    Chiaramonte Gulfi an experience of urban morphology

    This paper is aimed to describe a project for the city of Chiaramonte Gulfi elaborated during the International Laboratory of Architecture_LId’A coordinated by Laura Thermes. In one of the laboratories – directed by the author with Federica Visconti and coordinated to those by Carlo Moccia and Marco Mannino – the team worked on the redefinition of the “city walls”. Over the 1970s, it resulted from the construction of a continuous line of buildings in relationship to agricultural divisions and peri-urban plots. The thin strip determined, on one hand, a new “urban artifact” of morphological value – even if of less architectural value – as a kind of city walls of the ancient city, and, on the other, it opposed the typical nebulized growth of the sprawl-city. The project aims to propose a comprehensive reformulation for this wall, working in some of the significant places, highlighting discontinuities, openings the city to the landscape and establishing new relationships between the Comiso’s plain below and the perched character – like in the acropolis – of the medieval centre, re-entering portions of nature still existing in the urban fabric. The project combines two ways of architectural composition, the stereotomic and the tectonic, and develops the theme of the bastion corresponding to a hiatus of the wall, proposing a new condition overlooking towards the landscape of the Comiso plain and, at the same time, representing an element of a distinctive and interconnected system of “places” able to establish long distance relationships and summarize the whole urban morphology and the geography of Chiaramonte

    An Integrated Pharmacophore/Docking/3D-QSAR Approach to Screening a Large Library of Products in Search of Future Botulinum Neurotoxin A Inhibitors

    Botulinum toxins are neurotoxins produced by Clostridium botulinum. This toxin can be lethal for humans as a cause of botulism; however, in small doses, the same toxin is used to treat different conditions. Even if the therapeutic doses are effective and safe, the adverse reactions could be local and could unmask a subclinical impairment of neuromuscular transmissions. There are not many cases of adverse events in the literature; however, it is possible that sometimes they do not occur as they are transient and, if they do occur, there is no possibility of a cure other than to wait for the pharmacological effect to end. Inhibition of botulinum neurotoxin type A (BoNT/A) effects is a strategy for treating botulism as it can provide an effective post-exposure remedy. In this paper, 13,592,287 compounds were screened through a pharmacophore filter, a 3D-QSAR model, and a virtual screening; then, the compounds with the best affinity were selected. Molecular dynamics simulation studies on the first four compounds predicted to be the most active were conducted to verify that the poses foreseen by the docking were stable. This approach allowed us to identify compounds with a calculated inhibitory activity in the range of 316–500 nM

Luego propone una segundatesis que cuestiona el uso naturalizado de la idea de federalismo en la historia pos independencia en el Río de La Plata y por último destaca la tesis de la existencia de una “Constitución antes de la Constitución”, o la “Antigua Constitución”: idea que cuestiona los prejuicios históricos respecto a la existencia de una anarquía o anomia en el periodo posindependencia contraponiéndola a la de una época donde un cuerpo de leyes y normas, no siempre escritas, ordenaban la vida de estos pueblos, en la que se destaca la más importante de las normas del derecho natural: el libre consentimiento.The following conference proposes a journey through the main thesis present in the work of the author. It begins with the central thesis, the denial of the Argentine nation preexistence related to the revolution of colonial independence. Later proposes a second thesis that questions the naturalized use of the idea of federalism in the post-independence history in Río de La Plata and finally it highlights the idea of the existence of a “Constitution before the Constitution” or an “Ancient Constitution”: that challenges the historic prejudices over the existences of anarchy or anomy during the post-independence period against the idea of a time when a body of laws and rules, usually unwritten, ordered the life of the people.     Effects of high-flow nasal therapy on swallowing function: a scoping review

    Background High-flow nasal therapy (HFNT) is widely used in patients with respiratory failure in different clinical settings, but the effect of HFNT on respiratory-swallow coordination is unknown. Understanding this relationship is crucial, considering the necessity for patients to maintain adequate nutrition during daytime HFNT. Purpose This scoping review aims to synthesise available data on the effects of HFNT flow rates on swallowing function and the possible risk of aspiration during treatment, focusing on knowledge and evidence gaps. Methods PubMed, Scopus, Web of Science and Google Scholar databases were searched from inception to May 30th, 2023, for studies reporting data on swallowing assessment in healthy adults or patients with acute or chronic respiratory failure receiving HFNT. Data on study design, patients’ characteristics, and quality outcomes were extracted, and risk of bias was assessed. Results Eight studies were included, four including cohorts of healthy volunteers (n=148) and four including patients with acute or chronic respiratory failure (n=151). Study designs, patient populations, and quality outcome measures were heterogeneous. Two studies indicated improvement, while four articles showed impairment in swallowing function during HFNT; two studies showed that patients’ overall clinical picture and underlying medical conditions influence swallowing-breathing coordination rather than HFNT per se. Risk of bias was judged low for all the included studies. Conclusion This scoping review found limited and controversial evidence of the impact of HFNT on swallowing function. Remarkably, methods for swallowing function assessment were quite heterogeneous. Additional research is required to test the effect of HFNT on respiratory-swallowing coordination

    Rehabilitation approach in robot assisted total knee arthroplasty: an observational study

    Background The purpose of this study is to evaluate the impact of total knee arthroplasty (TKA) with the aid of Navio Robot, comparing it with standard prosthetic surgery on the functional outcomes of patients after an intensive rehabilitation program. Method A case–control observational study was conducted on patients undergoing TKA for severe KOA. All patients underwent the same intensive hospital rehabilitation program of 14 daily sessions lasting 3 h. The following rating scales were administered: Numeric Rating Scale (NRS), Knee Society Score (KSS) and 12-Item Short Form Survey scale. Patient assessments were performed 1 week post-surgery (T0), 1 month post-surgery (T2), and 3 months post-surgery (T3). The primary outcomes were active knee extension and fexion and pain severity. The secondary outcomes were functional capacity and quality of life. Results Using repeated measures ANOVA, we observed at T1 a statistically diferent diference for the treatment group compared to the control group about KSS (p<0.05), pain (p<0.05), and knee fexion (p<0.05). No statisti cally signifcant diference between the two groups was observed for knee extension (p=0.09) and the SF-12 scale (p=0.52). At T2 instead, we observed a statistically signifcant diference for the treatment group compared to the control group as regards KSS (p<0.05) and knee fexion (p<0.05), while no statistically signifcant diference was observed for pain (p=0.83), knee extension (p=0.60), and the SF-12 scale (0.44). Conclusions Our study has demonstrated that robot-NAVIO assisted knee prosthesis surgery, associated with a spe cifc intensive rehabilitation treatment, in the short and medium term, determines good pain control, better fexion recovery and a improvement of functional capacit
