The Severe Respiratory Insufficiency Application (SRI App): a pilot randomised controlled trial
An application (app) for the Severe Respiratory Insufficiency Questionnaire (SRI) has been designed and developed for mobile devices. In a randomised controlled trial comprising 60 patients with chronic respiratory failure, the app was compared with the classic paper SRI. Thereby, it was shown that the SRI app is a practical tool that is well accepted. Missing values can be completely avoided by using the SRI app. Finally, reliability, convergent and discriminant validities were established. Accordingly, for the individual SRI subscales, Cronbach's alpha ranged between 0.56 and 0.81 (app) and between 0.54 and 0.83 (paper), respectively. The multilingual SRI app is accessible free of charge for non-profit research purposes
Epidemiology of Neuralgic Amyotrophy—A Retrospective Analysis of Data From a Large German Health Insurance Company
ABSTRACT Introduction/Aims Neuralgic amyotrophy (NA, Parsonage–Turner syndrome) is a common, multifocal, autoimmune inflammatory disease that predominantly affects proximal nerve segments of the shoulder girdle. Despite the high incidence of 100/100,000, epidemiologic data based on larger cohorts are still lacking. This study aims to address this issue. Methods A retrospective evaluation of billing data from an average of approximately 26,000,000 insured persons of a large German health insurance company was performed from 2013 to 2022. In addition to descriptive statistical methods, regression analyses were carried out. Results The incidence and prevalence of NA were 7.7–12.8 and 19.7–21.6 per 100,000 people, respectively. During the study period, a steady decline in incidence and, to a lesser extent, prevalence was observed. The diagnosis was made significantly more frequently in the first quarter of the year than in any other quarter. The prevalence of a simultaneously coded diaphragmatic paresis (indicating the involvement of the phrenic nerve) was 0.45%. Discussion Compared with prospectively collected data, the data in this study revealed an approximately 90% lower incidence of NA. It is likely that the majority of cases are not correctly diagnosed and are therefore not captured in the billing data. The involvement of the phrenic nerve in patients with NA also appears not to be recognized in most cases. According to the current pathophysiological model of NA, an immunological trigger is necessary to initiate the disease process. Our data support the accuracy of this model
Lebensqualität im Langzeitverlauf nach prolongiertem Weaning in Abhängigkeit vom Weaning-Status
Zusammenfassung Einleitung Die Prognose von Patienten im prolongierten Weaning ist insbesondere in Abhängigkeit vom Weaning-Outcome eingeschränkt. Daten zur gesundheitsbezogenen Lebensqualität nach prolongiertem Weaning im Langzeitverlauf existieren bis heute jedoch nur spärlich. Methodik Es wurde die gesundheitsbezogene Lebensqualität nach prolongiertem Weaning mittels SRI-Fragebogen erfasst (SRI = Severe Respiratory Insufficiency Questionnaire). Ergebnisse Insgesamt haben 39 von 83 Patienten mit prolongiertem Weaning den SRI-Fragebogen ausgefüllt. Der zeitliche Abstand zur Entlassung aus dem Weaning-Zentrum betrug im Median 3,5 Jahre (IQR 2,4–4,5 Jahre). In der Gesamtgruppe lag der Median des Summenscores bei 56,4 (IQR 38,8–73,5). Patienten mit invasiver außerklinischer Beatmung (N=15) hatten eine deutliche schlechtere Lebensqualität im Summenscore als Patienten mit erfolgreicher Entwöhnung ohne Fortsetzung der maschinellen Beatmung (N=13) und solchen mit Fortsetzung einer nicht-invasiven Beatmung (N=11); Kruskal-Wallis-Test: H (2, N=39) = 7,875446; P=0,0195. Statistisch signifikante Unterschiede im Sinne einer schlechteren Lebensqualität bei Patienten mit invasiver außerklinischer Beatmung zeigten sich insbesondere auf den Subdomänen der sozialen Beziehungen (P=0,0325), der krankheitsbezogenen Ängste (P=0,0096) und des psychischen Befindens (P=0,0079). Schlussfolgerung Patienten mit erfolglosem prolongierten Weaning und invasiver außerklinischer Langzeitbeatmung haben eine deutlich schlechtere gesundheitsbezogene Lebensqualität als Patienten mit erfolgreicher Entwöhnung vom Respirator. Weitere Studien mit höheren Fallzahlen sind notwendig, um weitere Einflussgrößen auf die gesundheitsbezogene Lebensqualität im prolongierten Weaning zu untersuchen
