    Methods of connecting primary care patients with community-based physical activity opportunities : a realist scoping review

    Funding: NHS Fife Endowment Fund (Grant Number(s): FIF142).Deemed a global public health problem by the World Health Organization, physical inactivity is estimated to be responsible for one in six deaths in the United Kingdom (UK) and to cost the nation's economy £7.4 billion per year. A response to the problem receiving increasing attention is connecting primary care patients with community-based physical activity opportunities. We aimed to explore what is known about the effectiveness of different methods of connecting primary care patients with community-based physical activity opportunities in the United Kingdom by answering three research questions: 1) What methods of connection from primary care to community-based physical activity opportunities have been evaluated?; 2) What processes of physical activity promotion incorporating such methods of connection are (or are not) effective or acceptable, for whom, to what extent and under what circumstances; 3) How and why are (or are not) those processes effective or acceptable? We conducted a realist scoping review in which we searched Cochrane, Medline, PsycNET, Google Advanced Search, National Health Service (NHS) Evidence and NHS Health Scotland from inception until August 2020. We identified that five methods of connection from primary care to community-based physical activity opportunities had been evaluated. These were embedded in 15 processes of physical activity promotion, involving patient identification and behaviour change strategy delivery, as well as connection. In the contexts in which they were implemented, four of those processes had strong positive findings, three had moderately positive findings and eight had negative findings. The underlying theories of change were highly supported for three processes, supported to an extent for four and refuted for eight processes. Comparisons of the processes and their theories of change revealed several indications helpful for future development of effective processes. Our review also highlighted the limited evidence base in the area and the resulting need for well-designed theory-based evaluations.Peer reviewe

Aim This study aimed to identify methods, and explore barriers and facilitators, of connecting primary care patients with PA opportunities from the perspectives of both health professionals (HPs) and patients, using the example of jogs cotland. Design & setting An exploratory study utilising semi-structured interviews with primary care patients (n = 14) and HPs (n = 14) from one UK NHS board was conducted. Method Patient and HP transcripts were analysed separately using thematic analysis. Potential methods of connection were identified. The Capability, Opportunity, Motivation, behavioural (COM-B) model and theoretical domains framework (TDF) were employed to facilitate identification of barriers and facilitators for connecting primary care to community jogscotland groups. Results Three methods of connecting patients to community-based groups were identified: informal passive signposting, informal active signposting, and formal referral or prescribing. Barriers and facilitators for patient connection fell into five TDF domains for HPs and two COM-B model components for patients. Conclusion For patients, HPs raising the topic of PA can help to justify, facilitate, and motivate action to change. The workload associated with connecting patients with community-based opportunities is central to implementation by HPs. Integrative resource solutions and social support for patients can provide a greater variety of PA options and the vital information and support for connecting with local opportunities, such as jogscotland.Peer reviewe

Sub-themes for patients and HPs were mapped onto relevant components of the capability, opportunity, motivation behavioural (COM-B) model and theoretical domains framework (TDF) to identify barriers and facilitators for connecting primary care to community jog scotland groups.Results: Three potential methods of connecting patients to community-based jog scotland groups were identified: informal passive signposting, informal active signposting, and formal referral or prescribing. Barriers and facilitators to connecting patients to jog scotland groups fell into five TDF domains for HPs and two COM-B model components for patients.Conclusions: Our findings suggest that for patients, the acknowledgement and raising of the topic of physical activity improvement by their HP can help to justify as well as facilitate and motivate action to change.     Computer algebra and transputers applied to the finite element method

    Recent developments in computing technology have opened new prospects for computationally intensive numerical methods such as the finite element method. More complex and refined problems can be solved, for example increased number and order of the elements improving accuracy. The power of Computer Algebra systems and parallel processing techniques is expected to bring significant improvement in such methods. The main objective of this work has been to assess the use of these techniques in the finite element method. The generation of interpolation functions and element matrices has been investigated using Computer Algebra. Symbolic expressions were obtained automatically and efficiently converted into FORTRAN routines. Shape functions based on Lagrange polynomials and mapping functions for infinite elements were considered. One and two dimensional element matrices for bending problems based on Hermite polynomials were also derived. Parallel solvers for systems of linear equations have been developed since such systems often arise in numerical methods. Both symmetric and asymmetric solvers have been considered. The implementation was on Transputer-based machines. The speed-ups obtained are good. An analysis by finite element method of a free surface flow over a spillway has been carried out. Computer Algebra was used to derive the integrand of the element matrices and their numerical evaluation was done in parallel on a Transputer-based machine. A graphical interface was developed to enable the visualisation of the free surface and the influence of the parameters. The speed- ups obtained were good. Convergence of the iterative solution method used was good for gated spillways. Some problems experienced with the non-gated spillways have lead to a discussion and tests of the potential factors of instability

Recent advances in transcriptional profiling methods have enabled classification of cell types by their gene expression. While exceptionally powerful and high throughput, the ground-truth validation of these methods is difficult: If cell type is unknown, how does one assess whether a given analysis accurately captures neuronal identity? To shed light on the capabilities and limitations of solely using transcriptional profiling for cell-type classification, we performed 2 forms of transcriptional profiling—RNA-seq and quantitative RT-PCR, in single, unambiguously identified neurons from 2 small crustacean neuronal networks: The stomatogastric and cardiac ganglia. We then combined our knowledge of cell type with unbiased clustering analyses and supervised machine learning to determine how accurately functionally defined neuron types can be classified by expression profile alone. The results demonstrate that expression profile is able to capture neuronal identity most accurately when combined with multimodal information that allows for post hoc grouping, so analysis can proceed from a supervised perspective. Solely unsupervised clustering can lead to misidentification and an inability to distinguish between 2 or more cell types. Therefore, this study supports the general utility of cell identification by transcriptional profiling, but adds a caution: It is difficult or impossible to know under what conditions transcriptional profiling alone is capable of assigning cell identity. Only by combining multiple modalities of information such as physiology, morphology, or innervation target can neuronal identity be unambiguously determined.We thank members of the D.J.S., H.A.H., and E.M. laboratories for helpful discussions.     Developing a partnership of indigenous peoples, conservationists, and land use planners in Latin America

    Illustrating from a rich body of case material, Poole's report reflects a shift away from the traditional view - represented by certain national parks and similar protected areas - that indigenous peoples be allowed to occupy and use an area's resources following rules set by conservationists. Under the new paradigm that is developing, indigenous peoples are seen as an integral part of protected area planning through agreements worked out in partnership with conservation authorities. An example of this new approach is the role that indigenous peoples are playing in the design of biosphere reserves. Poole suggests that the Bank and other development organizations pay more attention to vernacular economies - economies based on local resources, used either for subsistence or as a source of revenue. Different kinds of impairments have very specific, context-defined, meanings that, in turn, have different implications for how gender and disability might intersect. Rather than diminishing masculinity in all instances – some bodily differences, as the article demonstrates, might even be enacted as hyper-masculine – impairments are shown rather to reshape understandings of the masculine in sometimes unexpected ways. And while my informants were constrained both by ableist norms and by the biological limitations of their own bodies, ambivalence towards certain forms of masculinity also afforded them space to perform their identities more creatively, sometimes to potentially positive effect.The Economic and Social Research Council and the British Academy
