Важным событием для отечественной латинской словесности стал приезд в 2013 г. в Москву выдающегося немецкого филолога-классика Вильфрида Штро. Свой визит он рассматривает на фоне долговременной истории российской классической филологии. В заключение автор делится своими общими соображениями по поводу многовековых германо-российских связей. Wilfried Stroh’s visit to Moscow in 2013 was an important event for the Russian classical philology. The famed German classicist considers his travel in the context of the centuries-old history of the Latin and Greek studies in Russia. Hemodynamic pathways of gestational hypertension and preeclampsia
Gestational hypertension and preeclampsia are the 2 main types of hypertensive disorders in pregnancy. Noninvasive maternal cardiovascular function assessment, which helps obtain information from all the components of circulation, has shown that venous hemodynamic dysfunction is a feature of preeclampsia but not of gestational hypertension. Venous congestion is a known cause of organ dysfunction, but its potential role in the pathophysiology of preeclampsia is currently poorly investigated. Body water volume expansion occurs in both gestational hypertension and preeclampsia, and this is associated with the common feature of new-onset hypertension after 20 weeks of gestation. Blood pressure, by definition, is the product of intravascular volume load and vascular resistance (Ohm's law). Fundamentally, hypertension may present as a spectrum of cardiovascular states varying between 2 extremes: one with a predominance of raised cardiac output and the other with a predominance of increased total peripheral resistance. In clinical practice, however, this bipolar nature of hypertension is rarely considered, despite the important implications for screening, prevention, management, and monitoring of disease. This review summarizes the evidence of type-specific hemodynamic profiles in the latent and clinical stages of hypertensive disorders in pregnancy. Gestational volume expansion superimposed on an early gestational closed circulatory circuit in a pressure- or volume-overloaded condition predisposes a patient to the gradual deterioration of overall circulatory function, finally presenting as gestational hypertension or preeclampsia-the latter when venous dysfunction is involved. The eventual phenotype of hypertensive disorder is already predictable from early gestation onward, on the condition of including information from all the major components of circulation into the maternal cardiovascular assessment: the heart, central and peripheral arteries, conductive and capacitance veins, and body water content. The relevance of this approach, outlined in this review, openly invites for more in-depth research into the fundamental hemodynamics of gestational hypertensive disorders, not only from the perspective of the physiologist or the scientist, but also in assistance of clinicians toward understanding and managing effectively these severe complications of pregnancy.The author reports no conflict of interest. Contemporary choice of glucose lowering agents in heart failure patients with type 2 diabetes
Background: The choice of glucose lowering agent in heart failure (HF)-patients can have a strong effect on HF-related adverse events, with some classes increasing and other classes reducing the risk. Little data is available about the choice of glucose lowering agents in HF-patients with type-2-diabetes. Methods: We performed a cross-sectional single centre point analysis of all patients with both a diagnoses of HF and type-2-diabetes followed in a tertiary HF-clinic. Medical records were used to determine the choice of current glucose lowering agent. Data at the time of cross-sectional analysis was used to determine potential eligibility to a sodium-glucose-linked-transporter-2-inhibitor (SGLT2-inhibitor) based on the enrolment criteria of the EMPAREG-OUTCOME-trial. Results: A total of 571 HF-patients with diabetes were assessed on June the first 2017. The majority of patients were either managed with one or two glucose lowering agents (43% respectively 34%), with metformin (N = 391;61%), Insulin (N = 278;49%) and sulfonylurea (N = 259;45%) being the most frequently employed treatments. SGLT2-inhibitor use was low (N = 7;1%). According to trial criteria 184 patients (32%) qualified for an SGLT2-inhibitor. With main reasons for ineligibility being a HbA1C = 2 glucose lowering agents from a class other than SGLT-2-inhibiton. Conclusion: Despite potential eligibility, SGLT2-inhibition remains an underused glucose lowering agent in this contemporary HF-population. Additional research is necessary on optimising its implementation in clinical practice, which might include switching glucose lowering therapies in patients at HbA1C-target.Pieter Martens is supported by a doctoral fellowship by the Research Foundation – Flanders (FWO, grant-number: 1127917N). On the Visit of the Munich Classicist Wilfried Stroh to Moscow
Важным событием для отечественной классической филологии стал визит в 2013 г. в Москву Вильфрида Штро. Латинские лекции, прочитанные им в МГУ, РГГУ и некоторых других высших и средних учебных заведениях столицы, были посвящены античной литературе, риторике и философии. Автор статьи приводит также общие сведения о жизни, научной и популяризаторской деятельности выдающегося немецкого филолога-классика. Wilfried Stroh’s visit to Moscow in 2013 was an important event for the classical philology of Russia. His Latin lectures delivered at the Lomonosov Moscow State University, Russian State University for the Humanities and in some other higher and secondary schools dealt with some problems of the ancient Greek and Roman literature, rhetoric and philosophy.
