The study of Dantean prophetism and of its relationship with the “she-wolf” (that is greed-lust for power), is carried out through the analysis of the most intensely prophetic moments of the poem: from the eschatological, political and social aspects of the background, through the intense reminiscences from Ezekiel and Apocalypse re-contextualized in Purgatory XIX and XX, to the great eschatological-political visions of Purgatory XXXII and XXXIII, to the quiet and sorrowful hope expressed in Beatrice's last cantos. Dante's prophetic cry is inseparable from the different images of the poet inside the Comedy : the pilgrim poet (the agens); the scribe poet, assigned to reproduce the experiences lived by the pilgrim; the author, that is the prophet poet, who also says “I” and intervenes from the present of the writing to teach the reader and execrate the vices of led astray Christianity. Then Dante's position is examined between the ancient auctores- poets and the auctoritas due to the holy books of the Old and New Testament
Sant'Ambrogio nella letteratura religiosa slava orientale del Sei e Settecento : per uno studio preliminare delle citazioni ambrosiane nell'omiletica rutena barocca
The aim of this study is to investigate the reception of Ambrose in the writings of three Ukrainian baroque preachers, viz. Ioannikij Galjatovs’kyj, Antonij Radyvylovs’kyj and Dimitrij Rostovskij. The method that has been adopted in the presentation of the results of our survey can be outlined as follows. First, we focused on the thematic aspects of the process of reception, which generally fall into two categories: 1. Exegetical themes, based on motives drawn from the Scriptures; 2. Philosophical-doctrinal themes. Furthermore, for all the writers which we examined we tried to determine whether an author made a direct or indirect usage of Ambrose’s oeuvre. Indirect use is generally to be ascribed to the influence of either exegetic Comments on the Bible, such as those of Cornelius A Lapide and Didacus de Baeza, or of emblematic literature produced in the West, such as Hemannus Hugo’s Pia desideria (1624).
A second question to be addressed is how the intellectual and ecclesiastical context of the Ukrainian lands, whose distinctive features were pluriconfessionalism and the close ties to Western Renaissance and Baroque, influenced our authors in the way they made use of Ambrose, especially as far as the relationship between Orthodoxy and Catholicism is concerned
Fundal pressure in the second stage of labor: time to face the invisible enemy
Sir,
We read with interest the paper written by Hasegawa and colleagues analyzing the obstetric factors associated with uterine rupture [1]. The authors found that fundal pressure in the second stage (Kristeller’s maneuver) was applied in approximately one every four women with uterine rupture, and in one every three women with rup- ture on unscarred uterus.
Kristeller’s maneuver remains one of the most contro- versial and underreported obstetric practices [2,3]. Very little evidence exist on the benefits and safety of the Kristeller’s maneuver. Recently, the World Health Organization issued a document stating fundal pressure in the second stage is not recommended [4]. The authors developing the document declared to have “serious con- cerns about the potential for harm to mother and baby with this procedure” [4]. In addition to these concerns, one of the main problems in research performed on the maneuver is that it is frequently unreported. In a recent study published by our group on Kristeller’s maneuver, we found that in 90% of cases it is not reported in the women’s file [5]. We believe that under- or the total absence of reporting of the maneuver is as challenging and risky as the maneuver itself. Over the past 5 years, in our University Hospital we aimed at reducing the rate of Kristeller’s maneuver and to increase its reporting using a systematic approach. This approach included conducting research to investigate the risks of the maneuver on the pelvic floor, increasing caregivers’ awareness on the con- cerns and evidence regarding the maneuver, and audit- ing the incidence of the application of the maneuver and the rate of its reporting over time
Transperineal ultrasound assessment of maternal pelvic floor at term and fetal head engagement
Objectives: To evaluate the association between pelvic floor dimensions in nulliparous women at term and fetal head engagement, as assessed by transperineal ultrasound. Methods: This was a prospective observational study of nulliparous women at term. Before the onset of labor, transperineal ultrasound was used to measure the anteroposterior diameter (APD) of the levator hiatus and the angle of progression (AoP) at rest, on maximum pelvic floor muscle contraction and on maximum Valsalva maneuver (before and after visual feedback). We assessed the correlation between pelvic floor static and dynamic dimensions (levator hiatal APD and levator ani muscle coactivation) and AoP, which is an objective index of fetal head engagement. Results: In total, 282 women were included in the analysis. Among these, 211 (74.8%) women had a vaginal delivery while 71 (25.2%) had a Cesarean delivery. AoP was narrower in the Cesarean-delivery group at rest, on maximum pelvic floor muscle contraction and on maximum Valsalva, whereas no differences in levator hiatal APD were found between the two groups. We found a negative correlation between levator hiatal APD at rest, on maximum pelvic floor muscle contraction and on maximum Valsalva and the duration of the second stage of labor. There was a positive correlation between AoP and levator hiatal APD on maximum Valsalva maneuver after visual feedback (r = 0.15, P = 0.01). Women with levator ani muscle contraction on Valsalva maneuver (i.e. coactivation), both pre and post visual feedback, had a narrower AoP at rest and on maximum Valsalva. After visual feedback, women with levator ani muscle coactivation had a longer second stage of labor than did those without (80.8 ± 61.4 min vs 62.9 ± 43.4 min (P = 0.04)). Conclusions: Smaller pelvic floor dimensions and levator ani muscle coactivation are associated with higher fetal head station and with a longer second stage of labor in nulliparous women at term.
The contribution considers the development of the possibility of existence of vulgar poets and authors (beside and in competition with the classics, mostly Latin) in Dante's works prior to the Comedy.
