The role of surgery in the treatment of older women with breast cancer
A significant proportion of women diagnosed with breast cancer are over the age of 70 years and there is evidence that these patients frequently do not receive standard treatments, including surgical procedures and adjuvant therapies, which would be routine practice in younger age groups. The factors underlying this may include the physiological effects of ageing, differences in the biology and stage of the tumour at presentation, patient co-morbidities and patient and clinician preferences. The interaction of all these factors needs to be considered when individualising treatment plans for patients. For some patients this will need to be undertaken in the context of an extended multidisciplinary team setting with additional input from geriatricians, in addition to surgeons and oncologists, in defining a treatment plan. Little is known about the preferences of older patients in their choice of surgical treatment for breast cancer and further research is required to increase the evidence base for the rational management of older women with breast cancer

PDT-induced cell death, by either apoptosis or necrosis may vary with cell type or PDT dose. 5 cell types were treated with varying doses of aminolaevulinic acid-induced PDT and the type of cell death analysed. Leucocyte interactions with the mouse cremaster muscle microcirculation in vivo in response to tumour-conditioned medium
Leucocyte interactions with the cremaster muscle microcirculation in vivo were investigated in response to culture medium conditioned with different cell types in 25 adult male Swiss mice. Animals were divided into five groups. Three groups received ex vivo fluorescently labelled lymphokine activated killer (LAK) cells systemically and had either tumour (murine melanoma K1735)-conditioned medium (TCM), fibroblast (murine 3T3)-conditioned medium (FCM) or fresh culture medium administered topically to the cremaster muscle. In the two remaining groups, the host leucocytes were labelled fluorescently by systemic administration of acridine red, and either TCM or FCM was applied topically to the cremaster muscle. There was an immediate but transient increase in the frequency of rolling and adherent LAK cells, and a subsequent (90-120 min later) increase in rolling and adherent host leucocytes, demonstrating temporal differences in the response to topical administration of TCM. These increases in contact with the vascular endothelium occurred in all vessel types, venules, arterioles and capillaries, with the greatest response observed in the venules. The FCM and normal culture medium did not affect the distribution and localization of either LAK cells or host leucocytes. These data suggest that there are one or more soluble tumour-specific chemoattractants for leucocytes present in the conditioned medium. The mouse cremaster muscle microcirculation is therefore a useful model to investigate the mechanism of leucocyte-endothelium interactions in tumour biology

The effect of aminolaevulinic acid-induced, protoporphyrin IX-mediated photodynamic therapy on the cremaster muscle microcirculation in vivo
The effect of photodynamic therapy on normal striated muscle was investigated using 30 adult male rats. Animals were divided into six groups. Three control groups received phosphate-buffered saline by gavage and violet light at 105, 178 and 300 mW cm-2 respectively. Three experimental groups received aminolaevulinic acid (ALA; 200 mg kg-1) and violet light at 105, 178 and 300 mW cm-2 respectively. After exposure of the cremaster muscle animals were allowed to equilibrate and vessel diameters and bloodflow assessed. Following photoactivation measurements were taken every 10 min over a 2 h period. Photoactivation of experimental groups at the two higher power densities resulted in an initial decrease in both arteriolar and venular diameters, and a concomitant decrease in blood flow. The magnitude of these changes and the degree of recovery by the end of the observation period was related to power density. No effects were observed in the control groups. These results suggest that microcirculatory damage may contribute to the mechanism of action of photodynamic therapy with ALA
