Niklaus Wirth is not only a master of language design but also a pioneer of compiler construction. For four decades he has refined his techniques for building simple, efficient and reliable compilers. This paper tries to collect some general principles behind his work. It is not a paper about new compilation techniques but a reflection about Wirth's way to write compilers.

Chirurgische Massnahmen bei therapierefraktärem Ulcus beim diabetischen Fuss
Bei Patienten mit Diabetes mellitus kommt es aufgrund einer vorliegenden Polyneuropathie sowie Makro- und Mikrozirkulationsstörungen gehäuft zu Fussulzerationen. Werden diese Ulcera nicht früh genug mittels guter Wundpflege konservativ behandelt, kann es zu Infektionen bis hin zu einer Osteomyelitis kommen. Oft liegt den chronischen Ulzerationen aber auch eine ossäre Fehlstellung des Fusses zugrunde. Dann ist es möglich, dass trotz guter Wundpflege die Ulzerationen nicht konservativ beherrschbar sind. Bei ausgeprägten Fehlstellungen kann eine orthopädische Schuhversorgung zudem deutlich erschwert bis verunmöglicht sein. In diesen Situationen ist es mittels chirurgisch-orthopädischen Massnahmen möglich, die Fussfehlstellung dahingehend zu korrigieren, dass die Druckspitzen aufgrund ossärer Fehlstellungen beseitigt werden können. Dadurch können auch Ulzera zur Abheilung gebracht werden.
Im folgenden Artikel soll auf die verschiedenen zur Verfügung stehenden chirurgischen Korrekturmöglichkeiten eingegangen werden.
Summary Patients with diabetes mellitus often also suffer from a polyneuropathy or a macro- and microangiopathy. This often leads to foot ulcers. If these ulcers are not treated appropriately with comprehensive wound care, infection or even osteomyelitis may result. Chronic ulceration is often caused by bony prominences due to malalignment of the foot. In this case, it is possible that comprehensive wound care alone will not be successful. If a complex malalignment of the foot is present, fitting of orthopedic shoes becomes increasingly difficult.
In these situations, surgical procedures can help to even out the pressure peaks due to bony prominence