Then it argues the reasons why Dante, in the Comedy, declares himself a poet but he never calls himself an author (such is particularly Virgil)
The role of a new three-dimensional ultrasound technique in the diagnosis of levator ani muscle avulsion
Aims: To evaluate the intermethod agreement between the tomographic ultrasound imaging (TUI), considered as the gold standard, and the OmniView-VCI in the diagnosis of levator ani muscle (LAM) avulsion and in the measurement of levator-urethral gap (LUG). Methods: We acquired dynamic 4D transperineal ultrasound volumes from 114 women. Each data set was analyzed on maximal pelvic floor contraction by TUI and OmniView-VCI techniques to check for LAM avulsion. Moreover, we measured LUG using both TUI and OmniView-VCI, twice by an operator and once by another to assess intraobserver and interobserver reproducibility. Reproducibility and intermethod agreement were studied by means of intraclass correlation coefficient (ICC) and Cohen's kappa coefficient. Results: In the diagnosis of ani levator avulsion, the two techniques showed a good agreement (Cohen's κ = 0.691, 95% confidence interval [CI], 0.522-0.860; P <.001); we also reported a good intraobserver and interobserver agreement (Cohen's κ = 0.738, 95% CI, 0.597-0.879; P <.001, and Cohen's κ = 0.864, 95% CI, 0.750-0.978; P <.001, respectively). LUG measurements by OmniView-VCI technique showed high intraobserver (ICC 0.895; 95% CI, 0.866-0.918) and interobserver (ICC 0.821; 95% CI, 0.774-0.858) reproducibility. High intermethod agreement was demonstrated between the two methods (ICC 0.813; 95% CI, 0.764-0.853). The area under the receiver-operating characteristic curve of LUG in predicting avulsion was 0.931 (0.868-0.994, 95% CI; P <.001) with 24 mm showing the best sensitivity (82%) and specificity (97%). Conclusions: OmniView-VCI is a reliable method for LUG measurement and for levator avulsion diagnosis
Green Open Access added to TU Delft Institutional Repository 'You share, we take care!' - Taverne project https://www.openaccess.nl/en/you-share-we-take-care Otherwise as indicated in the copyright section: the publisher is the copyright holder of this work and the author uses the Dutch legislation to make this work public.Housing Institutions & Governanc
Maternal hemodynamic assessment by USCOM® device in the first trimester of pregnancy
Background: Preeclampsia is a main obstetric disease and could be caused by the inability of the woman hemodynamic system to face the cardiovascular effort required by gestation. Objective: The aim of the present study is to evaluate the correlation between maternal hemodynamic parameters detected by UltraSonic Cardiac Output Monitor (USCOM®) and ultrasound or biochemical parameters in women during the first-trimester screening of chromosomal abnormalities. Methods: This was a prospective observational study with 162 women enrolled during the first-trimester ultrasound, recording demographic, biochemical and ultrasound data, including the pulsatility index of uterine arteries (UTPI). Hemodynamic indices were obtained using the USCOM® system. We also analyzed the outcomes of pregnancy of those women who delivered in our clinic. Results: As for the correlation between biochemical and ultrasound parameters, pregnancy-associated plasma protein A (PAPP-A) relates inversely with the mean UTPI (r s −0.298, p <.001). We also reported a statistically significant correlation between the mean UTPI and the maternal haemodynamic parameters detected by USCOM® (higher values of mean UTPI are associated with lower values of indexed cardiac output, CO, and higher indexed peripheral resistances, TVR). When comparing women with higher UTPI and TVR or lower CO to patients who had regular values of these parameters, we noticed that between these groups there were significant differences in all the remaining hemodynamic parameters, including inotropy index and stroke volume, as well as in the biochemical values of PAPP-A. Analyzing postpartum data, if we combine the onset of hypertensive disorders and fetal growth restriction, the most predictive parameter is indexed TVR. Conclusion: Our study confirmed the correlation between PAPP-A and placental function, expressed through the determination of the pulsatility index of the uterine arteries. Moreover, we found a clear correlation between biochemical markers, placental function and maternal hemodynamics, since the first trimester of pregnancy. The inclusion of maternal hemodynamic evaluation could be useful in the screening protocols of preeclampsia. If we combine the onset of hypertensive disorders and of fetal growth restriction, the most predictive parameter is indexed total vascular resistances
A Review of "Housing wealth and welfare", By Caroline Dewilde and Richard Ronald
Book review: Housing wealth and welfare, By Caroline Dewilde and Richard Ronald, Cheltenham, Edward Elgar Publishing, 2017, ISBN 978 78536 095 4.This is a very interesting edited volume comprising an introduction chapter reflecting on so-called housing asset-based welfare and nine chapters written by a nice selection of authors. It is a more than worthwhile joint effort of two excellent ERC laurates who have both been working on the topic for years which is demonstrated in the very comprehensive overview of the state of the art in housing wealth and welfare. They explain different models across time and across Europe: some models are more familialised, relying on family capital, while others are more financialised, relying on mortgages. The introduction is followed by a multi-angle, multi-country collection of well-written chapters. And last but not least, there is an epilogue by John Doling, ‘the father’ of housing asset-based welfare.Green Open Access added to TU Delft Institutional Repository 'You share, we take care!' - Taverne project https://www.openaccess.nl/en/you-share-we-take-care Otherwise as indicated in the copyright section: the publisher is the copyright holder of this work and the author uses the Dutch legislation to make this work public.OLD Housing System
