Is Systemic (Interstitial Fluid) Acidosis an Initial Event in the Etiopathogenesis of Dental Caries? A Hypothesis
Introduction: Dental caries is a microbial disease which affects retentive areas of the teeth. It has shown significant increase in incidence and prevalence in modern population. Various theories have been postulated in an attempt to explain the etiopathogenesis of dental caries. Despite their popularity, there are many questions which various theories haven’t been able to answer. And still there is no definite cure for dental caries. Consequently, in most cases the process can’t be reversed. Therefore, to tackle this issue other risk factors involved in the etiopathogenesis need to be explored. The Hypothesis: It could be suggested that there might be additional systemic factors, e.g. systemic acidosis, in pathogenesis of dental caries, in addition to proven localized causative factors. Evaluation of the Hypothesis: Various hypotheses have been discussed, on the basis of histopathology, and various research articles. The demineralization of bone osteoporosis and tooth demineralization of dental caries have been correlated to evaluate the hypothesis. Few parameters have been added in the proven local factors of etiopathogenesis of dental caries
Intraosseous malignant peripheral nerve sheath tumor of maxilla: A case report with review of the literature
Malignant peripheral nerve sheath tumor (MPNST), the principle malignancy of peripheral nerve origin, though rare in the general population, occurs with excessive frequency among patients with neurofibromatosis. This tumor always arises in soft-tissues, usually found in the lower extremities and only 10-12% of all lesions occur in the head and neck region, which makes it a rare entity. The primary intraosseous MPNST is rare and has been reported most frequently in the mandible. This article discusses a case report of MPNST of the left maxilla without a history of benign nerve tissue tumor and the diagnostic difficulties associated with MPNST

Odontoblasts in health and disease with an additional note on its three-Dimensional histological perspective
Teeth are considered to be the prime organs for mastication, speech, and esthetics. The basic structure of tooth involves enamel, dentin, and pulp. Each of these is formed by specialized structures during the various phases of tooth development. One of such specialized structures forming dentin of the tooth are odontoblasts. Being present at the periphery of the pulp chamber, they are in close contact with dentin through cytoplasmic extensions which travel into the entire length of dentin through dentinal tubules. In addition to dentin formation, they play a vital role in performing the various functions of the tooth including nerve sensation. They are functionally active throughout the life span of a healthy tooth and are subjective to numerous injuries causing various oral lesions. Oral leukoplakia: Transmission electron microscopic correlation with clinical types and light microscopy
Background: Leukoplakia, is a precancerous lesion that is most commonly encountered in the oral cavity. The grade of dysplasia is presumed to be the most important indicator of malignant potential. There are many promising aspects in advanced methods for the evaluation of oral precancer and cancer. Among these methods, electron microscopic examination predicts the true biologic potential more accurately than conventional histology and has some success in the early detection of potentially malignant lesions. It has been reported in the literature that there is some correlation between clinical, histopathological, and transmission electron microscopic features.
Materials and Methods: In this cohort study (prospective research), from the total of 9 subjects, 3 had homogenous leukoplakia, 3 had ulcerative type of oral leukoplakia, and 3 had nodular type of oral leukoplakia. Two patients were selected as control patients. Transmission electron microscopic examination was carried for all the cases and controls. All the findings were correlated with clinical features and light microscopy.
Results: Clinically and histologically, mild leukoplakia showed break in basement membrane, which can only be observed under transmission electron microscope (TEM). Additional dysplastic features were observed under transmission electron microscope, which are indicative of neoplastic process.
Conclusions: Thus, it is finally concluded that nodular leukoplakia seems to be the most severe clinical type of leukoplakia showing highest risk of malignant transformation. Homogenous leukoplakia might show break in basement membrane under TEM
Pemphigus vulgaris
Pemphigus vulgaris is a chronic autoimmune mucocutaneous disease that initially manifests in the form of intraoral lesions, which spread to other mucous membranes and the skin. The etiology of pemphigus vulgaris is still unknown, although the disease has attracted considerable interest. The pemphigus group of disease is characterized by the production of autoantibodies against intercellular substances and is thus classified as autoimmune diseases. Most patients are initially misdiagnosed and improperly treated for months or even years. Dental professionals must be sufficiently familiar with the clinical manifestations of pemphigus vulgaris to ensure early diagnosis and treatment, since this in turn determines the prognosis and course of the disease. This article presents a case report with unknown etiology along with an overview of the disease
Comparing the efficacy of hematoxylin and eosin, periodic acid schiff and fluorescent periodic acid schiff-acriflavine techniques for demonstration of basement membrane in oral lichen planus: A histochemical study
Background: Basement membrane (BM) is a thick sheet of extracellular matrix molecules, upon which epithelial cells attach. Various immunohistochemical studies in the past have been carried out but these advanced staining techniques are expensive and not feasible in routine laboratories. Although hematoxylin and eosin (H-E) is very popular among pathologists for looking at biopsies, the method has some limitations. This is where special stains come handy. Aims and Objectives: The aim of the present study was to demonstrate and compare the efficacy of H-E, periodic acid Schiff (PAS) and fluorescent periodic acid-acriflavine staining techniques for the basement membrane and to establish a histochemical stain which could be cost effective, less time consuming, and unambiguous for observation of the basement membrane zone. Materials and Methods: A total number of 40 paraffin-embedded tissue sections of known basement membrane containing tissues including 10 - Normal oral mucosa (NOM) and 30 - oral lichen planus (OLP) were considered in the study. Four-micron-thick sections of each block were cut and stained with H-E stain, PAS and fluorescent periodic acid-acriflavine stain. Sections were evaluated by three oral pathologists independently for continuity, contrast and pattern. Results: Though all the three stains showed favorable features at different levels, acriflavine stain was better than the other stains in demonstrating BM continuity, contrast and also the pattern followed by PAS stain. Acriflavine stain was the better in demonstrating a fibrillar pattern of a BM. Acriflavine stains a BM distinctly and is less time consuming and easy to carry out using readily available dyes as compared to other stains. Conclusion: The continuity and contrast along with the homogenous pattern and the afibrillar pattern of the BM was better demonstrated by acriflavine followed by the PAS stain
