Efficacy and safety of antithrombotic therapy with non-vitamin K antagonist oral anticoagulants after transcatheter aortic valve replacement: a systematic review and meta-analysis
Objective: A meta-analysis was performed to compare the efficacy and safety of antithrombotic therapy with non-vitamin K antagonist oral anticoagulants (NOACs) versus standard care in patients after successful transcatheter aortic valve replacement (TAVR). Methods: A systematic search of PubMed, Cochrane Central Register of Controlled Trials, and EMBASE databases and ClinicalTrials.gov website (through 21 October 2020) was performed. Risk ratios (RRs) with 95% confidence intervals (CIs) for all outcomes were calculated using random-effects models. Results: Twelve studies (two studies were randomized controlled trials) comprising 6943 patients were included (5299 had indications for oral anticoagulation (OAC) and 1644 had none). No significant differences were found between NOACs and the standard care in the incidences of all stroke, a composite endpoint, and major/life-threatening bleeding. NOACs were associated with lower all-cause mortality than vitamin K antagonists (VKAs) in post-TAVR patients with indications for OAC after more than 1 year of follow-up [RR = 0.64; 95% CI, (0.42, 0.96); p = 0.03], whereas NOACs exhibited poor outcomes than antiplatelet therapy (APT) in patients without indications for OAC [RR = 1.66; 95% CI, (1.12, 2.45); p = 0.01]. In the prevention of valve thrombosis, NOACs and VKAs were not significantly different in patients with indications for OAC [RR = 0.66; 95% CI, (0.24, 1.84); p = 0.43], whereas NOACs were better than APT in patients without indications for OAC [RR = 0.19; 95% CI, (0.04, 0.83); p = 0.03]. Conclusions: In patients with indications for OAC, post-TAVR antithrombotic therapy with NOACs was more favorable due to its lower all-cause mortality after more than 1 year of follow-up. In those without indications for OAC, NOACs presented poorer outcomes due to its higher all-cause mortality

DESIGN, IMPLEMENTATION AND ASSESSMENT OF NANOGRID BUILDING BLOCKS FOR ENERGY SECURE ELECTRICAL DISTRIBUTION SYSTEMS
This work presents zonal nanogrid building blocks (ZNBB) to enable a modular, scalable, cost-effective, and resilient solution to create \textit{ energy-secure} electrical power systems (EPS). The concept of ZNBB adapts zonal electrical distribution systems (ZEDS) from electrified shipboard power systems. ZNBBs can operate as separate islands within larger systems or can share power/energy if an electrical failure occurs within a ZNBB. This additional level of survivability is enabled through interzonal power sharing enabled by a novel distributed nanogrid controller for ZNBBs. The distributed controller used a pre-programmed state machine-driven events to coordinate power conversion and power distribution equipment to execute power routing when a zone experiences a loss of genset failure. The case study sets up the ZNBB in a three-zone system connected through an MVac ring bus. The distributed controller is validated through real-time controller hardware-in-the-loop (CHiL) simulation where Typhoon-HiL emulated the three-zone system and the distributed controllers are implemented in a National Instruments (NI) real-time controller. OPC UA (open platform communication unified architecture) is used for communication between the NI and Typhoon-HiL. Detailed simulation results are presented of the system transitions, synchronization events, switchgear and control actuation, and coordination during the loss of genset scenario. In addition, this thesis develops an integration framework to help bridge the gap between system-level metrics at the stakeholder level with low-level metrics at the equipment/subsystem through the adaptation of INCOSE (International Council on Systems Engineering) technical measures. This integration framework serves as a meta-level framework to help design and assess microgrid-based solutions for energy security. A microgrid protection design process is also introduced and validated through CHiL simulation based on a case study of a microgrid with parallel feeders. This protection design process provides a systematic process for designing and testing protection settings of power distribution equipment and ride-through setting for power conversion equipment. This process can be applied to the three-zone system and can be accessed through the integration framework. Lastly, this thesis provides simulation results of a digital twin of a BESS within a ZNBB to improve recovery under an electrical fault and loss of communications scenario

Axon specification is a critical step in neuronal development, and the function of glial cells in this process is not fully understood. Here, we show that C. elegans GLR glial cells regulate axon specification of their nearby GABAergic RME neurons through GLR-RME gap junctions. Disruption of GLR-RME gap junctions causes misaccumulation of axonal markers in non-axonal neurites of RME neurons and converts microtubules in those neurites to form an axon-like assembly. We further uncover that GLR-RME gap junctions regulate RME axon specification through activation of the CDK-5 pathway in a calcium-dependent manner, involving a calpain clp-4. Therefore, our study reveals the function of glia-neuron gap junctions in neuronal axon specification and shows that calcium originated from glial cells can regulate neuronal intracellular pathways through gap junctions
Value of miR-15b-5p combined with ultrasound imaging in early diagnosis of renal cell carcinoma
