Metabolic Syndrome Associated with Subclinical Hypothyroidism and Vit D Insufficiency - A Case Report and Discussion
Metabolic syndrome (MetS) associated with hypothyroidism, was reported in literature. Like-wise association between hypothyroidism and Vit. D deficiency was reported. Hypothyroidism, Vit. D deficiency in DM2 patients was also reported, in the literature. But the occurrence of MetS, hypothyroidism and Vit. D deficiency, together occurring in a case was unreported. The author presented such triple association in a male patient aged 54 years. While Met S itself was a risk factor for CVD (cardiovascular vascular disease) and so were independently, the hypothyroidism and hypovitaminosis D, the triple association would have cumulative risk for CVD. So, it would be profitable to screen all MetS cases with hypothyroidism, for Vit. D deficiency also, in view of the possible cumulative risk. Literature was reviewed as to the relationship of MetS to hypothyroidism and Vit D deficiency, as well as the changing diagnostic criteria of MetS and it’s pathogenesis
The association between vit D levels and BMD.
Allogeneic hematopoietic stem cell transplant (aHSCT) patients are well known to be at high risk of vitamin D (vit D) deficiency. This study assessed whether a loading dose (100,000 IU) of vitamin D3 pre-aHSCT could effectively achieve and maintain sufficient post-transplant vit D levels (serum total 25 hydroxy vitamin D (25(OH)D) ≥ 75nmol/L). Dual-energy X-ray absorptiometry (DXA) was also conducted for bone health evaluation. 74 patients were enrolled and randomly assigned, in a 1:1 ratio, either to the high vit D group (single loading dose (100,000 IU) plus 2,000 IU vit D3 daily) or the control group (2,000 IU vit D3 daily). Vit D levels were measured at three time points (baseline, day 30 and day 100 post-aHSCT). At baseline, fewer than 50% patients had a sufficient 25(OH)D (control: 42.9%; high vit D: 43.6%). The proportion of patients with sufficient 25(OH)D (nmol/L) was increased at day 30 and day 100, with a trend of higher proportion in the high vit D group at day 30 (high vit D vs. control: 89.7% vs. 74.3%, p = 0.08). The increased 25(OH)D was significantly higher in the high vit D group at day 30 (high vit D vs. control: 29±25.2 vs. 14 ±21.9, p = 0.01). Insufficient vit D level before transplant (baseline) was an independent risk factor for vit D insufficiency (serum 25(OH)D 3 significantly increased total 25(OH)D levels at day 30 post-transplant, and the intervention was especially beneficial for patients with baseline vit D insufficiency. We acknowledge that the primary outcome at day 100 post-aHSCT indicating superiority of loading dose versus daily dose supplementation was not met.</div

Comparison of Serum Vit D Level Between Psoriasis Patients and Normal Population
Introduction: Psoriasis is a common inflammatory skin disorder with humeral and cellular immune system involvement. On the other hands، Vitamin D has important immunomodulatory effects and its relations with many autoimmune diseases were shown. The relation between Serum Vit D Level and Psoriasis disease remained controversial issue that this study was done to show this relationship.
Methods and Materials: In this comparative cross sectional study، 30 psoriatic patients who attended to Razi dermatology clinic in Rasht، by sequential sampling underwent study as case group. 30 healthy people from hospital at the same age، gender and skin-colored also were selected as control group. Inclusion criteria were patients and people without Ca or Vit D supplements usage or underground diseases in relation to hypovitaminosis D. Demographical data were gathered by interviewing and then blood samples from all participants were sent for analysis of serum Ca، P، Vit D and parathormone level. Data analysis was done by SPSS ver. 18 using T-test and Chi 2 test.
Result: Mean and SD serum level of Vit D in case and control groups were 25.8± 12.6 ng/ mL and 23.7± 8.9 ng/ mL، respectively (ρ>0.05). Out of all، 24 participants (40%) suffered from Vit D Deficiency and 20 people (33.3%) from Vit D insufficiency ، only 26.7% study subjects had acceptable serum Vit D level. There were not any significant difference between Vit D deficiency rates and other measured biochemical indices in two groups (36.7% vs. 43.3%).
Conclusion: This study showed high level of Vit D deficiency in both psoriatic and normal people that must be notice
Effect of Replacement of Vit-D in Diabetic Painful Neuropathy among Chronic Type 2 Diabetic Patients
Background: Painful symptoms of diabetic neuropathy are more common in patients having type 2 diabetes. The objective in this trial was assessment of mean change in pain score upon giving Vit-D supplementation in patients with diabetic painful neuropathy.
Methodology: This study was conducted from December 2019 to June 2020 in OPD of Fauji Foundation Hospital, Rawalpindi after the ethical approval. A total of 251 diagnosed cases of diabetes (both male and female patients), with high HbA1c and low Vit-D levels were included in the study. Patients having Vit-D deficiency and chronic diabetic neuropathy were given Vitamin supplementation and results were assessed after 1 month through change in pain score according to Douleur Neuropathique 4. A document of consent form was filled by patients who were recruited for this study. Data analysis was done on SPSS version 16.Paired sample t- test, was used to compare pain score where p value less than 0.05 was considered statistically significant.
Results: Among the participants mean age was 48.22+17.06. There were 106 (42.2%) males and 145 (57.8%) female patients. A statistically significant (p<0.05)mean reduction in pain score of 2.20±1.19 was observed, upon giving Vit-D supplementation in Vit-D deficient patients having painful diabetic neuropathy.
Conclusion: Vit-D supplementation significantly reduced pain in patients having diabetic painful neuropathy.

To determine the association of serum VIT D level and uterine fibroid in premenopausal women
Background: Uterine Fibroid are monoclonal, benign tumors originating from the smooth muscle cell of uterus and predominantly located in the pelvis. Main hormones for uterine fibroid are estrogen and progesterone. Fibroids mainly consist of extracellular matrix that contain collagen, fibronectin and proteoglycans. vitamin D deficiency has been proven to be a major risk factor in the development of fibroid. Mean 25(OH)D serum levels are significantly low in uterine fibroid women as compared to healthy women. Aims & objectives: To estimate serum vitamin D level in the premenopausal women under age group of 30 to 45 years diagnosed with uterine fibroid and healthy women of same age, weight, locality and ethnicity and to compare serum vitamin D level among both groups. Materials and methods: An observational case control study done in the dept of Obstetrics & Gynaecology of MGM Medical College & M.Y Hospital, Indore Madhya Pradesh from 1st April 2020 to 31st march 2021 in the women attending outpatient dept, in the age group of 30-45 year. After obtaining informed consent, 3 ml of blood sample was collected from all the participants and vit D level measured in the serum by ELISA. Result: total 200 women (100 cases, 100 control) underwent the study. Most study participants among case and control are of age 30-35 years. None of them had BMI below normal and most of control and cases were overweight. Most common parity among them was 2. 91% of cases had vit D deficiency while 32% of control had vit D deficiency and 68% of control were having normal vit D levels. Conclusion: The present study shows there is positive correlation between vit D level and uterine fibroid (OR=21.48), which is suggestive of some contribution of hypovitaminosis of vit D in etiology of uterine fibroid
