cancer patients dates back to 1975 [1], its use has never gained wide acceptance among oncologists. Only in recent years has HPN prompted renewed interest [2-4], possibly because the availability of new anticancer drugs made imperative allowing completion of scheduled therapies to achieve full benefit despite treatment-associated toxicities. A major barrier to development and implementation of HPN programmes in cancer patients is the lack of statistically robust randomized controlled trials (RCTs), which inform strong recommendations in international guidelines. However, it has been already noted [5] that the peculiar design of nutrition trials may not fully fit the procedures required to issue guidelines. In particular, the inclusion of a no-nutrition arm may not be ethically possible when hypo- or aphagic cancer patients are studied. Furthermore, the growing awareness of the role of inflammation in the pathophysiology of cancer cachexia has led some oncologists to underestimate the potential of nutritional support as if the hypophagic cachectic patient ceased to require nutritive substrates to sustain the body's physiologic functions. Cardiotoxicity Induced by Immune Checkpoint Inhibitors: What a Cardio-Oncology Team Should Know and Do
Immune checkpoint inhibitors (ICIs) have revolutionized the therapeutic scenario for several malignancies. However, they can be responsible for immune-related adverse events (irAEs), involving several organs, with a pooled incidence ranging between 54% and 76%. The frequency of cardiovascular system involvement is <1%. Among the cardiovascular irAEs, myocarditis is the most common and the most dangerous but other, less common manifestations of ICI-related cardiotoxicity include pericardial disease, arrhythmias, Takotsubo-like syndrome, and acute myocardial infarction, all of which remain poorly explored. Both oncologists and cardiologists, as well as the patients, should be aware of the possible occurrence of one or more of these complications, which in some cases are fatal, in order to implement effective strategies of cardiac surveillance. In this review, we summarize the latest studies and recommendations on the pathogenesis, clinical manifestation, diagnosis, and management of ICI-related cardiotoxicity in order to realize a complete and updated overview on the main aspects of ICI-related cardiotoxicity, from surveillance to diagnosis to management, useful for both oncologists and cardiologists in their clinical practice. In particular, in the first part of the review, we realize a description of the pathogenetic mechanisms and risk factors of the main cardiovascular irAEs. Then, we focus on the management of ICI-related cardiotoxicity by analyzing five main points: (1) identifying and evaluating the type and severity of the cardiotoxicity; (2) deciding whether to withhold ICI therapy; (3) initiating steroid and immunosuppressive therapy; (4) starting conventional cardiac treatment; and (5) restarting ICI therapy. Finally, we discuss the existing evidence on surveillance for ICI-related cardiotoxicity and propose a surveillance strategy for both short- and long-term cardiotoxicity, according to the most recent guidelines

The article analyzes the relationship between history and the passions thorough the borrowings from the Storia d’Italia. Guicciardini by Francesco Guicciardini. Guicciardini's influence on the author of the Essais seems to extend to both the content and the form of the borrowed texts (their style, description techniques, and so on), and indeed to the treatment of the passions described. The analysis of the examples borrowed from Guicciardini in Book I fo the Essais leads to some initial conclusions ; although, there is further research yet to be done.L’étude analyse le rapport entre l’Histoire et les passions à travers les emprunts à la Storia d’Italia de Guichardin. Role for non-proteolytic control of M-phase-promoting factor activity at M-phase exit
M-phase Promoting Factor (MPF; the cyclin B-cdk 1 complex) is activated at M-phase onset by removal of inhibitory phosphorylation of cdk1 at thr-14 and tyr-15. At M-phase exit, MPF is destroyed by ubiquitin-dependent cyclin proteolysis. Thus, control of MPF activity via inhibitory phosphorylation is believed to be particularly crucial in regulating transition into, rather than out of, M-phase. Using the in vitro cell cycle system derived form Xenopus eggs, here we show, however, that inhibitory phosphorylation of cdk1 contributes to control MPF activity during M-phase exit. By sampling extracts at very short intervals during both meiotic and mitotic exit, we found that cyclin B1-associated cdk1 underwent transient inhibitory phosphorylation at tyr-15 and that cyclin B1-cdk1 activity fell more rapidly than the cyclin B1 content. Inhibitory phosphorylation of MPF correlated with phosphorylation changes of cdc25C, the MPF phosphatase, and physical interaction of cdk1 with wee1, the MPF kinase, during M-phase exit. MPF down-regulation required Ca++/calmodulin-dependent kinase II (CaMKII) and cAMP-dependent protein kinase (PKA) activities at meiosis and mitosis exit, respectively. Treatment of M-phase extracts with a mutant cyclin B1-cdk1AF complex, refractory to inhibition by phosphorylation, impaired binding of the Anaphase Promoting Complex/Cyclosome (APC/C) to its co-activator Cdc20 and altered M-phase exit. Thus, timely M-phase exit requires a tight coupling of proteolysis-dependent and proteolysis-independent mechanisms of MPF inactivation
