Comparison of Body Composition Bio Electrical Impedance Analysis of Type-1 Diabetes vs. Non-Diabetes in Children and Adolescent
Background: Analyzing the body composition of children and adolescents with diabetes is becoming more and more popular. For
managing weight changes that may emerge from treatment and evaluating treatment response, it is essential to comprehend the
body composition of diabetic children.
Objective: To compare body composition by Bioelectrical Impedance Analysis (BIA) among T1DM versus healthy children and
adolescents in Pakistan.
Methods: This comparative cross-sectional study was performed during Jan-March, 2023. Type 1 diabetic children and adolescents
were enrolled from the pediatric endocrinology outpatient clinic of the National Institute of Child Health whereas healthy children
and adolescents were enrolled from a nearby school to the hospital location. Evaluation of anthropometry indices and bioelectrical
impedance analysis was performed after obtaining the consent of parents.
Results: A total of 100 subjects were enrolled in the study, 50 each in the diabetic and healthy group. The mean age of participants
was 10.4 ± 2.4 years. The majority of study subjects were females (61%). The mean duration of T1DM was 4.7 ± 0.8 years. Height
(127.4 ± 11.7 versus 139.5 ± 15, p<0.001), Waist-hip ratio (0.8 ± 0.1 versus 0.7± 0.3, p=0.037) and resistance (686.2 ± 90.7 versus
651.1 ± 96.6, p=0.002) were significantly higher among T1DM group than healthy group. Percentage of muscle mass (45.1 ± 8.2
versus 50.3 ± 7.6, p=0.008), body cell mass (47.1 ± 3.8 versus 50.6 ± 4.5, p=0.045), reactance (56.3 ± 9.8 versus 62.4 ± 4.2,
p=0.017) and phase angle (4.3 ± 0.9 versus 5.7 ± 0.6, p=0.012) were significantly lower in T1DM patients than healthy individuals.
Conclusion: BIA analysis showed that body composition parameters and body functional status were lower among T1DM children
and adolescents than in the healthy group in terms of resistance, reactance, and phase angle
Correlation Between Body Mass Index And Percent Body Fat Estimated By BioElectrical Impedance Analysis In Children Of Karachi
Background: The current definition of obesity implies a rise in total body fat, which may only be evaluated through an examination
of body composition. Body Mass Index (BMI) cannot detect percent body fat (BF%) effectively, prompting the need for novel, simple,
affordable, and resource-constrained methods of body fat assessment and measurement.
Objective: To determine the correlation of BMI and BF% estimated by BIA in children of Karachi
Methods: This cross-sectional study was performed in a school located in Karachi. The study was conducted from January to April
2023. The permission to conduct this study was taken from the National Institute of Child Health and School Administration. Healthy
children of age 6-15 years of either gender were enrolled in this study.
Results: Total 132 children were studied with a mean age of 10.8 ± 2.5 years. The age range was 6-15 years. There was almost
equal representation of both gender with 50.8% males and 49.2% females. Median weight, height, BMI, and BF% of study subjects
were 30.5 (IQR=23.4-42.4) Kg, 139.5 (IQR=128-153) cm, 15.4 (IQR=13.8-17.6) Kg/m2 and 10.2 (IQR=6.1-16.7) % respectively. A
significant strong positive correlation was seen for overall data (rs=0.645, p<0.001). Out of 132 observations, concordance between
BMI and BF% was seen in 46.2% of observations yielding a weighted agreement of 0.2961 which was significant (p<0.001).
Conclusion: This study analyzed that there was a strong positive correlation between the two body measures. However, the
classification in terms of BMI and BF% ranges showed a difference for the same individual with slight agreement. The finding for the
Pakistani population should be validated in a larger study
Frequency and Risk Factors of Diabetic Retinopathy in Children having Type-1 Diabetes Mellitus
Abstract: Background: Diabetic retinopathy is the most prevalent complication and the primary cause of blindness in people with type 1 diabetes mellitus. There are two categories of risk factors: modifiable and non-modifiable for development of diabetic retinopathy.
Objective: To determine the frequency and risk factors of diabetic retinopathy in children having type-1 diabetes mellitus.
