However, few studies have examined the differences in the frequency of vital sign measurement between patients with and without emergency tracheal intubation in the general ward. Thus, this study aimed to investigate the differences in the frequency of vital sign measurements between patients with and without emergency tracheal intubation.Patients and methods: This is a single-center, retrospective, observational study of unplanned intensive care unit (ICU) admissions from the general wards between December 2015 and February 2017. This study included patients with emergency medical intubations in the general ward who were then transferred to the ICU. Vital signs measured within 24 hours prior to ICU admission were compared between patients who did and did not require emergency tracheal intubation in the general ward. A survey was also conducted to explore the nurses’ attitudes toward vital sign measurements. Results: Compared with other vital signs, the respiratory rate was significantly less frequently measured. Moreover, the frequency of respiratory rate measurement was lower in the 38 patients who were intubated than in the 102 patients who were not intubated in the general ward (P=0.07). The survey revealed that 54% of the participating nurses considered assessment of the respiratory rate as the most troublesome nursing task and ~15% of nurses did not routinely measure respiratory rates. Conclusion: Respiratory rate was less frequently assessed in deteriorating patients in the general ward, possibly because it was considered a troublesome task. Keywords: emergency intubation, vital sign measurements, general ward, nurse’s attitudes toward vital signs, respiratory rat

INTRODUCTION New Perspectives on Horror Manga The primary objective of this study is to analyze Japanese horror comics, known as manga, through the works of Junji Ito, particularly Tomie. This author has become a central figure in the genre, and his work is essential for understanding how horror in manga achieves an impact that goes beyond its graphic elements, utilizing a distinctive visual and narrative style that creates a unique atmosphere and evokes profound emotions in its readers. The analysis begins with a comparison between sequential horror media, such as happenings, and editorial media like comics, where the resources available are different: while the former include sound, characters in motion, and dynamic storytelling, comics must rely on visual strategies to achieve equally powerful horror effects. This research aims to explore the artistic evolution of Junji Ito and how, through the use and modification of lines and styles, he constructs atmospheres that immerse the reader in his narratives. Throughout this study, the artistic styles that characterize horror manga will be examined, detailing the visual techniques that intensify the atmosphere, generate tension, and build unsettling narratives. The research seeks to answer a fundamental question: how is it possible to achieve, within editorial media—specifically horror comics—the same atmospheric effect that is present in sequential media? This analysis of Junji Ito delves into the visual and narrative elements that define his style and have earned him a prominent place in horror manga. The study also considers the direct and indirect consequences of modifications in illustration on atmosphere and story development, highlighting the use of line work, onomatopoeia, and other essential resources for creating visual impact. This work not only describes the influence of Junji Ito but also aims to serve as a guide for design and art students interested in illustration, providing tools to understand and construct effective atmospheres that instill in audiences a constant desire to explore further. In this sense, understanding Junji Ito’s style and techniques is fundamental for learning how to depict horror and tension within the realm of printed and editorial media. This project aimed to create a series of works based on two concepts, the German term unheimliche interpreted as weird, sinister or creepy, and a reflection on the work of the author Junji Ito, who has captured in his works fantastic representations of anguish. It also attempts to show how the concept of unheimliche is capable of being found within art, horror works and today's society. Finally, it explores the execution of the oil technique, to find ways to interpret social issues and transform them into surreal scenarios.PregradoLicenciado(a) en Artes Plástica

The Sedative Effect of Propranolol on Critically Ill Patients: A Case Series

    No full text
IntroductionRecent studies have examined the effectiveness of alpha-2 adrenergic agonists for controlling delirium and agitation. Propranolol, a non-selective beta-adrenergic antagonist with good penetration of the blood–brain barrier, has not been investigated for this purpose.Materials and methodsWe retrospectively reviewed the medical records of all patients who were prescribed propranolol in our Medical Surgical ICU from January 1, 2010, to December 31, 2013. We recorded the sedation level and daily dose of sedatives, analgesics, and antipsychotics administered each day for 6 days after starting propranolol, and compared them to the day before starting propranolol.ResultsSixty-four patients met inclusion criteria. Thirty-eight episodes met exclusion criteria, leaving 27 patients (31 episodes). The administration of propranolol was associated with significant reductions in fentanyl equivalents (65%, p = 0.009), midazolam equivalents (57%, p = 0.048), propofol (16%, p = 0.009), and haloperidol (44%, p = 0.024) on day 2 after starting propranolol compared with baseline. A stratified analysis showed that these decreases were seen regardless of clinical improvement or deterioration.ConclusionThe use of propranolol was associated with a significant reduction in doses of sedatives and analgesia. Further studies are needed to determine whether propranolol may be a useful adjuvant for managing delirium and agitation in the ICU

