    Comparison between five indices of adherence to the Mediterranean dietary pattern and their association with systemic inflammation and common mental disorders in adults and elderly people living in the city of São Paulo in 2015

    Introdução: A adesão ao padrão da dieta mediterrânea (PDM) é fortemente reconhecida pelo seu papel na prevenção de doenças crônicas. A sua relevância, além de outras funções, tem sido atribuída ao seu desempenho anti-inflamatório. Uma alta adesão ao PDM pode atenuar vias de sinalização inflamatória envolvendo os sistemas nervoso, endócrino e imune, relacionadas ao desenvolvimento de transtornos mentais. Na literatura são descritas algumas propostas de análise baseadas nos índices de adesão ao PDM que se diferenciam quanto às suas estruturas e metodologias, e pouco se investiga sobre a concordância entre eles, especialmente em populações não-mediterrâneas. Os diferentes índices disponíveis são construídos a partir de diferentes abordagens com relação à população avaliada, o que pode resultar em achados significativamente diferentes. Objetivos: Avaliar e comparar cinco propostas de índices de adesão ao PDM; investigar a associação entre o PDM, inflamação sistêmica e os TMC entre adultos e idosos residentes no município de São Paulo. Métodos: Foram utilizados dados da população adulta e de idosos do Inquérito de Saúde de São Paulo de 2015 (ISA-Capital 2015), estudo transversal de base populacional. O Self Reporting Questionnaire-20 foi utilizado para identificar a presença de TMC. A adesão ao PDM foi avaliada a partir de cinco diferentes índices dietéticos (Mediterranean diet scale (MDS), Mediterranean diet Score (MedDietscore), Mediterranean dietary pattern (MDP), Mediterranean Adequacy Index (MAI) and Mediterranean-Style Dietary Pattern Score (MSDPS)), utilizando dados de consumo alimentar obtidos a partir de dois Recordatórios alimentares de 24 horas (R24h). Esses índices foram posteriormente comparados quanto à correlação e concordância entre seus resultados e a validade convergente de cada um foi testada por meio de análise fatorial confirmatória (AFC). O perfil inflamatório foi investigado por um Escore de inflamação sistêmica de baixo grau (LGSI score), construído com base nos níveis plasmáticos de biomarcadores pró e anti-inflamatórios de inflamação proteína C-reativa (PCR), fator de necrose tumoral alfa (TNF-&alpha;), interleucina-10 (IL-10) e adiponectina). Inicialmente foi investigada a associação entre o índice de adesão ao PDM com melhor validade convergente e perfil inflamatório por meio de modelos (ajustados e não ajustados) de regressão linear. Em seguida, foram estimados os efeitos direto, indireto e total do PDM sobre os TMC, incluindo a inflamação sistêmica como variável mediadora. Foi adotado nível de significância de 5% (p < 0,05). Resultados: Manuscrito 1 - As maiores correlações foram encontradas entre os índices MDP e MAI (r = 076; IC 95% 0.74-0.79) e entre MDP e MDS (r = 0.72; IC 95% 0.69-0.75). As maiores concordâncias observadas foram moderadas, entre MDP v. MAI (&kappa; = 0.57, P < 0.001) e MDP v. MDS (&kappa; = 0.48, P < 0.001). A qualidade do ajuste do modelo que definiu o constructo do MedDietscore (RMSEA = 0.033, IC 90% 0.02-0.042; SRMR = 0.042) e MSDPS (RMSEA = 0.028, IC 90% 0.019- 0.037; SRMR = 0.031) apresentou valores aceitáveis para índices de ajuste absoluto. Leguminosas, azeite de oliva, relação MUFA:SFA e cereais integrais com leguminosas foram mais relevantes para caracterizar o PDM (cargas fatoriais &ge;0&middot;50). Manuscrito 2 - Somente nos idosos, a alta adesão ao PMM significou uma menor pontuação do LGSI de 11,5%. Idosos classificados com alta adesão ao PMM diferiram por consumirem menor consumo de carnes e laticínios integrais. Entre os idosos, a ingestão de vegetais e azeite esteve inversamente associada aos níveis de inflamação sistêmica. Manuscrito 3 - O maior efeito observado nos modelos testados foi o MDP sobre o LGSI em idosos. O PDM apresentou efeitos diretos (SC = -0,247; p = 0,001) e totais (SC = -0,315; p<0,001) negativos e médios sobre a inflamação sistêmica. O PDM também teve um efeito indireto pequeno e negativo sobre o nível de inflamação mediado pelo IMC (SC = -0,068; p = 0,017). O segundo maior efeito observado foi o efeito direto (pequeno e negativo) da adesão ao PMM sobre os TMC em idosos (SC = -0,182; p = 0,048). O efeito indireto do MDP sobre o TMC mediado pela inflamação não foi significativo (SC = 0,001; p = 0,948). O mesmo resultado foi encontrado em relação ao efeito direto da inflamação sistêmica sobre os TMC (SC = -0,007; p = 0,948). Quanto ao ajuste do modelo, os índices de ajuste indicaram o bom ajuste dos dois modelos (adultos e idosos). Conclusões: O presente estudo mostrou que seguindo as etapas metodológicas apresentadas, é possível identificar os índices mediterrâneos que melhor refletem o seu objetivo em avaliar a adesão ao padrão da dieta mediterrânea em populações não-mediterrâneas e dessa forma guiar os elementos desse padrão alimentar que podem ser encorajados para garantir o seu papel protetor sobre a saúde mental e o nível de inflamação sistêmica, especialmente em idosos.Introduction: Adherence to the Mediterranean dietary pattern (MDP) is strongly recognized for its role in the prevention of chronic diseases. The relevance of this dietary pattern, in addition to other functions, has been attributed to its anti-inflammatory performance. A high adherence to the MDP can attenuate inflammatory signaling pathways involving the nervous, endocrine and immune systems, related to the development of mental disorders. In the literature there are different proposals for analysis based on indexes of adherence to the MDP. These proposals differ in terms of their structures and methodologies, and little is investigated about the agreement between them, especially in non-Mediterranean populations. The different available indexes are constructed from different approaches in relation to the population evaluated, which can result in different findings. Objectives: Evaluate and compare five proposals for adherence rates to the MDP; investigate the association between MDP, systemic inflammation and CMD among adults and elderly people living in the city of São Paulo. Methods: Data from the adult and elderly population from the 2015 São Paulo Health Survey (ISA-Capital 2015), a population-based cross-sectional study, were used. The Self Reporting Questionnaire-20 was used to identify the presence of CMD. Adherence to the PDM was assessed using five different dietary indexes (Mediterranean diet scale (MDS), Mediterranean diet Score (MedDietscore), Mediterranean dietary pattern (MDP), Mediterranean Adequacy Index (MAI) and Mediterranean-Style Dietary Pattern Score (MSDPS)), using food consumption data obtained from two 24-hour dietary recalls (R24h). These indexes were subsequently compared regarding the correlation and agreement between their results and the convergent validity of each one was tested using confirmatory factor analysis (CFA). The inflammatory profile was investigated using a Low-Grade Systemic Inflammation Score (LGSI score), constructed based on plasma levels of pro- and anti-inflammatory biomarkers, C-reactive protein (CRP), tumor necrosis factor alpha (TNF-&alpha;), interleukin-10 (IL-10) and adiponectin). Initially, the association between the MDP index with better convergent validity and the inflammatory profile was investigated using linear regression models (adjusted and unadjusted). Next, the direct, indirect and total effects of MDP on CMD were estimated, including systemic inflammation as a mediating variable. A significance level of 5% (p < 0.05) was adopted for the statistical tests (Stata v.14 and Mplus Version 87) Results: Manuscript 1 - The highest correlations were found between the MDP and MAI indexes (r = 076; 95% CI 0.74-0.79) and between MDP and MDS (r = 0.72; 95% CI 0.69-0.75). The greatest agreements observed were moderate, between MDP v. MAI (&kappa; = 0.57, P < 0.001) and MDP v. MDS (&kappa; = 0.48, P < 0.001). The goodness of fit of the model that defined the MedDietscore construct (RMSEA = 0.033, 90% CI 0.02-0.042; SRMR = 0.042) and MSDPS (RMSEA = 0.028, 90% CI 0.019- 0.037; SRMR = 0.031) presented acceptable values for absolute fit indices. Legumes, olive oil, MUFA:SFA ratio and whole grains with legumes were most relevant to characterize the MDP (factor loadings &ge;0&middot;50). Manuscript 2 - Only among the elderly, high adherence to the MDP meant a lower LGSI score of 11.5%. Elderly people classified as having high adherence to the MDP differed consuming less meat and full-fat dairy products. Among the elderly, vegetable and olive oil intake was inversely associated with levels of systemic inflammation. Manuscript 3 - The greatest effect observed in the tested models was the MDP on the LGSI in the elderly. MDP showed direct (SC = -0.247; p = 0.001) and total (SC = -0.315; p<0.001) negative and medium effects on systemic inflammation. MDP also had a small, negative indirect effect on the level of inflammation mediated by BMI (SC = -0.068; p = 0.017). The second largest effect observed was the direct effect (small and negative) of adherence to the PDM on CMD in older adults (SC = -0.182; p = 0.048). The indirect effect of MDP on CMD mediated by the LGSI was not significant (SC = 0.001; p = 0.948). The same result was found in relation to the direct effect of systemic inflammation on CMD (SC = -0.007; p = 0.948). Regarding model fit, the fit indices indicated the good fit of the two models (adults and elderly). Conclusions: The present study showed that following the methodological steps presented, it is possible to identify the Mediterranean indexes that best reflect the objective of evaluating adherence to the Mediterranean diet pattern in a non-Mediterranean population and thus guide the elements of this dietary pattern that can be encouraged to ensure its protective role on mental health and the level of systemic inflammation, especially in the elderly

