Clinical, histological, immunohistochemical and lysosomal function study in inclusion body myositis
A miosite por corpos de inclusão (inclusion body myositis - IBM), na sua forma esporádica, é considerada a miopatia adquirida mais comum após os 50 anos de idade. Embora seja incluída no grupo das miopatias inflamatórias, estudos recentes mostram um processo particular de degeneração muscular caracterizado por deposição anormal de agregados de proteínas nas fibras musculares e funcionamento anormal dos principais sistemas de degradação proteica. O objetivo deste estudo foi o de avaliar os aspectos clínicos, histológicos e imunoistoquímicos de pacientes com IBM. Avaliamos 18 casos com diagnóstico de IBM de dois dos principais centros de doenças neuromusculares do Brasil (25 biópsias musculares). Na tentativa de diferenciar os casos de IBM das outras miopatias inflamatórias, determinamos o padrão de expressão tecidual da p-tau (p62), alfa-sinucleína e TDP-43. Também foi avaliada a função lisossomal através da reação da fosfatase ácida (marcação da atividade lisossomal global) e determinação da marcação para LC3B (marcador de autofagia). Foi observado que a IBM predominou no sexo masculino (61% dos casos), da cor branca, com início das manifestações clínicas ao redor dos 59 anos de idade e os sintomas mais frequentes foram fraqueza muscular, instabilidade postural com quedas da própria altura, disfagia e perda ponderal, podendo ainda apresentar dispneia. O diagnóstico demorou em média 7,4 anos após o início dos sintomas e frequentemente esteve associada às seguintes comorbidades: hipertensão arterial sistêmica, diabetes mellitus tipo 2, osteopenia / osteoporose, dislipidemia e hiperuricemia / gota. O padrão de comprometimento muscular na IBM foi caracterizado por tetraparesia de predomínio proximal em membros inferiores e distal em membros superiores. Os valores séricos da creatinofosfoquinase em pelo menos uma das medições foram elevados em todos os pacientes, porém sem ultrapassar 10 vezes o limite superior da normalidade. O uso de imunossupressão não se mostrou eficaz nos pacientes com IBM. Os achados histológicos na IBM incluíram alterações distróficas variáveis com a presença de inflamação endomisial, assim como a ocorrência de vacúolos marginados, além da elevada frequência de alterações mitocondriais. Outros achados histológicos musculares característicos na IBM foram o aumento da atividade lisossomal (aumento global da marcação para fosfatase ácida), a presença de marcação positiva para beta-amilóide (marcação intra-vacuolar pelo vermelho-Congo), o aumento na degradação muscular (relacionada com ativação de LC3B, p-tau, e p62/SQSTM1) e a degeneração muscular (marcação para anti-phospo TDP-43 e para ?-sinucleína). Tais alterações apresentaram alta sensibilidade e especificidade. Sugerimos que a redução do critério de idade do início dos sintomas de mais de 45 anos para mais de 35 anos aumentaria a sensibilidade diagnóstica para os casos com IBM deste estudo de 83% para 100%. Com este estudo, foi possível caracterizar clínica e histológicamente pacientes com IBM em nosso meio, e fornecer indícios do benefício do uso de marcadores de degeneração e autofagia para o diagnóstico e para a determinação de vias ou sistemas celulares envolvidos na patogênese da doençaSporadic inclusion body myositis (sIBM) is considered the most common acquired myopathy affecting adults aged over 50 years. Although included in the group of inflammatory myopathies, recent studies show a particular process of muscle degeneration characterized by abnormal deposit of protein aggregates in muscle fibers and abnormal operation of the main protein degradation systems. The aim of this study was to evaluate the clinical, histological and immunohistochemical patients with IBM. We evaluated 18 cases with IBM diagnostic of two of the main centers of neuromuscular diseases in Brazil (25 muscle biopsies). In an attempt to differentiate the IBM cases of other inflammatory myopathies, we determined the pattern of tissue expression of p-tau (p62), alfa-synuclein and TDP-43. Also evaluated the lysosomal function by acid phosphatase reaction (marking global lysosomal activity) and determining the markup for LC3B (autophagy marker). It was observed that IBM was predominant in males (61% of cases), white colored, with onset of clinical manifestations around 59 years old and the most common symptoms are muscle weakness, postural instability with high falls, dysphagia and weight loss, and may also present dyspnea. The diagnosis took an average of 7.4 years after the onset of symptoms and was often associated with the following comorbidities: hypertension, type 2 diabetes mellitus, osteopenia / osteoporosis, dyslipidemia and hyperuricemia / gout. The muscular damage pattern at IBM was characterized by tetraparesis predominantly proximal lower limbs and distal upper limbs. Serum creatine kinase levels in at least one of the measurements were elevated in all patients, but not exceeding 10 times normal. Immunosuppression was not effective in patients with IBM. The IBM histological findings included diversify dystrophic changes, endomysial inflammation, as well as the occurrence of rimmed vacuoles, in addition to high frequency of mitochondrial changes. Other characteristic muscle histological findings in IBM were increased lysosomal activity (overall increase in labeling for acid phosphatase), the presence of positive staining for beta-amyloid (intra-vacuolar by Congo red marking), increased muscle degradation (related to activation of LC3B, p-tau and p62 / SQSTM1) and muscle degeneration (marking for anti-phospo TDP-43 and ?-synuclein). Such changes have a high sensitivity and specificity. which makes these important complementary analyzes for accurate pathological diagnosis. We suggest that lowering the age of the onset of symptoms of greater than 45 years to older than 35 years would increase the diagnostic sensitivity for cases with IBM this study from 83% to 100%. With this study, it was possible to characterize clinically and histologically the patients with IBM in our centers, and provide evidence of the benefit of using degeneration and autophagy markers for diagnosis and for determining pathways or cellular systems involved in the pathogenesis of the diseas

INTRODUÇÃO:A neuraminidase 1 (chamada a seguir por Neu1) regula o catabolismo de sialoglicoconjugados nos lisossomos. A deficiência congênita da Neu1 é a base da sialidose, doença neurossomática grave associada a deformidades osteoesqueléticas, hipotonia e fraqueza muscular. Camundongos com deficiência de Neu1 desenvolvem uma forma atípica de degeneração muscular caracterizada porproliferação anormal de fibroblastoslevando a invasão nas fibras musculares, expansão da matriz extracelular (MEC), fragmentação do citoplasma, formação vacuolar e atrofia muscular. A ocorrência de atrofia muscular indica que a deficiência da Neu1 podeestar relacionada com o controle da massa muscular, a qual é dependente do equilíbrio entre síntese e degradação proteica.Uma característica principalnaMacroautofagia (denominada a seguir por autofagia) é a principal via de degradação intracelular sendo expressamente essencial para a homeostase celular e remoção de materiais citoplasmáticos. Objetivos: Avaliarseos efeitos da deficiência da Neuraminidase 1 afetama indução de autofagia e a formação de autofagossomos e determinar os efeitos do bloqueio da função lisossomal sobre o fenótipo muscular