Intercostal artery pseudoaneurysm in pulmonary tuberculosis – A rare cause of hemoptysis: A case report with review of the literature
Pulmonary tuberculosis (TB) is one of the common causes of life-threatening hemoptysis, particularly in developing countries. Bronchial and pulmonary arteries are the major source of hemoptysis in pulmonary TB. Intercostal artery pseudoaneurysm as a cause of hemoptysis in pulmonary TB is extremely rare, with only a few reported cases. We describe a case of intercostal artery pseudoaneurysm in a patient of pulmonary TB, presenting as massive hemoptysis. Computed tomography bronchial angiography (CTBA) revealed a contrast-filled outpouching in a left upper lobe cavity. Catheter angiography revealed a pseudoaneurysm arising from one of the left intercostal arteries, which was managed subsequently by endovascular embolization. Pseudoaneurysm from visceral arteries is an extremely rare but important cause of massive hemoptysis in pulmonary TB, which if not managed promptly may prove fatal
Bronchial artery embolization in chronic pulmonary thromboembolism: A therapeutic dilemma
Bronchial artery embolization is the treatment of choice for the management of life-threatening massive hemoptysis. Chronic pulmonary thromboembolism (PTE) is one of the rare causes of hemoptysis. Management of hemoptysis in chronic PTE is a point of debate. In this article, we have reported one case of hemoptysis in chronic PTE managed successfully with bronchial artery embolization
How and When Leaders’ Self-Sacrifice Backfires: Examining the Impact on Subordinate Unethical Pro-Organizational Behavior
Self-sacrificial leadership has generally been studied in terms of its beneficial effects on organizations. However, recent research has challenged this notion by highlighting that leaders’ self-sacrifice can also produce dysfunctional outcomes for leaders and their followers. The present study draws on social identity theory and extends this novel perspective by examining whether self-sacrificial leader behaviors can inadvertently encourage unethical pro-organizational behavior (UPOB) among subordinates. The study further explores the mediating role of organizational identification and the moderating role of followers’ unethical tolerance and ethical organizational climate. Using time-lagged data from 271 employees, results confirm that leaders’ self-sacrifice is indirectly related to UPOB through organizational identification. The indirect relationship gets weakened when followers have low unethical tolerance, and the organizational climate is highly ethical. This study augments our understanding of the antecedents of UPOB and the boundary conditions under which UPOB may be curtailed. Implications for theory and practice are discussed
Current placenta previa management and outcome in a rural tertiary care centre
Background: The reported incidence for placenta previa averages 0.3% or 1 case in 300 to 400 deliveries. Multiparity, advanced maternal age, cigarette smoking, previous caesarean delivery, history of abortions or uterine surgical procedures are some of the risk factors contributing to the development of placenta previa. Massive obstetrical haemorrhage in placenta previa is associated with severe maternal morbidity and mortality. A significant number of mothers can be saved if right approach of management is followed in placenta previa. Methods: The present case study is a retrospective analysis carried out in a tertiary care centre to study the risk factors and maternal and perinatal outcome in cases of placenta previa. The study included antenatal patients diagnosed as placenta previa on sonography at or >26 weeks of pregnancy. Data was compiled and statistically analysed. Results: Incidence of Placenta Previa in our study was 0.54%. 73.2% patients had history of previous cesarean sections. 53.6% patients were referred from outside. 98.2% patients delivered by caesarean section. Obstetric hysterectomy was required in 3.5% of total cases. 10.7% cases required the ICU admission after delivery. There was no maternal mortality in the study group. The mean gestational age at delivery was 35±2.4weeks. The mean APGAR at 5 min was 9±2.2.Conclusions: Increasing rates of caesarean sections in present era indicate that incidence of Placenta Previa is expected to rise. Good antenatal care, availability of emergency obstetric services, infrastructure, blood bank facility, HDU and ICU care and NICU services can improve maternal and neonatal outcome.</jats:p
Association of obesity and bone disease among older adults in India: Role of modifiable risk factors
Background/objectives: Obesity and bone diseases have increased substantially worldwide in the last three decades, particularly among older adults. This study assesses the association between obesity and bone disease among older adults in India. Methods: The data from the Longitudinal Ageing Study in India (LASI) was considered for analysis. A total of 65,173 individuals above the age of 45 years were analyzed. The descriptive statistics, multivariable logistic regression and mediation analysis are done to analyse the determinants of obesity and bone diseases. Results: Sixteen percent of the older adults in India had bone diseases. Females (OR = 1.453, 95%CI = 1.254,1.684) with obesity (OR = 1.571, 95%CI = 1.344,1.837), those aged more than 70 years (OR = 1.834, 95% CI = 1.540,2.184), not in union, from SC category, from rural areas, consumed tobacco (OR = 1.269, 95%CI = 1.209, 1.331) and alcohol (OR = 0.902, 95%CI = 0.867, 0.950), and with no physical activity (OR = 1.228, 95%CI = 1.087, 1.387) in daily life had higher risks of bone diseases. The results of the mediation analysis showed the relationship between bone disease and overweight/obesity was significantly mediated by the three modifiable risk factors: alcohol intake, smoking, and physical activity. Conclusion: The association between obesity and bone disease was found to be statistically significant in this study. Women, not-in-union, the older, and those without any education, those have lack of health awareness are more likely to develop bone-related issues in their later life. Research investigating the relationship between obesity and bone health has the potential to identify novel targets for the treatment of bone-related illnesses, such as osteoporosis, fractures, and rheumatoid arthritis
Antimicrobial Effectiveness of Different Root Canal Irrigants on Viablility of Root Canal Flora
Introduction: Cleaning and shaping of sophisticated root canal system plays an important role in the success of endodontic treatment. In addition to root canal instrumentation, irrigation also becomes a crucial aspect in the effective disinfection of root canal system. The chemical substances like acids, chelating agents, alkaline solutions, Sodium Hypochlorite (NaOCl), oxidative agents, and normal saline are used as root canal irrigants. But each irrigant has its own limitations. With the advent of novel materials and techniques, the search for the optimal root canal irrigant still continues.