    Associação do estado nutricional e da dieta com o microbioma intestinal em idosos hospitalizados agudamente enfermos

    Introduction - Ageing is characterised by changes associated with an increased risk of developing chronic diseases and syndromes, of which malnutrition is one of the most common in older adults. The gut microbiota has an important role in the host\'s health and is determined by several factors, including nutritional status and diet. Therefore, the gut microbiota may be associated with malnutrition and dietary intake in acutely ill older adults. Objectives - to identify- 1) the prevalence of malnutrition; 2) the association of malnutrition with the composition and metabolic potential of the gut microbiota and its impact on clinical outcomes; 3) the effect of habitual diet on the gut microbiota of acutely ill hospitalised older adults according to the nutritional status. Methods - a longitudinal analysis secondary to a prospective cohort was performed on 108 participants aged 65+ years old admitted to the hospital due to acute conditions. Clinical, demographic, nutritional, and anthropometric data and rectal swab samples were collected at admission and after 72 hours of hospitalisation. The food intake was estimated using the dietary history and the nutritional status diagnosed using the Global Leadership Initiative on Malnutrition (GLIM) criteria, preceded by the Mini-Nutritional Assessment Short-form (MNA-SF). Rectal swab samples were obtained to analyse the gut microbiota via 16S rRNA gene sequencing. The microbiota diversity, overall composition and differential abundances were calculated and compared between well-nourished and malnourished groups. Microbiome features potentially associated with malnutrition were selected by penalised models and confirmed by additive models. The prediction of clinical outcomes was investigated from the \"malnourished microbiota\" using decision trees. The dietary intake was explored through multivariate methods and investigated with the microbiota using tests of association and mediation analysis. Results - Malnourished patients (51%) had a different overall microbiota composition compared to well-nourished during hospitalisation (R= 0.079, p= 0.003). Severely malnourished (32.4%) showed a poorer diversity at admission (Shannon p= 0.012, Simpson p= 0.018) and after 72 hours (Shannon p= 0.023, Chao1 p= 0.008). Subdoligranulum, Lachnospiraceae NK4A136 group and Faecalibacterium prausnitzii, short-chain fatty acids producers, had significantly lower abundance and negative association with malnutrition, while Fusobacterium, Corynebacterium and Ruminococcaceae Incertae Sedis were highly increased and positively associated with malnutrition. Corynebacterium, Ruminococcaceae Incertae Sedis and the overall composition were important predictors of critical care in malnourished during hospitalisation. Malnourished patients had a significantly lower intake of plant protein, carbohydrates, dietary fibre, Fe, Mn, folate, campesterol, beta-sitosterol, and &alpha;-linolenic acid, and higher intake of Vitamin D. There were no significant correlations of nutrients, foods or food groups with the microbiota structure, but for individual taxa and &alpha;-diversity metrics. Mediation analysis revealed a significant indirect effect of the nutritional status on the differential abundance of several bacterial taxa partially mediated in a positive direction by dietary fibre, plant-based protein, onions, and olive oil, and negative by vitamin D. Conclusion - Malnourished patients had a significantly lower intake of key dietary compounds and substantial gut microbial disturbances during hospitalisation, pronounced in the severe stage. Some plant-based compounds might confer beneficial effects on the gut microbiota profile. A \"malnourished microbiota\" may be able to predict critical illness in hospitalised malnourished older patients. Bench-to-bedside investigations are necessary to confirm these findings.Introdução - O envelhecimento é caracterizado por mudanças associadas a uma maior chance de desenvolver síndromes e doenças crônicas, das quais a desnutrição é uma das mais comuns em idosos. A microbiota intestinal tem um papel importante na saúde e é determinada por vários fatores, incluindo o estado nutricional e a dieta. Portanto, a microbiota intestinal pode estar associada à desnutrição e à ingestão alimentar em idosos gravemente doentes. Objetivos - identificar- 1) a prevalência de desnutrição; 2) a associação da desnutrição com a composição e o potencial metabólico da microbiota intestinal e seu impacto nos desfechos clínicos; 3) o efeito da dieta habitual sobre a microbiota intestinal de idosos hospitalizados com doenças agudas de acordo com o estado nutricional. Métodos - uma análise longitudinal secundária a uma coorte prospectiva foi realizada em 108 participantes com mais de 65 anos de idade admitidos no hospital devido a condições agudas. Foram coletados dados clínicos, demográficos, nutricionais, antropométricos e swab retal na admissão e após 72 horas de internação. O consumo alimentar foi estimado por meio do histórico alimentar e o estado nutricional foi diagnosticado pelos critérios da Iniciativa de Liderança Global sobre Desnutrição (GLIM), precedido da triagem pela Mini-Avaliação Nutricional versão curta (MAN-SF). Amostras de swab retal foram obtidas para analisar a microbiota intestinal via sequenciamento do gene 16S rRNA. A diversidade da microbiota, composição geral e a abundâncias diferenciais foram calculadas e comparadas entre os grupos bem nutrido e desnutrido. Variáveis do microbioma potencialmente associadas à desnutrição foram selecionadas por modelos penalizados e confirmadas por modelos aditivos. A predição para os desfechos clínicos foi investigada a partir da \"microbiota desnutrida\" utilizando árvores de decisão. O consumo alimentar foi explorado por meio de métodos multivariados e investigado com a microbiota por meio de testes de associação e análise de mediação. Resultados - Os pacientes desnutridos (51%) apresentaram composição geral da microbiota diferente comparado com os bem nutridos durante a hospitalização (R= 0,079, p= 0,003). Os sevemente desnutridos (32,4%) apresentaram menor diversidade na admissão (Shannon p= 0,012, Simpson p= 0,018) e após 72 horas (Shannon p = 0,023, Chao1 p= 0,008). Subdoligranulum, Lachnospiraceae NK4A136 e Faecalibacterium prausnitzii, produtores de ácidos graxos de cadeia curta, tiveram abundância significativamente menor e associação negativa com a desnutrição, enquanto Fusobacterium, Corynebacterium e Ruminococcaceae Incertae Sedis tiveram maior abundância e associação positiva. Corynebacterium, Ruminococcaceae Incertae Sedis e a composição geral foram importantes preditores de cuidados intensivos em desnutridos durante a hospitalização. Pacientes desnutridos tiveram ingestão significativamente menor de proteína vegetal, carboidratos, fibra dietética, Fe, Mn, folato, campesterol, beta-sitosterol e ácido &alpha;-linolênico, e maior de vitamina D. Não houve correlações significativas entre nutrientes, alimentos ou grupos de alimentos com a estrutura da microbiota, mas sim para táxons individuais e métricas da &alpha;-diversidade. A análise de medição revelou um efeito indireto significativo do estado nutricional sobre a abundância diferencial de vários táxons bacterianos parcialmente mediada em uma direção positiva pela fibra alimentar, proteína vegetal, cebola, e azeite de oliva, e negativa pela vitamina D. Conclusão - Pacientes desnutridos tiveram menor ingestão de compostos dietéticos chaves e distúrbios microbianos intestinais substanciais durante a hospitalização, pronunciados no estágio grave. Alguns compostos de origem vegetal podem conferir efeitos benéficos no perfil da microbiota intestinal. A \"microbiota desnutrida\" pode ser capaz de predizer doenças críticas em idosos desnutridos hospitalizados. Investigações pré-clínicas e translacionais são necessárias para confirmar esses achados

