    Is the Small Changes programme clinically effective for adults who are obese?

    This thesis concerns the treatment of (N=148) male and female clients in a weight management programme: Small Changes the thesis compares treatment as usual with a new approach using motivational interviewing. Firstly a preface sets out the historical context of Small Changes and how it developed and was originally funded before the aims of the thesis and the detail encompassed in each chapter are set out. Following this an introduction chapter presents the key constructs and theories surrounding motivational interviewing. This leads into study 1, a systematic review addressing the question: What should be included in a comprehensive weight management programme? The conclusion to this question is a mixture of input from: nutrition and exercise expertise combined with a clear behavioural approach, measures that encompass important clinical parameters, other than just weight, and sufficient length of follow-up post treatment. This conclusion forms the basis of the study 2 literature review which focusses on the contribution of motivational interviewing for affecting exercise and nutrition behaviours with the aim of improving weight management outcomes. The literature review leads into the methodology for study 2 which outlines the measures used, the follow-up period and the detail of what was done in study 2; sufficiently clearly for others to replicate what was done. From this chapter a separate results chapter reveals the statistical results for the outcome measures included in study 2. The discussion chapter then follows and focusses on the interpretation, critique and analysis of the results and relates these back to the literature presented in the literature review for study 2. Finally conclusions and short-comings of the study 2 investigation are explored and further research considerations made

    The invisible artist: Arrangers in popular music (1950-2000): Their contribution and techniques

    This thesis was submitted for the degree of Doctor of Philosophy and was awarded by Brunel University.This thesis is based on the research conducted by the author for the series, Richard Niles' History of Pop Arranging, seven thirty-minute documentary programmes for BBC Radio 2, researched, written and presented by the author and broadcast in 2003. It also draws on interviews conducted by the author (and other research) between 2002 and 2007 both for the radio series and for this thesis and on the author's experience as a professional arranger in popular music working with many of the genre's significant recording artists including Paul McCartney, Ray Charles, Cher, Tina Turner, Westlife, Tears For Fears, Dusty Springfield, James Brown, Pet Shop Boys, Kylie Minogue and producers including Trevor Hom, Steve Lipson, Steve Mac and Steve Anderson. It will be argued that the role of the arranger in popular music has often been undervalued and that during a critical period of popular music history (1950-2000) arrangers played a significant part in the evolution of musical content. This thesis is, to the best of the author's knowledge, the first time (apart from the above mentioned documentary) the subject has ever been examined. The arranger is "invisible" because musical arrangers are often un-credited on record liner notes or in books or articles concerning popular music. A considerable amount of research has been necessary to determine who wrote many of the arrangements considered herein. Motown's Berry Gordy purposely kept the names of musicians and arrangers off the records because he feared others might 'poach' the trademark 'Motown Sound'. Other record labels considered the job of the arranger to be reminiscent of an earlier era, diluting the Rock 'n' Roll image of emotion and spontanaeity they wished to promote. Some producers and recording artists disliked sharing credit for their work. Motown arranger David Van dePitte told the author that arranging was "thankless and anonymous - a very service-oriented profession where others often take credit for what you've done." Arranging has therefore remained an intrinsically unseen art created by 'invisible' artists. By analyzing many recordings, revealing the techniques and concepts they have used in their work to create popular records, arrangers and their art will be made more 'visible'

    Nutritional peak week and competition day strategies of competitive natural bodybuilders

    Bodybuilders utilize peaking strategies in a bid to fine-tune their aesthetics for competition day. The most prevalent peaking strategies utilized by natural bodybuilders are unreported in the current literature. Eighty-one (M - 59, F - 22) natural bodybuilders were recruited from competitions during the 2016 and 2017 British Natural Bodybuilder Federation seasons. Competitors completed a 34-item questionnaire designed to investigate peaking and contest day strategies. The questionnaire listed commonly utilized peaking strategies and provided additional space for qualitative information. Analysis of the data indicated that carbohydrate (CHO), water and sodium manipulation were the most commonly utilized peak week strategies. The consumption of high glycemic index CHO was the most common competition day strategy. Only 6.2 % of competitors reported following their regular diet the week prior to competition. The CHO manipulation strategies were similar to classical CHO loading, whereby bodybuilders attempt to maximize muscle glycogen concentrations. Furthermore, bodybuilders attempted to remove superfluous water by exploiting the diuretic/polyuria effect associated with water loading/restriction. The potentially deleterious effects of peaking on bodybuilders' health is considered and the efficacy of these strategies to enhance appearance is discussed. The findings of the present investigation are likely to be of interest to bodybuilders and their coaches

