    Das Raynaud-Syndrom ist mit einer Prävalenz von 3-5% ein häufiges klinisches Problem. Dennoch ist die Wirkung der meisten Therapiemöglichkeiten nur unzureichend durch kontrollierte Studien belegt. Zu den Therapien mit höherem Evidenzgrad gehört der Kalziumantagonist Nifedipin, für den in Metaanalysen sowohl bei primärem als auch bei sekundärem Raynaud-Syndrom eine verbesserte periphere Durchblutung sowie eine Abnahme der Frequenz und des Schweregrades der Raynaud-Attacken nachgewiesen werden konnte. Ähnliches gilt für intravenös appliziertes Iloprost in der Therapie des sekundären Raynaud-Syndroms bei systemischer Sklerose. Intravenös verabreichtes Iloprost verbessert darüber hinaus das Abheilen von Fingerkuppenulzera bei Patienten mit systemischer Sklerose. Vielversprechende Therapieansätze stellen Angiotensin-II-Rezeptor-1-Antagonisten (Losartan), die Kalziumantagonisten Felodipin und Amlodipin, Serotonin-Reuptake-Hemmer (Fluoxetin) und Phosphodiesterase-V-Hemmer (Sildenafil, Vardenafil) dar, die sich in kontrollierten Einzelstudien als wirksam erwiesen haben. Jedoch fehlen Erfahrungen mit größeren Patientenzahlen und längeren Anwendungszeiten, um diese Therapiemöglichkeiten abschließend zu beurteile

However, the mechanism of the role of BoNT/A in antagonizing the constriction of arteriola in RP remains unclear. Materials and methods: We tested the constriction of arteriole diameter and the distribution of adrenergic receptors on the rat cremaster modle. Moreover, we measured the secretion of norepinephrine (NE), protein level changes and related receptors on cultured rat superior cervical ganglia neurons(SCGNs), a model of sympathetic neuron. Results: Based on our results, the inhibition of arteriole vasoconstriction was increased with increasing doses of BoNT/A. BoNT/Α, prazosin, and BQ123 treatment can result in significant inhibition of arteriole vasoconstriction with the same electrical stimulation. The inhibition effect of prazosin was equivalent to BoNT/A, while BQ123 has a synergistic effect with BoNT/A. After treating SCGNs using BoNT/A for 30 min, the decrease in fluorescence intensity of FM1-43 slowed down which was correlated with the doses of BoNT/A. Furthermore, release of NE in the supernatant was significantly decreased as measured by enzyme-linked immunosorbent assay, 24 h after a high dose of BoNT/A (25 µ/mL). Cleaved-SNAP-25 was detected by western blotting 24 h following BoNT/A (50 µ/mL) treatment. Moreover, receptor SV2C, GM1, and FGFR3 were detected on sympathetic neurons, similarly to cholinergic neurons. Conclusion: Our study showed that BoNT/A could significantly inhibit electrical stimulation-induced arteriole vasoconstriction through the sympathetic pathway. The mechanism was similar to the cholinergic one, in which the vesicle release of sympathetic neurons could be inhibited by cleavage of SNAP-25. The end result was blocked vesicle fusion with the presynaptic membrane after BoNT/A treatment, inhibiting the release of the NE.     BACKGROUND: Raynaud\u27s phenomenon is an episodic, reversible spasm of the digital arterioles. Previous studies have suggested that Raynaud\u27s phenomenon may be associated with alterations in the ocular circulation. The authors used the blue field entoptic simulation technique to study macular retinal capillary hemodynamics in patients with Raynaud\u27s phenomenon. METHODS: Forty-two volunteers participated in the study: 18 with a connective tissue disease and secondary Raynaud\u27s phenomenon, 8 with a connective tissue disease and no Raynaud\u27s phenomenon, and 16 healthy volunteers. The mean of three blue field entoptic density and velocity measurements was determined before as well as during and 10 minutes after the immersion of one hand in ice water. RESULTS: Patients with severe Raynaud\u27s phenomenon demonstrated a statistically significant 30% decrease in retinal capillary flow during immersion of a hand in ice water (P less than 0.05), which remained 17% reduced (P less than 0.05) for at least 10 minutes after removal. The decrease in capillary flow was not observed in control individuals or in individuals with connective tissue disease who demonstrated minimal or no Raynaud\u27s phenomenon. CONCLUSION: The prolonged reduction in retinal capillary flow observed in subjects with Raynaud\u27s phenomenon is probably caused by retinal arteriolar vasoconstriction and is similar to concurrent arteriolar vasoconstriction reported in other systemic vascular beds.     Free-form, form finding and anisotropic grid shell

    p. 966-876The new geometrical developments open new perspectives for free-form design, making it possible to escape from planar triangular or quadrilateral discretizations. Recent advances in theory algorithms allow for the discretization of any surface using only single curvature panels thus allowing the realisation of smooth double curvature glazed envelops of any form. Grid shell structures usually present a nearly in plane uniform behaviour, but previous realisations have shown that grid shells can be designed also according to an anisotropic inplane arrangement. The control of principal direction and the fine tuning of the stiffness of the different structural elements (arcs, cables etc.) is a tool for adjusting the form-finding thus controlling the resulting geometry. Moreover, the form-finding can also be performed without researching a constant stress (self weight); in this case an even wider range of forms become possible. These new geometrical and structural approaches have been coupled together and tested in re-designing, as a case study, the glazed roof of the Neumunster Abbey in Luxembourg. Such approach allowed for the conception of an efficient structure supporting a smooth double curvature glass skin, made out of only single curvature panels, perfectly coherent with the perimeter of the courtyard i.e. matching all the edges without any gaps.Baldassini, N.; Raynaud, J.