    Effects of branched chain amino acid supplementation for increasing muscle mass and reducing body fat: a systematic review

    A busca pelo aumento da força muscular, em paralelo à diminuição da gordura corporal e melhora do rendimento esportivo, tem levado muitas pessoas ao uso de suplementos de proteínas e/ou aminoácidos, associados com a prática de exercícios físicos. Dentre os inúmeros suplementos de proteínas ou aminoácidos disponíveis no mercado, têm merecido destaque nas últimas décadas os aminoácidos essenciais de cadeia ramificada, também chamados de branched chain amino acids (BCAA\'s). O suplemento chamado de BCAA é uma combinação de três aminoácidos essenciais - L-Leucina, L-Valina e L-Isoleucina. As alegações feitas a esses aminoácidos giram em torno de seus efeitos sobre a síntese proteica no músculo esquelético, diminuição dos danos musculares, redução da gordura corporal e melhora do desempenho físico. O presente estudo teve como objetivo realizar uma revisão sistemática de estudos clínicos na utilização desses aminoácidos no intuito de aumentar a massa muscular, reduzir a gordura corporal e aumentar o rendimento esportivo, avaliando os resultados obtidos e que comprovem seu uso e segurança. A busca dos artigos nas bases de dados resultou em 7502 artigos. Seguindo todos os critérios de inclusão e exclusão, 11 artigos foram selecionados para esta revisão sistemática. A performance foi avaliada em 6 artigos. A massa muscular foi avaliada em 5. Não foram encontrados artigos visando a redução de gordura corporal. A dose de BCAA utilizada foi de 1,2 g até 10g e os estudos ministraram os suplementos na forma de pó, cápsulas e infusão. A melhor relação entre leucina, valina e isoleucina foi de 2:1:1, respectivamente. Considerando que o número de estudos com resultados benéficos praticamente se iguala ao de resultados negativos, mais estudos são necessários para que se comprove os reais benefícios do uso de BCAA como suplemento estratégico para aumentar a massa muscular, reduzir a gordura corporal e aumentar o rendimento esportivo.The search for increased muscle strength, in parallel to decreased in body fat and improved sports performance has led many people to use protein and/or amino acid supplements associated with the practice of physical exercises. Among the numerous supplements of proteins or amino acids available in the market, the branched chain amino acids (BCAA\'s) have deserved prominence in the last decades. The supplement called BCAA is a combination of three essential amino acids - L-Leucine, L-Valine and L-Isoleucine. The claims made to these amino acids revolve around their effects on protein synthesis in skeletal muscle, decreased muscle damage, reduced body fat and improved physical performance. The present study aimed to perform a systematic review of clinical studies in the use of these amino acids in order to increase muscle mass, reduce body fat and increase sports performance, evaluating the results obtained and proving its use and safety. Search for articles in databases resulted in 7502 articles. Following all the exclusion criteria, 11 articles were selected for the present systematic review. The performance was evaluated in 7 articles. The muscle mass was evaluated in 4. The used dose of BCAA ranged from 1.2 g to 10 g and studies have given supplements in the form of powder, capsules and infusion. The best ratio of leucine, valine and isoleucine was 2:1:1, respectively. Considering that the number of studies with beneficial results almost equals that of negative results, more studies are needed to prove the real benefits of using BCAAs as a strategic supplement to increase muscle mass, reduce body fat and increase sports performance

