Pacemaker guided screening for severe sleep apnea, a possible option for patients with atrial fibrillation: A systematic review and meta‐analysis

    No full text
Introduction Obstructive sleep apnea is often underdiagnosed in atrial fibrillation (AF) patients although it is an important risk factor. A systematic review and meta-analysis was performed to assess which techniques cardiac implantable electronic devices (CIED) and Holter monitors use to screen for sleep apnea (SA), and to evaluate if these are suitable for AF patients from a diagnostic accuracy perspective. Methods The search was conducted in accordance with the PRISMA-guidelines. PICO was defined as (P) patients with AF, (I) Holter monitors or CIED suitable for screening for SA, (C) overnight polysomnography (PSG), (O) positive screening with subsequent positive polysomnographic diagnosis of SA. Optimal index test cut-off points corresponding to reference test cut-off for severe SA (PSG-AHI >= 30) were compared. Meta-analysis was conducted for the diagnostic odds ratio (DOR), with forest plot and ROC-curve for summary DOR.Results A total of five prospective cohort studies (n = 192) were included in the systematic review of which four studies (n = 132) were included in the meta-analysis. All included studies use transthoracic impedance measurement as a screening parameter. No studies evaluating Holter monitors were included. The population consisted of patients indicated for pacemaker implantation. The summary DOR was 27.14 (8.83; 83.37), AUC was 0.8689 (0.6872; 0.9456) and Q* was 0.8390 (0.7482; 0.9013).Conclusion At optimal pacemaker-cut-off, pacemaker-guided screening for severe SA in patients with AF can be an effective triage tool for clinical practice. Further studies with larger sample sizes are needed to strengthen the evidence for this conclusion. This study is part of Limburg Clinical Research Center, supported by the foundation Limburg Sterk Merk, province of Limburg, Flemish government, Hasselt University, Ziekenhuis Oost-Limburg and Jessa Hospital

An analysis of temperature changes in Dutch winters

    No full text
It is said that Dutch winters are becoming milder and that we experience less severe winters nowadays. In this thesis it is attempted to test meteorological data on whether there is a significant trend. This is done by using the Mann-Kendall test and Sen’s slope estimator. In the last chapter the cultural impact of the changing winters on the Eleven cities tour is also explored.Applied Mathematic

These personal reminiscences were set down from memory by the author in 1968 after he had participated with other veterans of the labor camp program in recording experiences upon tapes now deposited in State Archives. Winters\u27 very active career with the California Division of Forestry covered a period of 35 years. This article was presented in 1972 essentially for the information of employees of this department. However, because of its historical value, demands for copies from libraries and educational institutions led us to believe that this reproduction would be truly justified. Asian Economic Integration - Perspectives from a Chinese Scholar

    Get PDF
Since the 1990s, regional economic integration has become an unstoppable trend throughout the world. As the most telling feature of that, regional trade agreements (RTAs) have expanded explosively in the past fifteen years (see figure 1). According to the latest WTO data, the number of RTAs of all kinds (including those in implementation or to be signed) had reached almost 300 by the end of 2005. 250 of them have been notified to WTO, among which 130 were notified after January 1995, with most signed recently. So far, 173 RTAs have been notified to WTO and taken effect. It is assessed that there are still over 70 RTAs which havent been notified to WTO but about to come into effect.Asian Economic Integration

Northern hemisphere storm tracks in strong AO anomaly winters

    No full text
Northern Hemisphere winter 300 hPa storm tracks/baroclinic waves and their energetics during strong Arctic Oscillation (AO) anomaly events are investigated. It is shown that during strong positive AO anomaly winters, the Atlantic storm track intensifies significantly and shifts northward and far downstream while Pacific storm track extends westward. Baroclinic waves over the Atlantic propagate along central North Atlantic during strong negative AO anomaly winters while during strong positive AO anomaly winters, waves over the Atlantic split into two branches. Distributions of the kinetic energy and energy conversions of different spatial scale over the storm track regions assume different characteristics for different AO phases. Gift Days by K.-L. Winters

Winters, Kari-Lynn. Gift Days. Illus. Stephen Taylor. Markham, ON: Fitzhenry & Whiteside, 2012. Print.A young Ugandan girl, Nassali, watches her brother go to school every day, and wishes she could go too.  She wants to learn to read but her time is filled with taking care of her family, her responsibility since the death of her mother.  How her life changes, and how she learns to read is told through this picture book. The author, Kari-Lynn Winters, is a Canadian author and literacy researcher and a faculty member in Teacher Education at Brock University.  She is a well-known author of numerous picture books. The book is illustrated by Stephen Taylor, a graduate of the Ontario College of Art, and frequent Illustrator of children’s books.  The illustrations are charming and add a great deal of depth to the story.  They do an excellent job of conveying how alone and left out Nassali feels as her brother goes to school every day, and her joy and sense of pride when she learns to read. The story unfortunately, is not as engaging as the illustrations.  The language is quite dry and somewhat didactic, seemingly at odds with the picture book format.  There are some disconnections in the story. It is unclear, for example, why the brother decides to teach Nassali to read.  For this reason it is difficult to determine the intended audience.  The format suggests primary school students, but the content suggests an older audience. This book would be useful as an instructional resource in an elementary school setting for a unit on gender issues or development. It is not, however, likely to engage a self-directed reader of any age.Reviewer:  Margaret Law Recommended with reservations: 2 out of 4 starsMargaret Law is the Associate University Librarian (International Relations) at the University of Alberta, responsible for developing international library partnerships.  Previously, she was a public librarian, primarily involved with the development of rural libraries in Alberta
