surgery, they have not been fully elucidated. Early identification of high-risk patients and active preoperative intervention are considered essential for the prevention of POD. Recently, psychiatric consultation intervention have been shown to prevent delirium. This study investigated the effect of preoperative psychiatric interventions on preventing POD in our specific surgical context. Materials and Methods: This retrospective, single-center observational study included 86 patients who underwent major oral and maxillofacial surgery with free flap reconstruction between 2016 and 2023. The effect of psychiatric intervention were compared between patients with and without delirium. Results: Preoperative psychiatric intervention did not reduce the incidence of POD. The incidence of POD was 29.1 %. Univariate analyses showed no significant associations between POD and any clinical variables. Conclusion: There was no difference in the incidence of POD between patients who received preoperative psychiatric intervention and those who did not, and further investigation is needed to determine the efficacy of preoperative psychiatric intervention in the prevention of POD.