Aim: To evaluate and compare the antimicrobial effectiveness of various root canal irrigant solutions such as combination of 5.25% NaOCl and 2% Chlorhexidine gluconate, 2% Chlorhexidine gluconate, Miswak extract and normal saline for root canal irrigation.
Materials and Methods: In this prospective observational study was conducted from February 2021 to April 2021, on 10 patients with persistent endodontic infection on single rooted teeth. After obtaining access to the root canals, paper points were inserted and the soaked paper points were placed immediately in a microtube with two mL of Reduced Transport Fluid (RTF) to obtain the bacterial suspension. The sensitivity of gram negative anerobic bacteria and E.faecalis to different irrigants was assessed with disc diffusion test and bacterial load was evaluated with Colony Forming Unit (CFU) assay. The study was analysed statistically using Kruskal-Wallis test and Dunn test (post-hoc test).
Result: The results of disk diffusion method showed that the zone of inhibition for 2% chlorhexidine gluconate was 29 mm for gram negative anerobic bacteria and 22 mm for E.faecalis, and the zone of inhibition for Miswak extract was 8 mm for gram negative anerobic bacteria and 10 mm for E.faecalis. Gram negative anerobic bacteria on the culture plates treated with Miswak extract had 6.41 CFU/mL, and E.faecalis had 5.21 CFU/mL. Gram negative anerobic bacteria on the culture plates treated with normal saline had a CFU/mL of 6.77, and E.faecalis had a CFU/mL of 5.77. The antibacterial activity of miswak extract was lower than that of other irrigants. The normal saline lacked any antimicrobial qualities.
Conclusion: The antibacterial effectiveness of the combination of 5.25% NaOCl and 2% Chlorhexidine gluconate was found to be higher than, 2% Chlorhexidine gluconate, Miswak extract and normal saline for root canal irrigation. A combination of 5.25% NaOCl and 2% Chlorhexidine gluconate can be used as a better choice over the other three irrigants
Modeling the impact of networks and dependence on male and female entrepreneurs in India
One of the key challenges facing entrepreneurs in the early stages of their venture is the process of resource mobilization. Social networks are critical for entrepreneurs particularly in emerging economies as they serve as means for acquiring essential resources from suppliers in the early stage. Existing literature on gender and entrepreneurship has focused on the differences in the characteristics and composition of male and female entrepreneurial networks. However, less is known about the impact these differences in the types of social networks have on the entrepreneur's decision-making and the operations of their ventures. The thesis contributes to the entrepreneurship literature by synthesizing social network and resource dependence perspectives to explore the types of networks male and female entrepreneurs have for resource mobilization in an emerging economy context. The thesis then examines how these types of social networks—in terms of tie strength, diversity, density and size—interact with resource dependence to impact the business decision making processes and operations of male and female entrepreneurs in an emerging economy context, namely India. Thus, the primary research questions guiding the study are the following: (1) What are the types of networks possessed by male and female entrepreneurs in an emerging economy context? and (2) What is the impact—in terms of power relations and dependencies—that these social networks create for female entrepreneurs versus male entrepreneurs in the early stages of their ventures? A novel theoretical framework is developed of two types of networks—richer vs. poorer—to assess how networks differ for male and female entrepreneurs in India. Using this classification of networks, the thesis then explores how these types of networks and associated resource dependencies interact to influence the business decision making processes and operations of Indian male and female entrepreneurs. For the empirical component of the thesis, the author conducted 50 interviews with male and female IT entrepreneurs in India to assess the hypotheses developed from the literature review. To reduce variation in the educational background of respondents and variation in the industry sector of their ventures, the interviewees were drawn from graduates of specific programs/specializations from one of the Indian Institutes of Technology who set up entrepreneurial ventures in India in a related field: the software industry. The study finds that male entrepreneurs tend to have richer networks while their female counterparts tend to have poorer networks. Moreover, the social networks of Indian female entrepreneurs tend to have a greater degree of influence on the resource acquisition and operations of the venture compared to the male entrepreneurs in the study.L'un des défis auquel doivent faire face les entrepreneurs lors des premiers instants de leurs entreprise est le processus de mobilisation de ressource.
