    Stapled anopexy versus transanal haemorrhoidal dearterialization for haemorrhoidal disease: a three-year follow-up from a randomized study

    BACKGROUND: This randomized study compared the medium-term results of stapled anopexy (SA) and transanal hemorrhoidal dearterialization with anopexy (THD) in 4 homogeneous groups of patients, 2 with third- and 2 with fourth-degree hemorrhoids. METHODS: Forty patients with third-degree and 30 with fourth-degree hemorrhoids were randomly submitted to SA (N.=20+15) and THD (N.=20+15), respectively. Clinical controls were done every 6 months from 1 to 42 months after the operation, with incidence of recurrent hemorrhoids as primary outcome measure. Operative time, complications, pain, time to return to normal activity, costs, Short Form-36, and overall patient satisfaction were also evaluated. RESULTS: Frequencies of preoperative obstructed defecation symptoms and prolapse recurrence were higher in patients with fourth-degree hemorrhoids, and SA was more effective than THD in reducing the risk of recurrence at 36±6 months follow-up (P=0.049). Operative time, complications, pain, and time of return to normal activity were similar in the 4 groups. Costs were significantly higher for SA in patients with fourth-degree hemorrhoids (P>0.01). A significant improvement of quality of life was observed in all groups, and no significant difference was found in overall patient satisfaction. CONCLUSIONS: Both techniques are safe and effective in the mid-term period. SA is more effective in reducing prolapse and obstructed defecation symptoms in fourth-degree hemorrhoids, with the disadvantage of higher costs. Prolapse size and presence of obstructed defecation symptoms could be predictive criteria for choice of the best surgical technique (Cite this article as: Venturi M, Salamina G, Vergani C.     Il ramonismo di Orio Vergani: riscritture ipertestuali e traduzioni

    In 1927 Orio Vergani wrote the introduction to the first authorized Italian translation of Ramón Gómez de la Serna’s Senos (1917) and in 1942 he republished Soste del capogiro, a book of short stories from 1921–1926. The author of this article demonstrates how some of them retrace the short fiction of Gómez de la Serna in style, themes and language. Some were expanded and published by Vergani in 1942 in the fascist magazine “Legioni e Falangi”, however, their anonymous translation for the Spanish twin edition departs from the ramonian model of the origins.W 1927 roku Orio Vergani napisał wstęp do pierwszego autoryzowanego przekładu włoskiego Senos (1917) Ramóna Gómeza de la Serna (1917), a w roku 1942 ponownie wydał Soste del capogiro, zbiór własnych opowiadań z lat 1921–1926. Autorka artykułu pokazuje ich stylistyczne, tematyczne i językowe nawiązania do krótkiej fikcji hiszpańskiego pisarza. Niektóre z tych opowiadań zostały rozszerzone i opublikowane przez Verganiego w 1942 r. w faszystowskim czasopiśmie „Legioni e Falangi”, jednak ich anonimowe tłumaczenie dla hiszpańskiego wydania bliźniaczego oparte jest na ramońskim modelu

    Diego and Giorgina Vergani : The two hearts of translational autoimmunity

    Since the publication of the first textbook on autoimmune diseases in 1963, the knowledge in the field has exponentially grown into numerous tracks of research, particularly at benchside. Systemic and organ-specific autoimmune diseases, as in the case of the liver, have witnessed notable advances in terms of epidemiology, genetics, effector and regulatory mechanisms, and ultimately treatment. While the available tools for communication have provided accelerating progress rates, we recognize that key opinion leaders continue to provide significant contributions to the field. The present issue is dedicated to celebrate Giorgina Mieli-Vergani and Diego Vergani as two of the finest examples of excellence in autoimmune liver diseases and the broader field of autoimmunity. Diego and Giorgina are extremely well-liked Colleagues who fully represent the translational efforts between laboratory research and clinically relevant questions in the practice of pediatric liver diseases and autoimmune hepatitis
