The impact of breast reduction surgery on the ability to breastfeed
Hintergrund: Brustverkleinerungsoperationen werden zunehmend bei Frauen im gebärfähigen Alter durchgeführt. Aufgrund der anatomischen Veränderungen durch unterschiedliche Operationstechniken bestehen Bedenken hinsichtlich der späteren Stillfähigkeit. Ziel: Ziel dieser Arbeit war es, eine systematische Literaturrecherche zu den Auswirkungen von Brustverkleinerungsoperationen auf die Stillfähigkeit durchzuführen und verschiedene Operationstechniken sowie Vergleichsgruppen zu analysieren. Methodik: Die systematische Literaturrecherche erfolgte zwischen Januar und Juli 2025 anhand vordefinierter Einschlusskriterien. Es wurden acht Kohortenstudien, eine Querschnittsstudie und eine systematische Übersichtsarbeit einbezogen. Die methodische Qualität wurde mit den Checklisten von STROBE, CASP, PRISMA und ROBIS bewertet. Der Zusammenhang zwischen Operationstechnik, Studiendesign und Stillerfolg wurde analysiert. Ergebnisse: Die Mehrheit der Studien zeigte, dass Brustverkleinerungen die Stillfähigkeit beeinträchtigen können, wobei die Auswirkungen stark von der verwendeten Technikabhängen. Techniken, die das subareoläre Drüsengewebe erhalten, zeigten tendenziell bessere Stillraten. Allerdings variierten die Studien stark in ihrer Qualität und der Definition von „erfolgreichem Stillen“. Schlussfolgerung: Brustverkleinerungsoperationen können die Stillfähigkeit einschränken, insbesondere wenn die Milchgänge oder der Mamillen-Areola-Komplex beeinträchtigt werden. Frauen im gebärfähigen Alter sollten präoperativ umfassend über mögliche Auswirkungen auf das Stillen aufgeklärt werden.Background: Breast reduction surgery is increasingly performed on women of childbearing age. Given the anatomical alterations associated with various surgical techniques, concerns exist regarding the impact on future breastfeeding ability. Objective: To analyze and compare existing studies on the effect of breast reduction surgery on the ability to breastfeed, considering different surgical techniques and comparison groups. Methods: A systematic literature search was conducted between January and July 2025 using predefined inclusion criteria. Eight cohort studies, one cross-sectional study, and one systematic review wit meta-analysis were included. The methodological quality of the studies was assessed using the STROBE, CASP, PRISMA, and ROBIS checklists. Breastfeeding outcomes were evaluated in relation to surgical techniques and study design. Results: Most studies showed that breast reduction surgery can impair breastfeeding ability, with variation depending on the surgical technique used. Techniques that preserve the subareolar parenchyma tend to be associated with better outcomes. However, study quality and definitions of "successful breastfeeding" varied considerably. Conclusion: Breast reduction surgery may reduce the ability to breastfeed, especially when the integrity of milk ducts or the nipple-areolar complex is compromised. The River Lea 1571-1767: a river navigation prior to canalisation
PhDIn pre-industrial England river navigations were subject
to improvement by canalisation, the introduction of
artificial navigation cuts and pound locks. Along the Lea
this did not happen until 1767. Before that the navigation,
except for one short period, relied upon a less efficient
technology, the provision of flashes from fishing weirs,
turnpikes and mills. Yet the river was still an important
transport route, particularly for the supply of grain, meal
and malt to London. It had been this during the mediaeval
period, but not by the middle of the sixteenth century. Then
in 1571 the City of London sponsored legislation to construct
a canal from the Lea to London. Parliamentary opposition
thwarted the original ambitious scheme, so two cheaper,
shorter canals were considered, but never built. Instead
an ambitious and unique river improvement scheme was
successfully implemented. This experimental navigation
(reducing reliance on flashes to a minimum) survived 20
years, before persistent and violent opposition from land
carriers closed it. A Star Chamber case upheld the rights
of the bargemen, but the experimental navigation was not
restored. Instead the traditional flash-lock navigation
re-appeared, and was to last, with only minor improvementg
until 1767. In the intervening years the navigation
continued to expand and prosper., This despite the admitted
problems of relying on flashes and tides, and despite a
series of major disputes with the New River Companyq the
