    Obiettivi: Valutare l'incidenza e i fattori di rischio per il rialzo degli enzimi di citolisi epatica (liver enzyme elevation, LEE) nei pazienti che iniziano una terapia antiretrovirale di prima linea (ART) nella coorte osservazionale prospettica ICONA. Pazienti e metodi: sono stati selezionati per lo studio 6575 pazienti naive alla ART, che hanno iniziato, tra giugno 2009 e dicembre 2017, un regime terapeutico costituito da 2 inibitori nucleosidici della trascrittasi inversa (NRTI) associati ad un inibitore della proteasi potenziato (PI/b) N=2436 (37.1%), ad un inibitore non nucleosidico della trascrittasi inversa (NNRTI) N=2384 (36.3%), o ad un inibitore dell'integrasi (INSTI) N=755 (26.7%). La presenza di co-infezione da parte del virus dell’HBV (HBsAg positività) o dell’HCV (HCV-RNA positività) è stata rilevata nel 3.9% e nel 5.8% della popolazione dello studio. I LEE sono stati definiti come aumento di ALT o AST di ≥ 2.5 x ULN (limite superiore della normale) per i pazienti con valori di base nei limiti della norma o ≥ 2.5 x il valore basale, per i pazienti con valori di base >ULN. L'analisi di regressione Cox inverse probability weighted è stata utilizzata per calcolare gli hazard ratio (HR) ed i relativi intervalli di confidenza al 95% (95%CI) per LEE, sulla base del regime terapeutico di prima linea utilizzato e delle caratteristiche basali dei partecipanti allo studio. Risultati: Durante un follow-up complessivo di 20722 anni di osservazione, si sono verificati 183 casi di LEE. Dopo l'aggiustamento per i principali fattori confondenti, il rischio di LEE è risultato significativamente ridotto nei pazienti trattati con INSTI rispetto a coloro che ricevevano NNRTI (HR 0.46, 95%CI 0.25-0.86), con una significativa riduzione del rischio nel gruppo di pazienti trattati con raltegravir (HR 0.11, 95%CI 0.02-0.84, utilizzando la classe degli NNRTI come riferimento). L'HR per LEE è risultato significativamente più elevato nei soggetti con co-infezione da HBV o HCV, nei pazienti con infezione da HIV scarsamente controllata e in quelli che hanno contratto l'HIV attraverso la trasmissione omosessuale. Conclusioni: Nel nostro studio, l'utilizzo di INSTI riduce del 54% il rischio di LEE rispetto ad altri regimi. Questo dato potrebbe essere particolarmente importante per la scelta dell'ART in pazienti con fattori di rischio per tossicità epatica come le co-infezioni HCV e HBV

    Introduction: Despite the unquestionable success of antiretroviral therapy achieved in recent years, there are still cases of heavily treated patients who do not achieve or struggle to maintain undetectable HIV-RNA due to drug resistance. New antiretroviral options are needed to address this issue.Area covered: The authors first provide an overview of fostemsavir and its role in the treatment of HTE PLWH. Data from pre-clinical and clinical studies are reviewed and the pharmacokinetic and farmacodynamic properties are highlited. Drug-drug interactions and safety data from available clinical studies are also discussed.Expert opinion: Fostemsavir is a promising antiretroviral belonging to the class of entry inhibitors; its novel mechanism of action represents a very important innovation. Its use will be limited to the heavytreatment-experienced patient population. This use will have to be monitored to avoid abuse and waste of a molecule that for some patients may represent a life-saving drug

    B/F/TAF forgiveness to non-adherence

    Background ART forgiveness is the ability of a regimen to maintain HIV-RNA suppression despite a documented imperfect adherence. We explored forgiveness of bictegravir/emtricitabine/tenofovir alafenamide (B/F/TAF). Methods In this retrospective cohort study pharmacy drug refills were used to calculate the proportion of days covered (PDC) as a proxy of adherence. Forgiveness was defined as the possibility to achieve a selected HIV-RNA threshold by a given level of imperfect adherence. A logistic model was applied to verify the impact of baseline variables and adherence on the virologic outcomes. Results We enrolled 420 adults. From them, 787 one-year time-periods were derived for a median cohort follow-up of 873 person/years. Most of them were males (73.1%); the most frequent risk factor for HIV infection was heterosexual contacts (49.5% of cases), followed by 22.5% MSM and 22.5% intravenous drug users. The median age of enrolled persons with HIV was 51 years (IQR 45-57 years); the median duration of HIV infection was 7.9 years (IQR 4-18 years) and the median nadir of CD4 cells was 277 cells/mcL (IQR 100-513 cells/mcL). Adherence showed a median of 0.97 (IQR 0.91-1.00), consequently only 17 time-periods (2.2%) in 17 different individuals (4.0%) showed HIV-RNA blood levels above 200 copies/ml. A PDC of 0.75 was sufficient to obtain in > 90% of cases the virologic outcome for both 200 copies/ml or 50 copies/ml. An adherence value of 0.85 obtained a positive response in virtually all subjects either for a cut-off of 50 or 200 copies/ml. Conclusions Long-term success of ART needs effective, well tolerated, friendly regimens. Adherence remains a crucial determinant of long-term success, but suboptimal adherence levels are relatively common. Given this, an elevated forgiveness plays a relevant role to further improve long-term outcomes and should be considered a fundamental characteristic of any antiretroviral regimen. B/F/TAF has been proved to have all of these characteristics

    This work focused on Schistosoma spp. as a potential agent of gross haematuria in non endemic areas. This change in epidemiology is mainly due to recent migratory flows. Moreover it emphasized the needfor cultural action (aimed at Urologists, Dermatologists, General Practitioners and Emergency Medicine doctors) to provide the elements for a correct and timely diagnosis. But the most important issue raised by this paper is the call for a fast track supply of drugs (usually not available in Italy) to field operators for treating tropical diseases
