    Recent Origin and Spread of a Common Lithuanian Mutation, G197del LDLR, Causing Familial Hypercholesterolemia: Positive Selection Is Not Always Necessary to Account for Disease Incidence among Ashkenazi Jews

    G197del is the most prevalent LDL receptor (LDLR) mutation causing familial hypercholesterolemia (FH) in Ashkenazi Jew (AJ) individuals. The purpose of this study was to determine the origin, age, and population distribution of G197del, as well as to explore environmental and genetic effects on disease expression. Index cases from Israel (n=46), South Africa (n=24), Russia (n=7), The Netherlands (n=1), and the United States (n=1) were enlisted. All trace their ancestry to Lithuania. A highly conserved haplotype (D19S221:104-D19S865:208-D19S413:74) was identified in G197del chromosomes, suggesting the occurrence of a common founder. When two methods were used for analysis of linkage disequilibrium (LD) between flanking polymorphic markers and the disease locus and for the study of the decay of LD over time, the estimated age of the deletion was found to be 20 ± 7 generations (the 95% confidence interval is 15–26 generations), so that the most recent common ancestor of the mutation-bearing chromosomes would date to the 14th century. This corresponds with the founding of the Jewish community of Lithuania (1338 a.d.), as well as with the great demographic expansion of AJ individuals in eastern Europe, which followed this settlement. The penetrance of mutation-linked severe hypercholesterolemia is high (94% of heterozygotes have a baseline concentration of LDL cholesterol (LDL-C) that is >160 mg/dl), and no significant differences in the mean baseline lipid level of G197del carriers from different countries were found. Polymorphisms of apolipoprotein E and of scavenger-receptor class B type I were observed to have minor effects on the plasma lipid profile. With respect to determinative genetic influences on the biochemical phenotype, there is no evidence that could support the possibility of a selective evolutionary metabolic advantage. Therefore, the founder effect in a rapidly expanding population from a limited number of families remains a simple, parsimonious hypothesis explaining the spread of G197del-LDLR–linked FH in AJ individuals

    Evaluating an mRNA based body fluid identification test using SYBR green fluorescent dye and real-time PCR

    The requirement to have more definitive and wider ranging body fluid identification (BFID) tests has resulted in a range of mRNA based real-time PCR BFI assays utilising Taqman fluorescent dye. An attempt to make a reliable and cost effective BFI test utilising the alternative SYBR Green fluorescent dye was carried out. RNA was extracted from blood and saliva stains from both male and female donors, this was then reverse transcribed using M-MLV and random hexamers. Using real-time PCR, relative quantitation of blood and saliva specific markers was carried out on the cDNA from the blood and saliva samples using the SYBR® Green fluorescent dye. Melting curve analysis was also performed immediately following PCR amplification. The relative quantitation values were calculated using the formula 2-ΔΔCT and all samples were normalised to reference gene 18s rRNA. The results revealed good specificity for a number of markers using this chemistry, however some markers were undetected. Blood markers NCF2, SPTB, PBDG and saliva specific markers HTN3, SPRR1A, KRT4 and KRT13 were investigated. In the SYBR green studies, the most specific markers were NCF2, KRT4, KRT13 and SPRR1A, showing reproducible results in a number of studies. Blood marker SPTB also appeared to be specific to blood however the melt curve data for this marker in each study was questionable given the low melting temperature for the amplified products. Blood specific marker PBGD, and saliva specific marker HTN3 were not detected using SYBR Green and saliva marker STATH was detected however in each case appeared to be non-specific in nature when them melt curves were analysed. Analysis of the 18s rRNA Ct values showed a higher expression in saliva than in blood in almost all instances, this may be due to collection of a higher number of cells when using a buccal swab, coupled with the inability to accurately quantify the RNA extracts before reverse transcription. Taqman assays were run on all markers as an additional test, to compare with the SYBR green data. All markers except SPTB showed very good specificity for their respective body fluids. SPTB, like in the SYBR green studies was detected in blood more than saliva, however detection was never consistent in each sample. It can therefore be said that real-time PCR using SYBR Green dye was capable of identifying specific mRNA markers blood and saliva however, the lack of specificity for this type of assay makes its use as a routine identification of body fluids in forensic casework not suitable. The main aim of this study was to develop a more cost effective BFID and as such involved the use of SYBR Green as a cheaper alternative to TaqMan. However, throughout these studies, it appeared to be quite costly in terms of validating a SYBR Green experiment, as more reagents were required in the long run due to vast amount of no template controls required per experiment. It therefore would appear that while SYBR Green is cheaper to buy, the cost to validate these type of experiments can be quite high, due to the non-specific nature of the dye itself. The SYBR Green studies were also much more time consuming with regards to data interpretation as post analysis of the amplification plot and melt curves is a necessity with this detection chemistry to ensure successful interpretation of the data