    Sarcopenia-relate factors in people living with human immunodeficiency virus

    As terapias antirretrovirais (TARVs) aumentaram a expectativa de vida de pessoas vivendo com HIV (PVHIV). Contudo, os efeitos residuais do HIV e a toxicidade das TARVs parecem prejudicar a integridade do músculo esquelético (ME), deixando as PVHIV mais suscetíveis à sarcopenia. Assim, os objetivos deste estudo foram: (i) compreender os mecanismos fisiopatológicos que explicam o motivo pelo qual PVHIV são mais suscetíveis à sarcopenia; (ii) rastrear entre as PVHIV e que não vivem com HIV (PNVHIV) a prevalência de estar sob risco de sarcopenia, a qualidade de vida relacionada à sarcopenia e os fatores associados; (iii) verificar a prevalência de sarcopenia e dinapenia entre as PVHIV, comparar os componentes da sarcopenia em relação às PNVHIV e verificar os fatores associados aos componentes da sarcopenia; (iv) identificar na literatura científica os estudos que avaliaram o efeito de intervenções nutricionais sobre os parâmetros relacionados ao ME de PVHIV. Assim, quatro artigos foram redigidos para responder cada um dos objetivos descritos acima, incluindo uma revisão narrativa (RN), dois estudos transversais e uma revisão sistemática (RS). Para a RN, estudos pré-clínicos e clínicos foram identificados no MEDLINE/PubMed utilizando as palavras-chave \"HIV\", \"Sarcopenia\" e \"Antirretrovirais\". Nos estudos de delineamento transversal, foram avaliadas PVHIV atendidas no Serviço de Extensão ao Atendimento de Pacientes HIV/AIDS que foram comparadas às PNVHIV. No inquérito virtual foram aplicados o SARC-F e o SarQoL® para identificar a prevalência de estar sob risco de sarcopenia (&ge; 4 pontos) e a qualidade de vida relacionada à sarcopenia (abaixo da mediana do escore geral), respectivamente. No estudo presencial foram avaliados a força de preensão manual (FPM), o índice de massa magra apendicular (IMMA), a velocidade de marcha (VM) e os fatores associados à sarcopenia. Em seguida, foi comparada a prevalência de sarcopenia, dinapenia e obesidade dinapênica entre as PVHIV e PNVHIV. Ambos os estudos foram compostos por homens e mulheres com idade &ge;18 anos, residentes no estado de São Paulo. Finalmente, a RS foi registrada no prospero CRD42019139981 e seguiu os critérios PICOS para condução da busca. Os dados da RN revelaram que fatores como a inflamação sistêmica, desordens metabólicas e disfunção mitocondrial proveniente das TARVs aumentam a chance de PVHIV desenvolverem sarcopenia. O inquérito virtual (n= 344; 45,5±13,1 anos; 211 PVHIV) revelou que 10,43% (95 % IC: 6,6 - 15,4%) e 0,75% (95% IC: 1,90e-4 - 4%) das PVHIV e PNVHIV estão sob risco de sarcopenia, respectivamente. Similar, PVHIV apresentam pior qualidade de vida relacionada à sarcopenia e os problemas com o sono (OR 7,343; 95 % IC: 3,261 - 16,536; p< 0,001) e a obesidade (OR 4.945; 95% IC: 1,597 - 15,304; p= 0.006) são os principais fatores positivamente associados, ao passo que a prática regular de exercícios físicos está associada a melhor qualidade de vida relacionada à sarcopenia (OR 0.173; 95% IC: 0,080 - 0,375; p< 0,001). O estudo presencial (n= 70; 45,5±7,73 anos; n= 35 PVHIV) revelou que PVHIV e PNVHIV não atenderam os critérios para sarcopenia; entretanto, as PVHIV apresentaram menor FPM (-6,68 kg; p= 0,005) comparativamente às PNVHIV. Ainda, a razão massa gorda: massa magra foi o principal fator associado à menor FPM (ß= -15,476; p< 0,001). O IMMA não diferiu entre os grupos (-0,138 kg/m2; p= 0,641). Finalmente, na RS, nenhuma intervenção nutricional proposta entre os estudos promoveu aumento da massa muscular em PVHIV.Antiretroviral therapies (ARTs) have increased the life expectancy of people living with HIV (PLWH). However, residual effects of HIV and toxicity from ARTs appear to impair skeletal muscle (SkM) integrity, leaving PLWH more susceptible to sarcopenia. Thus, this study\'s purpose was: to comprehend the pathophysiological mechanisms that explain why PLWH are more susceptible to sarcopenia; (ii) to verify among PLWH and not living with HIV (PNLWH) the prevalence of living at risk of sarcopenia, sarcopenia-related quality of life, and associated factors; (iii) to verify the prevalence of sarcopenia and dynapenia among PLWH, to compare the components of sarcopenia with PNVHIV and to verify the factors associated with the features of sarcopenia; (iv) to identify the studies that evaluated the effect of nutritional interventions on the SkM-related parameters. Thus, four articles were written to answer each of the purposes, including one narrative review (NR), two cross-sectional studies, and one systematic review (SR). For the NR, preclinical and clinical studies were identified in MEDLINE/PubMed using the keywords \"HIV,\" \"Sarcopenia,\" and \"Antiretrovirals.\" In the cross-sectional design studies, PLWH was evaluated and compared to PNLWH. In the web-based survey, SARC-F and SarQoL® were applied to identify at risk of sarcopenia (&ge; 4 points) prevalence and lower sarcopenia-related quality of life (below overall score median), respectively. In the second study, handgrip strength (HGS), appendicular lean mass index (ApLMI), walking speed (WS), and factors associated with sarcopenia were evaluated. Consequently, the prevalence of sarcopenia, dynapenia, and dynapenic obesity among PLWH and PNLWH was compared. Both studies were composed of men and women aged &ge;18 years, residing in the state of São Paulo. Finally, the SR was registered in the PROSPERO CRD42019139981 and followed the PICOS criteria for searching. The data from the NR revealed that factors such as systemic inflammation, metabolic disorders, and mitochondrial dysfunction arising from ARTs increase the odds of PLWH being sarcopenic. The web-based survey (n= 344; 45.5±13.1 years; 211 PLHIV) revealed that 10.43% (95% CI: 6.6 - 15.4%) and 0.75% (95% CI: 1.90e-4 - 4%) of PLWH and PNLWH are at risk of sarcopenia, respectively. Similar, PLWH have worse sarcopenia-related quality of life and sleep problems (OR 7.343; 95% CI: 3.261 - 16.536; p< 0.001) and obesity (OR 4.945; 95% CI: 1.597 - 15.304; p= 0.006) are the main positively associated factors, whereas regular exercise is associated with lower odds to poor sarcopenia-related quality of life (OR 0.173; 95% CI: 0.080 - 0.375; p< 0.001). The second transversal study (n= 70; 45.5±7.73 years; n= 35 PLHIV) revealed that PLWH and PNLWH did not meet criteria for sarcopenia; however, PLWH had lower HGS (-6.68 kg; p= 0.005) compared to PNLWH. Also, the fat mass: lean mass ratio was the main factor associated with lower HGS (ß= -15.476; p< 0.001). ApLMI did not differ between groups (-0.138 kg/m2; p= 0.641). Finally, no nutritional intervention in SR promoted increased muscle mass in PLWH

    Characterization of nutritional status of elderly from an outpatient clinic of geriatric neuropsychiatry: utilization of the Mini-nutritional assessment (MNA)

    INTRODUÇÃO: O cuidado nutricional é parte essencial para o acompanhamento integral da crescente população idosa, principalmente quando na presença de doenças crônicas e neurodegenerativas e morbidades psiquiátricas. A Mini Avaliação Nutricional (MAN®), tem sido utilizada para este fim, e em algumas situações, informações adicionais a essa ferramenta são necessárias. OBJETIVOS: Caracterizar o estado nutricional de idosos acompanhados em ambulatório de neuropsiquiatria geriátrica. MÉTODOS: Foi obtida uma amostra de conveniência selecionada aleatoriamente entre pacientes idosos com diagnóstico de depressão (DEP), doença de Alzheimer (DA) e outros transtornos psiquiátricos (OTP). O estado nutricional foi avaliados através da MAN®, pelo índice de massa corporal (IMC= peso/altura2) e pelas circunferências do braço (CB) e panturrilha (CP). Os dados referentes ao consumo de medicamentos também foram obtidos, considerando-se como polifarmácia o consumo de mais de três medicamentos/dia. Os dados de diagnóstico médico e exames laboratoriais foram obtidos do prontuário médico. Foram avaliados 217 indivíduos no total, distribuídos conforme o diagnóstico clínico: grupo DEP= 107 (49,3%); grupo DA=59 (27,2%) e grupo OTP=51 (23,5%). RESULTADOS: O escore da MAN® identificou risco nutricional em 67,3%, 69,5% e 54,9%, e desnutrição em 14,0%, 13,6% e 27,5% dos pacientes em DEP, DA e OTP respectivamente. O baixo peso, avaliado pelo IMC foi observado em 29,0% (DEP), 24,1% (DA) e 45,1% (OTP) dos avaliados. A prevalência de anemia foi de 11,2% (DEP), 10,2% (DA) e 19,6% (OTP); sendo mais frequente em mulheres em DEP e OTP e em homens em DA. A média para vitamina D em todos os grupos encontrava-se na insuficiência. A presença de colesterol total acima dos intervalos de normalidade foi de 47,0% (DEP), 36,3% (DA) e 31,8% (OTP). Foi observada hiperglicemia em 52,9% (DEP), 48,1% (DA) e 41,6% (OTP) dos pacientes. A polifarmácia foi observada em 18,7% (DEP), 15,2% (DA) e 19,6% (OTP). Não houve associação significativa entre o uso de medicamentos e o estado nutricional, embora tenha sido apontado um direcionamento ao risco de desnutrição com o uso de medicamentos para o tratamento da depressão (inibidores seletivos de receptação de serotonina), demências (inibidores de colinesterase), cloridrato de omeprazol e indutores do sono. CONCLUSÕES: o risco nutricional e a desnutrição são prevalentes na população estudada, independente do grupo específico de doença neuropsiquiátrica ou demência. Esses achados demandam e justificam um monitoramento nutricional permanente para esses indivíduos, uma vez que vem acompanhados de outras deficiências específicas que comprometem a saúde de forma geral, como a anemia e insuficiência de vitamina D. A elaboração de planos educativos envolvendo os pacientes, cuidadores e a equipe médica, poderá contribuir para a melhora dessas condições.INTRODUCTION: Nutritional care is an essential part for the integral follow - up of the growing elderly population, especially in the presence of chronic and neurodegenerative diseases and psychiatric morbidities. The Mini Nutrition Assessment (MAN®) has been used for this purpose, and in some situations, additional information to this tool is necessary. OBJECTIVES: To characterize the nutritional status of elderly patients in a geriatric neuropsychiatry clinic. METHODS: A randomly selected convenience sample was obtained among elderly patients diagnosed with depression (DEP), Alzheimer\'s disease (AD) and other psychiatric disorders (OTP). The nutritional status was evaluated through MAN®, body mass index (BMI = weight / height2) and circumference of arm (CB) and calf (CP). The data referring to the consumption of medicines were also obtained, considering as polypharmacy the consumption of more than three medications/day. The data of medical diagnosis and laboratory tests were obtained from the medical record. A total of 217 individuals were evaluated, distributed according to the clinical diagnosis: DEP = 107 (49.3%); Group DA = 59 (27.2%) and OTP group = 51 (23.5%). RESULTS: The MAN® score identified nutritional risk in 67.3%, 69.5% and 54.9%, and malnutrition in 14.0%, 13.6% and 27.5% of the patients in DEP, DA and OTP respectively. Low weight, evaluated by BMI, was observed in 29.0% (DEP), 24.1% (AD) and 45.1% (BTP) of the evaluated patients. The prevalence of anemia was 11.2% (DEP), 10.2% (AD) and 19.6% (OTP); Being more frequent in women in DEP and OTP and in men in AD. The mean for vitamin D in all groups was in the insufficiency. The presence of total cholesterol above the normality intervals was 47.0% (DEP), 36.3% (DA) and 31.8% (OTP). Hyperglycemia was observed in 52,9% (DEP), 48,1% (AD) and 41,6% (OTP) of the patients. Polypharmacy was observed in 18.7% (DEP), 15.2% (DA) and 19.6% (OTP). There was no significant association between drug use and nutritional status, although there was a trend towards malnutrition with the use of drugs to treat depression (selective serotonin reuptake inhibitors), dementias (cholinesterase inhibitors), Omeprazole hydrochloride and sleep inducers. CONCLUSIONS: nutritional risk and malnutrition are prevalent in the population studied, regardless of the specific group of neuropsychiatric disease or dementia. These findings demand and justify a permanent nutritional monitoring for these individuals, since they are accompanied by other specific deficiencies that compromise health in general, such as anemia and insufficiency of vitamin D.     Association Between Depressive Symptoms, Nutritional Aspects of Elderly Residents of a City of São Paulo: A Population-Based Study

