Case report: Robotically-treated spontaneous interstitial pregnancy on tubal stump
To report a rare case of a right interstitial pregnancy spontaneously occurring in a patient who had previously undergone homolateral salpingo-oophorectomy, and to propose possible explanations for the mechanisms involved in the genesis of this rare scenario. A 32-year-old G3P1 female presented to our emergency room with symptoms related to a suspected ectopic interstitial pregnancy managed in another hospital using a conservative pharmacological approach. After discussing the risks, firstly she underwent a transvaginal ultrasound examination, then a diagnostic hysteroscopy to clarify the unclear ultrasound finding, followed by a successful robot-assisted laparoscopic cornual resection. Hysteroscopy demonstrated an empty uterine cavity, confirming the suspect of pregnancy localization into the interstitial portion of the tubal stump. Through the robot-assisted laparoscopic approach, all the trophoblastic tissue was removed without causing significant damage to the surrounding myometrium and preserving the patient’s fertility. No post-operative complications were recorded. The robotic approach successfully allowed the cornual resection, with minimal blood loss and optimal suturing of the uterine defect. Although our knowledge is still limited, it is possible that the pregnancy nested in the tubal residue after being properly fertilized into the intact tube. However, it cannot be ruled out that there have been remodeling phenomena of the tubal residue so that it has acquired the ability to intercept the oocyte

An Unfinished Errata. The “twisted” poems of Rafał Wojaczek
The article is a critical analysis of a collection of unpublished texts by Rafał Wojaczek entitled Nie te czasy. Utwory nieznane [Not These Times. Unknown Poems]. In the first part of the paper, the author places the book within a broader context, revealing the specific nature of its reception. He regards the analyzed volume as an element of an ongoing process of complementing the overall work of the Wroclavian artist that began a few years ago. In the second part, the author focuses on the editing of selected poems. An in-depth comparative analysis of published versions and archival records makes it possible to detect numerous editorial mistakes made during the preparation of the volume, concerning both the question of the correct reading of the autographs and typescripts, and the reconstruction of the creative process necessary for understanding the available archival material. The author of the paper puts forward a claim that this supplementation of the poet’s canon is dangerous, for it distorts not only the poetic oeuvre of Rafał Wojaczek, but also the contents of the archive itself. The scarce editorial commentary justifying the final shape of the poems on the basis of the adopted criteria of evaluating the archival records cannot efficiently replace the sophisticated critical apparatus obligatory in such cases, and it turns out to be not only insufficient, but also contrary to the facts.
Pregnancy-Preserving Laparoscopic Treatment of Acute Hemoperitoneum Following Lutein Cyst Rupture in Early Gestation
Uterine Tumors Resembling Ovarian Sex Cord Tumors (UTROSCTs): A Scoping Review of 511 Cases, Including 2 New Cases
Uterine Tumors Resembling Ovarian Sex Cord Tumors (UTROSCTs) are rare uterine mesenchymal neoplasms with uncertain biological potential. These tumors, which affect both premenopausal and postmenopausal women, usually have a benign clinical course. Nevertheless, local recurrences and distant metastases have been described. By analyzing 511 cases retrieved from individual reports and cases series, we provide here the most comprehensive overview of UTROSCT cases available in the literature, supplemented by two new cases of UTROSCTs. Case 1 was an asymptomatic 31-year-old woman who underwent a laparoscopic resection of a presumed leiomyoma. Case 2 was a 58-year-old postmenopausal woman with abnormal vaginal bleeding who underwent an outpatient hysteroscopic biopsy of a suspicious endometrial area. In both cases, immunohistochemical positivity for Calretinin and Inhibin was noted, typical for a sex cord differentiation. In both cases, total laparoscopic hysterectomy with bilateral salpingo-oophorectomy was performed. In light of the available literature, no pathognomonic clinical or imaging finding can be attributed to UTROSCT. Patients usually present with