Materials and Methods: Children aged 7 to 18 years participated in this cross-sectional study, which was carried out in the National Institute of Child Health’s endocrinology department. Clinical, anthropometric, and demographic data of the patients were gathered. To identify diabetic retinopathy, a fundus examination involved examining the optic disc, macula, retinal blood vessels, background, and fundus periphery.
Result: With an average age of 14.07±2.82 years, 157 patients were examined. 88(56.1%) were male and 69(43.9%) were female. Average HbA1C level was 10.10±2.63%. Total 116(73.9%) patients had diabetic retinopathy, of which 103(88.8%) had mild non-proliferative, 10(8.6%) had moderate non-proliferative, 1(0.9%) had severe non-proliferative, and 2(1.7%) had proliferative diabetic retinopathy. Diabetic retinopathy was significantly associated with age group (p=0.025), random blood sugar (p=0.050) and insulin regimen type (p=0.018). Male patients had a lower risk of diabetic retinopathy (OR=-0.497, p=0.069). Individuals with age 14 years or less had a lower probability of developing diabetic retinopathy (OR=0.437, p=0.027). Patients with High HbA1c had more risk to develop diabetic retinopathy (OR=1.314, p=0.003). Patients on basal bolus insulin had a higher risk of developing diabetic retinopathy (OR=2.378, p=0.020).
Conclusion: The results of our investigation showed that a significant portion of the study group had diabetic retinopathy. The most common kind of retinopathy was mild non-proliferative. Males who were less than 14 years old, had a low HbA1c, had the disease for less than ten years, and used regular insulin and neutral protamine Hagedorn (NPH) were less likely to develop diabetic retinopathy
Global age-sex-specific mortality, life expectancy, and population estimates in 204 countries and territories and 811 subnational locations, 1950–2021, and the impact of the COVID-19 pandemic: a comprehensive demographic analysis for the Global Burden of Disease Study 2021
Background Estimates of demographic metrics are crucial to assess levels and trends of population health outcomes. The profound impact of the COVID-19 pandemic on populations worldwide has underscored the need for timely estimates to understand this unprecedented event within the context of long-term population health trends. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 provides new demographic estimates for 204 countries and territories and 811 additional subnational locations from 1950 to 2021, with a particular emphasis on changes in mortality and life expectancy that occurred during the 2020–21 COVID-19 pandemic period. Methods 22¿223 data sources from vital registration, sample registration, surveys, censuses, and other sources were used to estimate mortality, with a subset of these sources used exclusively to estimate excess mortality due to the COVID-19 pandemic. 2026 data sources were used for population estimation. Additional sources were used to estimate migration; the effects of the HIV epidemic; and demographic discontinuities due to conflicts, famines, natural disasters, and pandemics, which are used as inputs for estimating mortality and population. Spatiotemporal Gaussian process regression (ST-GPR) was used to generate under-5 mortality rates, which synthesised 30¿763 location-years of vital registration and sample registration data, 1365 surveys and censuses, and 80 other sources. ST-GPR was also used to estimate adult mortality (between ages 15 and 59 years) based on information from 31¿642 location-years of vital registration and sample registration data, 355 surveys and censuses, and 24 other sources. Estimates of child and adult mortality rates were then used to generate life tables with a relational model life table system. For countries with large HIV epidemics, life tables were adjusted using independent estimates of HIV-specific mortality generated via an epidemiological analysis of HIV prevalence surveys, antenatal clinic serosurveillance, and other data sources. Excess mortality due to the COVID-19 pandemic in 2020 and 2021 was determined by subtracting observed all-cause mortality (adjusted for late registration and mortality anomalies) from the mortality expected in the absence of the pandemic. Expected mortality was calculated based on historical trends using an ensemble of models. In location-years where all-cause mortality data were unavailable, we estimated excess mortality rates using a regression model with covariates pertaining to the pandemic. Population size was computed using a Bayesian hierarchical cohort component model. Life expectancy was calculated using age-specific mortality rates and standard demographic methods. Uncertainty intervals (UIs) were calculated for every metric using the 25th and 975th ordered values from a 1000-draw posterior distribution.Research reported in this publication was supported by the Gates Foundation, UK Department of Health and Social Care, the Norwegian Institute of Public Health, and the New Zealand Ministry of Health.