na deficiência da Neuraminidase1.Metodologia:Camundongos Neu1+/-foram cruzados e os filhotes genotipados, onde camundongos Neu1-/-(nocaute) e Neu1+/+(normal),foram utilizados no estudon total=90, sendo 10 em padronizações, 20 para coleta de fibroblastos e 60 para procedimentos in vivo, os grupos experimentais foram divididos em privação alimentar por 2 dias que, por meio da inibição do mTOR, induz a autofagia;grupo detratamento com Colchicina por 4 dias onde irá impedir a junção autofagossomo/lisossomonão havendo a degradaçãolisossomaleadicionado nos dois últimos dias de privação alimentar e o grupo de tratamento comRapamicina por 7 dias, droga com função de inibir seletivamente o mTOR.Após os tratamentos, os animais foram eutanasiadospara coleta dos músculos gastrocnêmios e tibiais anterior. Os músculos foram analisados apósas coloraçõeshistológicas porHematoxilina & Eosina e Fosfatase Ácida; Imunofluorescência de LC3-I/II; Western Blottingdas proteínas LC3-I/II, Lamp-1 e p62/SQSTM e a análise Ultra-estrutural. Além disso, foi realizada cultura de fibroblastos, os quais foram submetidos à privação de nutrientes e ao tratamento com Rapamicina, seguidos dasmesmas metodologias de análise invivo. Resultados:As análises histológicas através de H&E e Fosfatase Ácida não revelaram alterações consistentes na musculatura esquelética de animais submetidos aos tratamentos, mostrando em animais com deficiência de Neu1 um aumento anormal do espaço endomisial, e aumento da atividade lisossomalintracitoplasmáticos. Na análise ultra-estruturalobservou-se em todos os grupos, a presença de diversas estruturas com aspecto autofágico, de diferentes tamanhos, formas e constituintes. Naanálise da expressão do LC3 através de Western Blottingmostrou importante ativação do LC3-II na privação alimentar (com e sem administração de Colchicina) tanto em animais controles quanto em animais com deficiência de Neu1 e uma importante ativação do LC3-I em Rapamicina em ambos os grupos mostrando assim que houve um aumento da via da autofagia através do bloqueio do mTOR. Já na análise de Imunofluorescência não foi possível observar diferença consistenteentre os grupos.A análise in vitrode Western Blottingmostrou que tal ativação foi similar entre fibroblastos Neu1+/+e Neu1-/-. Conclusão:Os experimentos relacionados com a ativação ou inibição da autofagia, não resultaram em diferenças consideráveis entre músculos normais e músculos com deficiência de Neu1. Desta forma, podemos concluir com estes experimentos que aparentemente a Neu1, apesar de ser uma importante enzima lisossomal, não interfere com o processo de autofagiaIntroduction:Neuraminidase 1 (hereinafter Neu1) regulates the catabolism of sialoglycoconjugates in lysosomes. The congenital deficiency of Neu1 is the basis of sialidosis, a severe neurosomatic disease associated with osteo-skeletal deformities, hypotonia, and muscle weakness. Mice with Neu1deficient develop an atypical form of muscle degeneration characterized by abnormal fibroblast proliferation leading to muscle fiber invasion, extracellular matrix expansion (ECM), cytoplasm fragmentation, vacuolar formation, and muscle atrophy. The occurrence of muscle atrophy indicates that deficiencyofNeu1 may be related to the control of muscle mass, which is dependent on the balance between synthesis and protein degradation. A major feature in Macroautophagy (hereinafter referred to as autophagy) is the main pathway of intracellular degradation being expressly essential for cellular homeostasis and removal of cytoplasmic materials. Objectives:To evaluate whether the effects of neuraminidase 1 deficiency affect autophagy induction and autophagosome formation and to determine the effects of lysosomal function block on muscle phenotype in neuraminidase 1 deficiency. Methodos:Mice Neu1+/-were crossbred and the genotyped pups, where mice Neu1-/-(knockout) and Neu1+/+(normal), were used in the study ntotal = 90, 10 for standardization, 20 for fibroblast collection and 60 for in vivo procedures, experimental groups were divided into food deprivation for 2 days that, through mTOR inhibition, induces autophagy; Colchicine treatment group for 4 days whereit will prevent autophagosome/lysosome junction without lysosomal degradation and added in the last two days of food deprivation and Rapamycin treatment group for 7 days, drug with function to selectively inhibit mTOR. After the treatments, the animals were euthanized to collect the anterior gastrocnemius and tibial muscles. The muscles were analyzed after histological staining by Hematoxylin & Eosin and Acid Phosphatase; LC3-I/II immunofluorescence; Western Blottingof LC3-I/II, Lamp-1 and p62/SQSTM proteins and Ultra-structural analysis. In addition, fibroblasts were cultured and subjected to nutrient deprivation and Rapamycin treatment, followed by the same in vivo analysis methodologies. Results:Histological analyzes by H&E and Acid Phosphatase did notreveal consistent changes in skeletal muscle of animals submitted to treatments, showing in animals with Neu1 deficiency an abnormal increase in endomysial space and increased intracytoplasmic lysosomal activity.In ultrastructural analysis, it was observed in all groups, the presence of several structures with autophagic aspect, of different sizes, shapes and constituents. The analysis of LC3 expression by Western Blottingshowed important activation of LC3-II in food deprivation (with and without Colchicine administration) in both control and Neu1 deficient animals and an important activation of LC3-I in Rapamycininboth groups,thus showing an increase in the autophagy pathway through mTOR blockade. In the immunofluorescence analysis, it was not possible to observe consistent difference between the groups. In vitro Western Blottinganalysis showed that such activation was similar between Neu1+/+and Neu1-/-fibroblasts. Conclusion: Experiments related to activation or inhibition of autophagy did not result in considerable differencesbetween normal muscles and Neu1 deficient muscles. Effects of different glucocorticoids on molecular pathways regulating muscle trophism in rats and the effect of EPA / DHA on muscle atrophy induced by dexamethasone
Várias condições podem estar relacionadas com a atrofia muscular, tais como inatividade, envelhecimento, septicemia, diabetes, câncer e uso de glicocorticoides. Em tentativa prévia de prevenir tal condição catabólica secundário ao uso de glicocorticoide, através da suplementação de ômega-3 (N-3), observamos um agravamento da atrofia muscular, afetando mais tipos de fibras musculares, usualmente poupadas pelo glicocorticoide, fibras tipo 1 por exemplo. Entretanto, não foi possível determinar quais as propriedades dessa interação. Portanto, o objetivo deste estudo foi de avaliar a ação do Ômega-3 associada a dexametasona e de diferentes glicocorticoides em dose equipotente sobre o peso corporal; área de secção transversa muscular; perfil de ácidos graxos; expressão gênica de fatores de transcrição musculares e atrogenes (Atrogina 1 e MuRF-1); expressão proteica de componentes das vias do IGF-1/Akt/mTOR, Ras/Raf/MEK/ERK e Miostatina/Smad2/3; e expressão de receptores de glicocorticoides na musculatura esquelética de ratos. Metodologia: Ratos Wistar suplementados ou não com ômega-3 (100mg/kg/dia de EPA) por 40 dias receberam dexametasona (DX) subcutânea (2,5 e 1,25mg/kg/dia) nos últimos 10 dias de suplementação. Para estudo dos demais glicocorticoides, ratos sem suplementação