    Influence of caries infiltrant contamination on shear bond strength of different adhesives to dentin

    OBJECTIVES: To analyze whether the contamination with a caries infiltrant system impairs the adhesive performance of etch-and-rinse and self-etching adhesives on dentin. MATERIALS AND METHODS: Dentin contamination with the caries infiltrant system (Icon, DMG) was simulated by applying either hydrochloric acid (15 % HCl, Icon Etch, 15 s), the resin infiltrant (Icon infiltrant, 4 min), or both prior to the application of the respective adhesives (each group n = 10). In the control groups, the etch-and-rinse adhesive (Optibond FL, Kerr) and the self-etching adhesive (iBOND Self Etch, Hereaus) were applied without former contamination with the infiltrant system. Additionally, the adhesive performance of the resin infiltrant alone was tested. Shear bond strength of a nano-hybrid composite was analyzed after thermocycling (5,000×, 5-55°C) of the specimens and analyzed by ANOVA/Scheffé post hoc tests (p < 0.05) and Weibull statistics. Failure mode was inspected under a stereomicroscope at × 25 magnification. RESULTS: Contamination with the resin infiltrant alone did not impair shear bond strength, while contamination with hydrochloric acid or with hydrochloric acid and the resin infiltrant reduced shear bond strength (MPa) of the adhesives (Optibond FL: 20.5 ± 3.6, iBOND Self Etch: 17.9 ± 2.6) significantly. Hydrochloric acid contamination increased the number of adhesive failures. The adhesive performance of the caries infiltrant system alone was insufficient. CONCLUSION: The contamination with the caries infiltrant system impaired the shear bond strength of conventional dental adhesives. CLINICAL RELEVANCE: Contamination of the caries infiltrant system on dentin should be avoided due to the detrimental effect of hydrochloric acid etching

    Influence of study design on the impact of bleaching agents on dental enamel microhardness: a review

    OBJECTIVE: Numerous studies investigated the impact of bleaching procedures on enamel microhardness. The outcomes of these studies reveal inconsistencies regarding the fact whether a microhardness reduction due to bleaching occurs or not. Aim of the present review was to summarize the existing literature of external bleaching therapies, which used microhardness tests for evaluation of possible effects on enamel and to weigh up different parameters of the study designs with respect to the outcome of these studies. METHODS: The data from original scientific full papers listed in PubMed or ISI Web of Science (search term: enamel and (bleaching or peroxide) and (hardness or microhardness or Knoop or Vickers)) and received by additional hand-search meeting the inclusion criteria were included in the review. Influences of different parameters on the outcome of the bleaching treatments were analyzed with the Fisher's-exact-test. RESULTS: A total of 55 studies were identified with 166 hardness measurements conducted directly after bleaching and 69 measurements performed after a post-treatment episode. Directly after bleaching, 84 (51%) treatments showed microhardness reduction compared to baseline, whereas 82 (49%) did not yield microhardness reduction. After the post-treatment episode, 20 (29%) treatments showed hardness reduction and 49 (71%) did not. A significant higher number of bleaching treatments resulting in enamel microhardness reduction were observed, when artificial instead of human saliva was used for storage of the enamel samples in the intervals between the bleaching applications and when no fluoridation measures were applied during or after the bleaching phase. SIGNIFICANCE: The review shows that in those studies, which simulated the intraoral conditions as closely as possible, the risk of enamel microhardness decrease due to bleaching treatments seems to be reduced. Nevertheless more in situ- and in vivo-studies are needed to verify this observation

    Einführung: Der restaurative Aufbau verloren gegangener Zahnhartsubstanz und die Wiederherstellung der ursprünglichen vertikalen Bisslage bei einem durch Zahnerosionen oder -abrasionen stark in Mitleidenschaft gezogenen Gebiss stellt den Zahnarzt immer wieder vor Probleme bei der Umsetzung der Therapie. Traditionellerweise erfolgt diese Therapie mit laborgefertigten Werkstücken, die i.d.R. eine Präparation der verbliebenen Zahnsubstanz erfordern und für den Patienten hohe Kosten verursachen, sodass alternative Verfahren in Betracht gezogen werden sollten. Dabei gilt es hervorzuheben, dass jedwede, auch minimalinvasive restaurative Arbeiten bei diesen Patienten nur dann indiziert sind, wenn der präventive Ansatz, weiteren Zahnhartsubstanzverlust zu verhindern, gleichzeitig erfolgreich ist. Behandlungsmethode: Der folgende Beitrag stellt die Verwendung direkter adhäsiver Kompositrestaurationen als eine Möglichkeit zur Rekonstruktion einer erosiv stark veränderten Gebisssituationen anhand eines Falls dar. Zur Rekonstruktion der Okklusalflächen werden dabei Übertragungsschienen eingesetzt, die auf der Basis von individuellen Wax-up-Modellen angefertigt werden. Schlussfolgerung: Das beschriebene Verfahren ist eine gut untersuchte und erprobte Methode zur Versorgung von Zähnen mit erosionsbedingtem Zahnhartsubstanzverlust. Wie bei allen neuen Verfahren wird sich für den praktischen Zahnarzt eine gewisse Lernkurve ergeben, nach der hochwertige Restaurationen mit dieser Technik umgesetzt werden können.     Clinical pilot series of non-self-contained periodontal infrabony defects treated with a slowly resorbable bovine pericardium membrane in combination with low-temperature-treated decellularized bovine bone particles

