    Canagliflozin (Invokana®): a First-in-Class Anti-diabetic Drug

    The US Food and Drug Administration have approved (on March 29, 2013) a novel glucose-lowering agent, canagliflozin (Invokana, Janssen Pharmaceuticals) for the treatment of adults with type 2 diabetes. Canagliflozin is the first in a new class of drug, an oral inhibitor of sodium glucose cotransporter 2 (SGLT2). Inhibition of SGLT2 decreases glucose reabsorption in the renal tubule and increases urinary glucose excretion (UGE), with a consequent lowering of plasma glucose levels as well as weight loss.The profile of effective glucose lowering, weight loss, improved b-cell function, and low risk of hypoglycemia suggest that canagliflozin may be a clinically useful new antihyperglycemic agent

    Quantifying the effects of spatial resolution on the mesoscale modelling of the Dutch storm of 1953

    The Weather Research and Forecasting (WRF) model is used to investigate the horizontal spatial resolution sensitivity by simulating the 1953 Dutch storm, the grid sizes are 27, 9, 3, 1, 0.5 km from domain 1 to domain 5 respectively. Overall, the the probability density functions of wind speed of all time do not show higher resolution corresponding to higher maximum wind speed and more possibility to detect stronger winds. Because finer resolution domains feature a relatively weak model of wind intensity than coarser resolution domains. In the case, simulations are used two PBL scheme, YSU and Shin-Hong scheme. Three measurement stations with historical meteorology observation in 1953 are selected, Schiphol Netherlands, Leeuwarden Netherlands and Bentwaters United Kingdom. By comparison the measured data and all simulated data, the result from Shin-Hong scheme at the inner domain (d05) perform the best wind speed.Geoscience and Remote Sensin

    A pregnancy with GDM, hypothyroidism, polyhydrramnios with history of treatment for subfertility

    Gestational diabetes (GDM) is defined as carbohydrate intolerance that begins or is first recognized during pregnancy. Although it is a well-known cause of pregnancy complications, its epidemiology has not been studied systematically. There are several identifiable predisposing factors for GDM, and in the absence of risk factors, the incidence of GDM is low. Low thyroid hormone levels in early pregnancy are a risk factor for GDM incidence. Although gestational hyperthyroidism is uncommon (0.2%), hypothyroidism (autoimmune disease or suboptimal iodine intake) occurs in 2.5% of women and is predictive of reduced neonatal and child neuropsychological development and maternal obstetric complications.  It is well documented that thyroid disorders are associated with maternal and fetal complications during gestation and sequelea after delivery. The case-finding strategy does not solve the serious problem of leaving undiagnosed and untreated patients without risk factors. In the absence of strong evidence that support universal thyroid screening in pregnancy, current guidelines suggest a case-finding approach targeting thyroid function testing in high-risk groups. A couple have history of sub-fertility. Patient conceived following treatment. During pregnancy, GDM developed and later half of pregnancy was complicated by polyhydramnios.     Background: Oral lesions are common among Human Immuno deficiency Virus (HIV)-positive patients. The pattern of oral features in these persons may differ in separate settings. Aims: To find out the spectrum of oral manifestations among a section of seropositive individuals attending the antiretroviral therapy (ART) centre of a referral hospital. Materials and Methods: A total of three hundred and twenty-one newly diagnosed seropositive patients were enrolled in this study. Of these, ninety-four patients who demonstrated lesions related to the oral cavity were examined intra-orally by a clinician. HIV testing was done with ELISA and CD4 counts were measured with the help of fluorescence activated cell sorter (FACS) system. Results: Fifty-four respondents presented exclusively with oral lesions. Candidiasis patients were the largest group (38.30&#x0025;). Conclusion: HIV disease presents a plethora of oral manifestations, which provide valuable diagnostic and prognostic information

    A realistic simulation of Leipzig Wind Profile

    Over the years, the Leipzig Wind Profile observed under near neutral condition has been considered as an essential benchmark for idealized friction layer models. However, the general weather condition for the Leipzig Wind Profile still remains a mystery after nearly 90 years. In order to simulate this event and try to recreate the weather conditions at that time, the WRF model driven by two types of reanalysis data setting up with two domains is launched. The model captures the wind physics well, except for a small deviation around 800 meters. Additionally, the simulated surface friction velocity and surface heat flux at Leipzig are close to the validation value. The simulated temperature slope at Lindenberg has some deviation compared to the documented value, which may be caused by the vertical coarse resolution and sensitive temperature fluctuations over the height. Moreover, there may have been rain or drizzle when the observation took place and this may have contributed to the near neutral condition feature.
