Myoepithelioma of the parotid gland: Case report and review of literature
SummaryThe present paper describes the clinical and pathologic features of a benign myoepithelioma of the parotid gland. Through 1985, only 42 other cases had been reported in the literature: three malignant and 39 benign. Fewer than 100 cases had been reported through 1993. The number of case reports of myoepitheliomas is increasing as pathologists have become more aware of their existence. This uncommon tumor is most commonly diagnosed in the parotid gland and in the minor salivary glands of the palate. The complex and varied morphologic expression of neoplastic myoepithelium have attracted numerous investigators who have presented valuable but often contradictory data. It is our opinion that these tumors are not as rare as is generally believed, but they are simply not well recognized. Together with a review of the literature, consideration are presented on the clinical evaluation, differential diagnosis and treatment of these lesions
[Clinico-therapeutic considerations in a serious case of amelogenesis imperfecta]
: The Authors present a case of a young female patient affected with a serious hypoplasic form of Amelogenesis Imperfecta, involving all the teeth which has been completely restabilished using composite resin. The therapy used has been developed thanks to the chemical-physical qualities of last generation of composites, which allow the compete reconstruction of teeth. Such a therapy has not to be considered definitive, however it has been used as a temporary solution, waiting for final prosthetic rehabilitation. The Authors estimate that such a temporary long term treatment, that utilize composite resins, represent the ideal therapeutical choice thanks to its characteristics of endurance, cost and benefice. In our opinion this therapy prevents tooth deterioration, secures a better mastication, a partial reduction of the teeth sensibility, facilitates teeth cleaning and solves the esthetical problems with great psychological advantage
A simplified post preparation techinque after Thermafil obturation: evaluation of apical microleakage and presence of voids using methylene blue dye penetration
AIM: To evaluate the apical microleakage of Thermafil obturations after three different post space preparation techniques.
MATERIALS AND METHODS: 33 root canals of mono-radiculated extracted teeth were prepared with M two and then obturated with Thermafil. Teeth so treated were then divided into three groups and the post space to middle root was prepared using three different techniques. In samples in group A the housing for the post was created using a Torpan bur, and the carrier was partially removed only in the coronal portion. In samples in group B the carrier was completely removed and gutta-percha was hand compacted, before canal preparation using a Torpan bur. In samples in group C the carrier was completely removed, without guttapercha compaction, before canal preparation using a Torpan bur. The roots were immersed for 72 hours in methylene blue dye solution and sectioned transversely at 1-3-5 mm from the apex for evaluation of dye penetration using a stereomicroscope. The data collected were processed using Win CAD software and subjected to statistical analysis using the Student t test for p<0.05.
RESULTS: There were no significant differences between the three groups, except for the presence of voids in the intermediate section of teeth in groups B and C.
CONCLUSIONS: Post space preparation did not influence the apical seal, and gutta-percha without voids was always found in the last millimetre of the canal obturation. This study proposes a post preparation technique which provides for complete carrier removal using pliers, hand compaction of residual gutta-percha with a manual plugger and enlargement of the root canal, using appropriate post space burs, free of any interference from the carrier. Operating time is reduced, as is the risk of creating ledges or iatrogenic perforations
Whitening: is Omnichroma universal composite unchanging? Spectrophotometric evaluation
Background: This study aimed to evaluate the color stability of Class V anterior restorations with universal composite after professional bleaching using a spectrophotometer. Material and Methods: Class V cavities were prepared and restored with universal composite in twenty-eight extracted anterior teeth. One week after restoration, color analysis was performed using the spectrophotometer. In-office bleaching was performed. Color analysis was performed 24 hours, 72 hours and 30 days after bleaching. The parameters evaluated were L* values of each tooth and ΔE values between tooth and restoration at different timepoints. Data were analyzed using a statistical software. A P-value ≤0.05 was used to indicate statistical significance of the results. Results: There was a non-statistically significant but noticeable difference between the L* values in term of tooth bleaching effect. Non-statistically significant differences were found between the L* values and the ΔE values at different timepoints when evaluating the chromatic difference between the tooth surface and the restoration. The ΔE value one week after the restoration is higher than the ΔE values at the following timepoints, therefore the color difference between the restoration and the adjacent tooth decreases with time after bleaching. Conclusions: Universal Composite was found to be able to match the color of the surrounding tooth even after the bleaching procedur

Telediagnosis of dental caries: possible or impossible? A pilot cross-sectional study
Objectives: The aims of this study were to evaluate the effectiveness of teledentistry (based on a home intraoral imaging protocol) in detecting dental caries and to assess the accuracy of this method compared to clinical examination. Methods: Forty-three patients were recruited for the study. Using a protocol for taking intraoral photographs at home with a smartphone proposed by the Dental School of Verona, a remote diagnosis of dental caries (TD) was performed by an experienced dentist. The same caries sites were also assessed by clinical diagnosis (CD) by a second experienced dentist. Ten photos were taken at home in five different perspectives, with and without flash, and emailed to one of the authors. The best five photos were selected for telediagnosis. The International Caries Detection and Assessment System (ICDAS II) score was used for caries diagnosis. Statistical tests were performed: Sensitivity and specificity of TD, the positive and negative predictive value of TD (PPV-NPV), and Spearman correlation to evaluate the relationship between the scores of TD and CD. Results: A total of 430 photographs were submitted; TD was performed on 215 photographs and 43 patients were visited. A total of 1201 teeth were analyzed. The sensitivity of TD was 74.0, the specificity was 99.1, the PPV of TD was 91.7, and the NPV was 96.4. The Spearman correlation was 0.816, showing a very strong correlation between the values obtained with TD and CD. Conclusions: The study showed good potential for TD, which proved to be a feasible method to combine with routine caries diagnosis in daily preventive dentistry practice
