    Rhinitis in children: common clinical presentations and differential diagnoses

    Rhinitis is a common presentation in childhood. Acute virally induced rhinitis is generally self-limiting and usually does not require medical attention. Whilst allergic rhinitis is the focus of the paediatric allergist, the presentation of other diseases or comorbidities that can complicate or mimic allergic rhinitis needs to be considered. Effects on the child's quality of life also need to be addressed. Rhinitis can be associated with asthma and other significant comorbidities: importantly, non-allergic rhinitis can sometimes be a consequence of systemic immune impairment. The diagnosis of rhinitis is based on clinical findings with directed investigations. Nasal nitric oxide measurement is an emerging diagnostic tool and helpful particularly in relation to evaluating the differential diagnosis in more difficult rhinitis. Successfully identifying the cause of rhinitis in childhood and associated comorbidities can ensure that the patient is successfully treated as described in the recently published EAACI Pediatric Rhinitis Position Paper

    Background:Sarcoidosis staging has primarily relied on the Scadding chest radiographic system, although chest CT is finding increased clinical use.Research Question:Whether standardized CT assessment provides additional understanding of lung function beyond Scadding stage and demographics is unknown and the focus of this study.Study Design and Methods:We used the NHLBI study Genomics Research in Alpha-1 Anti-Trypsin Deficiency and Sarcoidosis (GRADS) sarcoidosis cases(N=351) with Scadding stage and Chest CT scans obtained in a standardized manner. One chest radiologist scored all CT scans with a visual scoring system, with a subset read by another chest radiologist. We compared demographic features, Scadding stage and CT findings, and correlation between these measures. Associations between spirometry and DLCO and CT and Scadding stage were determined using regression analysis (N=318). Agreement between readers was evaluated using Cohen\u2019s Kappa.Results:CT features were inconsistent with Scadding stage in about ~40% of cases. Most CT features assessed on visual scoring were negatively associated with lung function. Associations persisted for FEV1 and DLCO when adjusting for Scadding stage, although some CT feature associations with FVC became insignificant. Scadding stage was primarily associated with FEV1 and inclusion of CT features reduced significance in association between Scadding and lung function. Multivariable regression modeling to identify radiologic measures explaining lung function included Scadding stage for FEV1 and FEV1/FVC (P<0.05) and marginally for DLCO (P<0.15). Combinations of CT measures accounted for Scadding stage for FVC. Correlations among Scadding and CT features were noted. Agreement between readers was poor to moderate for presence/absence of CT features and poor for degree/location of abnormality.Interpretation:CT features explained additional variability in lung function beyond Scadding stage, with some CT features obviating the associations between lung function and Scadding. Fecha de lectura: 20-11-2020El objetivo de la investigación es analizar los elementos retóricos y poéticos de los relatos que componen la colección policiaca del padre Brown de G.K. Chesterton. A través de la aplicación de la Teoría del Lector Modelo, de Umberto Eco y de la Teoría de los Mundos Posibles –en su versión de Tomás Albaladejo– se pretende descender al proceso creativo del autor en su concepción del entramado policial y estudiar el proceso comunicativo de Chesterton, su intencionalidad y objetivos, así como la recepción en sus lectores. La Teoría del Lector Modelo permite estimar el tipo de lector que concibió el autor y la Teoría de los Mundos Posibles esbozar las distintas perspectivas de los personajes ideados para que se produzca la sorpresa esperada en la resolución de un relato policiaco. Asimismo, se presta especial atención al narrador y la focalización. Para concluir establecemos las características policiacas propias del autor con lo que consideramos que es su aportación genuina al mundo del relato criminal, deteniéndonos de manera particular en el crimen, el detective y el proceso de investigaciónThe focus of this research is to analyse the retoric and poetic aspects of the tales which are part of the compilation of the detective Father Brown Stories, by G.K. Chesterton. In this academic work we implement the Umberto Eco's theory of textual cooperation –principally the Model Reader– and the Theory of Possible Worlds developed by Tomás Albaladejo. The investigation intends to deepen the commuticative process of G.K Chesterton. In addition, we attempt to study the creative process of the author, his intention and his aims. Particularly, to find his thoughts about the receipt of the detective novel in his readers. The Model Reader theory helps to guess the type of reader that the author conceived. The Theory of Possible Worlds lets consider the perspectives of the characters in order to get the expected surprise in resolving the mystery. We also pay special attention to the storyteller and the focalisation. To conclude, we determine the own characteristics of G.K. Chesterton with his genuine contribution to the detective novel world, especially about the crime, the detetive and the investigation proces

    Chronic Rhinosinusitis—Could Phenotyping or Endotyping Aid Therapy?