    Introdução: Depressão ou morbidade depressiva, é uma doença mental complexa que inclui diferentes fenótipos e sintomas, com desfechos negativos na saúde. Os diferentes distúrbios psiquiátricos, assim como a presença de sintomas depressivos, têm sido associados a dieta e hábitos alimentares. Objetivos: Investigar a associação entre sintomas depressivos e aspectos relacionados ao estado nutricional e alimentação, em idosos residentes na da Zona Oeste do município de São Paulo. Métodos: O presente estudo é epidemiológico, transversal, de base comunitária, com 2.643 idosos com idade igual ou superior a 60 anos. Foram consideradas como variáveis: a- Dependente: Sintomas Depressivos pela escala proposta pelo Centro de Estudos Epidemiológicos para identificação de sintomas depressivos (CES-D); Variáveis independentes (preditores): Estado nutricional a partir das medidas antropométricas (peso e altura com posterior cálculo do IMC, e circunferência da cintura); Respostas a algumas questões relacionadas ao hábito de consumir determinados alimentos ou grupos de alimentos, e hábito de fazer as refeições diárias. Foram ainda avaliadas as variáveis relacionadas a presença de doenças crônicas (hipertensão, diabetes e hipercolesterolemia); características sócio econômicas e demográficas (critério de Classificação Econômica Brasileira (CCEB). As variáveis foram analisadas por regressão logística binária, adotando significativos os valores de p <0,05. Resultados: Foi encontrada uma prevalência elevada de sintomas depressivos (38%); associaram-se positivamente aos sintomas depressivos o hábito em consumir frutas, feijões, leites e derivados, azeite, água e vinho. Após ajustes pelas variáveis clínicas, socioeconômicas e demográficas, permaneceram significantes no modelo final o hábito em consumir leites e derivados, azeite e vinho; o hábito em realizar o almoços e jantar; relatar a presença de HAS; pertencer ao sexo feminino e estar divorciado ou separado. Conclusões: O presente estudo mostrou a existência de associações significativas entre sintomas depressivos e o hábito em consumir determinados alimentos (leite e derivados, azeite e vinho). Porém tais dados ainda não são totalmente conclusivos para a população brasileira. Novos estudos fazem-se necessários para esclarecer a relação entre o hábito em consumir determinados grupos de alimentos e a chance em desenvolver sintomas depressivos em idosos.Background: Depression or depressive morbidity is a complex mental disease that includes different phenotypes and symptoms, with negative health outcomes. The variables were analyzed by binary logistic regression, adopting significant values of p <0.05. Results: There was found a high prevalence of depressive symptoms (38%); The habit of consuming fruits, beans, milks and derivatives, olive oil, water and wine were positively associated with depressive symptoms. After adjusting for the clinical, socioeconomic and demographic variables, the habit of consuming milk and derivatives, olive oil and wine remained significant in the final model; The habit of holding lunch and dinner; Have HAS; Belong to the female sex and be divorced or separated. Conclusions: The present study showed the existence of significant associations between depressive symptoms and eating habits (milk and dairy products, olive oil and wine). However, these data are not yet fully conclusive for a Brazilian population. New studies are needed to define a relationship between the habit and consumption of food groups and a chance to develop depressive symptoms in the elderly

    Adesão ao Padrão da dieta mediterrânea e Transtornos Mentais Comuns em idosos de São Paulo-SP-Brazil: estudo de base populacional

