Oralsurgical approach to excision of peripheral giant cell granuloma
Periferni gigantocelularni granulom (PGCG) lat. epulis gigantocellularis svrstava se u skupinu gigantocelularnih promjena među koje pripadaju i centralni gigantocelularni granulom, kerubizam, gigantocelularni tumor, fibrozna displazija i smeđi tumor kod hiperparatireoidizma. PGCG je najčešća oralna lezija divovskih stanica. Manifestira se kao ljubičasto-crveni promjena mekog tkiva u čijem su histološkom nalazu vidljive multinuklearne divovske stanice u mononuklearnoj stromi i ekstravazacija eritrocita. Ne smatra se pravom neoplazmom, nego reaktivnom lezijom potaknutom lokalnom iritacijom i traumom. Terapija izbora je ekscizija. U ovom radu prikazan je devetogodišnjak s lezijom u vestibulumu gornje čeljusti u regiji zuba 54, 55. Lezija je u cijelosti uklonjena u lokalnoj anesteziji, histopatološki nalaz bio je karakterističan za PGCG. It manifests as a purple-red soft tissue lesion, with histological findings showing multinuclear giant cells in a mononuclear stroma and extravasation of erythrocytes. It is not considered a true neoplasm, but rather a reactive lesion stimulated by local irritation and trauma. The treatment of choice is excision. This paper presents the case of a nine-year-old boy with a lesion in the vestibule of the upper jaw in the region of teeth 54 and 55. The lesion was completely removed under local anesthesia, and the histopathological findings were characteristic of PGCG. The postoperative course was without complications and recurrence
Contemporary surgical approach for treatment of large cyst of the jaw
Ciste čeljusti patološke su šupljine ovalna ili okrugla oblika, ispunjene tekućim ili polutekućim
sadržajem. Omeđene su cističnom ovojnicom koja se sastoji od unutarnje epitelne i vanjske vezivne
ovojnice. Prema podjeli Svjetske zdravstvene organizacije iz 2017. godine, ciste čeljusti i mekih
tkiva usne šupljine dijele se na odontogene ciste upalnog podrijetla te odontogene i neodontogene
razvojne ciste. Cistične promjene teško je dijagnosticirati samo na temelju kliničke slike i
rendgenskog nalaza, stoga je dijagnostički postupak nužno upotpuniti punkcijom cističnog sadržaja
te, prema potrebi, ostalim tomografskim radiološkim pretragama kao što su CBCT ili MR.
Patohistološka dijagnostika jedina je tehnika koja točno definira vrstu cistične lezije. U terapiji
cista glavni je cilj enukleacija ciste u potpunosti. Odabir operativne tehnike ovisi o veličini i
lokalizaciji ciste. Male ciste, do tri centimetra u promjeru, uklanjaju se postupkom enukleacije
(metoda Partsch II) neovisno o lokalizaciji. Velike ciste, promjera većeg od tri centimetra, u donjoj
se čeljusti mogu ukloniti postupkom enukleacije, marsupijalizacijom (metoda Partsch I),
dvofaznom tehnikom po Hermannu, Broschovom metodom te metodom Partsch II sa sukcijom.