receberam deflazacorte (DC), metilprednisolona (MP) em dose/volume equipotente ao de dexametasona (DC 10 e 20mg/kg/dia e MP6,7 e 13,3mg/kg/dia) por 10 dias. Constituindo 10 grupos: CT, N-3, DX1,25, DX2,5, DX1,25+N-3, DX2,5+N-3, MP6, MP13, DC10 e DC20. Através de estudo histológico, imuno-histoquímico, PCR em tempo real e Western blotting, foram avaliados a área transversa dos diferentes tipos de fibras musculares; a expressão de receptor de glicocorticoide na fibra muscular; a expressão gênica dos atrogenes e fatores de transcrição; expressão de proteínas das vias IGF-1, Miostatina e MEK/ERK. Resultados: A administração de N-3 influenciou a atrofia por DX causando maior atrofia em fibras do tipo 1 e 2A, aumento na expressão proteica de FoxO3a total, P-Smad3, LC3-II e gênica (mRNA) de REDD-1, Atrogina-1/MAFbx. De forma isolada o ômega-3 reduziu a expressão de P-FoxO3a, PGC1alfa, a quantidade de ácido araquidônico e a expressão de mRNA do IRS-1 com aumento na expressão de LC3-II. A comparação entre glicocorticoides mostrou que a MP (13mg/kg/dia) acarretou maior impacto no peso corporal e muscular; o DC (10mg/kg/dia) causou menor atrofia em fibras 2B em relação aos demais glicocorticoides. A DX causou maior impacto sobre o Akt total em comparação com os demais glicocorticoides, em P-Akt o grupo DX1,25 teve menor expressão em relação a outros glicocorticoides em dose equipotente. Todos os glicocorticoides afetaram a expressão de P-FOXO3a. Na expressão de ERK1/2 e P-ERK1/2, MP6 foi o grupo com maior prejuízo à fosforilação em relação aos demais em dose equipotente. Já na avaliação da via Miostatina/Smad2/3 os grupos MP 6, MP13 e DC20 mostraram maior expressão de Smad2/3 total e P-Smad3. A expressão gênica de REDD-1 e MYOD foi aumentada nos grupos MP6 e MP13 em relação aos demais grupos; REDD2 no grupo DC20 foi menor em relação ao grupo DX2,5. A expressão de Miostatina foi menor nos grupos DX2,5 e DC20, sendo o DC a droga com menor impacto sobre os atrogenes MuRF-1 e Atrogina-1. DX1,25 e DX2,5 causaram menor expressão de IRS-1 entre os grupos de glicocorticoides. Conclusões: Ômega-3 pode aumentar a atrofia muscular causada por DX em fibras 1 e 2A, possivelmente relacionado com aumento da expressão de FoxO3a, REDD-1 e Atrogina-1, diminuição na expressão de PGC1alfa e P-FoxO3a, nas quantidades de ácido araquidônico com aumento da atividade lisossomal. Comparando diferentes glicocorticoides, a MP tende a produzir maior impacto nos pesos corporal e muscular, o DC é menos prejudicial as fibras do tipo 2B, entretanto, afeta predominantemente fibras do tipo 1, da mesma forma que a DX na dosagem de 1,25mg/kg/dia. A DX tende a afetar mais a expressão de Akt total e fosforilado que os demais glicocorticoides. A MP afeta mais a via Ras/Raf/MEK/ERK e expressão de REDD-1 em relação aos demais glicocorticoides, e o DC e MP mostram maior expressão de Smad2/3 total e fosforilada em relação ao DX após 10 dias de administraçãoSeveral conditions may be related to muscle atrophy, such as inactivity, aging, septicemia, diabetes, cancer and use of glucocorticoids. In a previous attempt to prevent such glucocorticoid catabolic condition, through the supplementation of omega-3 (N-3), we observed a worsening of muscular atrophy, affecting more types of muscle fibers, usually spared by glucocorticoid, type 1 fibers for example. However, it was not possible to determine the properties of this interaction. Therefore, the objective of this study was to evaluate the action of omega-3 associated with dexamethasone and different glucocorticoids in equipotent dose on body weight; muscle cross-sectional area; fatty acid profile; gene expression of muscle transcription factors and atrogenes (Atrogin-1 and MuRF-1); protein expression of IGF-1/Akt/mTOR, Ras/Raf/MEK/ERK and Myostatin/Smad2/3 pathways components; and expression of glucocorticoid receptors in the skeletal musculature of rats. Methods: Wistar rats given orally or not with omega-3 (100mg/kg/day of EPA) for 40 days received subcutaneous dexamethasone (DX) (2.5 or 1.25mg/kg/day) during the last 10 days of supplementation. For the other glucocorticoids, rats without supplementation received deflazacorte (DC) or methylprednisolone (MP) in dose/volume equivalent to that of dexamethasone (DC 10 or 20mg/kg/day and MP6.7 or 13.3mg/kg/day) for 10 days. Comprising 10 groups: CT, N-3, DX1.25, DX2.5, DX1.25 + N-3, DX2.5 + N-3, MP6, MP13, DC10 and DC20. Through histological, immunohistochemical, real-time PCR and Western blotting, we evaluated the transverse area of the different muscle fibers; the expression of glucocorticoid receptor; the gene expression of atrogenes and transcription factors; protein expression of the IGF-1, Myostatin and MEK/ERK pathways. Results: N-3 administration influenced DEXA atrophy causing increased atrophy in type 1 and 2A fibers, increased protein expression of total FoxO3a, P-Smad3, LC3-II, and REDD-1 gene (mRNA), Atrogin-1/MAFbx isolated omega-3 reduced the expression of P-FoxO3a, PGC1alpha, the amount of arachidonic acid and the expression of IRS-1 mRNA with increased expression of LC3-II. The comparison between glucocorticoids showed that MP13 had a greater impact on body and muscle weight; the DC10 caused less atrophy in 2B fibers in relation to the other glucocorticoids. DX, caused greater impact on total Akt compared to the other glucocorticoids, in P-Akt the DX1,25 group had lower expression to other equipotent dose glucocorticoids. All glucocorticoids affect the expression of P-FOXO3a. In the of ERK1/2 and P-ERK1/2 protein expression, the MP6 was the group with the greatest damage to the phosphorylation in relation to the others in equipotent dose. In the evaluation of the Myostatin/Smad2/3 pathway MP 6, MP13 and DC20 showed higher expression of total Smad2/3 and P-Smad3. The gene expression of REDD-1 and MYOD was increased in the MP6 and MP13 groups compared to the other groups, REDD2 in the DC20 group was lower in relation to the DX2.5 group. Myostatin expression was lower in the DX2.5 and DC20 groups, with DC being the drug with less impact on atrogenes MuRF-1 and Atrogin-1. DX1.25 and DX2.5 caused lower IRS-1 expression among the glucocorticoid groups. Conclusions: Omega-3 may increase muscle atrophy caused by DX in fibers 1 and 2A, possibly related to increased expression of FoxO3a, REDD-1 and Atrogin-1, decreased expression of PGC1alpha and P-FoxO3a, in the amounts of acid arachidonic with increased lysosomal activity. Comparing different glucocorticoids, MP tends to produce a greater impact on body and muscular weights, DC is less harmful to type 2B fibers, however, it predominantly affects type 1 fibers, in the same way as DX in the dosage of 1.25mg/kg/day. DX tends to affect total and phosphorylated Akt expression more than other glucocorticoids. MP affects more the Ras/Raf/MEK/ERK pathway and REDD-1 expression in relation to the other glucocorticoids, and DC and MP show a higher expression of total and phosphorylated Smad2/3 compared to DX after 10 days of administratio
Histological and molecular analysis of the diaphragm muscle after treatment with different glucocorticoids and under the association of omega 3 fatty acid and dexamethasone in rats
