Pseudomonas aeruginosa: Recent Advances in Vaccine Development
Pseudomonas aeruginosa is an important opportunistic human pathogen. Using its arsenal of virulence factors and its intrinsic ability to adapt to new environments, P. aeruginosa causes a range of complicated acute and chronic infections in immunocompromised individuals. Of particular importance are burn wound infections, ventilator-associated pneumonia, and chronic infections in people with cystic fibrosis. Antibiotic resistance has rendered many of these infections challenging to treat and novel therapeutic strategies are limited. Multiple clinical studies using well-characterised virulence factors as vaccine antigens over the last 50 years have fallen short, resulting in no effective vaccination being available for clinical use. Nonetheless, progress has been made in preclinical research, namely, in the realms of antigen discovery, adjuvant use, and novel delivery systems. Nanotechnologies for tissue engineering and regeneration: Nanoengineered/nanopatterned materials for cell and tissue guidance
Stem cells (SCs) can self-renew or differentiate into different cell types, which makes them an ideal cell source for therapies based on tissue engineering. Despite these characteristics, the employment of SCs in clinics has seen alternating fortunes because of our limited understanding of the signals governing SC functions and fate, which impairs our ability to engineer systems to deliver SCs in vivo and to guide their correct biological processes. However, experimental evidence demonstrated that SCs are able to recognize biochemical and biophysical signals displayed by material surfaces; most importantly, cells integrate these signals to elaborate fate decisions. Although the mechanisms underlying signal recognition and response have not been thoroughly characterized, there is a general consensus that the cell adhesion process plays a central role. Adhesion represents a communication gate between exogenous signals and intracellular signaling cascades involving the cytoskeleton and the nucleus. In this work we present recent findings on materials engineered to control cell functions through adhesion processes. In particular, we emphasize the role of material signals on SC behavior. Finally, we discuss a few sensing and transductive molecular mechanisms in an effort to draw out unifying elements concerning cell recognition of and reaction to biophysical/biochemical material signals aimed at controlling cell fate through cell adhesion
Variation in stress levels in the nursing profession : a comparison between dementia and geriatric nurses
Background: Previous research has suggested a strong correlation between nurses’
occupational stress, and staff turnover (Hinshaw & Atwood, 1993) and suggested that Staff stress levels are affected by Occupational and resident characteristics.
Objectives: The aim of this current study is to a) investigate whether there is a correlation
between levels of stress and job satisfaction in nursing home staff providing care for patients with dementia and in nursing staff who care for the general ageing geriatric population in the nursing home, and b) to investigate associations between these variables, occupational and individual characteristics of staff. Method: A quantitative cross sectional research design was adopted, using a self-completion questionnaire survey of 42 nursing home staff members from Sancta Maria nursing home situated in Naas, Co. Kildare. Staff stress levels were measured using the Perceived Stress Scale and Job satisfaction was measured using the Job Satisfaction Survey. Results: The results show that nurses working with patients with dementia experienced significantly higher levels of stress than nurses working with a general ageing geriatric population but there were no significant differences between the two groups in terms of job satisfaction. Those working part time experienced higher levels of stress. Being more
extraverted in personality and having the support of a significant other were protective
factors. Conclusion: The results add support to the existing research findings that nurses working with patients with dementia experience more stress than those who work with a general ageing geriatric population in the nursing home setting. Microneedle arrays for vaccine delivery: the possibilities, challenges and use of nanoparticles as a combinatorial approach for enhanced vaccine immunogenicity
Introduction: Vaccination is one of the greatest breakthroughs of modern preventative medicine. Despite this, there remain problems surrounding delivery, efficacy and compliance. Thus, there is a pressing need to develop cost-effective vaccine delivery systems that could expand the use of vaccines, particularly within developing countries. Microneedle (MN) arrays, given their ease of use, painlessness and ability to target skin antigen presenting cells, provide an attractive platform for improved vaccine delivery and efficacy. Studies have demonstrated enhanced immunogenicity with the use of MN in comparison to conventional needle. More recently, dissolving MN have been used for efficient delivery of nanoparticles (NP), as a means to enhance antigen immunogenicity. Areas covered: This review introduces the fields of MN technology and nanotechnology, highlighting the recent advances which have been made with these two technologies combined for enhanced vaccine delivery and efficacy. Some key questions that remain to be addressed for adoption of MN in a clinical setting are also evaluated. Expert opinion: MN-mediated vaccine delivery holds potential for expanding access to vaccines, with individuals in developing countries likely to be the principal beneficiaries. The combinatorial approach of utilizing MN coupled with NP, provides opportunities to enhance the immunogenicity of vaccine antigens.</p