Velike ciste gornje čeljusti operiraju se rinološkim metodama: Caldwell-Lucovom tehnikom,
tehnikom prema Loebelu te čuvenom Denkerovom cistektomijom. Dekompresija je operativna
tehnika koja se može primijeniti u gornjoj i donjoj čeljusti. Unatoč cijeloj paleti kirurških
postupaka, Partsch II metoda je odabira u liječenju cista i gornje i donje čeljusti.Jaw cysts are pathological cavities of oval and round shape filled with liquid or semi-liquid contents. They are bounded by a cystic membrane which consists of an outer connective and an inner epithelial membrane. According to the classification of the World Health Organization from 2017. cyst of the jaw and soft tissues of the oral cavity are divided into: odontogenic inflammatory cysts and developmental odontogenic and non-odontogenic cysts. It is difficult to diagnose cystic lesions based merely on the physical findings and the radiological examinations. Therefore, it is necessary to complete the diagnostic procedure with a needle aspiration of cystic content and possibly other tomographic radiological examinations such as CBCT or MR. Pathohistological diagnosis is the only technique that accurately defines the type of cystic lesion. The principal purpose of the treatment is a complete enucleation of the cyst. The size and the localization of the cyst determines which operative technique is going to be used. Minor jaw cysts, up to 3 centimetres in diameter, are removed by the method of enucleation (Partsch II), regardless of the localization. Large cysts are cysts larger than 3 centimetres in diameter. Such cysts may be removed in the mandible by the enucleation technique (Partsch II), marsupialisation (Partsch I method) and Partsch II method with suction. Large maxilla cysts are operated with rhinological methods, i.e. Caldwell- Luc technique, technique according to Loebel and the already historical cystectomy according to Denker. The decompression method is an operative technique that may be used in both the maxilla and mandible. Notwithstanding the existence of a whole range of surgical techniques for the operation of cysts of the jawbone, the Partsch II technique is the method of choice for the treatment of cysts of both the maxilla and mandible
Pretprotetska priprema alveolarnog grebena bezube donje čeljusti vestibuloplastikom sa sekundarnom epitelizacijom
Pre-prosthetic preparation of the alveolar ridge is a crucial step in ensuring the long-term success and comfort of complete dentures. In the edentulous lower jaw, one of the challenges is the limited depth of the vestibule, which can compromise denture stability and retention. Vestibuloplasty with secondary epithelization is a surgical technique used to deepen the vestibule and increase the surface area of the attached mucosa. The procedure involves repositioning the mucosal tissue and allowing the wound to heal by secondary intention, meaning the tissue is left to naturally granulate and epithelialize over time. While this approach requires a longer healing period compared to other methods, it is relatively simple, cost-effective, and does not require grafting. The outcome often results in improved denture support and patient comfort. This method is especially useful in patients where other surgical techniques are contraindicated or not feasible. Overall, vestibuloplasty with secondary epithelization remains a valuable tool in the prosthodontic management of the edentulous mandible.Pretprotetska priprema alveolarnog grebena ključan je korak u osiguravanju dugoročnog uspjeha i udobnosti potpunih proteza. U bezuboj donjoj čeljusti, jedan od uobičajenih izazova je ograničena dubina vestibula, što može ugroziti stabilnost i retenciju proteze. Vestibuloplastika sa sekundarnom epitelizacijom je kirurška tehnika koja se koristi za produbljivanje vestibula i povećanje površine pričvršćene sluznice. Ovaj postupak uključuje repozicioniranje tkiva sluznice i omogućavanje zacjeljivanja rane sekundarnom intencijom, što znači da se tkivo ostavlja da se prirodno granulira i epitelizira tijekom vremena. Iako ovaj pristup zahtijeva dulje razdoblje zacjeljivanja u usporedbi s drugim metodama, relativno je jednostavan, isplativ i ne zahtijeva presađivanje. Immediate implant prosthetic repair of the lower jaw with "short" implants