Atrofia muscular é uma resposta debilitante e que pode ocorrer em diversas condições, como por exemplo na inatividade, imobilização e corticoterapia. Corticoides estão entre os medicamentos mais prescritos na prática médica, sendo amplamente utilizados no tratamento de doenças neuromusculares. No entanto, seu uso pode levar a fraqueza e atrofia muscular, dependendo do tempo de uso, dosagem e tipo de musculatura afetada, podendo variar, de acordo com cada tipo de corticoide. Omega-3 (N-3) é um ácido graxo amplamente recomendado e consumido, principalmente pelos benefícios cardiovasculares, contudo, quando associado a dexametasona (DX) pode aumentar seus efeitos colaterais no músculo periférico. Não há, no entanto, estudo quanto ao efeito desta associação sobre a musculatura diafragmática. Objetivos: Avaliar o impacto da associação de DX+N-3 e de diferentes glicocorticoides em equipotência, sobre o músculo diafragma, através da histologia, expressão gênica e proteica. Metodologia: Sessenta ratos Wistar foram divididos em seis grupos. Vinte ratos foram suplementados com N-3 por 40 dias (EPA/DHA-100mg/kg/dia), dos quais, 10 foram submetidos à administração de DX (1,25mg/kg/dia) nos 10 dias finais. Quarenta animais receberam de forma equipotente, DX (1,25mg/kg/dia), metilprednisolona (MP) (6,7mg/kg/dia), deflazacort (DC) (10 mg/kg/dia) ou solução salina (CT) nos últimos 10 dias. Avaliamos a área de secção transversa e proporção das fibras musculares por coloração metacromática de ATPase, expressão de RNAm de atrogenes por PCR em tempo real e análise proteica por Western Blotting. Resultados: Houve aumento na proporção de fibras tipo 2B e diminuição do tipo 1 no grupo N-3. O DC aumentou a proporção de fibras tipo 1 e juntamente com os grupos DX, DX+N-3 e MP, diminuiu a proporção de fibras 2A. A DX atrofiou fibras 1, 2A e 2B, não sendo influenciada pelo N-3. De forma isolada, o N-3 atrofiou as fibras 2A e o DC atrofiou fibras 2A e 2B. Entre os glicocorticoides a DX causou maior atrofia muscular de fibras 2A. A DX causou maior atrofia de fibras 2B vs DC. Todos os glicocorticoides diminuíram a expressão proteica de pAkt (ativo), sendo a DC a mais branda e apenas o grupo MP diminuiu a expressão total de Akt. A MP e DX diminuíram a expressão de pFoxO3a (inativo). Houve diminuição importante da expressão total de FoxO3a no grupo MP. Todos os glicocorticoides mostraram diminuição importante da expressão de pGSK3 beta (inativo) e aumento da expressão total no MP e DC. Notamos diminuição da expressão proteica de PGC1alfa nos grupos DX e DC e tendência de queda na expressão proteica de pFoxO3a no grupo DX+N-3 vs DX. A expressão de Atrogina-1, MuRF-1 e REDD-2 foi aumentada no grupo DX. Houve menor expressão de MuRF-1 e REDD-2 no grupo DX+N-3 vs DX. A expressão de Atrogina-1 foi aumentada em todos os grupos e com maior expressão no grupo MP. A expressão de MuRF-1 foi aumentada nos grupos MP e DX, enquanto REDD-2 foi aumentada nos grupos DC e DX. Conclusões: A administração de glicocorticoides por 10 dias em ratos mostrou ser lesiva ao tecido diafragmático, com efeitos mais brandos (tardios) no uso de DC comparados ao outros glicocorticoides. A associação de DX+N-3 parece ser favorável quanto a expressão proteica de PGC1alfa, pFoxO3a e RNAm de atrogenesMuscle atrophy is a debilitating tissue response that can occur in diverse conditions such as immobilization, inactivity and corticotherapy. Corticosteroids are one of the most prescribed drugs in medical practice, widely used in the treatment of neuromuscular diseases. Although its use can lead to weakness and muscle atrophy, depending on the time of use, dosage and muscle type, with different impact levels depending on the corticoid type. Omega-3 (N-3) is a fatty acid widely recommended mainly for its beneficial cardiovascular effects, however, when associated with dexamethasone (DX) can increase the side effects in the peripheral muscle, with no description effect on diaphragmatic muscle. Aim: We aimed to evaluate the impact of the DX in association with N-3 and different glucocorticoids in equipotency, on the diaphragm muscle, through histology, gene and protein expression. Methods: The experimental study was carried out with sixty Wistar rats, divided into six groups. Twenty Wistar rats were supplemented with N-3 for 40 days (EPA / DHA-100mg / kg / day), of which 10 were submitted to DX administration (1.25mg / kg / day) in the final 10 days. 40 animals received equipotent form, DX (1.25mg / kg / day), methylprednisolone (MP) (6.7mg / kg / day), deflazacort (DC) (10 mg / kg / day) or saline solution (CT) in the last 10 days. We assessed the crosssectional area and proportion of muscle fibers by ATPase metachromatic staining, atrogenes mRNA by real-time PCR and protein by Western Blotting technique. Results: There was an increase in the proportion of type 2B fibers and a decrease in type 1 in the N-3 group. The DC increased the proportion of fibers 1 and together with the groups DX, DX+N-3 and MP, decreased proportion of type 2A fibers. DX atrophied fibers 1, 2A and 2B, and was not influenced by N-3, which alone led to atrophy of 2A fibers. The DC atrophied fibers 2A and 2B. Among glucocorticoids, DX caused greater muscle atrophy in 2A and 2B fibers vs DC. All glucocorticoids decreased the protein expression of pAktT (active), with DC being the mildest; only the MP group decreased the total expression of Akt. DX decreased the expression of pFoxO3a (inactive), and in total expression (FoxO3a) there was an important decrease in the MP group. All glucocorticoids showed a significant decrease in the expression of pGSK3beta (inactive), when compared to CT, with an increase in total expression in the MP and DC. We noticed a decrease in PGC1alfa protein expression in the DX and DC. In pFoxO3a protein expression, we noted a downward trend in the DX + N-3 vs DX. The DX increased all atrogenes expression. There was less expression of MuRF-1 and REDD-2 in the DX+N-3 vs DX group. Atrogin- 1 expression was increased in all groups and with greater expression in the MP. The expression of MuRF-1 was increased in the MP and DX, and REDD-2 was increased in the DC and DX. Conclusions: The glucocorticoids administration for 10 days in rats proved to be harmful to diaphragmatic tissue, with milder effects in the use of DC compared to other glucocorticoids. The association of DX+N-3 was favorable for protein expression of PGC1alfa, pFoxO3a and reduction of mRNA atrogenes
Congenital myasthenic syndromes: clinical and molecular aspects
Síndromes miastênicas congênitas (SMC) constituem um grupo raro de doenças genéticas com fenótipo muitas vezes difícil de distinguir clinicamente entre elas. Há ainda sobreposição de aspectos clínicos de SMC com algumas miopatias. Dessa forma, uma melhor caracterização clínica das diferentes formas de SMC seria de grande importância. O presente estudo teve os seguintes objetivos: 1) caracterizar clinicamente os pacientes com SMC em nosso meio; 2) identificar mutações mais frequentes em genes que causam SMC em nosso meio; 3) correlacionar as mutações encontradas com o quadro clínico dos pacientes. Assim, foram analisadas características clínicas, histológicas e eletrofisiológicas de uma coorte de 79 pacientes (57% mulheres) com suspeita de SMC um serviço terciário no Brasil. Estes pacientes eram de 68 famílias não relacionadas, com idades entre 1 e 69 anos (média: 26,7). As características clínicas observadas foram correlacionadas com o diagnóstico molecular final. Foram coletados dados de avaliações clínicas e exames auxiliares e amostras de sangue foram submetidas a testes moleculares, incluindo sequenciamento de nova geração, tal qual exoma, para aqueles não diagnosticados por sequenciamento Sanger de regiões