    Introduction: The presence of Common Mental Disorders (CMD) becomes more common with advancing age. The literature shows the beneficial effects of the Mediterranean Diet on the mental health of the elderly, but few studies have been conducted in Western countries. In addition, studies conducted in non-Mediterranean regions generally do not take into account the peculiarities of food, which do not always meet all the Mediterranean characteristics. Objective: (i) to describe the adherence to the Mediterranean dietary pattern, considering two points of view: including only foods characteristic of the original standard Mediterranean diet and including foods with non-Mediterranean characteristics (ii) to verify the association between the adherence to the two Mediterranean Dietary Patterns (MDP) and CMD in the elderly. Methods: cross-sectional population-based study; Data from the 2015 Health Survey of São Paulo were used. The presence of CMD was identified through the Self Reporting Questionnaire-20. The MDP was constructed using the Mediterranean Diet Score (calculated from data from two 24-hour dietary recall), considering two points of view: Mediterranean pattern origin-like (MPOL), which included only foods from the original Mediterranean diet; Mediterranean pattern including foods with non-Mediterranean characteristics (MPNM), which included foods submitted to procedures that diverge from the original dietary pattern. The other variables were identified through a previously structured questionnaire. The association between TMC and adherence to MPOL and MPNM was investigated through logistic regression models. The covariates for the adjustment were selected for biological plausibility. A significance level of 5% (p <0.05) was considered for final statistical tests. All data analyses were performed using Stata software (version 14). To accomplish the objectives of the present research, two manuscripts were elaborated. The aims of the 1st manuscript were: (i) describe the degree of adherence to this dietary pattern by elders from São Paulo-SP-Brazil; (ii) to identify modifications made in the form of intake of the different components of the original MDP, investigating associations with demographic and socioeconomic variables. The aims of the 2nd manuscript were: (i) To evaluate the association between the adherence to the MDP with the presence of mental disorders in these elders; (ii) To investigate these associations taking into account the two different points of view of MDP. Results: The 1st manuscript showed a moderate adherence to the two views of MDP. The greater adherence to the two proposed dietary patterns was characterized by higher intakes of vegetables, fruits, nuts, cereals, legumes and greater proportion between monounsaturated and saturated lipids and, as well as lower consumption of meat and dairy products. MPOL and MPNM were associated with formal education and age, respectively. From the 2nd manuscript, we identified that moderate and high adherence to MPOL, compared to low adherence, was associated with a lower prevalence of CMD, after adjustment for gender, age, body mass index, number of chronic diseases, per capita household income, physical activity and smoking status. The presence of CMD was not associated with MPNM adherence. Additionally, the presence of CMD was different between gender, physical activity and number of referred chronic diseases. Main conclusions (from the two manuscripts): the protective effect of MDP on CMD in elderly was observed only when the particularities of this diet were fulfilled. Other factors, associated to lifestyle, showed to be important to improve the associations with CMD. The results support the importance of programs that encourage the maintenance of healthy eating habits among the elderly.Introdução: A presença de Transtornos mentais comuns (TMC) torna-se mais frequente com o avançar da idade. A literatura evidencia os efeitos benéficos da Dieta mediterrânea sobre a saúde mental do idoso, porém poucos estudos foram realizados em países ocidentais. Ademais, os estudos realizados em regiões não mediterrâneas geralmente não levam em consideração as peculiaridades dos alimentos, que nem sempre atendem a todas as características mediterrâneas. Objetivos: (i) descrever a adesão ao padrão da dieta mediterrânea considerando dois pontos de vista: incluindo apenas alimentos característicos do original padrão da dieta mediterrânea e incluindo alimentos com características não mediterrâneas (ii) verificar a associação entre a adesão aos dois padrões da dieta mediterrânea (PDM) construídos e TMC em idosos. Métodos: trata-se de um estudo transversal de base populacional; foram utilizados dados do Inquérito de Saúde de São Paulo 2015 (ISA-Capital). A presença de TMC foi identificada através do Self Reporting Questionnaire-20. O PDM foi construído através do Escore da dieta mediterrânea (calculado com base em dois recordatórios alimentares de 24h), considerando dois pontos de vista: Padrão mediterrâneo similar ao original (PMSO) - com a inclusão de apenas alimentos originais da dieta mediterrânea; Padrão mediterrâneo com características não mediterrâneas (PMNM) - com a inclusão de alimentos que foram submetidos a procedimentos que divergem do padrão mediterrâneo original. As demais variáveis foram identificadas por meio de um questionário previamente estruturado. A associação entre TMC e a adesão ao PMSO e PMNM foi investigada por meio de modelos de regressão logística. As covariáveis para o ajuste foram selecionadas por plausibilidade biológica. Adotou-se nível de significância de 5% (p < 0,05) para os testes estatísticos finais. Todas as análises de dados foram realizadas utilizando o software Stata (versão 14). Para atender os objetivos da presente pesquisa, foram elaborados dois manuscritos. O manuscrito 1 teve como objetivos (i) descrever o grau de adesão ao PMD por idosos de São Paulo-SP-Brasil; (ii) construir dois pontos de vista do PMD, de acordo com as modificações frequentes na forma de ingestão dos diferentes componentes desse padrão dietético, investigando associações com variáveis demográficas e socioeconômicas. O manuscrito 2 teve como objetivos: (i) avaliar a associação entre a aderência ao PMD e transtornos mentais comuns nesses idosos; (ii) investigar essas associações levando em consideração as duas visões construídas para esse padrão. Resultados: No manuscrito 1 foi encontrada uma adesão moderada pelos idosos às duas visões do PMD. A maior adesão a ambas as visões do PMD foi caracterizada pela ingestão mais elevada de vegetais, frutas, oleaginosas, cereais, leguminosas e maior proporção de lipídios monoinsaturados e saturados, além de um menor consumo de carne e laticínios. O PMSO e o PMNM apresentaram associação com escolaridade e idade, respectivamente. No Manuscrito 2, identificou-se que as adesões moderada e alta ao PMSO, em comparação a uma baixa adesão, foram associadas a uma menor prevalência de TMC, após ajuste para sexo, idade, índice de massa corporal, número de doenças crônicas, renda domiciliar per capita, atividade física e tabagismo. Por sua vez, a presença de TMC não mostrou associação significativa com nenhum grau de adesão ao PMNM. Adicionalmente, a presença de TMC foi significativamente diferente entre os gêneros, a atividade física e o número de doenças crônicas referidas. Conclusões principais (a partir de ambos os manuscritos): o efeito protetor de uma maior adesão ao PDM sobre os TMC em idosos foi observado apenas quando as particularidades originais desse padrão foram atendidas. Outros fatores, associados ao estilo de vida, mostraram-se importantes para melhorar as associações com TMC. Os resultados encontrados reforçam a importância de programas que incentivem a manutenção de hábitos alimentares saudáveis entre idosos

    The inflammatory environment and health involvments in persons in ages related to menopause and andropause