Abstract Background Recent studies have suggested that quick Sequential Organ Failure Assessment (qSOFA) scores have limited utility in early prognostication in high-mortality populations. The purpose of this study was to investigate the association between pre-ICU qSOFA scores and in-hospital mortality among patients admitted to the ICU with suspected sepsis. This study also aimed to describe detailed clinical characteristics of qSOFA-negative (< 2) patients. Methods This single center, observational study, conducted in a Japanese tertiary care teaching hospital between May 2012 and June 2016, enrolled all consecutive adult patients admitted to the ICU with suspected sepsis. We assessed pre-ICU qSOFA scores with the most abnormal vital signs during the 24-h period before ICU admission. The primary outcome was in-hospital mortality censored at 90 days. We analyzed the association between pre-ICU qSOFA scores and in-hospital mortality. Results Among 185 ICU patients with suspected sepsis, 14.1% (26/185) of patients remained qSOFA-negative at the time of ICU admission and 29.2% (54/185) of patients died while in hospital. In-hospital mortality was similar between the groups (qSOFA-positive [≥ 2]: 30.2% [48/159] vs qSOFA-negative: 23.1% [6/26], p = 0.642). The Cox proportional hazard regression model revealed that being qSOFA-positive was not significantly associated with in-hospital mortality (adjusted hazard ratio 1.35, 95% confidence interval 0.56–3.22, p = 0.506). Bloodstream infection, immunosuppression, and hematologic malignancy were observed more frequently in qSOFA-negative patients. Conclusions Among ICU patients with suspected sepsis, we could not find a strong association between pre-ICU qSOFA scores and in-hospital mortality. Our study suggested high mortality and bacterial diversity in pre-ICU qSOFA-negative patients

Chemistry of High Polymers with Net Work Structure

Folry's theory on the molecular weight distribution of three dimensional polymers is reviewed and its relationship with vulcanization of rubber is discussed. The elasticity of rubber is recently described by many workers. After Flory's theory, the elasticity is proportional to α-1/α2 where α is elongation ratio, and number of cress-linkages ν, while Kubo states that the elasticity is proportional to ν4/3, but Flory's opinion is more reasonable. The tensile strength of vulcanized rubber decreases linearly with reciprocal molecular weight of original rubbr, because of the network defect due to terminal chain. The vulcanzation lowers the concentration of terminal chain, but on the other hand has negative effect on account of its hindrance of crystallization on stretching. The theoretical equation for the tensile strength is propcsed, by which the maximum tensile strength is explained. The author discussed the reinforcing effect of filler, i.e. filler fixes rubber on its surface and acts as cross-linkage on stretching. The theoretical equation is introduced by author. Finally, the elastic property of plasticized polyvinyl-chloride is discussed. It is explained as visco-elasticity. Logarithm of elasticity decreases with prasticizer

Junji Ito is a vital artist and author of horror manga, with more and more of his works being published in Brazil and receiving adaptations worldwide. Considering his relevance, this work sought to explore qualitatively and analytically, based on interpretation and bibliographic analysis, the first three stories of volume 1 of the manga, analyzing the embodiments of Tomie, and by combining elements from other stories in the collection we sought to think about the character as an object that both terrifies and causes desire in those who get involved with any of her forms. We discussed the three basic concepts that we could attribute to Tomie's stories – horror, monster, and body horror –, defining the genre of horror by the ideas of Noël Carroll (1999), relying on the entries for "horror" and "monster" by Zanini (2019), in the Dictionary of Unusual Fiction, and we reflected on the studies of Xavier Aldana Reyes (2016) and Peter Hutchings (2008) regarding body horror and the construction of a monstrous body. We seek to understand what kind of monstrous manifestations are in Tomie and how they are articulated soon after her murder, in the short stories that begin the subsequent reappearances. We understand that the character is a vengeful spirit, an expression from Japanese folklore; that she is a metamorphic monster; and that her body materializes both the desire and the contempt for the female body felt by the male figures