estratégicas dos principais genes. Após análise molecular, os casos foram estratificados em quatro categorias: diagnóstico confirmado de SMC (51 pacientes), diagnóstico de condição não- SMC (cinco pacientes), diagnóstico provável de SMC (sete pacientes) e casos não resolvidos (16 pacientes). Os achados clínicos foram comparados entre o grupo com diagnóstico molecular confirmado de SMC e os grupos: não resolvido e não-SMC. Pacientes classificados como prováveis SMC foram excluídos da comparação. CHRNE foi o gene afetado em 22 famílias, seguido por RAPSN (n=5), COL13A1 (n=3), DOK7 (n=3), COLQ (n=2), GFPT1 (n=2), SCN4A (n=1), CHAT (n=1), CHRNA1 (n=1) e GMPPB (n=1). Foram significativamente mais frequentes nos casos confirmados de SMC: presença de história familiar; alterações motoras na primeira infância; presença de fraqueza de membros, fraqueza facial, fraqueza mastigatória, crises miastênicas frequentes, alterações eletrofisiológicas do defeito da junção neuromuscular e alterações miopáticas leves na biópsia muscular. Nenhum caso com sintomas exclusivamente oculares foi confirmado como SMC, o que também foi estatisticamente significante. Em contraste, nos casos não-SMC, a deficiência intelectual foi significativamente mais frequente e as alterações eletrofisiológicas menos frequentes. Concluímos que CHRNE foi o principal gene envolvido, e mutações em RAPSN foram relevantes em nossa população. A pesquisa de mutações comuns nos 3 genes mais relacionados a SMC (c.130dupG em CHRNE, p.N88K em RAPSN e c.1124_1127dupTGCC em DOK7) pode levar a um diagnóstico molecular de mais de 60% dos casos. Provavelmente há casos de CMS relacionados a genes ainda não descritos em nossa população, ou pode haver contribuição significativa de mutações intrônicas, ou grandes deleções/duplicações na etiologia de SMC, visto que cerca de 20% dos casos não foram solucionados. Algumas características clínicas, e principalmente alterações eletrofisiológicas típicas são capazes de predizer o diagnóstico molecular de SMC. Além disso, alterações miopáticas também podem ser uma característica frequente.Congenital myasthenic syndromes (CMS) comprise a rare group of genetic diseases with phenotype that can be difficult to be distinguished between their selves. Yet, overlap with clinical aspects of some myopathies is seen. Thus, more precise mapping of clinical features to final diagnosis is valuable to improve this situation. This study had the following objectives: 1) to characterize clinically patients with CMS in our environment; 2) to identify more frequent mutations in genes that cause CMS in our environment 3) to correlate the mutations found with the clinical condition of patients. The clinical, histological and electrophysiological characteristics of a large cohort of 79 patients (57% female) with suspected CMS from a tertiary center in Brazil were analyzed. They were from 68 unrelated families, and age varied from 1 to 68 years (mean: 26.7). Observed features were correlated with the final molecular diagnosis. Data from clinical evaluations and ancillary exams were collected and blood samples were submitted to molecular tests, which included new generation sequencing, like whole exome sequencing, for those not diagnosed by Sanger sequencing of hotspots of the main genes. Cases were stratified into four categories: confirmed diagnosis of CMS (51 cases), confirmed diagnosis of other condition (non-CMS) (five cases), probable diagnosis of CMS (seven cases), and unsolved cases (16 cases). Clinical findings were compared between the group with a confirmed molecular diagnosis of CMS and the groups: unsolved and non-CMS. Patients classified as probable were excluded from the comparison. CHRNE was the defective gene in 22 families, and was followed by RAPSN (n=5), COL13A1 (n=3), DOK7 (n=3), COLQ (n=2), GFPT1 (n=2), SCN4A (n=1), CHAT (n=1), CHRNA1 (n=1), and GMPPB (n=1). Were significantly more frequent in CMS confirmed cases the presence of: family history; early infancy clinical features; limb weakness, facial weakness, masticatory weakness, frequent myasthenic crisis, electrophysiological alterations of neuromuscular junction defect and mild myopathic alterations on muscle biopsy. No cases with exclusive ocular symptoms were confirmed as CMS, which was also statistically significant. In contrast, in non-CMS cases, intellectual disability was significantly more frequent, and electrophysiology alterations less frequent. In conclusion, CHRNE was the main gene involved in CMS, and mutations in the RAPSN gene were relevant in our population. Search for common mutations in the 3 genes most frequently related to CMS (c.130dupG in CHRNE, p.N88K in RAPSN, and c.1124_1127dupTGCC in DOK7) could lead to a molecular diagnosis of more than 60% of cases. There are probably cases of CMS related to genes not yet described in our population, or may be significant contribution of intronic mutations or major deletions/duplications in the etiology of CMS since approximately 20% of cases were unable to reach a diagnosis. Regulation of IGF and PDGF receptors in the skeletal muscle of neuraminidase 1 deficient mice
A neuraminidase 1 (Neu1) é a enzima que regula o catabolismo de sialoglicoconjugados nos lisossomos. A deficiência da Neu1 é a base da sialidose, doença grave associada a um amplo espectro de manifestações, incluindo hipotonia e fraqueza muscular. Camundongos com deficiência de Neu1 desenvolvem degeneração muscular caracterizada principalmente por atrofia, invasão das fibras musculares por fibroblastos e expansão da matriz extracelular. A Neu1 controla a proliferação de fibroblastos de pacientes por meio da desialilação dos receptores de PDGF e IGF. Além disso, há enzimas lisossomais que são moduladas pela Neu1, tais como as catepsinas, que são capazes de degradar componentes musculares e estariam excessivamente ou erroneamente ativas (sialiladas) em decorrência da deficiência de Neu1. O objetivo deste trabalho foi identificar se o fenótipo da musculatura esquelética de camundongos Neu1-/- poderia estar associado à atividade do IGF-1R, PDGFR e/ou à sialilação de catepsina B, através da análise histológica e proteica de músculos esqueléticos e fibroblastos de camundongos Neu1+/+ e Neu1-/- tratados com inibidores de IGF1-R e PDGFR. O estudo da expressão proteica de catepsina B foi realizado nos músculos tratados com os inibidores de IGF-1R e PDGFR, e nas frações citosólica e lisossomal de fibroblastos tratados com neuraminidase exógena. Em comparação com camundongos Neu1+/+, os músculos de animais Neu1-/- apresentam menor área de fibra, peso corporal, expressão de pAkt e maior expressão de catepsina B; e os fibroblastos Neu1-/- exibem maior proliferação e expressão de pAkt. A inibição do IGF-1R em camundongos Neu1-/- aumentou a área das fibras musculares, expressão de pAKt e diminuiu a expressão de catepsina B; em relação aos fibroblastos Neu1-/-, entretanto aumentou a proliferação celular com diminuição de pAkt. A inibição do PDGFR em músculos de camundongos Neu1-/- levou ao aumento da expressão de pAkt, da área das fibras, com diminuição de pERK e catepsina L, quando comparados com os controles Neu1-/-; a mesma inibição in vitro conduziu à diminuição da expressão de pAkt, pERK e proliferação. A catepsina B encontra-se bastante ativa na fração lisossomal e o tratamento com neuraminidase foi eficaz na correção de seu peso molecular e compartimentalização lisossomal. Neu1 deficiency is the basis of sialidosis, a severe disease