abnormal uterine bleeding or pelvic discomfort, but 20% of them are asymptomatic. In most cases, a simple hysterectomy appears to be the appropriate treatment, but for women who wish to become pregnant, uterus-preserving approaches should be discussed after excluding risk factors. Age, tumor size, lymphovascular space invasion, nuclear atypia, and cervical involvement are not reliable prognostic factors in UTROSCT. The current research suggests that aggressive cases (with extrauterine spread or recurrence) can be identified based on a distinct genetic and immunohistochemical phenotype. For instance, UTROSCTs characterized by GREB1::NCOA1-3 fusions and PD-L1 molecule expression appear to be predisposed to more aggressive behaviors and recurrence, with GREB1::NCOA2 being the most common gene fusion in recurrent tumors. Hence, redefining the criteria for UTROSCTs may allow a better selection of women suitable for fertility-sparing treatments or requiring more aggressive treatments in the future
The Translational Role of miRNA in Polycystic Ovary Syndrome: From Bench to Bedside—A Systematic Literature Review
MicroRNAs (miRNAs) are small, non-coding RNAs that are essential for the regulation of post-transcriptional gene expression during tissue development and differentiation. They are involved in the regulation of manifold metabolic and hormonal processes and, within the female reproductive tract, in oocyte maturation and folliculogenesis. Altered miRNA levels have been observed in oncological and inflammatory diseases, diabetes or polycystic ovary syndrome (PCOS). Therefore, miRNAs are proving to be promising potential biomarkers. In women with PCOS, circulating miRNAs can be obtained from whole blood, serum, plasma, urine, and follicular fluid. Our systematic review summarizes data from 2010–2021 on miRNA expression in granulosa and theca cells; the relationship between miRNAs, hormonal changes, glucose and lipid metabolism in women with PCOS; and the potential role of altered miRNAs in fertility (oocyte quality) in PCOS. Furthermore, we discuss miRNAs as a potential therapeutic target in PCOS and as a diagnostic marker for PCOS
Rafał Wojaczek: Advertisement in the Light of a Legend
The article entitled “Rafał Wojaczek: Advertisement in the Light of a Legend” is the first part of a draft which tries to describe a breakthrough moment, in which the biography of Rafał Wojaczek coincides with the beginning of his literary output. The author makes a central point of the formula: “advertisement is the engine of trade” (appearing in the notebook collecting his early poetical notes from the period of studies at Polish philology), which is placed in contrast to the statement by Julian Przyboś: “the art of a poem is the engine of poetry”. The author, examining the earlier editions of Wojaczek’s poems explains (using the sociological and philosophical apparatus) the mechanisms conditioning the phenomenon of the reception of his poetry. At the same time, he launches the notion of pre-reception, which—being a labile notion, situated in the borderland between reception and advertise-ment—indicates a dual position of an object, to which the pre-reception refers (presence and absence at the same time). This, in turn, announces the poetic strategy of Wojaczek, embracing the form and the subject matter of a poem (expressiveness against a precise structure of a poem), and foretells a subsequent part focusing on the question of the actual reception, taking into consideration the somatic character of his output as well as its autothematic involvement.</span
RAFAŁ WOJACZEK’S “SICKNESS UNTO NON-EXISTENCE”
The paper focuses upon the philosophical message rendered by Rafał Wojaczek’s poetry. The author regards
the issue of non‑existence as the central theme of Wojaczek’s creative output, and analyses it in the context of
existentialist philosophy, referring to Martin Heidegger’s concepts in particular. The following four aspects of
the leading idea of non‑existence are identified and discussed in the article: non‑existence understood as death
which not only constitutes everyone’s destiny but also validates individual life; non‑existence as nothingness
which is disclosed in anxiety, with the retreat of being in its entirety; non‑existence as a blurred personal
identity; and, finally, non‑existence as a condition of creativity. The unique form and style of this poetry are
considered to play an essential role in conveying the metaphysical content
