There Is No Place Like Home: The Impact of Public Home‐Based Care on the Mental Health and Well‐Being of Older People
Despite a significant policy shift from institutional to home-based care for older adults, evidence on the effectiveness of policies incentivizing home care is limited. This study provides novel evidence on the causal effect of public home-based care on the mental health and well-being of older people. To address endogenous selection, we implement a novel instrumental variable approach that exploits eligibility rules for long-term care as defined in national legislations. We link longitudinal data from the Survey of Health, Aging & Retirement in Europe (SHARE, 2004–2017) to national LTC eligibility rules in France, Germany, Spain and Belgium (disaggregated for Wallonia and Flanders regions) and examine how exogenous variation in the use of long-term care caused by varying eligibility rules impacts depressive symptoms (EURO-D scale), quality of life (CASP scale) and loneliness (R-UCLA scale). We find that receiving formal home-based care significantly reduces depressive symptom scores by 2.6 points (large effect size measured by Cohen's d) and the risk of depression by 13 percentage points. The use of home-based formal care also increases quality of life as measured by the CASP scale, particularly by increasing feelings of control over life. We show that one potential mechanism involves the impact of home-based care on loneliness: we estimate that receiving formal home-based care reduces the risk of loneliness by 6.7 percentage points. Our results provide evidence that an increase in home-based care coverage is justified in terms of improved mental health and well-being outcomes for older people
Has the UK Campaign to End Loneliness reduced loneliness and improved mental health in older age? A difference-in-differences design
Objective: To estimate the impact of the UK nationwide campaign to End loneliness on loneliness and mental health outcomes among older people in England. Design: Quasi-experimental design, namely, a difference-in-differences approach. Setting: Local authorities across England. Participants: Older adults aged 65 and over participating in waves 4–8 (2008–2017) of the English Longitudinal Study of Aging (ELSA) and waves 1–9 (2009–2019) of the UK Household Longitudinal Study (UKHLS). Main outcome measures: Loneliness was measured through the UCLA Loneliness scale. A social isolation scale with components of household composition, social contact and participation was constructed. Mental health was measured by The Centre for Epidemiological Studies of Depression (CES-D) score, the General Health Questionnaire (GHQ-12) score, and the Short-Form-12 Mental Component Summary (SF-12 MCS) score. Results: There was no evidence of change in loneliness scores over the study period. Difference-in-differences estimates suggest that explicitly developed and implemented antiloneliness strategies led to no change in loneliness scores (estimate = 0.044, SE = 0.085), social isolation caseness (estimate = 0.038, SE = 0.020) or levels of depressive symptoms (estimate = 0.130, SE = 0.165). Heterogeneity analyses indicate that antiloneliness strategies produced little impact on loneliness or mental health overall, despite small reductions in loneliness and increases in social engagement among well-educated and higher-income older adults. The results were robust to various sensitivity and robustness analyses. Conclusions: Antiloneliness strategies implemented by local authorities have not generated a significant change in loneliness or mental health in older adults in England. Generating changes in loneliness in the older population might require longer periods of exposure, larger scope of intervention or more targeted strategies.</p

The book is the third of the se-ries called Evaluation Globe. The review highlights the theoretical and an-alytical framework of the study, which analyses the institutionalization level of the evaluation from the political, social, and professional dimen-sions. The comparative analysis between countries with diverse sizes, economic development levels, and forms of government prompts a discus-sion on the definition of evaluation, its role in supporting more democratic and inclusive societies, and ways to counter purely formal evaluation pro-cesses. Rand, Erica - 2022 Follow Up
Erica Rand is a professor of Arts and Visual Culture at Bates College, an adult figure skater, author and activist. This is a follow-up interview to her previous interview for Querying the Past in 2017. Erica Rand was heavily involved with ACT- UP Portland and more specifically the branch of ACT UP called: Pissed Off Dyke Cell and Women’s Health Action Crew. But more recently she has been involved with a new form of activism through sports and writing. She took her Masters in English Literature at Flinders University and taught in Girls and Boys Technical Colleges for 40 years.