associated with a broad spectrum of manifestations, including hypotonia and muscle weakness. Neu1 deficient mice develop muscular degeneration characterized by atrophy, invasion of muscle fibers by fibroblasts, and expansion of the extracellular matrix. Neu1 controls the proliferation of fibroblasts from patients through the desialylation of PDGF and IGF receptors. In addition, lysosomal enzymes are modulated by Neu1, such as cathepsins, which degrade muscle components and are excessively or erroneously active (sialylated) as a result of Neu1 deficiency. The aim of this study was to identify whether skeletal muscle phenotype of Neu1-/- mice may be associated with IGF-1R, PDGFR and/or sialylation of cathepsin B, through protein and histological analysis of skeletal muscles and fibroblast from Neu1+/+ and Neu1-/- mice treated with IGF-1R and PDGFR inhibitors. The study of cathepsin B protein expression was performed in skeletal muscles treated with IGF-1R and PDGFR inhibitors, and in the cytosolic and lysosomal fractions of fibroblasts treated with exogenous neuraminidase. Compared with Neu1+/+ animals, Neu1-/- muscles showed smaller muscle fiber area, body weight, pAkt expression and higher cathepsin B expression; and Neu1-/- fibroblasts exhibited increased proliferation and expression of pAkt. The inhibition of IGF-1R Neu1-/- mice increased the area of muscle fibers, expression of pAkt and decreased expression of cathepsin B; but, considering Neu1-/- fibroblasts, there was increased cell proliferation with reduction of pAkt. The inhibition of PDGFR in muscles of Neu1-/- mice led to increased expression of pAkt, muscle fiber area, with decreased expression of pERK and cathepsin L, when compared with the Neu1-/- controls; the same inhibition in vitro led to reduced expression of pAkt, pERK and cell proliferation. Cathepsin B presented high activity in the lysosomal fraction and the treatment with neuraminidase was effective in the correction of its molecular weight and lysosomal compartmentalization. Molecular analysis of the CAPN3 and FKRP genes in patients with limb-girdle muscular dystrophy
Introdução: As distrofias musculares de cinturas (limb-girdle muscular dystrophies - LGMD) são causadas por mutações em uma grande variedade de genes que codificam proteínas musculares, podendo ser herdadas de forma autossômica dominante ou recessiva. O diagnóstico é feito tanto através de exame de biópsia muscular que mostra um padrão histológico distrófico ao lado de deficiência específica de proteínas musculares quanto por estudo genético. Em alguns subtipos de LGMD não é possível fazer o diagnóstico específico pela biópsia muscular, tais como na deficiência da calpaína-3 (CAPN3) e da proteína relacionada a fukutina (FKRP). Nestes casos, portanto, o exame molecular é de grande valor para a confirmação do diagnóstico. Objetivos: Analisar os genes CAPN3 e FKRP em pacientes com diagnóstico histológico de LGMD e verificar a expressão proteica da CAPN3 nesses pacientes, correlacionando com as mutações identificadas e com o quadro clínico e histológico dos mesmos. Resultados: Fizeram parte deste estudo 36 pacientes com LGMD provenientes do ambulatório de miopatias do HC-FMUSP em que a biópsia muscular não identificou deficiência de distrofina, disferlina, caveolina-3 e sarcoglicanas. Destes, nove (25%) foram diagnosticados com LGMD2A, seis (17%) com LGMD2I e em 21 (58%) não foi possível identificar o subtipo específico. Foram encontradas mutações patogênicas no gene CAPN3 em oito pacientes, sendo em homozigose em dois casos, heterozigose composta em cinco casos e em heterozigose em um caso. Em um caso o diagnóstico de LGMD2A foi realizado baseado apenas na análise da expressão da proteína CAPN3 no tecido muscular. Em seis pacientes foram identificadas mutações patogênicas no FKRP, sendo em homozigose em cinco casos e em heterozigose em um caso. A maioria dos pacientes com LGMD2I (cinco casos) apresentava a mutação c.826C > A. Foi observada ausência total ou parcial da expressão da CAPN3 em pacientes com LGMD2A. Conclusões: O presente estudo mostrou que mutações nos genes CAPN3 e FKRP são frequentes em pacientes com diagnóstico clínico e histológico de LGMD. A análise da expressão da CAPN3 se mostrou como uma importante ferramenta no diagnóstico da LGMD2AIntroduction: The Limb-Girdle Muscular Dystrophies (LGMD) are caused by mutations on a wide variety of genes that encode muscular proteins which can be inherited in dominant or recessive autosomal forms. The diagnosis is made either by genetic study or by muscle biopsy which shows a dystrophic histologic pattern with specific deficiency of muscular proteins. On some LGMD subtypes such as calpain-3 (CAPN3) and fukutin related protein (FKRP) deficiencies it is not possible to make a specific diagnosis by muscle biopsy. In these cases, the molecular exam is of great value to confirm the diagnosis. Objectives: Analyze the CAPN3 and FKRP genes in patients with histological diagnoses of LGMD, and verify the protein expression of CAPN3 on these patients correlating it with the identified mutations and their clinical and histological pattern. Results: Thirty-six patients with LGMD, where the muscular biopsy did not identify deficiency of dystrophin, dysferlin, caveolin-3 and sarcoglycans, from the Muscle Ambulatory of HC-FMUSP took part in this study. Of these, nine (25%) were diagnosed with LGMD2A, six (17%) with LGMD2I, and on 21 of them (58%), it was not possible to identify the specific subtype. Pathogenic mutations on CAPN3 were found in eight patients, being homozygous in two cases, compound heterozygous in five cases and heterozygous in one case. The diagnosis of LGMD2A in one patient was done based exclusively by CAPN3 protein analysis on the muscle tissue. Pathogenic mutations on FKRP were found in six patients, being homozygous in five cases and heterozygous in one case. Most of the patients with LGMD2I (five cases) presented the mutation c.826C > A. It was observed total or partial absence of the CAPN3 expression in patients with LGMD2A. Clinical, histological and molecular study of patients with distal and myofibrillar myopathies
As miopatias distais (MD) são um grupo heterogêneo de doenças musculares com predomínio de fraqueza nas extremidades distais, enquanto as miopatias miofibrilares (MMF) são doenças musculares esqueléticas e cardíacas com agregados proteicos do disco Z e desarranjo miofibrilar. Ambas as condições podem apresentar fraqueza distal predominante e demonstrar uma histologia vacuolar associada, levando a certa sobreposição fenotípica. Este estudo visa identificar pacientes diagnosticados com MD e MMF, com o objetivo de caracterizá-los sob os aspectos clínicos, histológicos e moleculares e comparar os diferentes subtipos dessas miopatias. Trata-se de um estudo observacional retrospectivo com pacientes identificados no ambulatório do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo com diagnóstico histológico de miopatia miofibrilar e diagnóstico clínico de miopatia distal. Os pacientes foram avaliados clinicamente após convite e consentimento informado. Biópsias musculares foram revisadas e analisadas quanto à desorganização de miofibrilas, presença de vacuolização e acúmulo de produtos de degradação proteica intracitoplasmático. Pacientes sem diagnóstico molecular foram submetidos ao sequenciamento de nova geração utilizando um painel customizado, com cobertura para os seguintes genes: DES, CRYAB, MYOT, LDB3, FLNC, TIA1, MYH7, GNE, BAG3, DNAJB6, TTN, PLEC, FHL1, LMNA. Aqueles pacientes com resultados negativos ou inconclusivos foram submetidos ao sequenciamento completo do exoma. Foram incluídos 34 pacientes (23 famílias) com MMF e 20 pacientes (19 famílias) com MD. Entre as famílias com MMF, as variantes em DES foram a principal causa (n=8), seguidas por TTN (n=4), FHL1 (n=3), FLNC (n=2), BAG3 (n=2), MYOT (n=1) e HNRNPA2B1 (n=1). Entre as famílias com MD, DYSF (n=6) e GNE (n=4) foram os genes mais frequentes, seguidos por MYH7 (n=2), NEB (n=2), TTN (n=1), ANO5 (n=1), RYR1 (n=1) e DNM2 (n=1). A gravidade do comprometimento motor ocorreu em proporção parecida entre a miopatia distal e a miopatia miofibrilar, sendo mais acentuado nos pacientes com variantes nos genes BAG3, FHL1, MYH7 e GNE. As miopatias miofibrilares apresentaram maior proporção e gravidade de comprometimento cardiorrespiratório. A miopatia por DES foi relacionada à maior frequência de cardiopatias. Variantes na TTN foram associadas a diferentes fenótipos e presença de insuficiência respiratória e cardiopatia. A miopatia por BAG3 se apresentou de forma precoce, com contraturas axiais e comprometimento cardiorespiratório. Os pacientes com FHL1 apresentaram miopatia do corpo redutor na infância, histologia inflamatória e um fenótipo escapuloperoneal entre as mulheres portadoras. Observamos o fenótipo clássico de Miyoshi na disferlinopatia e na anoctaminopatia. A miopatia por GNE, por sua vez, apresentou-se com envolvimento dos músculos tibiais anteriores. Alguns genes relacionados a miopatias congênitas também foram causadores de MD (TTN, RYR1, DNM2) e foram inicialmente diagnosticados como neuropatia hereditária, devido precocidade dos achados e deformidades distais. MD e MMF compreendem grupos heterogêneos, mas suas características fenotípicas permitem a relativa categorização em subgrupos. Reconhecer essas miopatias é importante devido à sua gravidade potencial, complicações cardíacas precoces e insuficiência respiratória, que pode levar à morte evitávelDistal myopathies (DM) are a heterogeneous group of muscle diseases featuring a predominance of weakness in the distal extremities, while myofibrillar myopathies (MFM) are skeletal and cardiac muscle diseases with Z-disk protein aggregates and myofibril disarray observable upon muscle biopsy. Both conditions may present with predominant distal weakness and demonstrate a vacuolar histology, leading to some phenotypic overlap. This study aims to identify patients diagnosed with distal myopathies and myofibrillary myopathies with the aim of characterizing them under clinical, histological and molecular aspects, classifying them and comparing the different subtypes of these myopathies. This is a retrospective observational study with patients identified at the outpatient clinic of Hospital das Clinicas, Faculty of Medicine, University of São Paulo with histological diagnosis of myofibrillar myopathy and clinical diagnosis of distal myopathy. The patients were evaluated clinically after an invitation and informed consent. Muscle biopsies were reviewed and analyzed for myofibril breakdown, presence of vacuolization and accumulation of protein degradation products in the intracytoplasmic environment. Patients without molecular diagnosis were submitted to new generation sequencing using a customized panel with coverage for the following genes: DES, CRYAB, MYOT, LDB3, FLNC, TIA1, MYH7, GNE, BAG3, DNAJB6, TTN, PLEC, FHL1, LMNA. Those patients with negative or inconclusive results underwent a complete exome sequencing. We included 54 patients from 42 families: 34 patients (23 families) with MFMs, and 20 patients (19 families) with DMs. Among the MFM families, variants in DES were the main cause (n=8), followed by TTN (n=4), FHL1 (n=3), FLNC (n=2), BAG3 (n=2), MYOT (n=1) and HNRNPA2B1 (n=1). Among the DM families, DYSF (n=6) and GNE (n=4) were the most frequent disease-causative genes, followed by MYH7 (n=2), NEB (n=2), TTN (n=1), ANO5 (n=1), RYR1 (n=1), and DNM2 (n=1). The severity of motor impairment occurred in a similar proportion between distal myopathy and myofibrillar myopathy, being more pronounced in patients with variants in the BAG3, FHL1, MYH7 and GNE. Myofibrillar myopathies had a higher proportion and severity of cardiorespiratory impairment. DES myopathy was related to more frequent heart disease. Variants in TTN were associated with different phenotypes, but the presence of respiratory failure and heart disease. BAG3 myopathy was an early subgroup with axial contractures, cardiac and respiratory impairment. FHL1 patients presented with reducing body myopathy in childhood, inflammatory histology, and a scapuloperoneal phenotype among female carriers. We observed the Miyoshi phenotype in dysferlinopathy and anoctaminopathy. GNE myopathy, in turn, presented with involvement of the anterior leg muscles and vacuolar histology. Some genes related to congenital myopathies were also causes of DM (TTN, RYR1, DNM2) and were confounded with hereditary neuropathy, due to early findings and distal deformities. Introdução: As miopatias inflamatórias (MI) são um grupo heterogêneo de condições caracterizadas principalmente por fraqueza muscular e infiltrado inflamatório na histologia muscular. A classificação das MI tem evoluído ao longo dos anos e recentemente uma condição denominada Miopatia Inflamatória com Alterações Mitocondriais (MI-Mito) foi descrita em poucas séries de casos, permanecendo incerto se é uma variante da Miosite por Corpos de Inclusão (MCI) ou uma forma isolada de MI. A Ressonância Nuclear Magnética (RNM) muscular tem adquirido crescente importância na avaliação de pacientes com MI, porém ainda não foi descrita sua realização em pacientes com MI-Mito. Objetivos: Analisar as características clínicas e da RNM de corpo inteiro em pacientes com MI-Mito, comparando com indivíduos portadores de MCI. Métodos: De forma retrospectiva, foram incluídos 14 pacientes com MI-Mito e 10 pacientes com MCI diagnosticados em dois centros de patologia muscular, sendo os casos definidos de acordo com a histologia muscular. MI-Mito foi determinada por infiltrado inflamatório endomisial, presença de pelo menos 1% de fibras com deficiência de Citocromo C Oxidase e ausência de vacúolos marginados. Por outro lado, MCI foi definida em pacientes com alterações musculares distróficas, infiltrado inflamatório endomisial e vacúolos marginados. Os pacientes foram submetidos a avaliação clínica e RNM de corpo inteiro para determinar presença de edema, lipossubstituição e atrofia em vários músculos. Resultados: A idade média dos pacientes com MI-Mito foi significativamente menor que participantes com MCI (56,9 anos x 69,1 anos, respectivamente, p=0,01). Foram encontrados mais pacientes do sexo feminino no grupo MI-Mito quando comparado ao grupo com MCI (71,5% e 20%, respectivamente, p=0,01). Clinicamente, pacientes com MI-Mito se apresentaram de forma mais heterogênea, com fenótipo oligossintomático (28,6%), fenótipo de fraqueza muscular proximal (35,7%), fenótipo de fraqueza muscular proeminente em quadríceps ou flexores dos dedos (28,6%) e fenótipo de fraqueza muscular difusa (7,1%). Por outro lado, indivíduos com MCI demonstraram presença significativamente maior do fenótipo de fraqueza muscular proeminente em quadríceps ou flexores dos dedos quando comparados àqueles com MI-Mito (90% e 28,6%, respectivamente, p<0,01). Com relação aos achados da RNM de corpo inteiro, foram encontradas diversas