    Objectives: We reviewed the phenotyping and endotyping of chronic rhinosinusitis (CRS) and treatment options. Methods: We searched PubMed, Google, Google Scholar, and the Proquest Central Database of the Kırıkkale University Library. Results: Phenotypes are observable properties of an organism produced by the environment acting upon the genotype, that is, patients with a particular disorder are subgrouped according to common characteristics. Currently, CRS is usually phenotyped as being with (CRSwNP) or without (CRSsNP) nasal polyps. However, this is not immutable as some individuals progress from nonpolyp to polypoid CRS over time. Phenotypes of CRS are also based on inflammatory patterns, generally CRSwNP is eosinophilic, CRSsNP neutrophilic; but there is a spectrum, rather than a clear-cut division into 2 types. An endotype is a subtype of a condition defined by a distinct functional or pathobiological mechanism. Endotypes of CRS can be (1) nontype Th2, (2) moderate type Th2, and (3) severe type Th2 immune reactions, based on cytokines and mediators such as IL4, 5, 13. CRS endotyping can also include a (1) type 2 cytokine-based approach, (2) eosinophil-mediated approach, (3) immunoglobulin E-based approach, and (4) cysteinyl leukotriene-based approach. Subdivisions of CRSwNP can be made into nonsteroidal anti-inflammatory drug-exacerbated respiratory disease, allergic fungal sinusitis, and eosinophil pauci-granulomatous arteritis by testing. General treatment for all CRS is nasal douching. The place of surgery needs careful reconsideration. Endotype-directed therapies include glucocorticosteroids, antibiotics, aspirin, antifungals, anticytokines, and immunoglobulin replacement. The recognition of united airways and the co-occurrence of CRSwNPs and severe asthma should lead to common endotyping of both upper and lower airways in order to better direct therapy. Conclusion: Endotyping can allow for the identification of groups of patients with CRS with a high likelihood of successful treatment, such as patients with a moderate type 2 immune reaction or those with acquired immune deficiency. © The Author(s) 2018.With exception of data collection, preparation of this paper including design and planning was supported by Continuous Education and Scientific Research Association.     BSACI guidelines for the management of allergic and non-allergic rhinitis

    This guidance for the management of patients with allergic and non-allergic rhinitis has been prepared by the Standards of Care Committee (SOCC) of the British Society for Allergy and Clinical Immunology (BSACI). The guideline is based on evidence as well as on expert opinion and is for use by both adult physicians and paediatricians practicing in allergy. The recommendations are evidence graded. During the development of these guidelines, all BSACI members were included in the consultation process using a web-based system. Their comments and suggestions were carefully considered by the SOCC. Where evidence was lacking, consensus was reached by the experts on the committee. Included in this guideline are clinical classification of rhinitis, aetiology, diagnosis, investigations and management including subcutaneous and sublingual immunotherapy. There are also special sections for children, co-morbid associations and pregnancy. Finally, we have made recommendations for potential areas of future research

    The institution of property is undeniably one of the most important elements of every social and economic system. It affects every human being, playing a vital role in everyday life of the whole human race. Nevertheless, its origin and true meaning is not always treated with due attention, which sometimes leads to practical errors and improper application of this essential idea in human societies. The article concerns the true importance of private property, based on works by two British writers of the early 20th century, H. Belloc and G.K. Chesterton. The author describes the economic freedom attached to private property and shows it as a condition necessary for maintaining basic human dignity. Then he presents two of the most common errors concerning perception and application of property – the limitation of its positive effects in capitalism and the futile attempt of its abolition in socialism.Instytucja własności stanowi jeden z najistotniejszych składników każdego systemu społecznego i ekonomicznego, ponieważ oddziałuje bezpośrednio na życie każdego człowieka, będąc praktycznym elementem zwykłej codzienności. Nie zawsze jednak poświęca się jej genezie należytą uwagę, a w toku dziejów nie brakowało rozmaitych błędnych poglądów przeinaczających właściwe znaczenie owego podstawowego urządzenia społecznego. W niniejszym artykule została podjęta próba zaprezentowania elementarnej wartości własności prywatnej w oparciu o twórczość dwóch brytyjskich pisarzy początku XX w. – H. Belloca i G.K. Chestertona. Autor zwraca uwagę na wynikającą z posiadania wolność ekonomiczną oraz ukazuje własność prywatną jako warunek konieczny szeroko pojętej godności człowieka.     Here is a copy of the 1969 An old Library of Congress book that was never taken out once and so is in excellent condition. Vastly superior in its illustrations to the Avenel facsimile (1975?). This is not a facsimile, for it omits the illustration of the blackamoor being scrubbed to death! Otherwise it fully represents the 1912 original except for its sturdy simple covers and binding. I will keep the dust-jacketed extra in the collection.