Potpuna bezubost u mandibuli predstavlja izazov u izboru terapijskog plana. Ne postoji univerzalan oblik terapije, pogodan za svakog pacijenta te superioran svim ostalim opcijama liječenja. Zahtjevi pacijenata individualni su te modificirani psihosocijalnim, ekonomskim, kulturološkim faktorima te spolom i dobi. Zbog neodstatka koštanog volumena te neposredne blizine donjeg alveolarnog živca kod ekstremne atrofije mandibule otežana je implantološka terapija konvencionalnim implantatima. U takvim kliničkim uvjetima indicirani su dodatni kirurški zahvati u kojima se prvo nadoknađuje volumen izgubljene kosti kao što su vođena koštana regeneracija, horizontalna i vertikalna augmentacija koštanim blokom, distrakcijska osteogeneza, transpozicija donjeg alveolarnog živca te ridge-split tehnika. Takvi zahvati produljuju tijek terapije, predstavljaju dodatan financijski izdatak,a mogu uzrokovati i brojne komplikacije. „Short“ implantati definirani su kao implantati sa 8 ili manje mm intrakoštane duljine. Djeluju kao predvidljiva minimalno invazivna alternativa augmentaciji grebena. Modifikacijom oblika i dizajna navoja, povećanjem mikroporoznosti površine donjeg dijela tijela implantata, te razvojem glatke cervikalne površine obogaćene ionima kalcija ili hidroksilapatita značajno se poboljšalo učinak oseointegracije. Dobra primarna stabilnost pri ugradnji implantata preduvjet je za protokol imedijatnog opterećenja te trenutnu obnovu funkcije stomatognatog sustava u potpuno bezubih pacijenata. Primarna stabilnost tijekom cijeljenja glavni je čimbenik koji utječe na kvalitetu oseointegracije te prognozu implantata. Primjenom osnovnih biomehaničkih principa u implantološkoj terapiji osigurava se predvidljiv i stabilan rezultat.Complete mandibular edentulism represents a challenge in choosing a therapeutic plan. There is no universal therapy form, suitable for every patient and superior to all other treatment options. Patient requirements are individual and modified by psychosocial, economic and cultural factors, as well as gender and age. Due to the lack of bone volume and the close proximity of the lower alveolar nerve in extreme mandibular atrophy, implant therapy with conventional implants is difficult. In such clinical conditions, additional surgical interventions are indicated in which the lost bone volume is first compensated for, such as guided bone regeneration, horizontal and vertical bone block augmentation, distraction osteogenesis, transposition of the lower alveolar nerve, and ridge-split technique. Such procedures prolong the course of therapy, represent an additional financial expense, and can cause numerous complications. "Short" implants are defined as implants with 8 or less mm of intraosseous length. They act as a predictable minimally invasive alternative to ridge augmentation. By modifying thread shape and design, increasing microporosity of the implant bodies' lower surface and developing a smooth cervical surface enriched with calcium or hydroxylapatite ions, the effect of osseointegration was significantly improved. Good primary stability during implant placement is a prerequisite for the immediate loading protocol and immediate restoration of the stomatognathic system function in completely edentulous patients. Primary stability during healing is the main factor affecting the quality of osseointegration and implant prognosis. By applying the basic biomechanical principles in implant therapy, a predictable and stable result is ensured
Modern oral surgical approach of patiens irradiated in the head and neck area
Napretkom medicine i podizanjem svijesti ljudi o malignim bolestima, sve više se pacijentima otkrivaju karcinomi, kako u ranoj fazi tako i u uznapredovaloj. Uz kirurško liječenje, najčešće se primjenjuje terapija zračenjem ili kemoterapija. Radioterapija za sobom povlači nuspojave. Da bi se nuspojave svele na minimum, ključan zadatak stomatologa je informirati pacijenta, obaviti zahvate što bezbolnije i u pravo vrijeme te olakšati sam proces zračenja. Naglasak se stavlja na održavanju oralne higijene te na redovnim kontrolama. Kirurški zahvati bi se trebali obaviti prije početka terapije, za vrijeme terapije samo hitni zahvati, a nakon terapije treba dobro procijeniti rizik od nastanka neželjene nuspojave, osteoradionekroze. U današnje vrijeme sve se više istražuju novi pristupi onkološkim pacijentima kako bi se pacijentima poboljšala kvaliteta života. Najboljim potpornim metodama očuvanja kosti su se pokazali korištenje hiperbarične komore te korištenje plazme obogaćene faktorima rasta. Još uvijek je potrebno provoditi daljnja istraživanja i tragati za boljim rješenjima.With the progress of medicine and raised awareness of malignant diseases, more patients are being diagnosed with cancer, both in the early and advanced stages. In addition to surgical treatment, radiotherapy and chemotherapy are most commonly used. However, radiotherapy entails certain side effects. In order to reduce its side effects to a minimum, it is crucial that dentists inform the patient, do their best to perform well-timed, painless procedures as well as facilitate the radiation process itself. Emphasis is placed on maintaining oral hygiene and regular check-ups. Surgical procedures should be performed before the start of therapy, whereas only emergency procedures should be carried out during therapy. After therapy, it is important to carefully assess the risk of an unwanted side effect, i.e. osteoradionecrosis occurring. Nowadays, there has been an increase in the research of new approaches to oncological patients in order to improve their quality of life. The use of hyperbaric chambers and plasma enriched with growth factors have proven to be the best supportive methods of bone preservation. However, it is still necessary to conduct further research and search for better solutions