semelhanças nos vários parâmetros radiológicos avaliados em pacientes com MI-Mito e MCI. Os músculos com maior grau médio de lipossubstituição nos dois grupos foram o quadríceps e gastrocnêmio medial. As alterações radiológicas foram assimétricas na maior parte dos pacientes com MI-Mito e MCI (71,4% e 80%, respectivamente, p=0,63). Não houve diferença estatisticamente significativa na presença de edema, atrofia ou lipossubstituição na ampla maioria dos músculos avaliados em pacientes com MI-Mito e MCI. A maior parte dos pacientes com MI-Mito e MCI apresentaram o padrão radiológico do músculo reto femoral relativamente poupado quando comparado aos outros músculos do quadríceps (92,9% e 80% respectivamente, p=0,55). Os participantes também demonstraram em sua maioria comprometimento radiológico dos músculos flexores profundos dos dedos, havendo uma tendência de maior acometimento em paciente com MCI em comparação àqueles com MI-Mito (100% e 64,3% respectivamente, p=0,05). Conclusão: O nosso estudo sugere que as características radiológicas na RNM muscular de corpo inteiro são semelhantes entre a MI-Mito e MCI. Esses dados reforçam a hipótese que a MI-Mito pode fazer parte do espectro de apresentação da MCI. Comparativamente, a MI-Mito se apresenta mais frequentemente no sexo feminino, em pacientes relativamente mais jovens e com um fenótipo clínico mais heterogêneo que a MCIIntroduction: Inflammatory myopathies (IM) are a heterogeneous group of conditions characterized mainly by muscle weakness and inflammatory infiltrates in muscle histology. The classification of IM has evolved over the years and recently a condition called Inflammatory Myopathy with Mitochondrial Pathology (IM-Mito) has been described in a few case series. It remains uncertain whether IM-Mito is a variant of Inclusion Body Myositis (IBM) or an isolated form of IM. Muscle Magnetic Resonance Imaging (MRI) has gained increasing importance in the evaluation of patients with IM, but its application in patients with IM-Mito has not yet been described. Objectives: To analyze clinical and whole-body MRI features in patients with IM-Mito compared to individuals with IBM. Methods: Retrospectively, 14 patients with IM-Mito and 10 patients with IBM diagnosed at two muscle pathology centers were included. Cases were defined according to muscle histology. IM-Mito was determined by endomysial inflammatory infiltrate, presence of at least 1% of Cytochrome C Oxidase negative fibers, and absence of rimmed vacuoles. On the other hand, IBM was defined in patients with dystrophic muscular abnormalities, endomysial inflammatory infiltrate, and rimmed vacuoles. Patients underwent clinical evaluation and whole-body MRI to determine the presence of edema, fatty replacement and atrophy in various muscles. Results: Mean age of patients with IM-Mito was significantly lower than participants with MCI (56.9 years vs 69.1 years, respectively, p=0.01). A higher presence of female patients was found in participants with IM-Mito when compared to the group with IBM (71.5% and 20%, respectively, p=0.01). Clinically, patients with IM-Mito presented more heterogeneously with oligosymptomatic phenotype (28.6%), proximal muscle weakness phenotype (35.7%), prominent quadriceps or finger flexors muscle weakness phenotype (28.6%) and diffuse muscle weakness phenotype (7.1%). On the other hand, individuals with IBM demonstrated significantly greater presence of prominent muscle weakness in quadriceps or finger flexors when compared to those with IM-Mito (90% and 28.6%, respectively, p<0.01). Regarding whole-body MRI findings, several similarities were found in radiological parameters evaluated in patients with IM-Mito and IBM. Muscles with highest average degree of fatty replacement in both groups were quadriceps and medial gastrocnemius. Radiological abnormalities were asymmetric in most patients with IM-Mito and IBM (71.4% and 80%, respectively, p=0.63). There was no statistically significant difference in edema, muscle atrophy or fatty replacement in the majority of muscles evaluated in patients with IM-Mito and IBM. Most patients with IM-Mito and IBM showed radiological pattern of rectus femoris muscle relatively spared when compared to other quadriceps muscles (92.9% and 80%, respectively, p=0.55). Most participants also demonstrated radiological involvement of flexor digitorum profundus muscles, with a trend towards greater involvement in patients with IBM compared to those with IM-Mito (100% and 64.3%, respectively, p=0.05). Conclusion: Our study showed that radiological features in whole-body MRI are similar between IM-Mito and IBM. These data support the hypothesis that IM-Mito may be part of the presentation spectrum of IBM. Electrophysiological study of neuromuscular junction in congenital myasthenic syndromes, congenital myopathies, and chronic progressive external ophthalmoplegia
Este estudo foi planejado para avaliar a sensibilidade, especificidade e acurácia dos parâmetros do jitter combinados com Estimulação Repetitiva (ER) em pacientes com Síndrome Miastênica Congênita (SMC), Oftalmoparesia Externa Progressiva Crônica (OEPC) e Miopatias Estruturais Congênitas (MCE). Jitter foi obtido por Eletrodo de Agulha Concêntrica (EAC), durante ativação voluntária do músculo Orbicularis Oculi em 21 pacientes com SMC, 20 pacientes com OEPC, 18 pacientes com MCE e 14 pacientes controles saudáveis. ER (3Hz) foi realizada em seis diferentes músculos (Abductor Digiti Minimi, Tibialis Anterior, Trapezius, Deltoideus, Nasalis e OO). ER demonstrou-se alterada em 90,5% dos pacientes com SMC e em apenas 1 paciente fora desse grupo (um paciente com MCE). Jitter foi anormal em 95,2% dos pacientes com SMC, 20% dos pacientes com OEPC e 11,1% dos pacientes com MCE. Nenhum paciente com OEPC ou CME apresentou jitter médio superior a 53.6 s ou mais de 30% de pares com jitter individual anormal (> 45 s). Nenhum paciente com OEPC ou MCE com jitter alterado apresentou anormalidades concomitantes na ER. Fica demonstrado que a ER e avaliação do Jitter são técnicas valiosas para estudo de anormalidades da junção neuromuscular em pacientes com SMC. Valores de jitter superiores a 53.6 s ou a presença de mais de 30% de pares com jitter individual anormal (> 45 s) sugerem o diagnóstico de uma doença primária da junção neuromuscular na nossa amostra, favorecendo o diagnóstico de SMCThis study was designed to analyze the sensitivity, specificity, and accuracy of jitter parameters combined with repetitive nerve stimulation (RNS) in congenital myasthenic syndrome (CMS), chronic progressive external ophthalmoplegia (CPEO), and congenital myopathies (CM). Jitter was obtained with a concentric needle electrode during voluntary activation of the Orbicularis Oculi muscle in CMS (n=21), CPEO (n=20), and CM (n=18) patients and in controls (n=14). RNS (3Hz) was performed in six different muscles for all patients (Abductor Digiti Minimi, Tibialis Anterior, upper Trapezius, Deltoideus, Orbicularis Oculi (OO), and Nasalis). RNS was abnormal in 90.5% of CMS patients and in only one CM patient. Jitter was abnormal in 95.2% of CMS, 20% of CPEO, and 11.1% of CM patients. No patient with CPEO or CM presented a mean jitter higher than 53.6 s or more than 30% abnormal individual jitter (> 45 s). No patient with CPEO or CM and mild abnormal jitter values presented an abnormal decrement. Jitter and RNS assessment are valuable tools for diagnosing neuromuscular transmission abnormalities in CMS patients.