    A Cholesterol-Lowering Gene Maps to Chromosome 13q

    SummaryA cholesterol-lowering gene has been postulated from familial hypercholesterolemia (FH) families having heterozygous persons with normal LDL levels and homozygous individuals with LDL levels similar to those in persons with heterozygous FH. We studied such a family with FH that also had members without FH and with lower-than-normal LDL levels. We performed linkage analyses and identified a locus at 13q, defined by markers D13S156 and D13S158. FASTLINK and GENEHUNTER yielded LOD scores >5 and >4, respectively, whereas an affected-sib-pair analysis gave a peak multipoint LOD score of 4.8, corresponding to a P value of 1.26×10−6. A multipoint quantitative-trait-locus (QTL) linkage analysis with maximum-likelihood binomial QTL verified this locus as a QTL for LDL levels. To test the relevance of this QTL in an independent normal population, we studied MZ and DZ twin subjects. An MZ-DZ comparison confirmed genetic variance with regard to lipid concentrations. We then performed an identity-by-descent linkage analysis on the DZ twins, with markers at the 13q locus. We found strong evidence for linkage at this locus with LDL (P<.0002), HDL (P<.004), total cholesterol (P<.0002), and body-mass index (P<.0001). These data provide support for the existence of a new gene influencing lipid concentrations in humans

Collaboration (FHSC): Mafalda Bourbon (INSA)Background: The European Atherosclerosis Society Familial Hypercholesterolaemia Studies Collaboration (FHSC) global registry provides a platform for the global surveillance of familial hypercholesterolaemia through harmonisation and pooling of multinational data. In this study, we aimed to characterise the adult population with heterozygous familial hypercholesterolaemia and described how it is detected and managed globally. Methods: Using FHSC global registry data, we did a cross-sectional assessment of adults (aged 18 years or older) with a clinical or genetic diagnosis of probable or definite heterozygous familial hypercholesterolaemia at the time they were entered into the registries. Data were assessed overall and by WHO regions, sex, and index versus non-index cases. Findings: Of the 61 612 individuals in the registry, 42 167 adults (21 999 [53·6%] women) from 56 countries were included in the study. Of these, 31 798 (75·4%) were diagnosed with the Dutch Lipid Clinic Network criteria, and 35 490 (84·2%) were from the WHO region of Europe. Median age of participants at entry in the registry was 46·2 years (IQR 34·3-58·0); median age at diagnosis of familial hypercholesterolaemia was 44·4 years (32·5-56·5), with 40·2% of participants younger than 40 years when diagnosed. Prevalence of cardiovascular risk factors increased progressively with age and varied by WHO region. Prevalence of coronary disease was 17·4% (2·1% for stroke and 5·2% for peripheral artery disease), increasing with concentrations of untreated LDL cholesterol, and was about two times lower in women than in men. Among patients receiving lipid-lowering medications, 16 803 (81·1%) were receiving statins and 3691 (21·2%) were on combination therapy, with greater use of more potent lipid-lowering medication in men than in women. Median LDL cholesterol was 5·43 mmol/L (IQR 4·32-6·72) among patients not taking lipid-lowering medications and 4·23 mmol/L (3·20-5·66) among those taking them. Among patients taking lipid-lowering medications, 2·7% had LDL cholesterol lower than 1·8 mmol/L; the use of combination therapy, particularly with three drugs and with proprotein convertase subtilisin-kexin type 9 inhibitors, was associated with a higher proportion and greater odds of having LDL cholesterol lower than 1·8 mmol/L. Compared with index cases, patients who were non-index cases were younger, with lower LDL cholesterol and lower prevalence of cardiovascular risk factors and cardiovascular diseases (all p<0·001). Interpretation: Familial hypercholesterolaemia is diagnosed late. Guideline-recommended LDL cholesterol concentrations are infrequently achieved with single-drug therapy. Cardiovascular risk factors and presence of coronary disease were lower among non-index cases, who were diagnosed earlier.
