Conditional Reducibility of Certain Unbounded Nonnegative Hamiltonian Operator Functions
Let J and J be operators on a Hilbert space H which are both self-adjoint and unitary and satisfy JJ = −JJ. We consider an operator function A on [0, 1] of the form A(t) = S + B(t), t ∈ [0, 1], where S is a closed densely defined Hamiltonian (= J-skew-self-adjoint) operator on H with iR ⊂ ρ(S) and B is a function on [0, 1] whose values are bounded operators on H and which is continuous in the uniform operator topology. We assume that for each t ∈ [0, 1]A(t) is a closed densely defined nonnegative (=J-accretive) Hamiltonian operator with iR ⊂ ρ(A(t)). In this paper we give sufficient conditions on S under which A is conditionally reducible, which means that, with respect to a natural decomposition of H, A is diagonalizable in a 2×2 block operator matrix function such that the spectra of the two operator functions on the diagonal are contained in the right and left open half planes of the complex plane. The sufficient conditions involve bounds on the resolvent of S and interpolation of Hilbert spaces.
Formation of the competitive potential of the agricultural territories
The purpose of the study is to develop a mechanism for the formation of the competitive potential of the agricultural territories of Kazakhstan as the basis for ensuring the dynamic development of agricultural business in Kazakhstan. The research methodology is based on strategic alternative modeling for agricultural territories competitiveness management with the help of fuzzy cognitive logic. The study used a cognitive approach to decision making in the management of poorly structured systems proposed by V. Silov and actively developed in the works of modern scientists. Fifteen competent experts were invited to select the concepts and build a fuzzy cognitive map of the formation of the competitive potential of the agricultural territories of Kazakhstan. To carry out calculations and justify the content of the strategy, the authors used the software product “Intelligent generation of the best alternatives” (“IGLA”). The result of the study is the developed strategy for the management of agricultural territories competitiveness in Kazakhstan with a set of strategic goals and the best option for managerial impact, ensuring the formation of agricultural export potential of the region. The proposed version of the content of the strategy for managing the competitiveness of the agricultural territories of Kazakhstan can be used either as a system or as its separate elements in managing the development of agricultural business in the country and in developing strategic planning programs for the agro-industrial complex of the region. The novelty of the study in theoretical terms is justified by the approach to the formation of the country's agricultural export potential through ensuring the competitiveness of its agricultural territories. In methodological terms, the novelty of the study is expressed by the use of cognitive modeling technologies to develop a strategy for managing the competitiveness of Kazakhstan's agricultural territories, the implementation of which will allow achieving sustainable dynamics of agricultural production and increase the efficiency of the agricultural economy. Urethral melanoma is a rare malignancy that has a poor prognosis. The paper presents the data available in the worldwide literature on urethral melanoma and analyzes the specific features of the disease occurring in males and females. It also describes clinical cases of malignant melanoma of the urethra in one woman and two men treated at the Russian Cancer Research Center.
Immunotherapy in urothelial cancer: recent data and perspectives
Immune-checkpoint inhibitors blocking the programmed death 1/programmed death-ligand 1 (PD-1/PD-L1) and cytotoxic T-lymphocyteassociated protein 4 (CTLA-4) have shown a prominent anti-tumor activity with long-term responses and an acceptable toxicity profile in clinical trials. Pembrolizumab, atezolizumab, nivolumab, avelumab, and durvalumab are anti-PD-1/PD-L1 agents that redefine the standard of care for advanced urothelial carcinoma. CTLA-4 inhibitors are also under investigation in this setting. Phase III trial KEYNOTE-045 has demonstrated significant survival benefit in patients treated with pembrolizumab comparing with the standard second-line chemotherapy. Atezolizumab, nivolumab, avelumab, and durvalumab were also recommended for platinum-pretreated urothelial carcinoma patients based on phase II data. Following investigations of biomarkers such as PD-L1 expression are needed to determine high-responders to immunotherapy. This review article describes the advances in immunotherapy with immune-checkpoint inhibitors.</jats:p
Cabazitaxel in the treatment of castration-resistant cancer: hitting the balance of efficacy and safety
Cabazitaxel, an antineoplastic agent from the third generation taxan group, has demonstrated efficacy in the treatment of metastatic castration-resistant prostate cancer (mCRPC) refractory to docetaxel. This article is devoted to a critical analysis of studies on the use of cabazitaxel in this category of patients and key aspects of management of treatment-related toxicity. The authors also reviewed possible scenarios for the use of cabazitaxel in the sequential therapy of mCRPC, including androgen receptor signalling inhibitors and systemic radiotherapy
What do we know about the possibility of predicting the potential efficacy and safety of methotrexate, by measuring the concentration of its metabolites in rheumatoid arthritis?
Rheumatoid arthritis (RA) is characterized by a progressive course with the formation of joint deformities and the development of severe functional disorders. The most favorable conditions for changing the course of the disease emerge in the first months after its onset. Methotrexate (MTX) occupies a leading position in the treatment of RA. There is a need to develop tools that will be able to predict the efficacy and tolerability of MTX at the earliest stages of RA treatment. The concentration of active metabolites of MTX is considered as a potential biomarker that enables one to predict what response to therapy with this drug will be.The paper analyzes the most relevant works devoted to the study of the concentration of active MTX metabolites polyglutamates (MTX PGs) in patients with RA. It is shown that the presented studies do not cover all the possibilities of therapeutic drug monitoring, in particular the determination of the levels of MTX PGs in mononuclear cells over time. Further investigation is needed to develop an objective approach to prescribing MTX in RA patients

Results of Monitoring over and Biological Properties of <I>Vibrio cholerae</I> Isolated from Ambient Environment Objects in the Khabarovsk Territory

COMPARATIVE EFFECTIVENESS OF THE COMBINATIONS OF DIURETICS AND DIHYDROPYRIDINE AND NONDIHYDROPYRIDINE CALCIUM ANTAGONISTS IN PATIENTS WITH ARTERIAL HYPERTENSION
In total, 54 patients, aged 42-65 years, with Stage II arterial hypertension (AH) were examined. At baseline and 16 weeks after the treatment phase, 24-hour blood pressure monitoring (BPM) and Doppler echocardiography were performed. The patients were randomised into 2 groups: Group 1 received combined therapy with indapamide and verapamil retard, while Group 2 was administered indapamide and amlodipine. Circadian BP rhythm was normalised in 82,8% and 76,0% of the patients from Groups 1 and 2, respectively. In Group 1, combined therapy was more effective among patients with Type I left ventricular diastolic dysfunction (LVDD), non-dipper circadian BP profile, and hyperkinetic circulation type. In Group 2, the therapy was more effective among participants with Type II-III LVDD and hypokinetic circulation type