Implantološke protetske mogućnosti rehabilitacije atrofičnih čeljusti
Edentulism presents a significant issue for both patients, impacting their psychophysiological well-being, and for dentists, posing challenges in planning appropriate treatments. Advances in technology, materials, and surgical methods have progressively improved individual quality of life. Given that there is no universal solution for all patients, treatment plans must be tailored to address each individual's specific needs in the most suitable manner. Alveolar ridge atrophy, resulting from extractions, plays a crucial role in planning implant prosthetic rehabilitation. Various techniques have been described to compensate for or correct the loss of bone structure. The most commonly used methods include bone augmentation and vestibuloplasty. Bone augmentation aims to provide sufficient bone material necessary for dental implant placement, utilizing biological materials such as bone grafts and membranes. Vestibuloplasty involves deepening the vestibular sulcus to increase the seating area for proper fit of complete and partial dentures. The application of these techniques ensures better and more predictable outcomes for the implant prosthetic rehabilitation of atrophic jaws.Bezubost kao stanje predstavlja značajan problem kako za pacijenta, utječući na njegove psihofiziološke aspekte, tako i za stomatologa, stvarajući izazov pri planiranju adekvatne terapije. Napretkom tehnologije i materijala, te usavršavanjem operativnih metoda, kvaliteta života pojedinca danas postaje sve bolja. S obzirom na to da ne postoji jedinstveno riješenje kojim bi se moglo pristupiti svim pacijentima, izrada plana terapije mora biti usmjerena prema pojedincu i riješavanju njegova problema na najprikladniji mogući način. Atrofija alveolarnog gebena nastala kao posljedica ekstrakcija može igrati značajnu ulogu pri planiranju implantoprotetske rehabilitacije, stoga su opisane brojne tehnike kojima se gubitak koštane strukture može nadomjestiti, odnosno korigirati. Najčešće korištene metode koje se upotrebljavaju u tu svrhu uključuju augmentaciju kosti i vestibuloplastiku. Ekcizija i patohistološka dijagnostika atipičnog perifernog osificirajućeg fibroma - prikaz slučaja
Peripheral ossifying fibroma (POF) is a benign, reactive growth that appears on the gingiva, often triggered by local irritants such as dental plaque, tartar, or dental devices. This thesis aims to explore the clinical and histological features of POF, understand its etiology, and review various treatment options. A retrospective study was conducted on patients diagnosed with POF to collect data on demographics, lesion characteristics, and recurrence rates. The tissue composition of the lesions was analyzed through pathohistological examinations. A comparative context for the findings was also provided through a literature review. POF primarily impacts the anterior maxilla, and according to the research, there is a female predilection. Typically, the lesions exhibit a proliferation of fibroblastic cells, as well as areas of calcification and ossification. This is frequently accompanied by chronic inflammation and occasional ulceration. The high recurrence rate observed confirms the necessity of complete surgical excision and the removal of local irritants to reduce the risk of recurrence. Early detection and management of recurring lesions requires consistent follow up.Periferni osificirajući fibrom (POF) je benigna, reaktivna izraslina koja se pojavljuje na gingivi, često izazvana lokalnim iritansima poput zubnog plaka, zubnog kamenca ili zubnih naprava. Ovaj rad ima za cilj istražiti kliničke i histološke značajke POF-a, razumjeti njegovu etiologiju i pregledati različite mogućnosti liječenja. Provedena je retrospektivna studija na pacijentima kojima je dijagnosticiran POF kako bise prikupili podaci o demografskim karakteristikama, karakteristikama lezija i stopama recidiva. Sastav tkiva lezija analiziran je histopatološkim pretragama. Usporedni kontekst za nalaze također je osiguran pregledom literature.