101-106Renal cancer is one of the most common malignancies, and its incidence is increasing year by year, second only to prostate and bladder cancers. Therefore, early screening is of great significance to prevent and improve the prognosis of patients with renal cancer. Therefore, this study intends to observe the early diagnostic value of miR-15b-5p combined with Colour ultrasound imaging in renal cell carcinoma. In this study, the clinical samples of 76 patients with renal cell carcinoma diagnosed by pathology in our hospital from April 2020 to June 2022 were retrospectively collected as the research objects. Another 100 healthy people who underwent general physical examination in our hospital during the same period were selected to detect the expression level of miR-15b-5 in serum by RT-PCR together with the 76 experimental samples. Colour ultrasound imaging was used to detect the blood flow distribution around and inside the kidney of the patients with renal cancer, and the parameters were measured and compared with the gold standard for statistical analysis. The expression level of miR-15b-5 in renal cell carcinoma group was significantly higher than that in control group. Most of them showed renal hamartoma (40.79%). Benign renal tumors were mainly characterized by type I and II blood flow, while malignant renal tumors were characterized by type III and IV blood flow. Compared with the pathological gold standard, the diagnostic accuracy of ultrasound imaging was founded to be about 81.25%. Further ROC analysis showed that ultrasound imaging combined with miR-15b-5 detection could effectively improve the early screening value of renal cell carcinoma. We observed that miR-15b-5 has a specific expression level in the serum of patients with renal cell carcinoma, and its combination with ultrasound imaging can significantly improve the early detection of renal cell carcinoma. This approach is envisaged to be highly useful for, early diagnosis, intervention and management of patients with renal cell carcinoma
Improved Attribute-based Encryption with Fpga for Automatic Appliance Control Application in Smart Grid
In this thesis, the author describes the privacy violation issues in smart grid with Automatic Appliance Control applications, and explains the security threats related to it. The smart grid is a sensitive and sophisticated system in real life operation. A mass of data including the remote control commands and users’ energy consumptions is transmitted between the utility companies and other devices in the smart grid such as the substations, smart meters, smart home appliances and much more. Without efficient cryptographic methods, an adversary may hack into the data or the remote control commands and extrapolates a resident’s activity model. Therefore, the Attribute-Based Encryption (ABE) is proposed to provide protection through generating the secret key of user based on a set of attributes which is used to identify different users in the smart grid. And the ciphertext, which is the encrypted remote command, obtains the access policy for decryption. However, ABE algorithm requires long computational time especially a large quantity of attributes are required in a smart grid. The idea of improved ABE system with FPGA is proposed to solve the problem. But the FPGA is only conceptual idea in this thesis and future work of it will be done by other co-worker
Analytical applications of ionic liquids and determination of cell viability using capillary electrophoresis coupled with laser-induced fluorescence detection
Newly developed ionic liquids that are air and moisture stable have been subject to an increasing number of scientific investigations. Their recent applications include novel solvent systems and catalysts for organic synthesis, versatile electrolytes for electrochemical studies, and liquid-liquid extraction solvents. The potential usage of ionic liquids could be vast. The purpose of the first part of this dissertation is to address the novel applications of ionic liquids in the field of analytical chemistry. In this part, the author's research can be divided into two directions: (a) examining the chromatographic performance of ionic liquids as gas chromatography (GC) stationary phases or solvents for GC stationary phases; (b) synthesizing new ionic liquids and testing their properties as matrices for matrix-assisted laser desorption/ionization (MALDI) mass spectrometry.;In addition to multiple applications of ionic liquids, we also became interested in developing an effective instrumental method to assess the viability of microorganisms and mammalian cells. Since existing techniques, such as plate count methods, flow cytometry, etc., are either laborious or too expensive, highly efficient and more affordable methods are needed. Therefore, the second part of this dissertation is focused on the feasibility of using capillary electrophoresis (CE), in combination with fluorescent labeling technique, to determine cell viability. The author first adapted the recently developed highly efficient microbial CE method and viable fluorescence staining method to determine the viability of bacteria and yeast, and then carried out the potency study of animal sperm using a similar CE approach.;This dissertation is presented as two independent parts. Each part begins with a general introduction and literature review of recent progress in the specific research area. The following chapters are arranged in such a way that the related published papers or manuscripts are presented as separate chapters. All these chapters are presented in publication format. References for each chapter are independent and appear at the end of the chapter. The last chapter is general conclusions covering both parts of this dissertation.</p