    Introdução: O aumento da expectativa de vida da população faz com que as pessoas passem a viver por mais tempo com doenças crônicas não transmissíveis (DCNT). Dentre as mulheres, as fases da vida que mais se destacam pelos acometimentos em saúde são aquelas relacionadas à menopausa; além do risco aumentado para o desenvolvimento da síndrome metabólica (SM), há destaque para os transtornos mentais comuns (TMC) e as doenças articulares (DA). Já entre os homens, uma vez que a andropausa não é tão claramente identificada como a menopausa, acaba-se atribuindo essa ocorrência à idade e às baixas concentrações séricas de testosterona. Todavia, em ambos os sexos, um processo de inflamação sistêmica crônica e de baixo grau (ISBG) tem sido apontado como um importante fator associado ao desenvolvimento e agravo de todas as condições acima mencionadas. A ISBG decorre de alterações próprias do envelhecimento no sistema imune (SI), particularmente a imunossenescência, mas também por outros fatores externos ao SI, em especial as modificações na gordura corporal e no ambiente intestinal. Nesse contexto, estudar a relação e fatores associados a essas condições permite o delineamento de estratégias de intervenção em saúde. Objetivos: Investigar, em pessoas a partir 40 anos de idade, a prevalência e as associações entre desfechos em saúde relacionados a DCNT, incluindo fatores relacionados à ISBG. Métodos: O presente estudo, que consistiu na elaboração de três manuscritos, foi desenvolvido a partir de dados do Inquérito de Saúde de São Paulo de 2015, um estudo transversal, de base populacional e com amostra representativa dos moradores da área urbana do município de São Paulo. No primeiro manuscrito, foi investigada a associação entre a presença de TMC, DA, Índice de massa corporal (IMC) e outras doenças crônicas. As análises incluíram também dados sociodemográficos (idade, escolaridade, raça/etnia); essas associações foram testadas por modelos de regressão logística múltipla. No segundo manuscrito foram testadas as associações entre TMC e DA com o potencial inflamatório da dieta (identificado a partir do cálculo do Índice Inflamatório da Dieta), o nível de atividade física (utilizando o International Physical Activity Questionnaire- IPAq) classificado conforme recomendação da Organização Mundial da Saúde (OMS), a presença de outras doenças crônicas e o IMC. As análises também incluíram variáveis sociodemográficas (faixa etária, escolaridade, raça/etnia). O terceiro manuscrito consistiu em uma subamostra não representativa do banco de dados do estudo ISA, com participantes que tiveram a composição corporal avaliada por DEXA (raios-x de dupla energia), de onde se obteve a massa magra apendicular e o total de gordura corporal. Foi também avaliada a força de preensão manual, que determina a qualidade do músculo esquelético. Foi realizada a dosagem de marcadores inflamatórios (TNF-&alpha;) e de permeabilidade intestinal (LPS, zonulina e iFABP). Ainda, esses participantes tiveram realizadas as dosagens de HDL-c plasmático, glicemia de jejum e triacilglicerol, além das medidas de pressão arterial. Esses parâmetros foram utilizados para a classificação da síndrome metabólica (SM). As associações, mediações e direções entre essas variáveis foram testadas a partir de modelos generalizados de equações estruturais. Principais Resultados: Manuscrito 1. A prevalência de TMC entre as mulheres investigadas no estudo esteve entre 26,9% e 38,0%, a de DA ficou entre 18,7% e 31,1%. Foram encontradas associações entre TMC e DA (OR = 1,998; p<0,001), idade entre 56 e 60 anos (OR= 0,542; p=0,018), e a presença de três diagnósticos de outras doenças crônicas não transmissíveis (OR= 2,696; p= 0,027). Manuscrito 2. Avaliando simultaneamente as associações entre TCM, DA, potencial inflamatório da dieta, e o nível de atividade física, observou-se que as associações entre TMC, DA e número de diagnósticos de outras doenças crônicas foram mantidas, e o maior tercil do escore do índice inflamatório da dieta se mostrou positivamente associado a presença de TCM (OR=2,240; p=0,006). O nível de atividade física não apresentou significância, porém permaneceu ajustando os modelos. Manuscrito 3. A síndrome metabólica foi identificada em 45,8% dos participantes, e associações diretas foram observadas entre TNF-&alpha; e massa gorda corporal, e entre a permeabilidade intestinal e a massa muscular apendicular. Conclusões: Os resultados aqui apresentados confirmaram uma associação significativa entre transtornos mentais e aspectos inflamatórios, representados pela presença de doenças articulares e outras doenças crônicas, além do potencial inflamatório da dieta. A atividade física mostrou uma associação marginal protetora em relação à inflamação sistêmica e consequentemente aos transtornos mentais. Finalmente, componentes da composição corporal, massa gorda e massa magra apendicular, se mostraram diretamente associados a marcadores inflamatórios e a presença de síndrome metabólica.Background: The increase in population life expectancy allows individuals to live longer time periods with noncommunicable diseases (NCD). Among women, the life phases that stand out the most by the health compliment are those related to menopause, with emphasis to the common mental disorders (CMD), and the joint diseases (JD), and an increased risk for the metabolic syndrome presentation. Among men, once andropause is not as clearly identified as menopause, the occurrence of these conditions is attributed to age and to the low level of circulating testosterone. The called low-grade systemic inflammation (LGSI) has been pointed as an important factor associated to the development and worsening of all the mentioned conditions. The LGSI results from the immune system (IS) proper alterations, but also from factor aside IS, especially the body fat and gut environment changes. In this context, investigating the relations and factors associated to those conditions allows designing health intervention strategies. Aims: To investigate, in persons aged 40+ years old, the prevalence and associations between CMD related outcomes, with emphasis on the factors associated to the LGSI. Methods: The present study, which is constituted of three manuscript elaboration, was developed from data of the 205 Health Survey of São Paulo, a population-based cross-sectional study, with representative sample of urban residents of the city of São Paulo. In the first manuscript, associations of CMD, JD, and other chronic conditions, and body mass index (BMI), were investigated. The analysis also included sociodemographic data (age, schooling, race/ethnicity); these associations were testes by multiple logistic regression models. In the second manuscript, associations were tested between CMD and JD, with the inflammatory potential of the diet (identified from the Dietary Inflammatory Index calculation), the leisure physical activity level [classified according to WHO (World Health Organization) proposition using the International Physical Activity Questionnaire- IPAq], the presence of other chronic conditions and BMI. Analysis also included sociodemographic variables (age intervals, schooling, race/ethnicity). Third manuscript consisted of a sub-sample, which was not representative of the Health Survey of São Paulo, with participants who had their body composition evaluated by DXA (dual energy x-ray), from which appendicular muscle mass and total body fat were obtained. Also, handgrip strength, that determinate skeletal muscle quality, was evaluated. Inflammatory (TNF-&alpha;) and gut permeability (LPS, zonulin and iFABP) were evaluated. Yet, these participants had plasmatic HDL-c, fasting blood glucose and triacylglycerol, and arterial blood pressure evaluated. These parameters were used for the metabolic syndrome (MS) classification. Associations, mediations, and directions among those variables were tested by Generalized Structural Equation Models. Main results: Manuscript 1. The prevalence of CMD among investigated women was found between 26.9% and 38.0%, and from 18.7% to 31.1% for JD. Associations were found between CMD and DA (OR= 1.998; p<0.0001), age from 56 to 60 years old (OR= 0.542; p=0,018), and the presence of the diagnostics of three chronic conditions other than JD (OR= 2.696; p=0.027). Manuscript 2. Simultaneously evaluating the associations between CMD, JD, and dietary inflammatory potential, physical activity level, and the number of other chronic conditions diagnosis, and the higher dietary inflammatory index score presented positively associated to the presence of CMD (OR= 2.240; p=0.006). Physical activity level did not present significant associations, but it adjusted the models. Manuscript 3. Metabolic Syndrome was identified in 45.8% of the participants, and direct associations were observed between TNF-&alpha; and body fat mass, and from gut permeability and the appendicular muscle mass. Conclusions: The here presented results confirm a significant association between common mental disorders and inflammatory aspects, represented by the presence of joint diseases and other chronic conditions, and also the dietary inflammatory potential. Physical activity presented marginal protective association in relation to systemic inflammation, and consequently to common mental disorders. Finally, body composition components, body fat and appendicular muscle mass, were directly associated to inflammatory markers and to the presence of metabolic syndrome

    Screening tools for HIV-associated neurological disorders: what is the best choice for the nutritionist? A systematic review

    Introdução: As terapias antirretrovirais (TARV) foram imprescindíveis para o aumento da expectativa de vida das pessoas que vivem com o HIV (PVHIV). Entretanto, ações residuais do vírus, combinadas com efeitos colaterais da TARV, mantêm um estado de imunoativação e, consequentemente, um quadro de inflamação sistêmica de baixo grau nessas pessoas. Nesse contexto, comprometimentos cognitivos são ainda presentes e podem evoluir para demências e incapacidades. Em 2007 foi estabelecida a denominação de HIV-associated neurological disorders (HAND) às alterações cerebrais decorrentes da infecção pelo HIV. Essas desordens são categorizadas em: Comprometimento Neurocognitivo Assintomático (ANI), Comprometimento Neurocognitivo Leve/moderado (MND) e Demência Associada ao HIV (HAD). O atendimento clínico das PVHIV por profissionais de saúde, e particularmente pelo nutricionista, demanda o rastreio desses comprometimentos, uma vez que a presença deles pode contribuir com uma baixa compreensão ou não adesão ao plano dietético, o que aumenta o risco nutricional dessas pessoas. Desta forma, estudos que indiquem as ferramentas mais adequadas para rastreio dos HAND contribui para o trabalho da equipe multiprofissional. Objetivos: o presente estudo tem como objetivo identificar estudos que compararam diferentes ferramentas utilizadas para rastreio do HAND com o procedimento considerado \"padrão-ouro\", a bateria de testes neuropsicológicos. Métodos: Este estudo consiste em uma revisão sistemática (RS) com registro no PROSPERO CRD42021227122. Foram incluídos estudos epidemiológicos ou clínicos com adultos PVHIV, publicados a partir de 2007, que tenham estudado a validação de ferramentas de rastreio para HAND frente a uma bateria neuropsicológica. As buscas foram realizadas em seis bases de dados (LILACS, PubMed, Embase, Web of Science, Scopus e Scielo). Resultados: a busca gerou 1.505 artigos. Após aplicação dos critérios de inclusão e exclusão, 15 estudos, em sua maioria classificados como de boa qualidade metodológica, fizeram parte da RS. Dentre esses estudos, cinco testaram a validação para a pontuação geral do HAND, e outros 10 testaram a validação de acordo com as três categorias do HAND. As ferramentas testadas pelos estudos foram: Escala Internacional de Demência do HIV (IHDS) , Escala de Demência do HIV (HDS) , Mini Exame do Estado Mental (MEEM), Avaliação Cognitiva de Montreal (MoCA), Questões de sintomas de Simioni (SSQ), Quatro perguntas-chave propostas pelo Conselho Consultivo HAND da Ásia, Austrália, África e Oriente Médio (AAAME), Questionário de sintomas cognitivos do HIV (HCSQ). Além disso, dois estudos dois estudos utilizaram como rastreio um teste integrante da bateria neurocognitiva, o Trail making Test A. As melhores combinações entre sensibilidade (Se) e especificidade (Sp) foram atribuídas às seguintes ferramentas de rastreio: HDS isoladamente (ponto de corte=14: Se= 88% e SP= 67%); IHDS+ Trail making test A (ponto de corte 10: Se= 86% e Sp =79%); MoCA (ponto de corte 26: Se=66% e Sp= 55%). Com relação ao MEEM, nenhuma combinação adequada de sensibilidade e especificidade foi indicada pelos estudos. Nenhum desses resultados foram considerados suficientemente sensíveis e específicos para rastreio das formas leves do HAND. Conclusão: Os resultados desta RS apontam que ainda é necessário o desenvolvimento de ferramentas mais apropriadas para o rastreio do HAND, principalmente das formas leve e moderada. Das ferramentas existentes, o HDS a que mostrou os resultados mais próximos de adequação.Introduction: Antiretroviral therapies (ART) have been essential for increasing the life expectancy of people living with HIV (PLHIV). However, residual effects of the virus, combined with the side effects of ART, provoke a state of immune activation and, consequently, a low-grade and systemic inflammation status. In this context, cognitive disorders are present and can progress to dementia and disabilities. In 2007, the nomenclature \"HIV-associated neurological disorders (HAND)\" was established for these HIV-related consequences. These disorders are categorized as follows: Symptomatic Neurocognitive Disorders (ANI), Mild/Moderate Neurocognitive Disorders (MND), and HIV-Associated Dementia (HAD). The clinical care of PLHIV by health professionals, including nutritionists, demands identifying these disorders since they can contribute to poor adherence to dietary plans. Poor adherence increases the nutritional risk of these people. Therefore, studies searching for most the appropriate tools for screening HAND can contribute to the work of the multidisciplinary team. Objectives: This study aimed to identify different HAND screening tools, and their comparison with the \"gold standard\" method\", the neuropsychological battery. Methods: This study consists of a systematic review (SR) registered in PROSPERO CRD42021227122. We included epidemiological or clinical studies with PLHIV adults, published from 2007 onwards, which have studied the validation of screening tools for HAND against a neuropsychological battery. Studies were searched in six databases (LILACS, PubMed, Embase, Web of Science, Scopus, and Scielo). Main Results: The search generated 1,505 articles; after applying the inclusion and exclusion criteria, 15 studies, mostly classified as having good methodological quality, took part in the RS. Five studies performed the validation for the overall HAND score and another 10 tested the validation according to the three HAND categories. The tools tested were: the International HIV Dementia Scale (IHDS), HIV Dementia Scale (HDS), Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Simioni Symptom Questions (SSQ), Four key questions proposed by the HAND Advisory Council of Asia, Australia, Africa, and the Middle East (AAAME), HIV Cognitive Symptoms Questionnaire (HCSQ). In addition, two studies tested as a screening tool, a test which is part of the neurocognitive battery (Trail making Test A). From these tools, the best combinations between sensitivity (Se) and specificity (Sp) were attributed to: HDS alone (cut-off point=14: Se=88% and SP=67%); IHDS+ Trail making test A (cut-off 10: Se=86%, and Sp=79%); and MoCA (cut-off point 26: Se=66% and Sp=55%). For the MMSE, no suitable combination of sensitivity was found according to the studies. None of these tools were considered sensitive enough for screening mild forms of HAND. Conclusion: The results of this SR indicate that it is still necessary to develop more appropriate tools for adequate HAND screening, especially in mild and moderate forms. Of the existing tools, HDS showed slightly satisfactory results

