    Primacy of effective communication and its influence on adherence to artemether-lumefantrine treatment for children under five years of age: a qualitative study.

    Prompt access to artemesinin-combination therapy (ACT) is not adequate unless the drug is taken according to treatment guidelines. Adherence to the treatment schedule is important to preserve efficacy of the drug. Although some community based studies have reported fairly high levels of adherence, data on factors influencing adherence to artemether-lumefantrine (AL) treatment schedule remain inadequate. This study was carried-out to explore the provider's instructions to caretakers, caretakers' understanding of the instructions and how that understanding was likely to influence their practice with regard to adhering to AL treatment schedule. A qualitative study was conducted in five villages in Kilosa district, Tanzania. In-depth interviews were held with providers that included prescribers and dispensers; and caretakers whose children had just received AL treatment. Information was collected on providers' instructions to caretakers regarding dose timing and how to administer AL; and caretakers' understanding of providers' instructions. Mismatch was found on providers' instructions as regards to dose timing. Some providers' (dogmatists) instructions were based on strict hourly schedule (conventional) which was likely to lead to administering some doses in awkward hours and completing treatment several hours before the scheduled time. Other providers (pragmatists) based their instruction on the existing circumstances (contextual) which was likely to lead to delays in administering the initial dose with serious treatment outcomes. Findings suggest that, the national treatment guidelines do not provide explicit information on how to address the various scenarios found in the field. A communication gap was also noted in which some important instructions on how to administer the doses were sometimes not provided or were given with false reasons. There is need for a review of the national malaria treatment guidelines to address local context. In the review, emphasis should be put on on-the-job training to address practical problems faced by providers in the course of their work. Further research is needed to determine the implication of completing AL treatment prior to scheduled time

    We present a qualitative comparison between the host and black hole properties of radio galaxies in the MeerKAT International GHz Tiered Extragalactic Exploration (MIGHTEE) survey with the radio galaxy population in the SIMBA suite of cosmological hydrodynamical simulations. The MIGHTEE data include a ∼1 deg2 pointing of the COSMOS field observed at 1.28 GHz with the MeerKAT radio telescope and cross-matched with multiwavelength counterparts to provide classifications of high- and low-excitation radio galaxies (HERGs and LERGs) along with their corresponding host properties. We compare the properties of the MIGHTEE HERGs and LERGs with that predicted by the SIMBA simulations where HERGs and LERGs are defined as radio galaxies dominated by cold or hot mode accretion, respectively. We consider stellar masses M∗, star formation rates SFR, AGN bolometric luminosity Lbol, and Eddington fraction fEdd, as a function of 1.4 GHz radio luminosity and redshift. In both MIGHTEE and SIMBA, the properties of HERGs and LERGs are similar across all properties apart from SFRs due to differences in host cold gas content in SIMBA. We predict a population of HERGs with low fEdd in SIMBA that are confirmed in the MIGHTEE observations and tied to the faint population at low z. The predictions from SIMBA with the MIGHTEE observations describe a regime where our understanding of the radio galaxy dichotomy breaks down, challenging our understanding of the role of AGN accretion and feedback in the faint population of radio galaxies

    The radio galaxy population in the SIMBA simulations

    We examine the 1.4 GHz radio luminosities of galaxies arising from star formation and active galactic nuclei (AGNs) within the state-of-the-art cosmological hydrodynamic simulation SIMBA. SIMBA grows black holes via gravitational torque limited accretion from cold gas and Bondi accretion from hot gas, and employs AGN feedback including jets at low Eddington ratios. We define a population of radio loud AGNs (RLAGNs) based on the presence of ongoing jet feedback. Within RLAGN, we define high and low excitation radio galaxies (HERGs and LERGs) based on their dominant mode of black hole accretion: torque limited accretion representing feeding from a cold disc, or Bondi representing advection-dominated accretion from a hot medium. SIMBA predicts good agreement with the observed radio luminosity function (RLF) and its evolution, overall as well as separately for HERGs and LERGs. Quiescent galaxies with AGN-dominated radio flux dominate the RLF at ≳1022−23 W Hz−1, while star formation dominates at lower radio powers. Overall, RLAGNs have higher black hole accretion rates and lower star formation rates than non-RLAGN at a given stellar mass or velocity dispersion, but have similar black hole masses. SIMBA predicts an LERG number density of 8.53 Mpc−3, ∼10× higher than for HERGs, broadly as observed. While LERGs dominate among most massive galaxies with the largest black holes and HERGs dominate at high specific star formation rates, they otherwise largely populate similar-sized dark matter haloes and have similar host galaxy properties. SIMBA thus predicts that deeper radio surveys will reveal an increasing overlap between the host galaxy demographics of HERGs and LERGs