    The aim of this case series was to present the clinical outcomes of non-contained intrabony periodontal defects (IPDs) treated by means of papillary preservation flaps in association with a slowly resorbable bovine pericardium membrane (BPM) and a low-temperature-treated bovine bone graft (BBG). Eight healthy, non-smoking patients (two males and six females, mean age 48 ± 8 years) with stage 3 periodontitis and at least one site with residual probing depth (PD) ≥ 6 mm associated with a non-contained IPD ≥ 3 mm were treated. Two weeks after surgery, no adverse events were observed, and an early wound healing score (EHS) of 8.1 ± 1.0 was recorded. After 1 year, the mean probing depth (PD) reduction and mean clinical attachment level gain (CAL-gain) accounted for 4.8 ± 0.7 and 3.5 ± 0.7 mm, respectively, whereas the mean gingival recession (REC) was of 1.2 ± 0.3 mm. Radiographic bone fill was observed in all cases. In conclusion, the treatment of non-contained IPDs with a slowly resorbable BPM and a low-temperature-treated BBG could be considered safe and may result in significant clinical improvements 1 year after surgery

    Direct restorative technique in posterior teeth to treat erosion induced tooth wear

    Introduction: The restorative reconstruction of lost tooth structure and the restoration of the original vertical bite position in a dentition that has been severely affected by tooth erosion or abrasion always presents the dentist with problems in implementing the therapy. Traditionally, this therapy is carried out with laboratory-produced restaurations, which usually require preparation of the remaining tooth substance and result in high costs for the patient, so that alternative procedures should be considered. It should be emphasized that any restorative work, even minimally invasive, is only indicated in these patients if the preventive approach of inhibiting further loss of tooth structure is successful at the same time. Treatment method: The following article presents a case study on the use of direct adhesive composite restorations as a possibility for reconstructing an erosively severely altered dentition. Transfer splints, which are fabricated on the basis of individual wax-up models, are used to reconstruct the occlusal surfaces. Conclusion: The procedure described is a well-studied and proven method for restoring teeth with erosion-induced tooth wear. As with all new procedures, there will be a certain learning curve for the practicing dentist, after which high-quality restorations can be implemented using this technique.     Influence of fluoride concentration and ethanol pre-treatment on the reduction of the acid susceptibility of enamel

    OBJECTIVE: To determine the association between KOH-soluble and structurally bound fluoride uptake and the erosion resistance of enamel, respectively. Additionally, the effect of enamel pre-treatment with ethanol before fluoridation was assessed. METHODS: Sixty bovine incisors (4 specimens/tooth) were randomly allocated to six groups (A-F). Samples 1 and 2 remained untreated, serving as control at baseline. Pre-treatment of the samples was performed for 5 min with 99% ethanol (groups A, B and C) or physiologic saline (groups D, E and F). Samples 3 and 4 were treated either with 0.5% (groups A and D), 1.0% (groups B and E) or 1.5% (groups C and F) fluoride solution. In samples 1 and 3, uptake of KOH-soluble and structurally bound fluoride was determined. Samples 2 and 4 were used for the determination of acid susceptibility by immersion in 1 ml HCl for 30s. Calcium release into HCl was assessed by atomic absorption spectroscopy. Differences between the groups were calculated by unpaired t-tests (p<0.05). RESULTS: Mode of pre-treatment showed no influence on fluoride acquisition. KOH-soluble and structurally fluoride uptake increased with increasing fluoride concentrations. Highest acid resistance was observed after treatment with 1% fluoride solution for both kinds of pre-treatment followed by 1.5% and 0.5% fluoride solution. CONCLUSION: Dose-dependency was observed for enamel fluoride acquisition but not for acid resistance

    Influence of two different fluoride compounds and an in vitro pellicle on the amount of KOH-soluble fluoride and its retention after toothbrushing

    Objective. To determine the influence of two different fluoride compounds and an in vitro pellicle on KOH-soluble fluoride formation - its retention and resistance to toothbrushing. Material and methods. Forty bovine incisors were randomly assigned to four groups (A-D). Of five samples prepared per tooth, one remained untreated and served as a baseline control. Groups A and B were pretreated with artificial saliva and groups C and D with human saliva. Groups A and C were treated with amine fluoride and groups B and D with sodium fluoride. After treatment, samples were brushed with 25, 50, and 75 brushing strokes. The amount of KOH-soluble fluoride formed on the enamel samples was measured at baseline, after application, and after 25, 50, and 75 brushing strokes. Fluoride uptake was calculated by unpaired t-tests and fluoride retention by paired t-tests. Results. No statistically significant differences in the KOH-soluble fluoride uptake of the groups that were pretreated (A vs B and C vs D) or treated equally (A vs C and B vs D) were observed. Retention of the KOH-soluble fluoride in the brushed samples was higher when the samples were pretreated with human saliva and treated with sodium fluoride than when the samples were pretreated with artificial saliva and treated with sodium fluoride. Conclusions. The fluoride compound and the acquired human in vitro pellicle have no influence on the uptake of KOH-soluble fluoride, but show a significant influence on abrasion resistance

    The Rigid-Shield Technique: A New Contour and Clot Stabilizing Method for Ridge Preservation

    Tooth extraction causes vertical and horizontal alveolar bone loss and consequent remodeling. Several methods have been introduced in terms of so-called &ldquo;ridge preservation&rdquo; techniques, which mostly resemble guided bone regenerative (GBR) procedures using filler materials and membranes in order to stabilize the respective sites. This conceptual case report describes a novel approach using a degradable polylactic acid membrane covered with a collagen matrix, which aims to reshape the resorbed alveolar wall and thereby to stabilize the soft tissues during matrix formation and socket mineralization. Clinical re-entry, radiographic (CBCT) and histologic evaluation proved adequate for osteoneogenesis despite an unfavorable initial situation: An implant could be ideally placed, which was circumferentially covered by bone. This minimally invasive method could offer a new method to approach socket preservation without using filler materials and coverage of the socket entrance. However, more controlled research on this topic is needed