    Nutrição e alimentação no manejo da doença de Alzheimer: revisão sistemática e meta-análise

    Alzheimer\'s disease (AD) is one of the main causes of dependency and disability in the elderly population. A number of investigations have been seeking its prevention and/or management. In this context, it is important to highlight the role of modifiable risk factors, such as nutrition. This study aims to conduct a systematic review and subsequent meta-analysis, to assess the effect of food and/or nutrients for the management of AD at different stages. This work was steered based on the Cochrane Handbook for systematic reviews of interventions and the PRISMA Statement. Electronic databases were searched up to 2014, in Portuguese, English or Spanish. Relevant publications were identified by title and abstract using key search terms referring to Alzheimer\'s disease, nutrition interventions and type of study. Trials\' risk of bias was appraised by applying the Cochrane\'s tool for assessing risk of bias. The main outcome measures comprise neuropsychological tests such as MMSE, ADCS-ADL, NPI and CDR-sob, biomarkers and brain imaging. Pairwise meta-analyses were performed in a random-effect model by estimating the weighted mean differences between treatment and placebo groups, with 95% confidence intervals for outcome measures by treatment. Network meta-analysis and the ranking probability of treatment for each nutrition intervention were undertaken on cognitive outcome. The strength and quality of evidence were rated according to the GRADE approach. From the whole research, 182 studies met the systematic review\'s purpose. Thirty-five clinical trials complied with eligibility criteria and risk of bias assessment. Included studies utilized: antioxidants, B-vitamin complex, carbohydrates, lipids, omega-3 fatty acids, polymeric formulas, polypeptide and vitamin D. Estimates treatment effects from pairwise meta-analyses show a significant positive effect from the supplementation with proline-rich polypeptide (WMD 12.00 [95% CI 10.20, 13.80] P < 0.00001) and B-vitamin complex (WMD 0.44 [95% CI 0.09, 0.79] P = 0.01) on cognitive function measured by the MMSE. Remaining nutrients supplementation did not show any significant effect on functional, behavioral, global performance, biomarkers or brain imaging outcomes. Isolated nutrient supplementations show no convincing evidence of providing a significant benefit on clinical manifestations or neuropathology of AD. As a treatment strategy, nutrients did not show any effect when delivered individually, probably due to their synergistic work on brain function at different domains. Nevertheless, nutrients represent a potential preventive approach and an adjuvant treatment for patients with AD at earlier stages.A doença de Alzheimer (DA) é uma das maiores causas de dependência e incapacidade na população idosa, o que tem levado a inúmeras investigações sobre sua prevenção e ou manejo. Neste contexto, é importante destacar o papel desempenhado pelos fatores de risco modificáveis, como a nutrição. Este estudo trata de uma revisão sistemática e meta-análise, para avaliar o efeito das intervenções nutricionais no manejo da DA, em seus diferentes estágios. Este trabalho segue as propostas da Colaboração Cochrane e a declaração PRISMA. Bases de dados eletrônicas foram pesquisadas a partir do seu início até o 2014, em Português, Inglês ou Espanhol. Estudos relevantes foram identificados por título e resumo usando as palavras-chave referente à doença de Alzheimer, intervenções nutricionais e tipo de estudo. A qualidade dos estudos foi avaliada mediante a ferramenta da Cochrane para avaliação do risco de viés. As principais medidas de desfechos compreenderam os testes neuropsicológicos MEEM, AVD, NPI e CDR-sob, biomarcadores e neuroimagem. As meta-análises em pares foram realizadas em modelo de efeito aleatório pela estimativa de diferença de médias ponderadas entre os grupos de tratamento e placebo, com 95% de intervalo de confiança para as medidas de desfecho segundo a intervenção. A meta-análise em rede e a probabilidade da posição do tratamento para cada intervenção nutricional foi realizada para o desfecho cognitivo. A força e a qualidade da evidência foram avaliadas de acordo com o método GRADE. Da busca total inicial, 182 estudos cumpriam com o propósito desta revisão sistemática. Ainda, 35 ensaios clínicos preencheram os critérios de elegibilidade e avaliação de risco de viés. Os estudos incluídos usaram: antioxidantes, vitaminas do complexo B, carboidratos, lipídeos, ácidos graxos ômega-3, formula poliméricas, polipeptídios e vitamina D. As estimativas de efeito do tratamento das meta-análises em pares mostraram um efeito positivo significativo a partir da suplementação com um polipeptídio rico em prolina (MD 12.00 [95% IC 10.20, 13.80] P < 0.00001) e com as vitaminas do complexo B (MD 0.44 [95% IC 0.09, 0.79] P = 0.01) na função cognitiva avaliada pelo MEEM. A suplementação com os demais nutrientes não mostrou um efeito significativo na funcionalidade, comportamento, desempenho global, biomarcadores da DA, nem desfechos de imagem. A suplementação com nutrientes isolados não mostrou um efeito significativo nas manifestações clínicas ou neuropatologicas da DA. Como estratégia de tratamento, os nutrientes não demonstraram um efeito separadamente, provavelmente devido a seu trabalho sinérgico nos diferentes domínios da função cerebral. Ainda assim, os nutrientes representam uma abordagem preventiva potencial e um tratamento adjuvante nas pessoas com DA nos estágios iniciais
