    Effect of intraoperative neuromonitoring on recurrent laryngeal nerve palsy rates after thyroid surgery—A meta-analysis

    Background/PurposeThough intraoperative nerve monitoring (IONM) during thyroid surgery has gained universal acceptance for localizing and identifying the recurrent laryngeal nerve (RLN), its role in reducing the rate of RLN injury remains controversial. In order to assess the effect of IONM during thyroid surgery, its value in reducing the incidence of RLN palsy was systematically evaluated.MethodsStudies were evaluated for inclusion in this analysis by researching PubMed, Embase, the Cochrane Central Register of Controlled Trials, and the references of included studies. The initial screening of article titles and abstracts was independently performed by five reviewers based on the research protocol criteria. Each article was then read in detail and discussed before inclusion in the meta-analysis. Data were independently extracted, including the level of evidence, number of at-risk nerves, allocation method, baseline equivalence between groups, definitions of transient and permanent vocal fold palsy, systematic application of electrodes, etc. The meta-analysis was then performed. Odds ratios were pooled using a random effects model.ResultsFive randomized clinical trials and 12 comparative trials evaluating 36,487 at-risk nerves were included. Statistically significant differences in terms of total recurrent laryngeal nerve palsy (3.37% with intraoperative nerve monitoring [IONM] vs. 3.76% without IONM [OR: 0.74; 95% confidence interval [CI]: 0.59–0.92]) and transient recurrent laryngeal nerve palsy (2.56% with IONM vs. 2.71% without IONM [OR: 0.80; 95% CI: 0.65–0.99]) were identified. The persistent incidence of recurrent laryngeal nerve palsy was 0.78% for IONM versus 0.96% for nerve identification alone (OR: 0.80; 95% CI: 0.62–1.03).ConclusionBased on this meta-analysis, statistically significant differences were determined in terms of the incidences of total and transient recurrent laryngeal nerve palsy after using IONM versus recurrent laryngeal nerve identification alone during thyroidectomy. However, no statistically significant differences were identified regarding the incidence of persistent recurrent laryngeal nerve palsy between groups

    This paper demonstrates that urban social exclusion in China does not only include restricted participation by the ¿underclass¿ in urban life, but also the deprivation of certain political, social and economic rights. In addition, the paper describes how the character of urban social exclusion has changed over time. The author also examines the social exclusion of rural workers living and working in urban areas. The paper concludes by arguing that urban social exclusion in China needs coordinated reforms that target the whole set of problems in the urban ¿underclass¿ lacking political rights, social protection and economic opportunities.social exclusion, urban China, rural to urban migrants

    This thesis focuses on introducing the development of, and changes in, the Chinese legal profession over this past twenty years, analysing the reasons that led to these changes, and considering both theoretical and practical problems. The Chinese legal profession has developed considerably since 1978 when the reform and opening up movement was begun. Major changes have taken place in the lawyer’s social status, business, work institutions and in the supervision system. Compared with the end of the 1970s, the legal profession nowadays is much more commercialised and professional. The changes have a symbiotic relationship with the changes in Chinese society: economic and ideological changes have transformed the role of lawyers and the law, which have in turn led to changes in society. The transformation of the Chinese society is still going on, and corresponding future changes are expected in the legal profession

    Deep Learning for Breast MRI Style Transfer with Limited Training Data

    In this work we introduce a novel medical image style transfer method, StyleMapper, that can transfer medical scans to an unseen style with access to limited training data. This is made possible by training our model on unlimited possibilities of simulated random medical imaging styles on the training set, making our work more computationally efficient when compared with other style transfer methods. Moreover, our method enables arbitrary style transfer: transferring images to styles unseen in training. This is useful for medical imaging, where images are acquired using different protocols and different scanner models, resulting in a variety of styles that data may need to be transferred between. Methods: Our model disentangles image content from style and can modify an image's style by simply replacing the style encoding with one extracted from a single image of the target style, with no additional optimization required. This also allows the model to distinguish between different styles of images, including among those that were unseen in training. We propose a formal description of the proposed model. Results: Experimental results on breast magnetic resonance images indicate the effectiveness of our method for style transfer. Conclusion: Our style transfer method allows for the alignment of medical images taken with different scanners into a single unified style dataset, allowing for the training of other downstream tasks on such a dataset for tasks such as classification, object detection and others.Comment: preprint version, accepted in the Journal of Digital Imaging (JDIM). 16 pages (+ author names + references + supplementary), 6 figure

    Zai zhongyin yu shengdiao de lianxuti shang kan Shixingyu (Between stress and tone: Tones in Shixing and other Qiangic languages)

    International audienceShixing, a little-studied language of Western Sichuan, is currently classified within the Qiangic subgroup of Tibeto-Burman languages. Previous studies (Sun 1983, Huang and Renzeng 1991) describe this language as tonal and claim it possesses these two characteristics: (1) among its four tones (55, 53, 35 and 33), only the former three contrast on monosyllables, whereas the fourth appears only in polysyllabic words; (2) its tones are not stable and often change in natural speech flow. The author's fieldwork in 2005 and 2006 confirms these two observations and uncovers a prosodic system with distinctive pitch (here analyzed as word-tone). This system is in many respects similar to those described for other Qiangic languages, such as Puxi Lavrung, Caodeng rGyal-rong and Shuitang Qiang, interpreted by Sun (2005) and Evans (2006) as combining prototypical properties of tone and accent. An examination of previous descriptions of tonal systems of other Qiangic languages shows that the same analysis in terms of tone and accent applies to the entire Qiangic subgroup. This combination of tonal and accentual features is treated by the author as an areal feature of Western Sichuan.属藏缅语族羌语支南支的史兴语(又称旭米藏语)分布在四川省凉山州木里县水洛乡部分村寨,使用人口大约2000人。 史兴语是有声调的语言,据孙宏开(1983)与黄布凡和仁增旺姆(1991)的介绍,其声调具备如下两个特点: 1. 史兴语四个声调(高平:55、高降:53、中升:35和中平:33)当中,只有前三者才呈现声调对立,而最后一个只出现在双音节和多音节词中; 2. 史兴语声调往往受连读变调和语气影响而有变化,在句中不大稳定。 在2005和2006年的田野调查资料的基础上,本文重点分析史兴语声调系统这些特点,讨论史兴语声调的一些变调规则,并提出史兴语声调负载单位不是音节,而据初步分析认定应该是词。 作者注意到,史兴语韵律系统,类型十分类似于蒲西拉乌绒语、草登嘉戎语(孙天心2005)和水塘羌语(Evans 2006),可看做为结合声调与重音的典范特征。把这一分析框架扩展到整个羌语支其他的语言,作者提出,羌语支的语言都不以音节作为声调负载单位,一律可分析为属于声调与重音过渡阶段。作者认为,该特点有可能是超羌语支语言境界之外的川西地区的区域特征