Enhancement in Site-Specific Delivery of Carvacrol against Methicillin Resistant Staphylococcus aureus Induced Skin Infections Using Enzyme Responsive Nanoparticles: A Proof of Concept Study
Methicillin resistant Staphylococcus aureus (MRSA) induced skin infections have become a challenging problem due to the escalating antibiotic resistance. Carvacrol (CAR) has been reported to be effective against MRSA. However, due to its characteristics, CAR exhibits low skin retention. In this study, CAR was formulated into site-specific nanoparticle (NPs) delivery system using poly(ε-caprolactone) (PCL), following incorporation into a hydrogel matrix to facilitate dermal delivery. The release study exhibited significantly higher release of CAR from PCL NPs in the presence of bacterial lipase, highlighting its potential for differential delivery. Moreover, encapsulation of CAR in PCL NPs resulted in a two-fold increase in its anti-MRSA activity. Dermatokinetic studies revealed that the NPs loaded hydrogel was able to enhance skin retention of CAR after 24 h (83.29 ± 3.15%), compared to free CAR-loaded hydrogel (0.85 ± 0.14%). Importantly, this novel approach exhibited effective antimicrobial activity in an ex-vivo skin infection model. Hence, these findings have proven the concept that the loading of CAR into a responsive NPs system can lead to sustained antimicrobial effect at the desired site, and may provide a novel effective approach for treatment of MRSA induced skin infections. However, further studies must be conducted to investigate in-vivo efficacy of the developed system in an appropriate infection model

Microneedles and nanomedicine are two out of several active innovative approaches used to enhance the transdermal drug and vaccine delivery. Their application individually or in combination have shown to be very promising and attracted considerable interest by researchers from both industry and academia over the last 2 decades. Combining the two technologies has growing interest and has shown to be promising approach not only in enhancing the transdermal drug and vaccine permeation for even difficult drug molecules, such as hydrophilic and macromolecules, but also impart protection and controlling the drug release rates. In Subchapter 2.2, we aim to highlight the advances, which has been made in using these two technologies as an individual or in combination with drug and vaccine delivery. Metal implants, in the form of screws, plates, or pins, are extensively used in the treatment of fractures and nonunions or as replacements for malfunctioned joints. These bones implants face many challenges for acceptance and survival upon insertion in the human body. These challenges include severe inflammation, bacterial invasion, and poor biointegration with the traumatized tissue. Titania (TiO2) nanotubes (TNTs) arrays engineered on the surface of Ti implants by simple and scalable electrochemical anodization process, which have been widely explored as a new nanoengineering approach to improve the process of osseointegration and at the same time to be used as depots for loading drugs and their controllable release for localized delivery, and therapeutic purpose. Several advanced functions can be introduced into these multifunctional implants, including biopolymers, nanoparticles or external stimulation (e.g., electrical, electromagnetic and ultrasound) to release the loaded therapeutic agents in a desired manner when required. Subchapter 2.3 highlights the developed concepts of drug releasing implants based on TNTs for enhancing osteogenesis at the bone-implant interface, as an alternative approach to systemic delivery of therapeutic agents. The reconstruction of skeletal defects is a continuing challenge. Bone and dentin are mineralized hard tissues. The primary inorganic component of these two tissues is crystalline hydroxyapatite and the primary organic component is type I collagen, and these two components are cell-manufactured materials. Tissue-engineering strategies using novel biomaterials have emerged as a promising potential for the treatment of mineralization defects. Some of the recent bioinspired biomaterials utilize proteins and peptides as nanoscale building blocks. A key functional property is that biomaterials need to be cell-compatible and mimic the dynamic nature of the extracellular matrix. ECM is a complex environment comprising a plethora of macromolecules. Another class of novel biomaterials that are currently being developed are peptide-based hydrogels. Peptide-based engineered scaffolds present several advantages over traditional protein scaffolds as the control over hydrogel properties and can be easily tailored to the requirement of the tissue. Biomaterials generated by the self-assembly process have varied applications as it mimics nature’s method of material synthesis. Therefore, concepts of protein-based self-assembly can be utilized for constructing useful biomaterials.</p