Eng. in Logistics)--Massachusetts Institute of Technology, Engineering Systems Division, 2009 [second author].Includes bibliographical references (leaves 69-70).As the public health system in Burundi transitions from a mode of emergency humanitarian response to a sustainable operation, there is a significant opportunity to reduce costs by eliminating the duplication of functions served by multiple parties. Numerous multi-lateral agencies and NGOs play a significant role in the public health supply chain. Additionally, Burundi has several vertical programs focused on specific communicable diseases such as HIV/AIDS, malaria and tuberculosis, which have parallel distribution systems. This thesis assesses the costs of multiplicity in the public health supply chain in Burundi, focusing on identifying the factors contributing to multiplicity and evaluating their cost impacts. We specifically analyze the transportation and inventory costs under four different distribution scenarios in the context of a proposed health sector structural reform while using the current system as a baseline. The analysis also discusses several other relevant factors that affect the costs of multiplicity. The research suggests that the Ministry of Health in Burundi can reduce costs, and improve availability of drugs by shifting from a pickup mode to a delivery mode of distribution, sharing assets between the central procurement agency and vertical programs, and improving the cash flow in the system.by Jeet N.     Better known as a novelist, William Trevor has — over the last twenty years and with five volumes to his credit — slowly but surely won a place in the forefront of short-story writers in English. As a thousand touches will show, these stories a fully -documented picture of social and cultural life in post-war England and Ireland. Of the sixty stories published to date, many take place in London ; some in the English country-side ; some in Ireland. Mainly restricted to the middle classes and with an emphasis on the old, the lonely, the eccentric and the unhappy, they cover a wide canvas of age and occupation ; and despite a predilection for gloomy, even tragic, situations, Trevor has provided us with many comic incidents and comic portraits in a language that is often deliciously funny . Yet compassion is never far away : rarely sentimental over his characters, the author makes us suffer with them. While the situations may sometimes recall those of Somerset Maugham, Graham Greene, V.S. Pritchett, Trevor's treatment of his material is very much his own, just as the style — occasionally reminiscent of James Joyce and Flann O'Brien — bears the author's original imprint. A masterly use of ironic detachment and playful understatement confer distinction on stories which are both deft and dense, moving and droll.Bien qu'il soit plus connu pour ses romans, William Trevor a publié cinq volumes de nouvelles en vingt ans et se place maintenant parmi les premiers en langue anglaise dans ce genre.     Cybocephalus iviei T. R. Smith (Fig. 85–92) Cybocephalus iviei T. R. Smith in Smith and Cave 2007a: 169–170. Distribution. West Indies (Puerto Rico, U.S. Virgin Islands). Hosts. Pseudococcidae: Hypogeococcus pungens Granara de Willink. Remarks. A large series of C. iviei was collected, sifting litter underneath cacti and agave. Cybocephalids are often associated with scale insects feeding on cacti, and it can be inferred that this was why so many of these beetles were found thus. Another series of this beetle was collected in flight intercept traps on Buck Island, U.S. Virgin Islands. This species (published as C. kathrynae) has been documented feeding on the pollen of Mammillaria nivosa Link ex N. D. Pfeiffer and was captured in Puerto Rico feeding on H. pungens (Curbelo-Rodrígues et al. 2012). This is a wingless species, unlike Cybocephalus randalli; no winged phenotypes have been found. Originally this species was determined to be C. kathrynae by the author (Curbelo-Rodrígues et al. 2012). However, upon further examination and with more specimens on hand for dissection, the author has determined that the species recorded in Puerto Rico by Curbelo-Rodrígues et al.
