    Patterns of care for chronic patients after the Italian psychiatric reform: A longitudinal case register study

    Identified chronic psychiatric patients in the community over a 6-yr period after the Italian psychiatric reform. 151 long-term patients (Ss in contact with psychiatric services) and 25 long-stay patients (Ss in hospitals) were followed to determine subsequent patterns of care. Six 1st-contact sociodemographic variables (sex, age, marital status, living situation, education, occupational status) and 2 clinical variables (International Classification of Diseases-9 diagnosis and past history of state psychiatric hospital admission) were studied for these Ss. 36.4% of postreform chronic Ss in the community remained in long-term contact with psychiatric services for 2 yrs after they were first identified. None of the sociodemographic and clinical variables studied individually or in combination were predictive of the probability of remaining in long-term contact with community psychiatric services.     Suicide and unemployment in Italy: Description, analysis and interpretation of recent trends

    Provides descriptive, longitudinal, and cross-sectional aggregate-level analyses and also trends in individual-level and population risks for suicide in relation to unemployment from 1977 to 1987. Evidence for an association between suicide and unemployment among women was not convincing. Individual-level analyses for each year confirmed that unemployed women were more likely to commit suicide than their employed counterparts, although the overall relative risk was low. Among men, the unemployment rate (UR) was positively correlated over time with the suicide rate (SR). However, change in the SR across 18 geographic regions of Italy was unrelated to change in the UR. Unemployment was also positively related to the SR among the employed and population attributable risk, but unrelated to the rate among the unemployed or the relative risk.     Geographical variation in the seasonality of suicide

    This paper reports the results of a study on the seasonal variation of suicides occurred in Italy during the years 1969-1981. We found evidence for a cyclical pattern with one cycle per year for North/Central as well as for South Italy with no difference between the two parts in the variance explained by it. However, urban regions showed a higher seasonality than the rural parts of Italy, independently of their geographical location. To account for these findings, the hypothesis of a strong seasonal related urban rhythm has been put forward

    Meal-induced thermogenesis in lean and obese prepubertal children

    The resting metabolic rate (RMR) and the thermic effect of a meal (TEM) were measured in a group of 16 prepubertal (8.8 +/- 0.3 y) obese children (43.6 +/- 9.2 kg) and compared with a group of 10 age-matched (8.6 +/- 0.4 y), normal-weight children (31.0 +/- 6.0 kg). The RMR was higher in the obese than in the control children (4971 +/- 485 vs 4519 +/- 326 kJ/d, P < 0.05); after the RMR was adjusted for the effect of fat-free mass (FFM) the values were not significantly different (4887 +/- 389 vs 4686 +/- 389 kJ/d). The thermic response to a liquid mixed meal, expressed as a percentage of the energy content of the meal, was significantly lower in obese than in control children (4.4 +/- 1.2% vs 5.9 +/- 1.7%, P < 0.05). The blunted TEM shown by the obese children could favor weight gain and suggests that the defect in thermogenesis reported in certain obese adults may have already originated early in life