millers, fishermen and riparian land-owners. Conflict there
certainly was, but also compromise. Ultimately all parties
were prepared to accept the conflicting rights of other
users, provided they could defend their own. commissions
of Sewers provided an effective administrative forum to
effect and authorise such compromise, even after the
appointment of a body of Trustees in 1739. That the Lea
was an adequate navigation before canalisation, despite
a 'second-best' technology and an unpaid part-time administrative
structure means' that a valid comparison with the
concept of Appropiate Technology, discussed in modern-day
development theory, is possible

Effects of Anorexia nervosa in women of reproductive age on fertility, pregnancy course, birth outcomes and postpartum period
Hintergrund: Essstörungen, insbesondere Anorexia nervosa (AN), treten zunehmend häufiger auf und betreffen vor allem junge Frauen im gebärfähigen Alter. Als eine der am weitesten verbreiteten psychiatrischen Erkrankungen in dieser Lebensphase hat AN bedeutende Auswirkungen auf die geburtshilfliche Versorgung – insbesondere im Hinblick auf Fertilität, Schwangerschaft, Geburt und die postpartale Phase. Ziel: Ziel dieser Arbeit ist es, die Auswirkungen von Anorexia nervosa auf die diese Auswirkungen systematisch darzustellen. Im Mittelpunkt stehen dabei Fertilitätsstörungen, Schwangerschaftsverläufe, geburtshilfliche Komplikationen sowie Herausforderungen des postpartalen Zeitraums. Darüber hinaus wird die Rolle von Hebammen und weiteren Gesundheitsberufen in der Betreuung betroffener Frauen beleuchtet. Methodik: Mittels systematischer Literaturrecherche wurden zwischen Oktober 2024 und Juni 2025 relevante Studien aus den Datenbanken PubMed, CINAHL, Cochrane und Psychiatrist identifiziert. Die Auswahl erfolgte anhand definierter Einschlusskriterien sowie einer Bewertung der methodischen Qualität. Insgesamt wurden neun Studien in die Analyse aufgenommen. Ergebnisse: Die Analyse zeigt, dass AN mit eingeschränkter Fertilität, ungeplanten Schwangerschaften sowie erhöhten Risiken für Frühgeburten, intrauterine Wachstumsverzögerungen, Plazentastörungen und postpartale Depressionen assoziiert ist. Pathophysiologisch spielen endokrine Dysfunktionen eine zentrale Rolle. In der klinischen Versorgung fehlt es häufig an strukturierten Screening-Instrumenten und an interdisziplinären Versorgungsmodellen. Schlussfolgerung: Die Auswirkungen von AN sind vielfältig. Die Etablierung eines interdisziplinären Netzwerks aus Hebammenwesen, Gynäkologie, Endokrinologie, Diätologie und Psychotherapie ist entscheidend, um gesundheitliche Risiken für Mutter und Kind zu minimieren. Neben der medizinischen Betreuung bedarf es sexualpädagogischer Aufklärung, psychosozialer Begleitung sowie strukturierter Nachsorge. Die Integration essstörungsspezifischer Inhalte in die Aus- und Weiterbildung von Gesundheitsberufen ist dringend erforderlich.Background: Eating disorders, particularly anorexia nervosa, are occurring with increasing frequency and primarily affect young women of childbearing age. Objective: This thesis aims to systematically explore the impact of anorexia nervosa on women of reproductive age. The focus lies on fertility disorders, pregnancy courses, obstetric complications, and challenges during the postpartum period. In addition, the role of midwives and other health professionals in the care of affected women is examined. Methodology: Through a systematic literature search conducted between October 2024 and June 2025, relevant studies were identified from the databases PubMed, CINAHL, Cochrane, and Psychiatrist. The selection was based on defined inclusion criteria and an assessment of methodological quality. A total of nine studies were included in the analysis. Results: The analysis shows that AN is associated with reduced fertility, unplanned pregnancies, and increased risks of preterm birth, intrauterine growth restriction, placental disorders, and postpartum depression. Endocrine dysfunctions play a central pathophysiological role. In clinical practice, there is often a lack of structured screening tools and interdisciplinary care models. Conclusion: The effects of AN are multifaceted. Establishing an interdisciplinary network consisting of midwifery, gynecology, endocrinology, dietetics, and psychotherapy is crucial to minimize health risks for both mother and child. In addition to medical care, there is a need for sexual education, psychosocial support, and structured follow-up care.
